Objective: To determine whether implementing a Facebook training program improves the effectiveness of computerized cognitive training (CCT) in older adults. Design: Randomized, controlled, double single-blind trial with parallel groups. Setting: Community centers. Subjects: Eighty-six adults between 60 and 90 years old. Interventions: Nine face-to-face 60-min sessions of CCT with VIRTRAEL for all participants. The experimental group received an additional 30 min of Facebook training per session. Main measures: Attention (d2 Test of Attention); learning and verbal memory (Hopkins Verbal Learning Test-Revised); working memory (Letter-Number Sequencing test), semantic and abstract reasoning (Similarities and Matrix Reasoning tests); and planning (Key Search test). Results: There was a significant Group*Time interaction in the Hopkins Verbal Learning Test-Revised-Trial 3, Letter-Number sequencing, and Matrix tests. Between groups, post-hoc analyses showed a difference in Matrix reasoning (p <.001; d = 0.893) at post-intervention in favor of the experimental group. Significant main effects of time were found in the CCT group between baseline and 3-month follow-up for Concentration (F = 26.431, p ≤.001), Letters and Numbers (F = 30.549, p ≤.001), Learning (F = 38.678, p ≤.001), Similarities (F = 69.885, p ≤.001), Matrix (F = 90.342, p ≤.001), and Key Search (F = 7.904, p =.006) tests. Conclusions: The utilization of CCT with VIRTRAEL, a freely accessible tool with broad applicability, resulted in enhanced attention, verbal learning, working memory, abstract and semantic reasoning, and planning among older adults. This triple-arm, double-blind, randomized clinical trial investigated the effect of multisession prefrontal transcranial direct current stimulation (tDCS) on social functioning in individuals with autism spectrum disorder (ASD). A total of 105 individuals (age 14–21 years) diagnosed with ASD were randomized into the active-tDCS, sham-tDCS, and waitlist control groups. The experimental group received 20 min of 1.5 mA tDCS stimulation (cathode: F3; anode: Fp2) for 10 days with concurrent computerized cognitive remediation training. Changes in overall social functioning, social communication, and restricted, repetitive behaviors (RRB) were measured by the Social Responsiveness Scale-2nd edition (SRS-2). Two-level hierarchical linear mixed-model analysis showed significant group*time interactions for SRS-2 total [ F (2, 107.09) = 7.82; p = 0.001] and RRB [ F (2, 90.26) = 5.62; p = 0.005] T-scores. When compared with the waitlist control group, the changes in scores were only significant in active-tDCS (SRS-2 total T-score p < 0.001, d = 0.61; SRS-2 RRB T-score p = 0.002, d = 0.88), but not sham-tDCS (SRS-2 total T-score p < 0.12, d = 0.30; SRS-2 RRB T-score p = 0.54, d = 0.17) group. Multiple sessions of prefrontal tDCS coupled with cognitive training is a safe and effective treatment for improving social functioning in people with ASD. Currently available pharmacological and behavioral interventions for adolescents and young adults with autism spectrum disorder (ASD) yield only modest effect in alleviating their core behavioral and cognitive symptoms, and some of these treatment options are associated with undesirable side effects. Hence, developing effective treatment protocols is urgently needed. Given emerging evidence shows that the abnormal connections of the frontal brain regions contribute to the manifestations of ASD behavioral and cognitive impairments, noninvasive treatment modalities that are capable in modulating brain connections, such as transcranial direct current stimulation (tDCS), have been postulated to be potentially promising for alleviating core symptoms in ASD. However, whether tDCS can reduce behavioral symptoms and enhance cognitive performance in ASD remains unclear. This randomized controlled trial involving 105 adolescents and young adults with ASD showed that multiple sessions of a tDCS protocol, which was paired up with computerized cognitive training, was effective in improving social functioning in adolescents and young adults with ASD. No prolonged and serious side effects were observed. With more future studies conducted in different clinical settings that recruit participants from a wider age range, this tDCS protocol may be potentially beneficial to a broad spectrum of individuals with autism. Objective: To assess the effectiveness of Wii sports-based strategy training on risk of falling, falls and quality of life in adults with idiopathic Parkinson's disease. Design: Single blind, Randomised comparative trial. Setting: Hospital and home Subjects: Diagnosed as idiopathic Parkinson's disease, Hoehn-Yahr stage 2.5 to 4, history of at least 3 falls in the last 3 months. Intervention: Experimental group participated in 30–40 min of Wii training, 3 session/week for 12 weeks and control group participated for the same duration in traditional balance training. During each session both the group received 30 min of conventional physiotherapy. Outcomes: Number of fallers, Fall rate, Berg balance scale, Timed up and go test, Parkinson's disease questionnaire 39, at baseline, 12th and 36th week after baseline. Results: We randomised 192 participants. Participant's characteristics were similar between Wii and control group at base line, in age 69.7 ± 10, 68.5 ± 9.8, disease severity 3.43 ± 0.56, 3.42 ± 0.59, and fall rate 10.47 ± 15.78, 11.80 ± 18.95 (in mean ± SD). At 12th week 28%(27), 51%(49) & 36th week 45%(42), 60%(57) in Wii-group, control group fell at least once, respectively. Wii group improved more than control group at 12th, 36th week in number of fallers (−23%, p < 0.001) (−15%, p < 0.039), fall rates −2.635(−5.38 to 0.112), −1.476(−5.09 to 2.142) (difference in mean, Confidence Interval) and bodily discomfort component of PDQ 39. Conclusion: A 12 weeks exercise training using the Wii sports-based strategy decreases the number of fallers, fall rate, measures of risk of falling but did not alter the quality of life in adults with idiopathic Parkinson's disease. Objective: To investigate an intensive asynchronous computer-based treatment delivered remotely with clinician oversight to people with aphasia. Design: Single-blind, randomized placebo-controlled trial. Setting: Free-standing urban rehabilitation hospital. Participants: Adults with aphasia (at least six months post-onset). Interventions: Experimental treatment was Web ORLA® (Oral Reading for Language in Aphasia) which provides repeated choral and independent reading aloud of sentences with a virtual therapist. Placebo was a commercially available computer game. Participants were instructed to practice 90 minutes/day, six days/week for six weeks. Main measures: Change in Language Quotient of the Western Aphasia Battery-Revised from pre-treatment to post-treatment and pre-treatment to six weeks following the end of treatment. Results: 32 participants (19 Web ORLA®, 13 Control) completed the intervention and post-treatment assessment; 27 participants (16 Web ORLA®, 11 Control) completed the follow-up assessment six weeks after treatment had ended. Web ORLA® treatment resulted in significant improvements in language performance from pre-treatment to immediately post-treatment (X = 2.96; SD = 4.32; P < 0.01; ES = 0.68) and from pre-treatment to six weeks following the end of treatment (X = 4.53; SD = 3.16; P < 0.001; ES = 1.43). There was no significant difference in the gain from pre-treatment to post-treatment for the Web ORLA® versus Control groups. However, the Web ORLA® group showed significantly greater gains at the six-week follow-up than the control group (X = 2.70; SD = 1.01; P = 0.013; ES = 1.92). Conclusion: Results provide evidence for improved language outcomes following intensive, web-based delivery of ORLA® to individuals with chronic aphasia.
