Patient navigation is a strategy for overcoming barriers to reduce disparities and to improve access and outcomes. The aim of this umbrella review was to identify, critically appraise, synthesize, and present the best available evidence to inform policy and planning regarding patient navigation across the cancer continuum. Systematic reviews examining navigation in cancer care were identified in the Cochrane Central Register of Controlled Trials (CENTRAL), PubMed, Embase, Cumulative Index of Nursing and Allied Health (CINAHL), Epistemonikos, and Prospective Register of Systematic Reviews (PROSPERO) databases and in the gray literature from January 1, 2012, to April 19, 2022. Data were screened, extracted, and appraised independently by two authors. The JBI Critical Appraisal Checklist for Systematic Review and Research Syntheses was used for quality appraisal. Emerging literature up to May 25, 2022, was also explored to capture primary research published beyond the coverage of included systematic reviews. Of the 2062 unique records identified, 61 systematic reviews were included. Fifty-four reviews were quantitative or mixed-methods reviews, reporting on the effectiveness of cancer patient navigation, including 12 reviews reporting costs or cost-effectiveness outcomes. Seven qualitative reviews explored navigation needs, barriers, and experiences. In addition, 53 primary studies published since 2021 were included. Patient navigation is effective in improving participation in cancer screening and reducing the time from screening to diagnosis and from diagnosis to treatment initiation. Emerging evidence suggests that patient navigation improves quality of life and patient satisfaction with care in the survivorship phase and reduces hospital readmission in the active treatment and survivorship care phases. Palliative care data were extremely limited. Economic evaluations from the United States suggest the potential cost-effectiveness of navigation in screening programs

INTRODUCTION: Vaccination is a key strategy to limit the impact of the COVID-19 pandemic, among vulnerable groups such as cancer patients. However, COVID-19 vaccine hesitancy is limiting vaccination uptake in this population as in others. This study aimed to synthesise the emerging literature on vaccine hesitancy in this population and in Oncology health professionals, reasons for and factors associated with hesitancy, and interventions that address hesitancy. METHODS: A rapid review was undertaken PubMed, Ovid and Google across all years up to October 2021 for articles in English, from any country or region, addressing the above issues. Individual case studies, opinion pieces, commentary articles and conference abstracts were excluded. Article screening, data extraction and bias assessment were conducted by two authors. A narrative synthesis of the data was undertaken. RESULTS: Eighteen eligible articles were identified. Reported COVID-19 vaccine hesitancy rates varied from 76.7 % to 3.9 %, with a mean of 38.4 %. A large international study (n > 20,000) reported a more conservative hesitancy rate of 19 %. Six broad, common reasons for hesitancy were identified. Oncologist advice was valued by patients. DISCUSSION: Vaccine hesitancy remains a significant concern in the oncology context. Oncologists are key to addressing hesitancy and providing tailored advice to cancer patients. PRACTICE IMPLICATIONS: Where possible, patients appreciate personalised, tailored information about vaccination which addresses its interaction with cancer and its treatment. Education programmes for oncologists to support effective communication in this context are needed. Webinars and peer-to-peer counselling may be useful but remain to be proven.

This article's main aim is to discuss research exploring how the self-governing practices found in Indigenous societies, biota and modern organisations can be embedded into the constitutions of legal entities to protect and share the wellbeing of humanity, biota and the planet. In this paper, we explore how Australian Indigenous knowledge and practices can be embedded into organisational entities and discuss how this can be achieved by reformatting Ostrom's design principles to be incorporated into corporate constitutions following an ecological form of governance practised by Indigenous Australians. This form of polycentric self-governance can aggregate the voices of minorities representing local environments up to a global level. We use case studies, system science and biomimicry to explore polycentric self-governance and how organisations can adopt it to focus on the wellbeing of all stakeholders. In particular, the paper highlights how Indigenous knowledge can contribute globally to achieving societal sustainability.

BACKGROUND: There are limited data comparing ablation outcomes in patients with low intraprocedural burden of ventricular arrhythmias (VA) undergoing a pace mapping (PM)-guided strategy vs those with high burden guided by standard activation mapping strategy (non-PM). OBJECTIVE: We sought to determine if catheter ablation-guided by PM of low-intraprocedural-burden idiopathic outflow tract VA would be noninferior compared to non-PM-guided ablation. METHODS: Outcomes of catheter ablation of idiopathic outflow tract VA in 22 patients with a low burden of intraprocedural VA using PM-guided ablation were compared to 44 patients with a high burden of intraprocedural VA undergoing ablation using standard techniques. RESULTS: Sixty-six patients were included (age 46.5 ± 14.8 years; 68% female, left ventricular ejection fraction 59% ± 5%). Within the PM group, 24-hour preprocedure premature ventricular complex (PVC) burden was 9.5% (interquartile range [IQR] 4%-13.8%), number of pace maps 33.6 ± 18.5, surface area of ≥95% pace map correlation 1.9 ± 1.2 cm2, with best pace map correlation 96% (IQR 92%-97%). Within the non-PM group, 24-hour preprocedure PVC burden was 13.5% (IQR 6.6%-30%), earliest activation time -33.7 ± 9.9 ms. Procedural duration, general anesthesia administration, fluoroscopy dose, and complications were all comparable. Following final procedure, 24-hour VA burden (PM 0% [IQR 0-2.4%] vs non-PM 0% [IQR 0-4.2%], P = .98), along with VA-free survival at 6-month follow-up (PM 77% vs non-PM 71%, P = .77), were both comparable. CONCLUSION: In patients with low intraprocedural burden of outflow tract VA, PM-guided catheter ablation can accurately identify the VA site of origin, leading to outcomes comparable to those achieved with standard ablation techniques.

INTRODUCTION: Minimal data exist on the Advisor HD Grid (HDG) catheter and the Precision electroanatomic mapping (EAM) system for ventricular arrhythmia (VA) procedures. Using the HDG catheter, the EAM uses the high-density (HD) wave mapping and best duplicate software to compare the maximum peak-to-peak bipolar voltages within a small zone independent of wavefront direction and catheter orientation. This study aimed to summarize the procedural experience for VAs using the HDG catheter. METHODS: Clinical and procedural characteristics of VA ablation procedures were retrospectively reviewed that used the HDG catheter and the Precision EAM over a 12-month period. RESULTS: A total of 22 patients, 18 with sustained ventricular tachycardia and 4 with premature ventricular contractions were included. Clinically indicated left and/or right ventricular (LV, RV, respectively), and aortic maps were created. LV substrate maps (n = 13) used a median 1700 points (interquartile range [IQR]25%-75% , 1427-2412) out of a median 18 573 (IQR25%-75% , 15 713-41 067) total points collected. RV substrate maps (n = 11) used a median 1435 points (IQR25%-75% , 1114-1871) out of a median 16 005 (IQR25%-75% , 11 063-21 405) total points collected. Total point utilization, used vs collected, was 9%. Mean mapping time was 43 ± 17 minutes (substrate, 34 ± 18 minutes; activation/pace mapping, 9 ± 13 minutes). Acute success was achieved in 56 (86%) and short-term success achieved in 16 patients (73%) at a median follow-up of 145 days (IQR25%-75% , 62-273 days). There were no procedural complications. CONCLUSION: HD wave mapping using the novel HDG catheter integrated with the Precision EAM is safe and feasible in VA procedures in the LV, RV, and aorta. Mapping times are consistent with other multielectrode mapping catheters.

The Association of Cancer Physicians in the United Kingdom has developed a strategy to improve outcomes for cancer patients and identified the goals and commitments of the Association and its members.

This paper serves as an introduction and scene-setter to a series of four papers (including this) that examines the different facets of scholarly publication in the area of media and communication (as well as its various cognate disciplines) in the ‘region’ that comprises Australia and New Zealand. The authors of these papers are active members of the Australian and New Zealand Communication Association (ANZCA), the peak professional and scholarly association of the region, and have all delivered a version of these papers at a plenary panel on ‘Publication in Australia and New Zealand’ at the International Communications Association (ICA) Regional Conference held at the Queensland University of Technology in Brisbane during 1–3 October 2014. This paper begins by looking at the role ANZCA plays in supporting and mentoring scholars, especially postgraduates or aspiring academics and early-career researchers, towards commencing their publication journey. It then turns its attention towards the globalising strategy of ‘parochial internationalisation’ that has defined much of Australia’s attempt to gain relevance and recognition in the global publications environment. The second part of the paper details the growth trajectory of one of Australia’s oldest and most reputable media and communications journal, Media International Australia, offering insights on how scholars and journal editors might think and operate more strategically in order to thrive in a new and demanding publications era.
