3. The Role of Movement-Specific Reinvestment in Visuomotor Control of Walking by Older Adults.

Abstract

OBJECTIVES:The aim of this study was to examine the association between conscious monitoring and control of movements (i.e., movement-specific reinvestment) and visuomotor control during walking by older adults. METHOD:The Movement-Specific Reinvestment Scale (MSRS) was administered to 92 community-dwelling older adults, aged 65-81 years, who were required to walk along a 4.8-m walkway and step on the middle of a target as accurately as possible. Participants' movement kinematics and gaze behavior were measured during approach to the target and when stepping on it. RESULTS:High scores on the MSRS were associated with prolonged stance and double support times during approach to the stepping target, and less accurate foot placement when stepping on the target. No associations between MSRS and gaze behavior were observed. DISCUSSION:Older adults with a high propensity for movement-specific reinvestment seem to need more time to "plan" future stepping movements, yet show worse stepping accuracy than older adults with a low propensity for movement-specific reinvestment. Future research should examine whether older adults with a higher propensity for reinvestment are more likely to display movement errors that lead to falling.

Abstract

Aim To examine whether differences emerged when the Chinese version of the Movement-Specific Reinvestment Scale ( MSRS- C) was administered to community-dwelling older adults with instructions to respond in the context of 'general' movements, walking, using chopsticks or dressing. Furthermore, the difference between the six-point Likert scale and four-point Likert scale response formats of the MSRS- C was investigated. Methods The study was implemented in the community of Hong Kong with 52 older adults (mean age 77.4 years). Telephone interviews were carried out on two occasions for each participant. Participants provided a verbal response to each of 10 questions from the MSRS- C with different response formats (i.e., six-point or four-point Likert Scales) and different instructions in the response context (i.e. general, walking, using chopsticks, dressing). The sequence of response format and context was randomized for each participant. Results Older fallers scored significantly higher on the MSRS- C (general) with six-point or four-point response formats than non-fallers. The MSRS- C (general) and MSRS- C (walking) were not statistically different, and showed good discriminative power for previous older fall status (older fallers or older non-fallers). However, MSRS- C (chopsticks) and MSRS- C (dressing) failed to differentiate older fallers from older non-fallers. Conclusion Both the MSRS- C (general) and MSRS- C (walking) with a six-point or a four-point response format showed good discrimination of older fallers from non-fallers. 6. Efficacy of diaphragmatic breathing exercise on respiratory, cognitive, and motor function outcomes in patients with stroke: a systematic review and meta-analysis.

Abstract

Background: Stroke disrupts the functions carried out by the brain such as the control of movement, sensation, and cognition. Disruption of movement control results in hemiparesis that affects the function of the diaphragm. Impaired function of the diaphragm can in turn affect many outcomes such as respiratory, cognitive, and motor function. The aim of this study is to carry out a systematic review and meta-analysis to determine the efficacy of diaphragmatic breathing exercise on respiratory, cognitive, and motor outcomes after stroke., Method: The study was registered in PROSPERO (CRD42023422293). PubMED, Embase, Web of Science (WoS), PEDro, Scopus, and CENTRAL databases were searched until September 2023. Only randomized controlled trials comparing diaphragmatic breathing exercise with a control were included. Information on the study authors, time since stroke, mean age, height, weight, sex, and the protocols of the experimental and control interventions including intensity, mean scores on the outcomes such as respiratory, cognitive, and motor functions were extracted. Cochrane risks of bias assessment tool and PEDro scale were used to assess the risks of bias and methodological quality of the studies. Narrative synthesis and meta-analysis were used to summarize the results, which were then presented in tables, risk-of-bias graph, and forest plots. The meta-analysis was carried out on respiratory function [forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1), FEV1/FVC, peak expiratory flow (PEF)] and motor function (trunk impairment, and internal and external oblique muscles activity)., Results: Six studies consisting of 151 participants were included. The results of the meta-analyses showed that diaphragmatic breathing exercise is only superior to the control at improving respiratory function, FVC (MD = 0.90, 95% CI = 0.76 to 1.04, P < 0.00001), FEV1 (MD = 0.32, 95% CI = 0.11 to 0.52, P = 0.002), and PEF (MD = 1.48, 95% CI = 1.15 to 1.81, P < 0.00001)., Conclusion: There is limited evidence suggesting that diaphragmatic breathing exercise may help enhance respiratory function, which may help enhance recovery of function post stroke., Systematic Review Registration: PROSPERO, identifier CRD42023422293., Competing Interests: The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest., (Copyright © 2024 Abdullahi, Wong and Ng.)
