1. Global Occurrence and Chemical Impact of Stratospheric Blue Jets Modeled With WACCM4

Abstract

In this work we present the first parameterizations of the global occurrence rate and chemical influence of Blue Jets, a type of Transient Luminous Event (TLE) taking place in the stratospheric region above thunderclouds. These parameterizations are directly coupled with five different lightning parameterizations implemented in the Whole Atmosphere Community Climate Model (WACCM4). We have obtained a maximum Blue Jet global occurrence rate of about 0.9 BJ per minute. The geographical occurrence of Blue Jets is closely related to the chosen lightning parameterization. Some previously developed local chemical models of Blue Jets predicted an important influence onto the stratospheric concentration of N$_2$O, NO$_x$ and O$_3$. We have used these results together with our global implementations of Blue Jets in WACCM4 to estimate their global chemical influence in the atmosphere. According to our results, Blue Jets can inject about 3.8 Tg N$_2$O-N yr$^{-1}$ and 0.07 Tg NO-N yr$^{-1}$ near the stratosphere, where N$_2$O-N and NO-N stand for the mass of nitrogen atoms in N$_2$O and NO molecules, respectively. These production rates of N$_2$O and NO$_x$ could have a direct impact on, for example, the acidity of rainwater or the greenhouse effect. We have found that Blue Jets could also slightly contribute to the depletion of stratospheric ozone. In particular, we have estimated that the maximum difference in the concentration of O$_3$ at 30 km of altitude between simulations with and without Blue Jets can be about -5 \% in Equatorial and Polar regions.

Abstract

In this work, we develop two spectroscopic diagnostic methods to derive the peak reduced electric field in Transient Luminous Events (TLEs) from their optical signals. These methods could be used to analyze the optical signature of TLEs reported by spacecraft such as ASIM (ESA) and the future TARANIS (CNES). As a first validation of these methods, we apply them to the predicted (synthetic) optical signatures of halos and elves, two type of TLEs, obtained from electrodynamical models. This procedure allows us to compare the inferred value of the peak reduced electric field with the value computed by halo and elve models. Afterward, we apply both methods to the analysis of optical signatures of elves and halos reported by GLIMS (JAXA) and ISUAL (NSPO) spacecraft, respectively. We conclude that the best emission ratios to estimate the maximum reduced electric field in halos and elves are the ratio of the Second Positive System (SPS) of N$_2$ to First Negative System (FNS) of N$_2^+$, the First Positive System (FPS) of N$_2$ to FNS of N$_2^+$ and the Lyman-Birge-Hopfield (LBH) band of N$_2$ to FNS of N$_2^+$. In the case of reduced electric fields below 150~Td, we found that the ratio of the SPS of N$_2$ to FPS of N$_2$ can also be used to reasonably estimate the value of the field. Finally, we show that the reported optical signals from elves can be treated following an inversion method in order to estimate some of the characteristics of the parent lightning.

Abstract

The spatial non-uniformity of the electric field in air discharges, such as streamers, can influence the accuracy of spectroscopic diagnostic methods and hence the estimation of the peak electric field. In this work, we use a self-consistent streamer discharge model to investigate the spatial non-uniformity in streamer heads and streamer glows. We focus our analysis on air discharges at atmospheric pressure and at the low pressure of the mesosphere. This approach is useful to investigate the spatial non-uniformity of laboratory discharges as well as sprite streamers and blue jet streamers, two types of Transient Luminous Event (TLE) taking place above thunderclouds. This characterization of the spatial non-uniformity of the electric field in air discharges allows us to develop two different spectroscopic diagnostic methods to estimate the peak electric field in cold plasmas. The commonly employed method to derive the peak electric field in streamer heads underestimates the electric field by about 40-50~\% as a consequence of the high spatial non-uniformity of the electric field. Our diagnostic methods reduce this underestimation to about 10-20\%. However, our methods are less accurate than previous methods for streamer glows, where the electric field is uniformly distributed in space. Finally, we apply our diagnostic methods to the measured optical signals in the Second Positive System of $N_2$ and the First Negative System of $N_2^+$ of sprites recorded by Armstrong et al. (1998) during the SPRITE's 95 and 96 campaigns.

Abstract

Halos and elves are Transient Luminous Events (TLEs) produced in the lower ionosphere as a consequence of lightning-driven electromagnetic fields. These events can influence the upper-atmospheric chemistry and produce optical emissions. We have developed different two-dimensional self-consistent models that couple electrodynamical equations with a chemical scheme to simulate halos and elves produced by vertical cloud-to-ground (CG) lightning discharges, Compact Intra-cloud Discharges (CIDs) and Energetic In-cloud Pulses (EIPs). The optical emissions from radiative relaxation of excited states of molecular and atomic nitrogen and oxygen have been calculated. We have upgraded previous local models of halos and elves to calculate for the first time the vibrationally detailed optical spectra of elves triggered by CIDs and EIPs. According to our results, the optical spectra of elves do not depend on the type of parent lightning discharge. Finally, we have quantified the local chemical impact in the upper atmosphere of single halos and elves. In the case of the halo, we follow the cascade of chemical reactions triggered by the lightning-produced electric field during a long-time simulation of up to one second. We obtain a production rate of NO molecules by single halos and elves of 10$^{16}$ and 10$^{14}$ molecules/J, respectively. The results of these local models have been used to estimate the global production of NO by halos and elves in the upper atmopshere at $\sim10^{-7}$ Tg~N/y. This global chemical impact of halos and elves is seven orders of magnitude below the production of NO in the troposphere by lightning discharges.

5. Whistler wave propagation through the ionosphere of Venus

Abstract

We investigate the attenuation of whistler waves generated by hypotetical venusian lightning occurring at the altitude of the cloud layer under different ionospheric conditions. We use the Stanford Full Wave Method (FWM) for stratified media of \cite{Lehtinen2008/JGR} to model wave propagation through the ionosphere of Venus. This method calculates the electromagnetic field created by an arbitrary source in a plane-stratified medium (i.e. uniform in the horizontal direction). We see that the existence of holes in electronic densities and the magnetic field configuration caused by solar wind play an important role in the propagation of electromagnetic waves through the venusian ionosphere.

6. Three-dimensional modeling of lightning-induced electromagnetic pulses on Venus, Jupiter and Saturn

Abstract

While lightning activity in Venus is still controversial, its existence in Jupiter and Saturn was first detected by the Voyager missions and later on confirmed by Cassini and New Horizons optical recordings in the case of Jupiter, and recently by Cassini on Saturn in 2009. Based on a recently developed 3D model we investigate the influence of lightning-emitted electromagnetic pulses (EMP) on the upper atmosphere of Venus, Saturn and Jupiter. We explore how different lightning properties such as total energy released and orientation (vertical, horizontal, oblique) can produce mesospheric transient optical emissions of different shapes, sizes and intensities. Moreover, we show that the relatively strong background magnetic field of Saturn can enhance the lightning-induced quasi-electrostatic and inductive electric field components above 1000 km of altitude producing stronger transient optical emissions that could be detected from orbital probes.

7. Mesospheric optical signatures of possible lightning on Venus

Abstract

A self-consistent two-dimensional model is proposed to account for the transient mesospheric nighttime optical emissions associated to possible intra-cloud (IC) lightning occurring in the Venusian troposphere. The model calculates the mesospheric (between 75 km and 120 km in altitude) quasielestrostatic electric field and electron density produced in response to IC lightning activity located beween 40 km and 65 km in the Venusian cloud layer. The optical signatures and the densities of perturbed excited atomic and molecular neutral and ionic species in the mesosphere of Venus are also calcutated using a basic kinetic scheme. The calculations were performed for different IC lightning discharge properties [Krasnopolsky (1980), Cosmic. Res.]. We found that the calculated electric fields in the mesosphere of Venus are above breakdown values and that, consequently, visible transient glows (similar to terrestrial Halos produced by lightning) right above the parent IC lightning are predicted. The transient optical emissions result from radiative de-excitation of excited electronic states of N$_2$ (in the ultraviolet, visible and near infrared ranges) and of O($^{1}$S) and of O($^{1}$D) in, respectively, the green (557 nm) and red (630 nm) wavelengths. The predicted transient lightning induced glows from O($^{1}$S) can reach an intensity higher than 167 R and, consequently, be above the detection threshold of the Lightning and Airglow Camera (LAC) instrument aboard the japanese Akatsuki probe orbiting Venus since Dec 2015. However, according to our model, successful observations of transient lightning-induced optical glows could only be possible for sufficiently close (300 km or maximum 1000 km) distances.

Abstract

We investigate the effect of halo activity on the electrostatic field measured at ground level. We use electrostatic arguments as well as self-consistent simulations to show that, due to the screening charge in the ionosphere, the distant electrostatic field created by the uncompensated charge in a thundercloud decays exponentially rather than as the third power of the distance. Furthermore, significative ionization around the lower edge of the ionosphere slightly reduces the electrostatic field at ground level. We conclude that halos do not extend the range of detectability of lightning-induced electrostatic fields.

9. Modeling streamer discharges as advancing imperfect conductors

Abstract

A major obstacle for the understanding of long electrical discharges is the complex dynamics of streamer coronas, formed by many thin conducting filaments. Building macroscopic models for these filaments is one approach to attain a deeper knowledge of the discharge corona. Here we present a one-dimensional, macroscopic model of a propagating streamer channel. We represent the streamer as an advancing finite-conductivity channel with a surface charge density at its boundary. This charge evolves self-consistently due to the electric current that flows through the streamer body and within a thin layer at its surface. We couple this electrodynamic evolution with a field-dependent set of chemical reactions that determine the internal channel conductivity. With this one-dimensional model we investigate how key properties of a streamer affect the channel's evolution. The ultimate objective of our model is to construct realistic models of streamer coronas in order to understand better the physics of long electrical discharges.

Abstract

Detailed observations of the solar system planets reveal a wide variety of local atmospheric conditions. Astronomical observations have revealed a variety of extrasolar planets none of which resembles any of the solar system planets in full. Instead, the most massive amongst the extrasolar planets, the gas giants, appear very similar to the class of (young) Brown Dwarfs which are amongst the oldest objects in the universe. Despite of this diversity, solar system planets, extrasolar planets and Brown Dwarfs have broadly similar global temperatures between 300K and 2500K. In consequence, clouds of different chemical species form in their atmospheres. While the details of these clouds differ, the fundamental physical processes are the same. Further to this, all these objects were observed to produce radio and X-ray emission. While both kinds of radiation are well studied on Earth and to a lesser extent on the solar system planets, the occurrence of emission that potentially originate from accelerated electrons on Brown Dwarfs, extrasolar planets and protoplanetary disks is not well understood yet. Abstract

Single- and multi-pulse blue corona discharges are frequently observed in thunderstorm clouds. Although we know they often correlate with Narrow Bipolar Events (NBEs) in Very Low Frequency/Low Frequency (VLF/LF) radio signals, their physics is not well understood. Here, we report a detailed analysis of different types of blue corona discharges observed by the Atmosphere-Space Interactions Monitor (ASIM) during an overpass of a thundercloud cell nearby Malaysia. Both single- and multi-pulse blue corona discharges were associated with positive NBEs at the top of the cloud, reaching about 18 km altitude. We find that the primary pulses of multi-pulse discharges have weaker current moments than the single-pulse discharges, suggesting that the multi-pulse discharges either have shorter vertical channels or have weaker currents than the single-pulse discharges. The subsequent pulse trains of the multi-pulse discharges delayed some milliseconds are likely from horizontally oriented electrical discharges, but some NBEs, correlated with both single-and multi-pulse discharges, include small-amplitude oscillations within a few microseconds inside their waveforms, which are unresolved in the optical observation and yet to be understood. Furthermore, by jointly analyzing the optical and radio observations, we estimate the photon free mean path at the cloud top to be ∼6 m.

Abstract

Edoxaban is used for venous thromboembolism (VTE) treatment. Real-life data are lacking about its use in long-term therapy. We aimed to assess the efficacy and the safety of edoxaban for long-term VTE treatment in a real-life setting. Patients with VTE included in the Registro Informatizado Enfermedad TromboEmbólica (RIETE) registry, receiving edoxaban 60 or 30 mg daily were prospectively followed up to validate the benefit of using different dosages. The main outcome was the composite of VTE recurrences or major bleeding in patients with or without criteria for dose reduction. Multivariable analysis to identify predictors for the composite outcome was performed. From October 2015 to November 2019, 562 patients received edoxaban for long-term therapy. Most (94%) of the 416 patients not meeting criteria for dose reduction received 60 mg daily, and 92 patients meeting criteria (63%) received 30 mg daily. During treatment, two patients developed recurrent VTE, six had major bleeding and nine died (2 from fatal bleeding). Among patients not meeting criteria for dose reduction, those receiving 30 mg daily had a higher rate of the composite event (hazard ratio (HR) 8.37; 95% confidence interval (CI) 1.12–42.4) and a significant higher mortality rate (HR 31.1; 95% CI 4.63–262) than those receiving 60 mg. Among patients meeting criteria for dose reduction, those receiving 60 mg daily had no events, and a nonsignificantly higher mortality rate (HR 5.04; 95% CI 0.54–133) than those receiving 30 mg daily. In conclusion, edoxaban seems to be effective and safe for long-term VTE treatment in real life. Criteria for dose reduction should be reformulated., No funding was received for this work

Abstract

In the current era of patient empowerment and precision medicine, access to timely information is critical to decision-making. Unfortunately, we currently lack patient-specific, real-time data about clinical presentation, risk of thrombotic or hemorrhagic events, key risk factors, and adverse outcomes in patients with venous thromboembolism (VTE). Accordingly, the Registro Informatizado Enfermedad TromboEmbólica (RIETE) investigators developed a tool to provide an open-source, real-time graphic representation of VTE-related data derived from over 90 000 patients with confirmed VTE. This information is intended to facilitate discussion in the informed decision-making process. The current article describes the aims, rationale, methods, and ongoing and future efforts of the real-time VTE infographics developed by the RIETE registry collaborators.

Abstract

Background Patients with venous thromboembolism (VTE) secondary to transient risk factors may develop VTE recurrences after discontinuing anticoagulation. Identifying at‐risk patients could help to guide the duration of therapy. Methods We used the RIETE database to assess the prognostic value of d‐dimer testing after discontinuing anticoagulation to identify patients at increased risk for recurrences. Transient risk factors were classified as major (postoperative) or minor (pregnancy, oestrogen use, immobilization or recent travel). Results In December 2018, 1655 VTE patients with transient risk factors (major 460, minor 1195) underwent d‐dimer measurements after discontinuing anticoagulation. Amongst patients with major risk factors, the recurrence rate was 5.74 (95% CI: 3.19–9.57) events per 100 patient‐years in those with raised d‐dimer levels and 2.68 (95% CI: 1.45–4.56) in those with normal levels. Amongst patients with minor risk factors, the rates were 7.79 (95% CI: 5.71–10.4) and 3.34 (95% CI: 2.39–4.53), respectively. Patients with major risk factors and raised d‐dimer levels (n = 171) had a nonsignificantly higher rate of recurrences (hazard ratio [HR]: 2.14; 95% CI: 0.96–4.79) than those with normal levels. Patients with minor risk factors and raised d‐dimer levels (n = 382) had a higher rate of recurrences (HR: 2.34; 95% CI: 1.51–3.63) than those with normal levels. On multivariate analysis, raised d‐dimers (HR: 1.74; 95% CI: 1.09–2.77) were associated with an increased risk for recurrences in patients with minor risk factors, not in those with major risk factors. Conclusions Patients with raised d‐dimer levels after discontinuing anticoagulant therapy for VTE provoked by a minor transient risk factor were at an increased risk for recurrences. Abstract

Knee arthroscopy is the most common orthopedic procedure worldwide. While incidence of post-arthroscopy venous thromboembolic events (VTE) is low, treatment patterns and patient outcomes have not been described. Patients from the "Registro Informatizado Enfermedad TromboEmbolica" who had confirmed post-arthroscopy VTE were compared to patients with provoked, post bone-fracture, and to patients with unprovoked VTE. Baseline characteristics, presenting signs and symptoms, treatment and outcomes including recurrent VTE, bleeds or death were compared. Abstract

BackgroundCurrent guidelines suggest treating cancer patients with incidental pulmonary embolism (PE) similarly to those with clinically suspected and confirmed PE. However, the natural history of these presentations has not been thoroughly compared.MethodsWe used the data from the RIETE (Registro Informatizado de Enfermedad TromboEmbólica) registry to compare the 3-month outcomes in patients with active cancer and incidental PE versus those with clinically suspected and confirmed PE. The primary outcome was 90-day all-cause mortality. Secondary outcomes were PE-related mortality, symptomatic PE recurrences and major bleeding.ResultsFrom July 2012 to January 2019, 946 cancer patients with incidental asymptomatic PE and 2274 with clinically suspected and confirmed PE were enrolled. Most patients (95% versus 90%) received low-molecular-weight heparin therapy. During the first 90 days, 598 patients died, including 42 from PE. Patients with incidental PE had a lower all-cause mortality rate than those with suspected and confirmed PE (11% versus 22%; OR 0.43, 95% CI 0.34–0.54). Results were consistent for PE-related mortality (0.3% versus 1.7%; OR 0.18, 95% CI 0.06–0.59). Multivariable analysis confirmed that patients with incidental PE were at lower risk of death (adjusted OR 0.43, 95% CI 0.34–0.56). Overall, 29 (0.9%) patients developed symptomatic PE recurrences, and 122 (3.8%) had major bleeding. There were no significant differences in PE recurrences (OR 0.62, 95% CI 0.25–1.54) or major bleeding (OR 0.78, 95% CI 0.51–1.18).ConclusionsCancer patients with incidental PE had a lower mortality rate than those with clinically suspected and confirmed PE. Further studies are required to validate these findings, and to explore optimal management strategies in these patients.

In order to simplify other laparoscopic techniques, a new standing laparoscopic technique for partial closure of inguinal canal (PCIC) has been developed. This technique uses a new anchoring device and can be performed without advanced laparoscopic skills. The aim of this study was to develop a prior evaluation of the effects of this new technique on the stallion reproductive capacity, assessing the sperm production and testicular perfusion. Abstract

Background: Survival outcomes are poor for patients with metastatic urothelial carcinoma who receive standard, first-line, platinum-based chemotherapy. We assessed the overall survival of patients who received durvalumab (a PD-L1 inhibitor), with or without tremelimumab (a CTLA-4 inhibitor), as a first-line treatment for metastatic urothelial carcinoma.Methods: DANUBE is an open-label, randomised, controlled, phase 3 trial in patients with untreated, unresectable, locally advanced or metastatic urothelial carcinoma, conducted at 224 academic research centres, hospitals, and oncology clinics in 23 countries. Eligible patients were aged 18 years or older with an Eastern Cooperative Oncology Group performance status of 0 or 1. We randomly assigned patients (1:1:1) to receive durvalumab monotherapy (1500 mg) administered intravenously every 4 weeks; durvalumab (1500 mg) plus tremelimumab (75 mg) administered intravenously every 4 weeks for up to four doses, followed by durvalumab maintenance (1500 mg) every 4 weeks; or standard-of-care chemotherapy (gemcitabine plus cisplatin or gemcitabine plus carboplatin, depending on cisplatin eligibility) administered intravenously for up to six cycles. Randomisation was done through an interactive voice–web response system, with stratification by cisplatin eligibility, PD-L1 status, and presence or absence of liver metastases, lung metastases, or both. The coprimary endpoints were overall survival compared between the durvalumab monotherapy versus chemotherapy groups in the population of patients with high PD-L1 expression (the high PD-L1 population) and between the durvalumab plus tremelimumab versus chemotherapy groups in the intention-to-treat population (all randomly assigned patients). The study has completed enrolment and the final analysis of overall survival is reported. The trial is registered with ClinicalTrials.gov, NCT02516241, and the EU Clinical Trials Register, EudraCT number 2015-001633-24.Findings: Between Nov 24, 2015, and March 21, 2017, we randomly assigned 1032 patients to receive durvalumab (n=346), durvalumab plus tremelimumab (n=342), or chemotherapy (n=344). At data cutoff (Jan 27, 2020), median follow-up for survival was 41·2 months (IQR 37·9–43·2) for all patients. In the high PD-L1 population, median overall survival was 14·4 months (95% CI 10·4–17·3) in the durvalumab monotherapy group (n=209) versus 12·1 months (10·4–15·0) in the chemotherapy group (n=207; hazard ratio 0·89, 95% CI 0·71–1·11; p=0·30). In the intention-to-treat population, median overall survival was 15·1 months (13·1–18·0) in the durvalumab plus tremelimumab group versus 12·1 months (10·9–14·0) in the chemotherapy group (0·85, 95% CI 0·72–1·02; p=0·075). In the safety population, grade 3 or 4 treatment-related adverse events occurred in 47 (14%) of 345 patients in the durvalumab group, 93 (27%) of 340 patients in the durvalumab plus tremelimumab group, and in 188 (60%) of 313 patients in the chemotherapy group. The most common grade 3 or 4 treatment-related adverse event was increased lipase in the durvalumab group (seven [2%] of 345 patients) and in the durvalumab plus tremelimumab group (16 [5%] of 340 patients), and neutropenia in the chemotherapy group (66 [21%] of 313 patients). Serious treatment-related adverse events occurred in 30 (9%) of 345 patients in the durvalumab group, 78 (23%) of 340 patients in the durvalumab plus tremelimumab group, and 50 (16%) of 313 patients in the chemotherapy group. Deaths due to study drug toxicity were reported in two (1%) patients in the durvalumab group (acute hepatic failure and hepatitis), two (1%) patients in the durvalumab plus tremelimumab group (septic shock and pneumonitis), and one (Interpretation: This study did not meet either of its coprimary endpoints. Further research to identify the patients with previously untreated metastatic urothelial carcinoma who benefit from treatment with immune checkpoint inhibitors, either alone or in combination regimens, is warranted.Funding: AstraZeneca.

Abstract

Registro Informatizado de Enfermedad Tromboembólica Investigators., [Objective] Overlap exists between the risk factors for coronary artery disease and venous thromboembolism (VTE). However, a paucity of data is available on the incidence of major acute cardiovascular events (MACE) and major adverse limb events (MALE) among patients presenting with VTE. Moreover, it is unknown whether the rate of cardiovascular outcomes differs among patients with unprovoked vs provoked VTE., [Methods] We analyzed the data from 2009 to 2017 in the Registro Informatizado de Enfermedad Tromboembólica registry, an ongoing, multicenter, international registry of consecutive patients with a diagnosis of objectively confirmed VTE. The query was restricted it to patients with data entry for the arterial outcomes. The baseline prevalence of coronary artery disease risk factors was compared between patients with provoked (ie, immobility, cancer, surgery, travel >6 hours, hormonal causes) and unprovoked VTE. After the initial VTE event, we followed up patients for the composite primary outcome of incident MACE (ie, stroke, myocardial infarction, unstable angina) and/or MALE (ie, major limb events). We used the χ2 test for baseline associations and a Cox proportional hazard for multivariate analysis. We used IBM SPSS, version 24 (IBM Corp, Armonk, NY) for statistical analysis. A P value of, [Results] We analyzed the data from 41,259 patients with VTE, of whom 22,633 (55.6%) had experienced a provoked VTE. During follow-up, the patients with provoked VTE were more likely to develop MACE or MALE than were patients with unprovoked VTE (hazard ratio [HR], 1.3; 95% confidence interval [CI], 1.1-1.5). The association of arterial events with recent immobility (HR, 1.4; 95% CI, 1.5-12.1) and cancer (HR, 1.7; 95% CI, 1.4-1.9) was strong. After adjusting for multiple conventional cardiovascular risk factors, provoked VTE, compared with unprovoked VTE, was significantly associated with an increased hazard for MACE (HR, 1.4; 95% CI, 1.1-1.7). Cancer remained a significant adjusted predictor for both MACE (HR, 1.7; 95% CI, 1.4-2.1) and MALE (HR, 2.1; 95% CI 1.01-4.6) in those with provoked VTE., [Conclusions] Among patients with VTE, provoked cases, specifically those with cancer-associated VTE, have an increased risk of major arterial events.

Abstract

The RIETE investigators., In young patients with acute pulmonary embolism (PE), the predictive value of currently available prognostic tools has not been evaluated. Our objective was to compare prognostic value of 7 available tools (GPS, PESI, sPESI, Prognostic Algorithm, PREP, shock index and RIETE) in patients aged, This study received funding: SEPAR (1/2016) grupo GeCIR. Abstract

Background Venous thromboembolism (VTE) is a major cause of morbidity and mortality postoperatively. The use of pharmacologic prophylaxis is effective in reducing the incidence of VTE. However, the prophylaxis is often discontinued at hospital discharge, especially for those with benign disease. The implications of this practice are not known. We assessed the data from a large, ongoing registry regarding the time course of VTE and outcomes after noncancer surgery. Methods We analyzed the RIETE (Computerized Registry on Venous Thromboembolism) registry, which includes data from consecutive patients with symptomatic confirmed VTE. In the present study, we focused on general surgical patients who had developed symptomatic postoperative VTE in the first 8 weeks after noncancer surgery. The main objective was to assess the interval between surgery and the occurrence of VTE. Additional variables included the clinical presentation associated with the event, the use of thrombosis prophylaxis, and unfavorable outcomes. Results The data from 3296 patients were analyzed. The median time from surgery to the detection of VTE was 16 days (interquartile range, 8-30 days). Of the VTE events, 77% were detected after the first postoperative week and 27% after 4 weeks. Overall, 43.9% of the patients with VTE had received pharmacologic prophylaxis after surgery for a median of 8 days (interquartile range, 5-14 days), and three quarters of the VTE events were detected after pharmacologic prophylaxis had been discontinued. Overall, 54% of the patients with VTE had presented with pulmonary embolism. For 15% of the patients, the clinical outcome was unfavorable, including 4% who had died within 90 days. Conclusions The risk of VTE after noncancer general surgery remains high for ≤2 months. More than one half of the patients had presented with symptomatic PE as the VTE event, and 15% had had unfavorable outcomes. Only 44% of these patients had received pharmacologic prophylaxis for around 1 week.

Abstract

BACKGROUND: Limited data exist about the clinical presentation, ideal therapy and outcomes of patients with hereditary hemorrhagic telangiectasia (HHT) who develop venous thromboembolism (VTE). METHODS: We used the data in the RIETE Registry to assess the clinical characteristics, therapeutic approaches and clinical outcomes during the course of anticoagulant therapy in patients with HHT according to initial presentation as pulmonary embolism (PE) or deep venous thrombosis (DVT). RESULTS: Of 51,375 patients with acute VTE enrolled in RIETE from February 2009 to January 2019, 23 (0.04%) had HHT: 14 (61%) initially presented with PE and 9 (39%) with DVT alone. Almost half (47.8%) of the patients with VTE had a risk factor for VTE. Most PE and DVT patients received low-molecular-weight heparin for initial (71 and 100%, respectively) and long-term therapy (54 and 67%, respectively). During anticoagulation for VTE, the rate of bleeding events (major 2, non-major 6) far outweighed the rate of VTE recurrences (recurrent DVT 1): 50.1 bleeds per 100 patient-years (95%CI: 21.6-98.7) vs. 6.26 recurrences (95%CI: 0.31-30.9; p = 0.020). One major and three non-major bleeding were epistaxis. No patient died of bleeding. One patient died shortly after being diagnosed with acute PE. CONCLUSIONS: During anticoagulation for VTE in HHT patients, there were more bleeding events than VTE recurrences. Abstract

Objective: Given its rarity, the management of primary upper extremity deep vein thrombosis is controversial. Although anticoagulation alone is commonly advocated for its treatment, it is unclear if this will reduce the risk of developing post-thrombotic syndrome (PTS). The aim of this "Evidence Driven" Clinical Scenario is to evaluate whether more aggressive treatments (including catheter directed thrombolysis or surgery) might help reduce the risk of PTS or recurrent venous thromboembolism in patients with primary upper extremity deep vein thrombosis (DVT)., Methods: An electronic systematic review of Ovid MEDLINE and Embase was conducted. Randomised controlled trials and observational studies were eligible. The primary outcome was PTS., Results: The initial search identified 146 articles, and 36 more were identified during a secondary search. In total, 25 studies, reporting the outcome of 1271 patients, were included. None of the studies included was a randomised controlled trial and the large majority of studies were retrospective cohorts. The use of anticoagulation alone was associated with a significant risk of PTS. In patients treated with surgery with or without thrombolysis the incidence of PTS was significantly reduced., Conclusion: Current evidence, albeit with some methodological limitations, suggests that anticoagulation may not be sufficient to prevent PTS in patients with primary upper extremity DVT and that surgery with or without thrombolysis to repair the anatomical defects is needed., (Copyright © 2017 European Society for Vascular Surgery.
