Objective. This study describes geometry-based and intensity-based tools for quality assurance (QA) of automatically generated structures for online adaptive radiotherapy, and designs an operator-independent traffic light system that identifies erroneous structure sets. Approach. A cohort of eight head and neck (HN) patients with daily CBCTs was selected for test development. Radiotherapy contours were propagated from planning computed tomography (CT) to daily cone beam CT (CBCT) using deformable image registration. These propagated structures were visually verified for acceptability. For each CBCT, several error scenarios were used to generate what were judged unacceptable structures. Ten additional HN patients with daily CBCTs and different error scenarios were selected for validation. A suite of tests based on image intensity, intensity gradient, and structure geometry was developed using acceptable and unacceptable HN planning structures. Combinations of one test applied to one structure, referred to as structure-test combinations, were selected for inclusion in the QA system based on their discriminatory power. A traffic light system was used to aggregate the structure-test combinations, and the system was evaluated on all fractions of the ten validation HN patients. Results. The QA system distinguished between acceptable and unacceptable fractions with high accuracy, labeling 294/324 acceptable fractions as green or yellow and 19/20 unacceptable fractions as yellow or red. Significance. This study demonstrates a system to supplement manual review of radiotherapy planning structures. Objective. To develop and benchmark a novel 3D dose verification technique consisting of polymer gel dosimetry (PGD) with cone-beam-CT (CBCT) readout through a two-institution study. The technique has potential for wide and robust applicability through reliance on CBCT readout. Approach. Three treatment plans (3-field, TG119-C-shape spine, 4-target SRS) were created by two independent institutions (Institutions A and B). A Varian Truebeam linear accelerator was used to deliver the plans to NIPAM polymer gel dosimeters produced at both institutions using an identical approach. For readout, a slow CBCT scan mode was used to acquire pre- and post-irradiation images of the gel (1 mm slice thickness). Independent gel analysis tools were used to process the PGD images (A: VistaAce software, B: in-house MATLAB code). Comparing planned and measured doses, the analysis involved a combination of 1D line profiles, 2D contour plots, and 3D global gamma maps (criteria ranging between 2%1 mm and 5%2 mm, with a 10% dose threshold). Main results. For all gamma criteria tested, the 3D gamma pass rates were all above 90% for 3-field and 88% for the SRS plan. For the C-shape spine plan, we benchmarked our 2% 2 mm result against previously published work using film analysis (93.4%). For 2%2 mm, 99.4% (Institution A data), and 89.7% (Institution B data) were obtained based on VistaAce software analysis, 83.7% (Institution A data), and 82.9% (Institution B data) based on MATLAB. Significance. The benchmark data demonstrate that when two institutions follow the same rigorous procedures gamma passing rates up to 99%, for 2%2 mm criteria can be achieved for substantively different treatment plans. The use of different software and calibration techniques may have contributed to the variation in the 3D gamma results. By sharing the data across institutions, we observe the gamma passing rate is more consistent within each pipeline, indicating the need for standardized analysis methods. [ABSTRACT FROM AUTHOR]

Purpose. Fast kV-switching (FKS) and dual-layer flat-panel detector (DL-FPD) technologies have been actively studied as promising dual-energy spectral imaging solutions for FPD-based cone-beam computed tomography (CT). However, cone-beam CT (CBCT) spectral imaging is known to face challenges in obtaining accurate and robust material discrimination performance. That is because the energy separation by either FKS or DL-FPD, alone, is still limited, along with apparently unpaired signal levels in the effective low- and high-energy projections in real applications, not to mention the x-ray scatter in cone-beam scan which will make the material decomposition almost impossible if no correction is applied. To further improve CBCT spectral imaging capability, this work aims to promote a source-detector joint multi-energy spectral imaging solution which takes advantages of both FKS and DL-FPD, and to conduct a feasibility study on the first tabletop CBCT system with the joint spectral imaging capability developed. Methods. For CBCT, development of multi-energy spectral imaging can be jointly realized by using an x-ray source with a generator whose kilo-voltages can alternate in tens of Hertz (i.e. FKS), and a DL-FPD whose top- and bottom-layer projections corresponds to different effective energy levels. Thanks to the complimentary characteristics inherent in FKS and DL-FPD, the overall energy separation will be significantly better when compared with FKS or DL-FPD alone, and the x-ray photon detection efficiency will be also improved when compared with FKS alone. In this work, a noise performance analysis using the Cramér–Rao lower bound (CRLB) method is conducted. The CRLB for basis material after a projection-domain material decomposition is derived, followed by a set of numerical calculations of CRLBs, for the FKS, the DL-FPD and the joint solution, respectively. To compensate for the slightly angular mismatch between low- and high- projections in FKS, a dual-domain projection completion scheme is implemented. Afterwards material decomposition from the complete projection data is carried out by using the maximum-likelihood method, followed by reconstruction of basis material and virtual monochromatic images (VMI). In this work, the first FKS and DL-FPD jointly enabled multi-energy tabletop CBCT system, to the best of our knowledge, has been developed in our laboratory. To evaluate its spectral imaging performance, a set of physics experiments are conducted, where the multi-energy and head phantoms are scanned using the 80/105/130 kVp switching pairs and projection data are collected using a prototype DL-FPD, whose both top and bottom layer of panels are composed of 550 μ m of cesium iodine (CsI) scintillators with no intermediate metal filter in-between. Results. The numerical simulations show that the joint spectral imaging solution can lead to a significant improvement in energy separation and lower noise levels in most of material decomposition cases. The physics experiments confirmed the feasibility and superiority of the joint spectral imaging, whose CNRs in the selected regions of interest of the multi-energy phantom were boosted by an average improvement of 21.9%, 20.4% for water basis images and 32.8%, 62.8% for iodine images when compared with that of the FKS and DL-FPD, respectively. For the head phantom case, the joint spectral imaging can effectively reduce the streaking artifacts as well, and the standard deviation in the selected regions of interest are reduced by an average decrement of 19.5% and 8.1% for VMI when compared with that of the FKS and DL-FPD, respectively. Conclusions. A feasibility study of the joint spectral imaging solution for CBCT by utilizing both the FKS and DL-FPD was conducted, with the first tabletop CBCT system having such a capability being developed, which exhibits improved CNR and is more effective in avoiding streaking artifacts as expected. Objective. The aim of this study was to reconstruct volumetric computed tomography (CT) images in real-time from ultra-sparse two-dimensional x-ray projections, facilitating easier navigation and positioning during image-guided radiation therapy. Approach. Our approach leverages a voxel-sapce-searching Transformer model to overcome the limitations of conventional CT reconstruction techniques, which require extensive x-ray projections and lead to high radiation doses and equipment constraints. Main results. The proposed XTransCT algorithm demonstrated superior performance in terms of image quality, structural accuracy, and generalizability across different datasets, including a hospital set of 50 patients, the large-scale public LIDC-IDRI dataset, and the LNDb dataset for cross-validation. Notably, the algorithm achieved an approximately 300% improvement in reconstruction speed, with a rate of 44 ms per 3D image reconstruction compared to former 3D convolution-based methods. Significance. The XTransCT architecture has the potential to impact clinical practice by providing high-quality CT images faster and with substantially reduced radiation exposure for patients. Objective. To demonstrate that complete cone beam CT (CBCT) scans from both MV-energy and kV-energy LINAC sources can reduce metal artifacts in radiotherapy guidance, while maintaining standard-of-care x-ray doses levels. Approach. MV-CBCT and kV-CBCT scans are acquired at half normal dose. The impact of lowered dose on MV-CBCT data quality is mitigated by the use of a 4-layer MV-imager prototype and reduced LINAC energy settings (2.5 MV) to improve photon capture. Additionally, the MV-CBCT is used to determine the 3D position and pose of metal implants, which in turn is used to guide model-based poly-energetic correction and interleaving of the kV-CBCT and MV-CBCT data. Certain edge-preserving regularization steps incorporated into the model-based correction algorithm further reduce MV data noise. Main results. The method was tested in digital phantoms and a real pelvis phantom with large 2.5″ spherical inserts, emulating hip replacements of different materials. The proposed method demonstrated an appealing compromise between the high contrast of kV-CBCT and low artifact content of MV-CBCT. Contrast-to-noise improved 3-fold compared to MV-CBCT with a clinical 1-layer architecture at matched dose (37 mGy) and edge blur levels. Visual delineation of the bladder and prostate improved noteably over kV- or MV-CBCT alone. Significance. The proposed method demonstrates that a full MV-CBCT scan can be combined with kV-CBCT to reduce metal artifacts without resorting to complicated beam collimation strategies to limit the MV-CBCT dose contribution. Cone-beam computed tomography (CBCT) is widely used in dental imaging, small animal imaging, radiotherapy, and non-destructive industrial inspection. The quality of CBCT images depends on the precise knowledge of the CBCT system's alignment. We introduce a distinct procedure, 'precision alignment loop (PAL)', to calibrate any CBCT system with a circular trajectory. We describe the calibration procedure by using a line-beads phantom, and how PAL determines the misalignments from a CBCT system. PAL also yields the uncertainties in the simulated calibration to give an estimate of the errors in the misalignments. From the analytical simulations, PAL can precisely obtain the source-to-rotation axis distance, and the geometric center G, 'the point in z -axis meets the detector', where the z -axis is coincident with the line from the x-ray source that intersects the axis of the rotation orthogonally. The uncertainties of three misalignment angles of the detector are within ±0.05°, which is close to ±0.04° for the results of Yang et al (2017 Med. Phys. 44 1692), but our method is easy and simple to implement. Our distinct procedure, on the other hand, yields the calibration of a micro-CT system and an example of reconstructed images, showing our calibration method for the CBCT system to be simple, precise, and accurate. Objective. To create two non-coplanar, stereotactic ablative radiotherapy (SABR) lung patient treatment plans compliant with the radiation therapy oncology group (RTOG) 0813 dosimetric criteria using a simple, isocentric, therapy with kilovoltage arcs (SITKA) system designed to provide low cost external radiotherapy treatments for low- and middle-income countries (LMICs). Approach. A treatment machine design has been proposed featuring a 320 kVp x-ray tube mounted on a gantry. A deep learning cone-beam CT (CBCT) to synthetic CT (sCT) method was employed to remove the additional cost of planning CTs. A novel inverse treatment planning approach using GPU backprojection was used to create a highly non-coplanar treatment plan with circular beam shapes generated by an iris collimator. Treatments were planned and simulated using the TOPAS Monte Carlo (MC) code for two lung patients. Dose distributions were compared to 6 MV volumetric modulated arc therapy (VMAT) planned in Eclipse on the same cases for a Truebeam linac as well as obeying the RTOG 0813 protocols for lung SABR treatments with a prescribed dose of 50 Gy. Main results. The low-cost SITKA treatments were compliant with all RTOG 0813 dosimetric criteria. SITKA treatments showed, on average, a 6.7 and 4.9 Gy reduction of the maximum dose in soft tissue organs at risk (OARs) as compared to VMAT, for the two patients respectively. This was accompanied by a small increase in the mean dose of 0.17 and 0.30 Gy in soft tissue OARs. Significance. The proposed SITKA system offers a maximally low-cost, effective alternative to conventional radiotherapy systems for lung cancer patients, particularly in low-income countries. The system's non-coplanar, isocentric approach, coupled with the deep learning CBCT to sCT and GPU backprojection-based inverse treatment planning, offers lower maximum doses in OARs and comparable conformity to VMAT plans at a fraction of the cost of conventional radiotherapy. Objective. This study aims at investigating a novel super resolution CBCT imaging approach with a dual-layer flat panel detector (DL-FPD). Approach. With DL-FPD, the low-energy and high-energy projections acquired from the top and bottom detector layers contain over-sampled spatial information, from which super-resolution CT images can be reconstructed. A simple mathematical model is proposed to explain the signal formation procedure in DL-FPD, and a dedicated recurrent neural network, named suRi-Net, is developed based upon the above imaging model to nonlinearly retrieve the high-resolution dual-energy information. Physical benchtop experiments are conducted to validate the performance of this newly developed super-resolution CBCT imaging method. Main Results. The results demonstrate that the proposed suRi-Net can accurately retrieve high spatial resolution information from the low-energy and high-energy projections of low spatial resolution. Quantitatively, the spatial resolution of the reconstructed CBCT images from the top and bottom detector layers is increased by about 45% and 54%, respectively. Significance. Objective. Metal artifact reduction (MAR) has been a key issue in CT imaging. Recently, MAR methods based on deep learning have achieved promising results. However, when deploying deep learning-based MAR in real-world clinical scenarios, two prominent challenges arise. One limitation is the lack of paired training data in real applications, which limits the practicality of supervised methods. Another limitation is that image-domain methods suitable for more application scenarios are inadequate in performance while end-to-end approaches with better performance are only applicable to fan-beam CT due to large memory consumption. Approach. We propose a novel image-domain MAR method based on the generative adversarial network with variable constraints (MARGANVAC) to improve MAR performance. The proposed variable constraint is a kind of time-varying cost function that can relax the fidelity constraint at the beginning and gradually strengthen the fidelity constraint as the training progresses. To better deploy our image-domain supervised method into practical scenarios, we develop a transfer method to mimic the real metal artifacts by first extracting the real metal traces and then adding them to artifact-free images to generate paired training data. Main results. The effectiveness of the proposed method is validated in simulated fan-beam experiments and real cone-beam experiments. All quantitative and qualitative results demonstrate that the proposed method achieves superior performance compared with the competing methods. Significance. The MARGANVAC model proposed in this paper is an image-domain model that can be conveniently applied to various scenarios such as fan beam and cone beam CT. At the same time, its performance is on par with the cutting-edge dual-domain MAR approaches. In addition, the metal artifact transfer method proposed in this paper can easily generate paired data with real artifact features, which can be better used for model training in real scenarios. In vivo dosimetry (IVD) is an important tool in external beam radiotherapy (EBRT) to detect major errors by assessing differences between expected and delivered dose and to record the received dose by individual patients. Also, in intraoperative radiation therapy (IORT), IVD is highly relevant to register the delivered dose. This is especially relevant in low-risk breast cancer patients since a high dose of IORT is delivered in a single fraction. In contrast to EBRT, online treatment planning based on intraoperative imaging is only under development for IORT. Up to date, two commercial treatment planning systems proposed intraoperative ultrasound or in-room cone-beam CT for real-time IORT planning. This makes IVD even more important because of the possibility for real-time treatment adaptation. Here, we summarize recent developments and applications of IVD methods for IORT in clinical practice, highlighting important contributions and identifying specific challenges such as a treatment planning system for IORT. HDR brachytherapy as a delivery technique was not considered. We add IVD for ultrahigh dose rate (FLASH) radiotherapy that promises to improve the treatment efficacy, when compared to conventional radiotherapy by limiting the rate of toxicity while maintaining similar tumour control probabilities. Objective. To compare a not adapted (NA) robust planning strategy with three fully automated online adaptive proton therapy (OAPT) workflows based on the same optimization method: dose mimicking (DM). The added clinical value and limitations of the OAPT methods are investigated for head and neck cancer (HNC) patients. Approach. The three OAPT strategies aimed at compensating for inter-fractional anatomical changes by mimiking different dose distributions on corrected cone beam CT images (corrCBCTs). Order by complexity, the OAPTs were: (1) online adaptive dose restoration (OADR) where the approved clinical dose on the planning-CT (pCT) was mimicked, (2) online adaptation using DM of the deformed clinical dose from the pCT to corrCBCTs (OADEF), and (3) online adaptation applying DM to a predicted dose on corrCBCTs (OAML). Adaptation was only applied in fractions where the target coverage criteria were not met (D98% < 95% of the prescribed dose). For 10 HNC patients, the accumulated dose distributions over the 35 fractions were calculated for NA, OADR, OADEF, and OAML. Main results. Higher target coverage was observed for all OAPT strategies compared to no adaptation. OADEF and OAML outperformed both NA and OADR and were comparable in terms of target coverage to initial clinical plans. However, only OAML provided comparable NTCP values to those from the clinical dose without statistically significant differences. When the NA initial plan was evaluated on corrCBCTs, 51% of fractions needed adaptation. The adaptation rate decreased significantly to 25% when the last adapted plan with OADR was selected for delivery, to 16% with OADEF, and to 21% with OAML. The reduction was even greater when the best plan among previously generated adapted plans (instead of the last one) was selected. Significance. Objective. CBCTs in image-guided radiotherapy provide crucial anatomy information for patient setup and plan evaluation. Longitudinal CBCT image registration could quantify the inter-fractional anatomic changes, e.g. tumor shrinkage, and daily OAR variation throughout the course of treatment. The purpose of this study is to propose an unsupervised deep learning-based CBCT-CBCT deformable image registration which enables quantitative anatomic variation analysis. Approach. The proposed deformable registration workflow consists of training and inference stages that share the same feed-forward path through a spatial transformation-based network (STN). The STN consists of a global generative adversarial network (GlobalGAN) and a local GAN (LocalGAN) to predict the coarse- and fine-scale motions, respectively. The network was trained by minimizing the image similarity loss and the deformable vector field (DVF) regularization loss without the supervision of ground truth DVFs. During the inference stage, patches of local DVF were predicted by the trained LocalGAN and fused to form a whole-image DVF. The local whole-image DVF was subsequently combined with the GlobalGAN generated DVF to obtain the final DVF. The proposed method was evaluated using 100 fractional CBCTs from 20 abdominal cancer patients in the experiments and 105 fractional CBCTs from a cohort of 21 different abdominal cancer patients in a holdout test. Main Results. Qualitatively, the registration results show good alignment between the deformed CBCT images and the target CBCT image. Quantitatively, the average target registration error calculated on the fiducial markers and manually identified landmarks was 1.91 ± 1.18 mm. The average mean absolute error, normalized cross correlation between the deformed CBCT and target CBCT were 33.42 ± 7.48 HU, 0.94 ± 0.04, respectively. Significance. In summary, an unsupervised deep learning-based CBCT-CBCT registration method is proposed and its feasibility and performance in fractionated image-guided radiotherapy is investigated. Objective. Existing dual-layer flat panel detectors (DL-FPDs) use a thin scintillator layer to preferentially detect low-energy x-rays, followed by a permanent Cu filter to absorb residual low-energy x-rays, and finally, a thicker scintillator layer to preferentially detect high-energy x-rays. The image outputs of the two scintillator layers can be jointly processed for dual-energy (DE) planar and cone-beam CT imaging. In clinical practice, a given FPD is often used for not only DE imaging but also routine single-energy (SE) imaging. With the permanent Cu layer, the total x-ray absorption is unsatisfactory for SE imaging since more than 30% of x-rays can be lost in the Cu layer. The purpose of this work was to demonstrate the benefits of using a removable filter material in DL-FPDs for SE and DE imaging applications. Approach. The proposed detector contains a removable filter between the two scintillator layers. The filter can be either a chamber filled with a liquid high- Z eff material or a removable solid filter. When DE imaging is not clinically indicated, the DL-FPD can switch to a high-efficiency SE imaging mode by retracting the filter from the inter-scintillator space. For commonly available filter materials (iodine, gadolinium, and Cu), their optimal area densities were theoretically calculated for both water-bone decomposition and water-iodine decomposition DE imaging tasks. Preliminary experimental studies were also performed to compare the SE performance of the proposed DL-FPD with the existing DL-FPD with the permanent Cu filter and study the stability of the liquid filter on a rotating gantry. Main results. The optimal filter material was found to be an iodine solution (approximately 375 mg cm−2). With this liquid filter in place, the proposed DL-FPD has equivalent or better DE imaging performance compared with the existing DL-FPD with the Cu filter. When the filter is removed from the inter-scintillator space for SE imaging, the total x-ray absorption efficiency of the proposed DL-FPD ranges from 73% (100 kVp) to 54% (140 kVp), compared with 51% (100 kVp) to 41% (140 kVp) for the existing DL-FPD with a permanent 1 mm Cu filter. Significance. The removable filter provides a boost to the total x-ray absorption efficiency of DL-FPDs for SE imaging without compromising DE imaging. This can facilitate the adoption of DL-FPDs in clinical x-ray imaging systems that usually perform more SE imaging procedures than DE imaging series. [ABSTRACT FROM AUTHOR]

Purpose. Target localization in pulmonary interventions (e.g. transbronchial biopsy of a lung nodule) is challenged by deformable motion and may benefit from fluoroscopic overlay of the target to provide accurate guidance. We present and evaluate a 3D–2D image registration method for fluoroscopic overlay in the presence of tissue deformation using a multi-resolution/multi-scale (MRMS) framework with an objective function that drives registration primarily by soft-tissue image gradients. Methods. The MRMS method registers 3D cone-beam CT to 2D fluoroscopy without gating of respiratory phase by coarse-to-fine resampling and global-to-local rescaling about target regions-of-interest. A variation of the gradient orientation ( GO) similarity metric (denoted G O ′ ) was developed to downweight bone gradients and drive registration via soft-tissue gradients. Performance was evaluated in terms of projection distance error at isocenter (PDEiso). Phantom studies determined nominal algorithm parameters and capture range. Preclinical studies used a freshly deceased, ventilated porcine specimen to evaluate performance in the presence of real tissue deformation and a broad range of 3D–2D image mismatch. Results. Nominal algorithm parameters were identified that provided robust performance over a broad range of motion (0–20 mm), including an adaptive parameter selection technique to accommodate unknown mismatch in respiratory phase. The G O ′ metric yielded median PDEiso = 1.2 mm, compared to 6.2 mm for conventional GO. Preclinical studies with real lung deformation demonstrated median PDEiso = 1.3 mm with MRMS + G O ′ registration, compared to 2.2 mm with a conventional transform. Runtime was 26 s and can be reduced to 2.5 s given a prior registration within ∼5 mm as initialization. Conclusions. MRMS registration via soft-tissue gradients achieved accurate fluoroscopic overlay in the presence of deformable lung motion. By driving registration via soft-tissue image gradients, the method avoided false local minima presented by bones and was robust to a wide range of motion magnitude. [ABSTRACT FROM AUTHOR]

Objective. Periodic respiratory motion and inter-fraction variations are sources of geometric uncertainty in stereotactic body radiation therapy (SBRT) of pulmonary lesions. This study extensively evaluates and validates the separate and combined dosimetric effect of both factors using 4D-CT and daily 4D-cone beam CT (CBCT) dose accumulation scenarios. Approach. A first cohort of twenty early stage or metastatic disease lung cancer patients were retrospectively selected to evaluate each scenario. The planned-dose (3DRef) was optimized on a 3D mid-position CT. To estimate the dosimetric impact of respiratory motion (4DRef), inter-fractional variations (3DAcc) and the combined effect of both factors (4DAcc), three dose accumulation scenarios based on 4D-CT, daily mid-cone beam CT (CBCT) position and 4D-CBCT were implemented via CT-CT/CT-CBCT deformable image registration (DIR) techniques. Each scenario was compared to 3DRef. A separate cohort of ten lung SBRT patients was selected to validate DIR techniques. The distance discordance metric (DDM) was implemented per voxel and per patient for tumor and organs at risk (OARs), and the dosimetric impact for CT-CBCT DIR geometric errors was calculated. Main results. Median and interquartile range (IQR) of the dose difference per voxel were 0.05/2.69 Gy and −0.12/2.68 Gy for 3 D Acc − 3 D Ref and 4 D Acc − 3 D Ref. For 4 D Ref − 3 D Ref the IQR was considerably smaller −0.15/0.78 Gy. These findings were confirmed by dose volume histogram parameters calculated in tumor and OARs. For CT-CT/CT-CBCT DIR validation, DDM (95th percentile) was highest for heart (6.26 mm)/spinal cord (8.00 mm), and below 3 mm for tumor and the rest of OARs. The dosimetric impact of CT-CBCT DIR errors was below 2 Gy for tumor and OARs. Significance. The dosimetric impact of inter-fraction variations were shown to dominate those of periodic respiration in SBRT for pulmonary lesions. Objective. Existing clinical C-arm interventional systems use scintillator-based energy-integrating flat panel detectors (FPDs) to generate cone-beam CT (CBCT) images. Despite its volumetric coverage, FPD-CBCT does not provide sufficient low-contrast detectability desired for certain interventional procedures. The purpose of this work was to develop a C-arm photon counting detector (PCD) CT system with a step-and-shoot data acquisition method to further improve the tomographic imaging performance of interventional systems. Approach. As a proof-of-concept, a cadmium telluride-based 51 cm Ă— 0.6 cm PCD was mounted in front of a FPD in an Artis Zee biplane system. A total of 10 C-arm sweeps (5 forward and 5 backward) were prescribed. A motorized patient table prototype was synchronized with the C-arm system such that it translates the object by a designated distance during the sub-second rest time in between gantry sweeps. To evaluate whether this multi-sweep step-and-shoot acquisition strategy can generate high-quality and volumetric PCD-CT images without geometric distortion artifacts, experiments were performed using physical phantoms, a human cadaver head, and an in vivo swine subject. Comparison with FPD-CT was made under matched narrow beam collimation and radiation dose conditions. Main results. Compared with FPD-CT images, PCD-CT images had lower noise and improved visualization of low-contrast lesion models, as well as improved visibility of small iodinated blood vessels. Fine structures were visualized more clearly by the PCD-CT than the highest-available resolution provided by FPD-CBCT and MDCT. No perceivable geometric distortion artifacts were observed in the multi-planar PCD-CT images. Significance. Objective. Image guidance and precise irradiation are fundamental to ensure the reliability of small animal oncology studies. Accurate positioning of the animal and the in-beam monitoring of the delivered radio-therapeutic treatment necessitate several imaging modalities. In the particular context of proton therapy with a pulsed beam, information on the delivered dose can be retrieved by monitoring the thermoacoustic waves resulting from the brief and local energy deposition induced by a proton beam (ionoacoustics). The objective of this work was to fabricate a multimodal phantom (x-ray, proton, ultrasound, and ionoacoustics) allowing for sufficient imaging contrast for all the modalities. Approach. The phantom anatomical parts were extracted from mouse computed tomography scans and printed using polylactic acid (organs) and a granite/polylactic acid composite (skeleton). The anatomical pieces were encapsulated in silicone rubber to ensure long term stability. The phantom was imaged using x-ray cone-beam computed tomography, proton radiography, ultrasound imaging, and monitoring of a 20 MeV pulsed proton beam using ionoacoustics. Main results. The anatomical parts could be visualized in all the imaging modalities validating the phantom capability to be used for multimodal imaging. Ultrasound images were simulated from the x-ray cone-beam computed tomography and co-registered with ultrasound images obtained before the phantom irradiation and low-resolution ultrasound images of the mouse phantom in the irradiation position, co-registered with ionoacoustic measurements. The latter confirmed the irradiation of a tumor surrogate for which the reconstructed range was found to be in reasonable agreement with the expectation. Significance. This study reports on a realistic small animal phantom which can be used to investigate ionoacoustic range (or dose) verification together with ultrasound, x-ray, and proton imaging. This work focuses on the determination of the radiation doses for a total sample of 916 children, categorised into four age groups (<1, 1â€"5, <5â€"10, <10â€"15 years) undergoing the most frequent paediatric CT scans performed in different scan facilities in Tunisia in order to establish the national diagnostic reference levels (DRLs). Dose evaluation concerned the dosimetric indicators: volume computed tomography dose index (CTDIvol) and doseâ€"length product (DLP). The different paediatric CT protocols and practices were also evaluated. The results show a large variation in doses between different radiology departments. For head scans, the respective DRLs for children aged <1, 1â€"5, 5â€"10 and 10â€"15 years were 26, 38, 51 and 51 mGy, respectively, for CTDIvol and 384, 664, 873 and 978 mGy cm, respectively, for DLP. For the chest, the equivalent respective DRLs were 8, 10, 12 and 15 mGy for CTDIvol and 118, 330, 442 and 526 mGy cm for DLP. For the abdomen, the respective DRLs were 9, 13, 19 and 18 mGy for CTDIvol and 353, 485, 592 and 1073 mGy cm for DLP. This study shows that the optimisation of paediatric CT procedures should be a priority, especially within regional hospitals. The implementation of corrective actions will take place after the initial DRLs. These actions, including recommendations and guidelines to good practice, should be a joint effort of all stakeholders, including health authorities, the radiation protection regulator, professional societies and universities. Objective. Cone-Beam CT (CBCT) often results in severe image artifacts and inaccurate HU values, meaning poor quality CBCT images cannot be directly applied to dose calculation in radiotherapy. To overcome this, we propose a cycle-residual connection with a dilated convolution-consistent generative adversarial network (Cycle-RCDC-GAN). Approach. The cycle-consistent generative adversarial network (Cycle-GAN) was modified using a dilated convolution with different expansion rates to extract richer semantic features from input images. Thirty pelvic patients were used to investigate the effect of synthetic CT (sCT) from CBCT, and 55 head and neck patients were used to explore the generalizability of the model. Three generalizability experiments were performed and compared: the pelvis trained model was applied to the head and neck; the head and neck trained model was applied to the pelvis, and the two datasets were trained together. Main results. The mean absolute error (MAE), the root mean square error (RMSE), peak signal to noise ratio (PSNR), the structural similarity index (SSIM), and spatial nonuniformity (SNU) assessed the quality of the sCT generated from CBCT. Compared with CBCT images, the MAE improved from 28.81 to 18.48, RMSE from 85.66 to 69.50, SNU from 0.34 to 0.30, and PSNR from 31.61 to 33.07, while SSIM improved from 0.981 to 0.989. The sCT objective indicators of Cycle-RCDC-GAN were better than Cycle-GAN’s. The objective metrics for generalizability were also better than Cycle-GAN’s. Significance. Objective. We develop a model-based optimization algorithm for ‘one-step’ dual-energy (DE) CT decomposition of three materials directly from projection measurements. Approach. Since the three-material problem is inherently undetermined, we incorporate the volume conservation principle (VCP) as a pair of equality and nonnegativity constraints into the objective function of the recently reported model-based material decomposition (MBMD). An optimization algorithm (constrained MBMD, CMBMD) is derived that utilizes voxel-wise separability to partition the volume into a VCP-constrained region solved using interior-point iterations, and an unconstrained region (air surrounding the object, where VCP is violated) solved with conventional two-material MBMD. Constrained MBMD (CMBMD) is validated in simulations and experiments in application to bone composition measurements in the presence of metal hardware using DE cone-beam CT (CBCT). A kV-switching protocol with non-coinciding low- and high-energy (LE and HE) projections was assumed. CMBMD with decomposed base materials of cortical bone, fat, and metal (titanium, Ti) is compared to MBMD with (i) fat-bone and (ii) fat-Ti bases. Main results. Three-material CMBMD exhibits a substantial reduction in metal artifacts relative to the two-material MBMD implementations. The accuracies of cortical bone volume fraction estimates are markedly improved using CMBMD, with ∼5â€"10Ă— lower normalized root mean squared error in simulations with anthropomorphic knee phantoms (depending on the complexity of the metal component) and ∼2â€"2.5Ă— lower in an experimental test-bench study. Significance. In conclusion, we demonstrated one-step three-material decomposition of DE CT using volume conservation as an optimization constraint. The proposed method might be applicable to DE applications such as bone marrow edema imaging (fat-bone-water decomposition) or multi-contrast imaging, especially on CT/CBCT systems that do not provide coinciding LE and HE ray paths required for conventional projection-domain DE decomposition. Objective. Modern preclinical small animal radiation platforms utilize cone beam computerized tomography (CBCT) for image guidance and experiment planning purposes. The resolution of CBCT images is of particular importance for visualizing fine animal anatomical structures. One major cause of spatial resolution reduction is the finite size of the x-ray focal spot. In this work, we proposed a simple method to measure x-ray focal spot intensity map and a CBCT image domain deblurring model to mitigate the effect of focal spot-induced image blurring. Approach. We measured a projection image of a tungsten ball bearing using the flat panel detector of the CBCT platform. We built a forward blurring model of the projection image and derived the spot intensity map by deconvolving the measured projection image. Based on the measured spot intensity map, we derived a CBCT image domain blurring model for images reconstructed by the filtered backprojection algorithm. Based on this model, we computed image domain blurring kernel and improved the CBCT image resolution by deconvolving the CBCT image. Main results. We successfully measured the x-ray focal spot intensity map. The spot size characterized by full width at half maximum was ∼0.75 Ă— 0.55 mm2 at 40 kVp. We computed image domain convolution kernels caused by the x-ray focal spot. A simulation study on noiseless projections was performed to evaluate the spatial resolution improvement exclusively by the focal spot kernel, and the modulation transfer function (MTF) at 50% was increased from 1.40 to 1.65 mmâˆ'1 for in-plane images and 1.05â€"1.32 mmâˆ'1 for cross-plane images. Experimental studies on a CT insert phantom and a plastinated mouse phantom demonstrated improved spatial resolution after image domain deconvolution, as indicated by visually improved resolution of fine structures. MTF at 50% was improved from 1.00 to 1.12 mmâˆ'1 for in-plane direction and from 0.72 to 0.84 mmâˆ'1 for cross-plane direction. Significance. Objective. 4D-CBCT provides phase-resolved images valuable for radiomics analysis for outcome prediction throughout treatment courses. However, 4D-CBCT suffers from streak artifacts caused by under-sampling, which severely degrades the accuracy of radiomic features. Previously we developed group-patient-trained deep learning methods to enhance the 4D-CBCT quality for radiomics analysis, which was not optimized for individual patients. In this study, a patient-specific model was developed to further improve the accuracy of 4D-CBCT based radiomics analysis for individual patients. Approach. This patient-specific model was trained with intra-patient data. Specifically, patient planning 4D-CT was augmented through image translation, rotation, and deformation to generate 305 CT volumes from 10 volumes to simulate possible patient positions during the onboard image acquisition. 72 projections were simulated from 4D-CT for each phase and were used to reconstruct 4D-CBCT using FDK back-projection algorithm. The patient-specific model was trained using these 305 paired sets of patient-specific 4D-CT and 4D-CBCT data to enhance the 4D-CBCT image to match with 4D-CT images as ground truth. For model testing, 4D-CBCT were simulated from a separate set of 4D-CT scan images acquired from the same patient and were then enhanced by this patient-specific model. Radiomics features were then extracted from the testing 4D-CT, 4D-CBCT, and enhanced 4D-CBCT image sets for comparison. The patient-specific model was tested using 4 lung-SBRT patients’ data and compared with the performance of the group-based model. The impact of model dimensionality, region of interest (ROI) selection, and loss function on the model accuracy was also investigated. Main results. Compared with a group-based model, the patient-specific training model further improved the accuracy of radiomic features, especially for features with large errors in the group-based model. For example, the 3D whole-body and ROI loss-based patient-specific model reduces the errors of the first-order median feature by 83.67%, the wavelet LLL feature maximum by 91.98%, and the wavelet HLL skewness feature by 15.0% on average for the four patients tested. In addition, the patient-specific models with different dimensionality (2D versus 3D) or loss functions (L1 versus L1 + VGG + GAN) achieved comparable results for improving the radiomics accuracy. Using whole-body or whole-body+ROI L1 loss for the model achieved better results than using the ROI L1 loss alone as the loss function. Significance. This study demonstrated that the patient-specific model is more effective than the group-based model on improving the accuracy of the 4D-CBCT radiomic features analysis, which could potentially improve the precision for outcome prediction in radiotherapy. Objective. Cone-beam CT (CBCT) in modern pre-clinical small-animal radiation research platforms provides volumetric images for image guidance and experiment planning purposes. In this work, we implemented multi-energy element-resolved (MEER) CBCT using three scans with different kVps on a SmART platform (Precision x-ray Inc.) to determine images of relative electron density (rED) and elemental composition (EC) that are needed for Monte Carlo-based radiation dose calculation. Approach. We performed comprehensive calibration tasks to achieve sufficient accuracy for this quantitative imaging purpose. For geometry calibration, we scanned a ball bearing phantom and used an analytical method together with an optimization approach to derive gantry angle specific geometry parameters. Intensity calibration and correction included the corrections for detector lag, glare, and beam hardening. The corrected CBCT projection images acquired at 30, 40, and 60 kVp in multiple scans were used to reconstruct CBCT images using the Feldkampâ€"Davisâ€"Kress reconstruction algorithm. After that, an optimization problem was solved to determine images of rED and EC. We demonstrated the effectiveness of our CBCT calibration steps by showing improvements in image quality and successful material decomposition in cases with a small animal CT calibration phantom and a plastinated mouse phantom. Main results. It was found that artifacts induced by geometry inaccuracy, detector lag, glare, and beam hardening were visually reduced. CT number mean errors were reduced from 19% to 5%. In the CT calibration phantom case, median errors in H, O, and Ca fractions for all the inserts were below 1%, 2%, and 4% respectively, and median error in rED was less than 5%. Compared to the standard approach deriving material type and rED via CT number conversion, our approach improved Monte Carlo simulation-based dose calculation accuracy in bone regions. Mean dose error was reduced from 47.5% to 10.9%. Significance. The MEER-CBCT implemented on an existing CBCT system of a small animal irradiation platform achieved accurate material decomposition and significantly improved Monte Carlo dose calculation accuracy. Objective. In this study we developed an automatic method to predict tumour volume and shape in weeks 3 and 4 of radiotherapy (RT), using cone-beam computed tomography (CBCT) scans acquired up to week 2, allowing identification of large tumour changes. Approach. 240 non-small cell lung cancer (NSCLC) patients, treated with 55 Gy in 20 fractions, were collected. CBCTs were rigidly registered to the planning CT. Intensity values were extracted in each voxel of the planning target volume across all CBCT images from days 1, 2, 3, 7 and 14. For each patient and in each voxel, four regression models were fitted to voxel intensity; applying linear, Gaussian, quadratic and cubic methods. These models predicted the intensity value for each voxel in weeks 3 and 4, and the tumour volume found by thresholding. Each model was evaluated by computing the root mean square error in pixel value and structural similarity index metric (SSIM) for all patients. Finally, the sensitivity and specificity to predict a 30% change in volume were calculated for each model. Main results. The linear, Gaussian, quadratic and cubic models achieved a comparable similarity score, the average SSIM for all patients was 0.94, 0.94, 0.90, 0.83 in week 3, respectively. At week 3, a sensitivity of 84%, 53%, 90% and 88%, and specificity of 99%, 100%, 91% and 42% were observed for the linear, Gaussian, quadratic and cubic models respectively. Overall, the linear model performed best at predicting those patients that will benefit from RT adaptation. The linear model identified 21% and 23% of patients in our cohort with more than 30% tumour volume reduction to benefit from treatment adaptation in weeks 3 and 4 respectively. Significance. Industrial x-ray computed tomography (CT) represents an established measurement technique in the field of nondestructive testing and dimensional metrology. However, the measurement accuracy is sometimes limited by measurement artifacts that cannot be avoided using a standard circular scan trajectory. This problem can be addressed with the aid of flexible 3D trajectories, but up to date, the application of these is mainly restricted to special CT devices using robot arms. In this paper, we present results using a hexapod as an additional positioning system in a commercial industrial CT scanner. In addition to the 360∘ rotation, task-specific tilting of the part during the scan is possible in this way. We used and adapted geometry calibration procedures based on a multi-sphere reference object to enable reconstruction with high accuracy. Using a demonstrator test fixture with high absorbing elements, we show that severe metal and truncation artifacts can be avoided for a region-of-interest scan. Furthermore, cone-beam artifacts, which are inherent to circular scan trajectories, can be reduced significantly. Using measurement objects that can be measured well with a circular trajectory, we found that applying a 3D trajectory leads to dimensional measurement deviations in the same range or even lower than those of a circular trajectory. This suggests that the pose repeatability of the hexapod is sufficient to perform complex scan paths without general loss of accuracy. The obtained results could be relevant for end users of conventional CT scanners, as upgrading existing devices is in principle possible. Proton therapy treatment for lungs remains challenging as images enabling the detection of inter- and intra-fractional motion, which could be used for proton dose adaptation, are not readily available. 4D computed tomography (4DCT) provides high image quality but is rarely available in-room, while in-room 4D cone beam computed tomography (4DCBCT) suffers from image quality limitations stemming mostly from scatter detection. This study investigated the feasibility of using virtual 4D computed tomography (4DvCT) as a prior for a phase-per-phase scatter correction algorithm yielding a 4D scatter corrected cone beam computed tomography image (4DCBCTcor), which can be used for proton dose calculation. 4DCT and 4DCBCT scans of a porcine lung phantom, which generated reproducible ventilation, were acquired with matching breathing patterns. Diffeomorphic Morphons, a deformable image registration algorithm, was used to register the mid-position 4DCT to the mid-position 4DCBCT and yield a 4DvCT. The 4DCBCT was reconstructed using motion-aware reconstruction based on spatial and temporal regularization (MA-ROOSTER). Successively for each phase, digitally reconstructed radiographs of the 4DvCT, simulated without scatter, were exploited to correct scatter in the corresponding CBCT projections. The 4DCBCTcor was then reconstructed with MA-ROOSTER using the corrected CBCT projections and the same settings and deformation vector fields as those already used for reconstructing the 4DCBCT. The 4DCBCTcor and the 4DvCT were evaluated phase-by-phase, performing proton dose calculations and comparison to those of a ground truth 4DCT by means of dose-volume-histograms (DVH) and gamma pass-rates (PR). For accumulated doses, DVH parameters deviated by at most 1.7% in the 4DvCT and 2.0% in the 4DCBCTcor case. The gamma PR for a (2%, 2 mm) criterion with 10% threshold were at least 93.2% (4DvCT) and 94.2% (4DCBCTcor), respectively. The 4DCBCTcor technique enabled accurate proton dose calculation, which indicates the potential for applicability to clinical 4DCBCT scans. [ABSTRACT FROM AUTHOR]

The aim of the present study was to describe patient dose indicator levels during intra-arterial catheter (IAC) implantation for liver chemotherapy, and to determine factors affecting the dose indicators. Between January 2017 and January 2019, 61 patients with hepatic metastases from colorectal cancer were retrospectively included. Interventions were carried out in a standardised manner by three experienced radiologists on the same angiographic table without changes in protocol parameters. For each patient, clinical, radiological and dosimetry data were collected, including the air kerma area product (KAP), part of KAP due to the fluoroscopy and fluoroscopy time (FT), total kerma at the reference interventional point and peak skin dose (PSD). Local dose reference levels (RLs) were determined as the third quartile of the patient dose distributions. Univariate and multivariate analysis of factors affecting dose indicators was performed. The mean KAP was 111 Gy cm2, the mean reference point air kerma (Ka,r) was 648 mGy, the mean PSD was 613 mGy, and the mean FT was 3190 s (62% of the KAP). The mean cone beam computed tomography dose was 37.3 ± 11.8 Gy cm2, which accounted for 37% of the KAP. The RL could be proposed taking into account the third quartiles (KAP = 164.6 Gy cm2, Ka,r = 904.5 mGy, FT = 4011 s and standard deviation = 772.7 mGy). The factors affecting dose indicators were related to the patients (sex, cardiovascular risk factors, weight, body mass index), to the vascular anatomies (coeliac trunk angulation) and to the procedures (number of embolised arteries). This study allowed a better understanding of dose indicators and factors affecting these indicators during the implantation of IACs for hepatic chemotherapy, which is a long and difficult procedure. Objective. Recently, dental cone-beam computed tomography (CBCT) methods have been improved to significantly reduce radiation dose while maintaining image resolution with minimal equipment cost. In low-dose CBCT environments, metallic inserts such as implants, crowns, and dental fillings cause severe artifacts, which result in a significant loss of morphological structures of teeth in reconstructed images. Such metal artifacts prevent accurate 3D bone-teeth-jaw modeling for diagnosis and treatment planning. However, the performance of existing metal artifact reduction (MAR) methods in handling the loss of the morphological structures of teeth in reconstructed CT images remains relatively limited. In this study, we developed an innovative MAR method to achieve optimal restoration of anatomical details. Approach. The proposed MAR approach is based on a two-stage deep learning-based method. In the first stage, we employ a deep learning network that utilizes intra-oral scan data as side-inputs and performs multi-task learning of auxiliary tooth segmentation. The network is designed to improve the learning ability of capturing teeth-related features effectively while mitigating metal artifacts. In the second stage, a 3D bone-teeth-jaw model is constructed with weighted thresholding, where the weighting region is determined depending on the geometry of the intra-oral scan data. Main results. The results of numerical simulations and clinical experiments are presented to demonstrate the feasibility of the proposed approach. Significance.
