Climate change potentially threatens the sustainable production of highly valued cold-water fish species in flow-through systems, such as salmonids. By analysing the relationship of water temperature to hydrological characteristics, air temperature, solar exposure, and precipitation, this study predicted temperature dynamics of five temperate cold-water aquaculture facilities under four projected climate change scenarios. Air temperature was found to be directly associated with facility site water temperature, and based on rational assumptions, two of the five facilities were predicted to face critical warming by mid-century. Extreme precipitation events induced acute short-term increases in water temperature of up to 5 °C. Significantly lower warming, roughly equal to the projected climate change-induced increase, was seen with artificial shading lowering temperature by 1 °C. Complementary niche modelling revealed that 37-77 % of current cold-water facilities will likely incur suboptimal climate conditions by the end of the century. This paper explores whether directed deceased organ donation should be permitted, and if so under which conditions. While organ donation and allocation systems must be fair and transparent, might it be "one thought too many" to prevent directed donation within families? We proceed by providing a description of the medical and legal context, followed by identification of the main ethical issues involved in directed donation, and then explore these through a series of hypothetical cases similar to those encountered in practice. Ultimately, we set certain conditions under which directed deceased donation may be ethically acceptable. Objective: The objective of this review was to collate and summarize the current literature on what is known about organ donation following medical assistance in dying (MAiD). Additionally, for this first part of a 2-part scoping review, the focus is on legal and ethical considerations regarding organ donation following MAiD., Introduction: Organ donation following MAiD is a relatively new procedure that has sparked much debate and discussion. A comprehensive investigation into the legal and ethical aspects related to organ donation following MAiD is needed to inform the development of safe and ethical practices., Inclusion Criteria: In this review, we included documents that investigated legal and/or ethical issues related to individuals who underwent organ donation following MAiD in any setting (eg, hospital or home) worldwide. We considered quantitative and qualitative studies, text and opinion papers, gray literature, and unpublished material provided by stakeholders., Methods: This scoping review followed JBI methodology. Published studies were retrieved from databases, including MEDLINE (Ovid), Embase (Ovid), CINAHL (EBSCOhost), PsycINFO (Ovid), Web of Science Core Collection, and Academic Search Complete (EBSCOhost). Gray and unpublished literature included reports from organ donation organizations in Canada, The Netherlands, and Belgium. Two independent reviewers screened all reports (both by title and abstract and by full text) against the inclusion criteria, extracted data, and completed a content analysis. Disagreements between the 2 reviewers were resolved through discussions among the reviewers and the lead reviewer., Results: We included 121 documents for parts I and II of our scoping review, 89 of which are included in part I. The majority of the 89 documents were discussion papers published in English and in Canada from 2019 to 2021. In the content analysis, we identified 4 major categories regarding ethical and legal aspects of organ donation following MAiD: i) legal definitions, legislation, and guidelines; ii) ethics, dilemmas, and consensus; iii) consent and objection; and iv) public perceptions. We identified the main legislation regulating the practices of organ donation following MAiD in countries where both procedures are permitted, the many ethical debates surrounding this topic (eg, eligibility criteria for organ donation and MAiD, disclosure of donors' and recipients' information, directed organ donation, death determination in organ donation following MAiD, ethical safeguards for organ donation following MAiD), as well as the public perceptions of this process., Conclusions: Organ donation following MAiD has raised many legal and ethical concerns regarding establishing safeguards to protect patients and families. The majority of the 107 documents were discussion papers, published in English, and in Canada from 2019 to 2021. Objective: To investigate whether observable differences exist between patterns of withdrawal of life-sustaining measures (WLSM) for patients eligible for donation after circulatory death (DCD) in whom donation was attempted compared with those patients in whom no donation attempts were made., Setting: Adult intensive care units from 20 centres in Canada, the Czech Republic and the Netherlands., Design: Secondary analysis of quantitative data collected as part of a large, prospective, cohort study (the Death Prediction and Physiology after Removal of Therapy study)., Participants: Patients ≥18 years of age who died after a controlled WLSM in an intensive care unit. Patients were classified as not DCD eligible, DCD eligible with DCD attempted or DCD eligible but DCD was not attempted., Primary and Secondary Outcome Measures: The process of WLSM (timing and type and, if applicable, dosages of measures withdrawn, dosages of analgesics/sedatives) was compared between groups., Results: Of the 635 patients analysed, 85% had either cardiovascular support stopped or were extubated immediately on WLSM. Of the DCD eligible patients, more were immediately extubated at the initiation of WLSM when DCD was attempted compared with when DCD was not attempted (95% vs 61%, p<0.0001). Initiation of WLSM with the immediate cessation of cardiovascular measures or early extubation was associated with earlier time to death, even after adjusting for confounders (OR 2.94, 95% CI 1.39 to 6.23, at 30 min). Other than in a few patients who received propofol, analgesic and sedative dosing after WLSM between DCD attempted and DCD eligible but not attempted patients was not significantly different. All patients died., Conclusions: Patients in whom DCD is attempted may receive a different process of WLSM. Introduction: Migration puts refugees in a completely new social context when simultaneously some have to deal with previously experienced traumatic events and post-migration stressors. Social capital and sense of coherence could be key resources to improve mental health of refugees. This study aims to examine the interplay between social capital (structural and cognitive), sense of coherence and mental health of refugees in the Netherlands., Objective: The present study was conducted to i) examine if social capital (structural and cognitive) and mental health are related in a population of Dutch refugees, and ii) test if sense of coherence has a moderating and/or a mediating effect on this relation., Method: Data were collected through questionnaires (n = 154) in a cross-sectional survey at different locations throughout the Netherlands. The data were analysed with multiple regression analyses and nonparametric bootstrapping using SPSS., Results: Social capital (structural and cognitive) was positively related to mental health. In addition a positive relation between sense of coherence and mental health of refugees was found. The relationship between cognitive social capital and mental health was completely mediated by sense of coherence. No moderation effect of sense of coherence on the relation between social capital and mental health was found., Conclusions: The current study contributed to understanding the social mechanism that determines refugee mental health: participating in social groups (structural social capital) and having supportive and trusting relationships (cognitive social capital), whilst experiencing life as comprehensible, manageable, and meaningful (sense of coherence) are positively related to better mental health of refugees. The Regulation on Registration, Evaluation, Authorisation and Restriction of Chemicals requires that the risks from the exposure to substances be controlled throughout the life cycle. This includes that conditions of safe use are established via risk assessments, documented and communicated to the downstream users of chemicals. This also applies to the environmental risks originating from downstream uses of chemicals, for instance, those from the industrial uses of adhesives and sealants. Upon application, these products form solid matrices with low emissions to the environment during the application. Hence, it is expected that environmental exposure is low, provided that good industrial practice is followed. To explore this, an environmental risk assessment for industrial uses of adhesives and sealants is performed for the environmentally most hazardous ingredients. These include several solvents, organotin catalysts, fillers, reactive resins, a pigment, and a preservative. Specific environmental release categories (SPERCs) developed by the Association of European Adhesives and Sealants Industries (FEICA) are used to derive emission estimates. In combination with multimedia fate modeling, the environmental risk in water, sediment, soil, and a sewage treatment plant is investigated. The assessment results indicate no environmental risk for any of the ingredients. The discussion evaluates the conservative nature of the assumed values of the use rates, the release factors, the fate modeling, and assessment factors. It concludes that their combination results in a sufficient degree of conservatism. In view of the conservative nature of the assessment and given that the worst-case ingredients of adhesives and sealants are sufficiently controlled under the generically defined use conditions, it is concluded that the SPERCs used represent safe conditions for use, irrespective of the ingredient substances of adhesives and sealants. The essential SPERC information elements are identified for the purpose of communicating the conditions of safe use. With over 80 million people forcibly displaced worldwide, providing safe, healthy, and supportive places for refugees has become an imperative for national governments, aid organizations, and host communities. While much has been written about the needs of these displaced people, organizations and practitioners tend to focus on essential material needs, medical care, and food and water provisioning. Yet a growing body of evidence points to the potential role of social capital - the bonding, bridging, and linking social ties that connect us to one another - as a critical resource for these refugees. We have little data about social capital interventions at individual and community levels to assist with mental health for this vulnerable population, and even less methodical evidence about such interventions' impact. This systematic review analyzes nearly 400 articles to find patterns in the literature on how social-capital-based interventions can improve the mental health of refugees. Within the studies of interventions that met our filtering criteria, the reinforcement or creation of social capital, especially bridging and linking types, serves as a crucial resource to help this vulnerable group. Specifically, our review showed that community and multilevel social capital interventions are key to curbing mental health symptoms among refugees. Families of organ donors play an important role in the deceased organ donation process. The aim of this study was to gain insight into donor family care by creating an inventory of practice in various European countries. A questionnaire about donor family care and contact between donor families and recipients was developed. Representatives of the organ donor professionals of 15 European countries responded (94%). The donor coordinator plays a key role in care for the donor family. All countries provide information about the donation results to the families, although diminished due to privacy laws. Anonymous written contact between donor families and recipients is possible in almost all countries and direct contact in only a few. Remembrance ceremonies exist in most countries. Half of the respondents thought the aftercare could improve. This first inventory shows that differences exist between countries, depending on the organisation of the donation process, the law and the different role of the professionals. Direct contact between donor families and recipients is rarely supported by the donation organisation. Physical inactivity increases the risk of many chronic diseases. Approximately 40% to 50% of college students are physically inactive. Research suggests that when students establish physical activity routines during their college careers, they have a greater chance of maintaining those specific behaviors over time. The current study sought to increase steps in sedentary college students by using a self-management intervention consisting of self-monitoring, goal setting, social media (Instagram), weekly meetings, and social feedback. Researchers used a multiple-baseline across-participants design with an embedded reversal to assess the effects of the packaged intervention. The intervention increased the physical activity levels of 3 of the 4 participants. It is well known that families frequently overrule the wishes of dying patients who had previously expressed a wish to donate their organs. Various strategies have been suggested to reduce the frequency of these 'family overrules'. However, the possibility of families overruling a patient's registered decision not to donate has not been discussed in the medical literature, although it is legally possible in some countries. In this article, we provide an ethical analysis of family overrule of a relative's refusal to donate, using the different jurisdictions of the UK, Switzerland, Germany and the Netherlands to provide some context. Background: Increasing attention is payed to the contribution of somatosensory processing in motor control. In particular, temporal somatosensory discrimination has been found to be altered differentially in common movement disorders. To date, there have only been speculations as to how impaired temporal discrimination and clinical motor signs may relate to each other. Prior to disentangling this relationship, potential confounders of temporal discrimination, in particular age and peripheral nerve conduction, should be assessed, and a quantifiable measure of proprioceptive performance should be established., Objective: To assess the influence of age and polyneuropathy (PNP) on somatosensory temporal discrimination threshold (STDT), temporal discrimination movement threshold (TDMT), and behavioral measures of proprioception of upper and lower limbs., Methods: STDT and TDMT were assessed in 79 subjects (54 healthy, 25 with PNP; age 30-79 years). STDT was tested with surface electrodes over the thenar or dorsal foot region. TDMT was probed with needle electrodes in flexor carpi radialis (FCR) and tibialis anterior (TA) muscle. Goniometer-based devices were used to assess limb proprioception during (i) active pointing to LED markers, (ii) active movements in response to variable visual cues, and (iii) estimation of limb position following passive movements. Pointing (or estimation) error was taken as a measure of proprioceptive performance., Results: In healthy subjects, higher age was associated with higher STDT and TDMT at upper and lower extremities, while age did not correlate with proprioceptive performance. Patients with PNP showed higher STDT and TDMT values and decreased proprioceptive performance in active pointing tasks compared to matched healthy subjects. As an additional finding, there was a significant correlation between performance in active pointing tasks and temporal discrimination thresholds., Conclusion: Given their notable impact on measures of temporal discrimination, age and peripheral nerve conduction need to be accounted for if STDT and TDMT are applied in patients with movement disorders. Background: Despite data from randomized trials supporting omission of radiation therapy (RT) for women ≥ 70 years of age with T1, estrogen receptor-positive (ER+) tumors undergoing breast-conserving therapy (BCT), RT usage remains high. We reviewed our institutional experience to determine if risk factors for local recurrence or comorbidities influenced use., Methods: Women ≥ 70 years of age with T1, ER+, human epidermal growth factor receptor 2-negative (HER2-) tumors undergoing BCT in 2010-2012 were identified from a prospectively maintained database. Ten-year estimated mortality was calculated using the Suemoto index. The associations of clinicopathological features and mortality risk on receipt of RT were examined., Results: Overall, 323 patients with 327 cancers were identified. Median age was 75 years, median tumor size was 1 cm, and all were clinically node negative; 53.7% of patients received RT. RT usage decreased with age (73.6%, age 70-74 years; 49.5%, age 75-79 years; 33.3%, age 80-84 years; 10.7%, ≥ 85 years; p < 0.001). Within age groups, estimated mortality did not impact RT usage. On multivariable analysis, only younger age and larger tumor size were associated with RT use. Recurrence-free survival was 98% versus 93% with and without RT, respectively (p = 0.011). Those who received adjuvant radiation also had improved overall survival (92% vs. 89%), although this effect did not reach statistical significance (p = 0.051)., Conclusion: Neither the factors associated with risk of local recurrence nor the estimated risk of death in 10 years were associated with use of adjuvant radiation in a large cohort of women ≥ 70 years of age with small ER+ breast cancers treated with breast-conserving surgery.

Despite two decades of research on social capital and health, intervention studies remain scarce. We performed a systematic review on social capital interventions in public health and searched the Pubmed and PsychInfo databases. The majority of interventions we identified focused on individual level change (e.g. encouraging social participation), as opposed to community level change. We included 17 manuscripts in the systematic review. We categorized studies according to the role of social capital in the interventions (as the direct target of intervention, as a channel/mediator, or as a segmenting variable) as well as the levels of interventions (individual, community levels vs. multilevel ). We conclude that the majority of interventions sought to directly strengthen social capital to influence health outcomes. Our review reveals (i) a lack of studies that incorporate a multilevel perspective and (ii) an absence of consideration of specific groups that might selectively benefit from social capital interventions (segmentation). Future research is needed on both questions to provide a more nuanced picture of how social capital can be manipulated to affect health outcomes., (Copyright © 2018 Elsevier Ltd. In this article, we analyse the potential benefits and disadvantages of permitting healthcare professionals to invoke conscientious objection to deceased organ donation. There is some evidence that permitting doctors and nurses to register objections can ultimately lead to attitudinal change and acceptance of organ donation. However, while there may be grounds for conscientious objection in other cases such as abortion and euthanasia, the life-saving nature of donation and transplantation renders objection in this context more difficult to justify. In general, dialogue between healthcare professionals is a more appropriate solution, and any objections must be justified with a strong rationale in hospitals where such policies are put in place.

Donation after circulatory death (DCD) is increasing in Europe, yet there is widespread variability in practice. Insight into actual practice is difficult to acquire simply by analyzing protocols and laws from each individual country. For this reason, the 3 DCD vignettes in this article have been constructed to outline routine and standard DCD practice in the United Kingdom, the Netherlands, and Spain. These imagined vignettes reflect a "typical" case, based on the authors' extensive experience with DCD but are not real patient cases. They are a resource aimed at stimulating discussion regarding European organ donation practice and provide a knowledge bank for those wanting to establish a DCD program in their country. It is our hope that by providing these vignettes, the wider organ donation and transplant community, as well as philosophers and the public, will have a better understanding of what DCD really is and what it really isn't.

Specific environmental release categories (SPERCs) are an instrument for lower-tier environmental emissions assessments. They support chemical safety assessments under the European Union (EU) regulation Registration, Evaluation, Authorisation, and Restriction of Chemicals. SPERCs have been developed by industry and subjected to regulatory review. Within the framework of the Chemical Safety Report/Exposure Scenario Roadmap, the EU Chemicals Agency (ECHA), the EU Member State authorities, and European industry sector associations collaborate to improve the quality of the SPERCs. Following up on the outcome of ECHA's SPERC Best Practice Project, industry, together with ECHA, developed an updated SPERC factsheet template and guidance on how to fill it out. In addition, industry developed 2 sets of SPERC factsheet examples and the corresponding SPERC background documents. These documents were submitted to a multistakeholder review process. The comments from the review were discussed at a workshop in spring 2016. The workshop participants acknowledged the revised factsheet format including the corresponding guidance, the 2 SPERC factsheets, and the 2 SPERC background documents as best practice examples. The package is expected to support further improvement of the quality of the SPERCs. A common understanding was achieved of the need to match the level of detail of the use conditions description with the risk to be controlled (i.e., the emission intensity and hazard profile of the substances) and with the level of conservatism of SPERC release factors. The complete and transparent documentation of the derivation of the release factors and of their conservatism is conceived as crucial for the credibility of the SPERCs, such that they can be trusted by partners in the chemicals supply chain and by regulators. To that end, background documents will include a dedicated section describing the conservatism of SPERCs. Millions of people want to donate their organs after they die for transplantation, and many of them have registered their wish to do so or told their family and friends about their decision. For most of them, however, this wish is unlikely to be fulfilled, as only a small number of deaths (1% in the United Kingdom) occur in circumstances where the opportunity to donate organs is possible. Even for those who do die in the "right" way and have recorded their wishes or live in a jurisdiction with a "presumed consent" system, donation often does not go ahead because of another issue: their families refuse to allow donation to proceed. In some jurisdictions, the rate of "family overrule" is over 10%. In this article, we provide a systematic ethical analysis of the family overrule of donation of solid organs by deceased patients, and examine arguments both in favor of and against allowing relatives to "veto" the potential donor's intentions. First, we provide a brief review of the different consent systems in various European countries, and the ramifications for family overrule. Next, we describe and discuss the arguments in favor of permitting donation intentions to be overruled, and then the arguments against doing so. The "pro" arguments are: overrule minimises family distress and staff stress; families need to cooperate for donation to take place; families might have evidence regarding refusal; and failure to permit overrules could weaken trust in the donation system. The "con" arguments are: overrule violates the patient's wishes; the family is too distressed and will regret the decision; overruling harms other patients; and regulations prohibit overrule. We conclude with a general discussion and recommendations for dealing with families who wish to overrule donation. Overall, overrule should only rarely be permitted.

The European Union (EU) chemicals regulation Registration, Evaluation, Authorisation and Restriction of Chemicals (REACH) requires a hazardous substance registration to identify the uses of a substance and the corresponding conditions of safe use. This requirement includes a human and an environmental safety assessment. Exposure scenarios are developed and employed for estimating emissions resulting from the uses of hazardous substances. To support the environmental assessments, the REACH guidance documents define 22 environmental release categories (ERCs) with conservative release factors (RFs) to water, air, and soil. Several industry associations target the ERCs to more specific uses and respective emission scenarios to enable more realistic emission estimations. They have developed more than 190 specific ERCs (SPERCs) as standardized descriptions of operational conditions (OCs) and risk management measures (RMMs). SPERCs reflect the current good practice and are documented in factsheets. These factsheets contain the information necessary for environmental emission modeling. Key parameters are the substance use rate, the efficiency of the risk management measures (if applicable), and the RFs. These parameters can be based on literature or measured company data or are justified by qualitative arguments. The majority of SPERCs have been implemented as realistic worst-case emission values in screening-level chemical safety assessment (CSA) tools. Three regulatory reviews in Europe have established requirements for documenting the SPERCs and for justifying the RFs. In addition, each of the reviews included recommendations for improving the SPERCs. The latest review proposed a condensed factsheet that focuses on the essentials for exposure assessment and subsequent communication in safety data sheets. It is complemented with a background document for providing details on the emission scenarios and justifications. In the EU the SPERCs will be further progressed in a consensus process using the multi-stakeholder expert network on exposure scenarios. This report deals with organ retrieval procedures in both controlled and uncontrolled DCD, looking at the ethical, legal, and psychosocial aspects during the different phases of the process. A recently published report by the UK Donation Ethics Committee (UKDEC) has served as an important reference document to outline the steps in the controlled DCD patient-donor pathway (Academy of Medical Royal Colleges. UK Donation Ethics Committee. An ethical framework for controlled donation after circulatory death. December 2011). For uncontrolled DCD, the UKDEC pathway description was adapted. At the 6th International Conference in Organ Donation held in Paris in 2013, an established expert European Working Group reviewed the UKDEC reports, which were then considered along with the available published literature. Regulation (EC) No 1907/2006 concerning the Registration, Evaluation, Authorisation and Restriction of Chemicals (REACH) requires that environmental exposure assessments be performed for all uses of dangerous substances that are marketed in the European Union in quantities above 10 tons per year. The quantification of emissions to the environment is a key step in this process. This publication describes the derivation of release factors and gives guidance for estimating use rates for quantifying the emissions from the manufacturing and application of adhesives and sealants. Release factors available for coatings and paints are read across to adhesives or sealants based on similarities between these 2 product groups with regard to chemical composition and to processing during manufacturing and application. The granular emission scenarios in these documents are mapped to the broad emission scenarios for adhesives or sealants. According to the mapping, the worst-case release factors for coatings or paints are identified and assigned to the adhesives or sealants scenarios. The resulting 10 specific environmental release categories (SPERCs) for adhesives and sealants are defined by differentiating between solvent and nonsolvent ingredients and between water-borne and solvent-borne or solvent-free products. These cover the vast majority of the production processes and uses and are more realistic than the 5 relevant emission estimation defaults provided in the REACH guidance. They are accompanied with adhesive or sealant consumption rates in the EU and with guidance for estimating conservative substance use rates at a generic level. The approach of combining conservative SPERC release factors with conservative estimates of substance rates is likely to yield emission estimates that tend to overpredict actual releases. Because this qualifies the approach for use in lower-tier environmental exposure assessment, the Association of the European Adhesive & Sealant Industry (FEICA) SPERCs are available in several exposure assessment tools that are used under REACH.
