2. Repeated exposure to low doses of light induces retinal damage in vivo in a wavelength-dependent manner

Abstract

The exposure of the general population to artificial light at night has dramatically increased in recent decades. Current standards for domestic lighting are based on acute exposure to light and consider blue wavelengths to be responsible for phototoxicity. However, meta-analyses pointed out the role of lifelong light exposure in the onset of age-related macular degeneration, suggesting a cumulative effect of light exposure. Here, we investigate the retinal phototoxicity of a repeated exposure to light emitting diodes of different spectral compositions in 6-week-old albino rats. Rats were exposed twice a day for 15 days to retinal doses that were safe in acute exposure (0.1 and 0.2 J/cm² for blue and white lights, 0.2 J/cm² for green light and 0.05 J/cm² for red light). We show that rats repeatedly exposed to blue and white lights display irreversible retinal damage, characterized by a degradation of the global retinal structure, a significant photoreceptor loss, and an increase of stress and inflammation markers. We highlight the role of green wavelengths in the phototoxicity of white light and show the protective effect of the addition of red light to mitigate the phototoxicity of blue light. All of this points out the need to rethink the current phototoxicity standards by taking into account the cumulative effect of the exposure to light and the role of the different parts of the emission spectrum.

7. Association between multidimensional prognostic index (MPI) and pre-operative delirium in older patients with hip fracture

Abstract

Abstract Pre-operative delirium may cause delay in surgical intervention in older patients hospitalized for hip fracture. Also it has been associated with higher risk of post-surgical complications and worst functional outcomes. Aim of this retrospective cohort study was to evaluate whether the multidimensional prognostic index (MPI) at hospital admission was associated with pre-operative delirium in older individuals with hip fracture who are deemed to require surgical intervention. Consecutive older patients (≥ 65 years) with hip fracture underwent a comprehensive geriatric assessment to calculate the MPI at hospital admission. According to previously established cut-offs, MPI was expressed in three grades, i.e. MPI-1 (low-risk), MPI-2 (moderate-risk) and MPI-3 (high risk of mortality). Pre-operative delirium was assessed using the four ‘A’s Test. Out of 244 older patients who underwent surgery for hip fracture, 104 subjects (43%) received a diagnosis of delirium. Overall, the incidence of delirium before surgery was significantly higher in patients with more severe MPI score at admission. Higher MPI grade (MPI-3) was independently associated with higher risk of pre-operative delirium (OR 2.45, CI 1.21–4.96). Therefore, the MPI at hospital admission might help in early identification of older patients with hip fracture at risk for pre-operative delirium.

Abstract

The spectral distribution is a fundamental property of non-monochromatic optical radiation. It is commonly used in research and practical applications when studying how light interacts with matter and living organisms, including humans. In the field of lighting, misconceptions about the spectral distribution of light are responsible for unfounded claims, which pervade the scientific and technical communities. Starting from the definition of the spectral distribution, this paper describes the ambiguities and errors associated with a purely graphical analysis of the spectral distribution. It also emphasizes the importance of considering the particle nature of light in research involving both visual and non-visual effects, which implies using the spectral distribution expressed in the photon system of units, a system that has been seldom used in lighting research for historical reasons. The authors encourage lighting engineers and researchers to determine which system is best suited to their work and then proceed with the correct use of spectral distributions and of spectral weighting functions for applications involving optical radiation. Abstract

O artigo tem como objetivo analisar a abordagem ou não da cultura afro-brasileira, por meio da dança nas aulas de Educação Física, pelos professores do Ensino Fundamental I das quatro escolas municipais de Santa Rosa do Sul –SC que atendem este nível de ensino. O período da pesquisa se deu entre julho de 2018 a junho de 2019 e foi desenvolvido por meio de uma pesquisa de campo, a partir de um questionário, com sete professores da rede. Constatou-se que há, em parte, a abordagem da cultura afro-brasileira nas aulas de Educação Física do Ensino Fundamental I. Porém, se evidencia a descontinuidade do planejamento, resultando em ações soltas e descontextualizadas durante as aulas. A abordagem da dança afro-brasileira como forma de conhecimento não foi evidenciada e os professores pouco compreendem sobre o conteúdo dança ou cultura afro-brasileira.

Abstract

Aims A previous randomized study demonstrated that the subcutaneous implantable cardioverter defibrillator (S-ICD) was noninferior to transvenous ICD with respect to device-related complications and inappropriate shocks. However, that was performed prior to the widespread adoption of pulse generator implantation in the intermuscular (IM) space instead of the traditional subcutaneous (SC) pocket. The aim of this analysis was to compare survival from device-related complications and inappropriate shocks between patients who underwent S-ICD implantation with the generator positioned in an IM position in comparison with an SC pocket. Methods and results We analysed 1577 consecutive patients who had undergone S-ICD implantation from 2013 to 2021 and were followed up until December 2021. Subcutaneous patients (n = 290) were propensity matched with patients of the IM group (n = 290), and their outcomes were compared. : During a median follow-up of 28 months, device-related complications were reported in 28 (4.8%) patients and inappropriate shocks were reported in 37 (6.4%) patients. The risk of complication was lower in the matched IM group than in the SC group [hazard ratio 0.41, 95% confidence interval (CI) 0.17-0.99, P = 0.041], as well as the composite of complications and inappropriate shocks (hazard ratio 0.50, 95% CI 0.30-0.86, P = 0.013). The risk of appropriate shocks was similar between groups (hazard ratio 0.90, 95% CI 0.50-1.61, P = 0.721). There was no significant interaction between generator positioning and variables such as gender, age, body mass index, and ejection fraction. Conclusion Our data showed the superiority of the IM S-ICD generator positioning in reducing device-related complications and inappropriate shocks.

Abstract

Adapting to the existent context in current days is a mandatory measure that implies understanding present times. It also implies comprehending an ontological critique that explains the role of education and knowledge, at the same time as supporting educational and connection strategies that restore to theory its fundamental role of guiding the practice. Contrary to zeitgeist, ontology and knowledge implications on the debate about education, explicitly or implicitly pose the question why ontology and why an ontological critique? In this article, we present the dialectical movement of the ontological and gnosiological dimension based on a conception of human beings that can generate the possibilities of banning the idea that the totalizing capitalist system cannot be surpassed. On the contrary, we need a form of knowledge that considers totality, as offered by critical ontology.

Abstract

Background: Implantable loop recorder (ILR) is still underutilized in clinical practice, especially in the setting of elderly patients with recurrent, traumatic, unexplained syncope. Data on the actual risk of traumatic syncopal recurrence during ILR monitoring in this specific patient setting are lacking. Research design and methods: Prospective, multicentre registry enrolling consecutive patients undergoing ILR insertion for unexplained, recurrent, traumatic syncope. In a proportion of enrolled patients, remote monitoring (RM) was used for device follow-up. The risk of traumatic and non-traumatic syncopal recurrences during ILR observation were prospectively assessed. Results: A total of 483 consecutive patients (68±14 years, 59% male) were enrolled. During a median follow-up of 18 months, a final diagnosis was reached in 270 patients (55.9%). The risk of syncopal and traumatic syncopal recurrence was of 26.5 and 9.3%, respectively. RM significantly reduced the time to diagnosis (19.7±10.3 vs. 22.1±10.8 months; p=0.015) and was associated with a significant reduction in the risk of syncope recurrence of 48% (p

Abstract

O artigo tem o objetivo de analisar as políticas de formação docente, a partir de um balanço das últimas décadas, a fim de problematizar as perspectivas que se apresentam diante de um contexto neoconservador que se configurou, sobretudo, a partir de 2019, no Brasil. A análise apresentada é resultado de estudos documentais e bibliográficos, sob a perspectiva ontoepistemológica marxista. Inicialmente apresentamos algumas premissas que acreditamos serem fundamentais, pois orientam a concepção de formação de professores(as) no contexto da formação humana, a concepção de educação e de conhecimento que defendemos e que contraria as propostas que permeiam a sociabilidade do capital. Posteriormente, recuperamos questões colocadas desde a década dos anos 1990 do século XX e, em seguida, no cenário neoconservador, procuramos analisar o que mudou nesses últimos anos para a formação e o trabalho docente. Subsequentemente, abordamos, de maneira específica a análise da “nova” proposta de formação, indicando perspectivas futuras, considerando a nova conjuntura política brasileira, a partir do ano de 2023.

Abstract

The present review draws together wide-ranging studies performed over the last decades that catalogue the effects of artificial-light-at-night (ALAN) upon living species and their environment. We provide an overview of the tremendous variety of light-detection strategies which have evolved in living organisms - unicellular, plants and animals, covering chloroplasts (plants), and the plethora of ocular and extra-ocular organs (animals). We describe the visual pigments which permit photo-detection, paying attention to their spectral characteristics, which extend from the ultraviolet into infrared. We discuss how organisms use light information in a way crucial for their development, growth and survival: phototropism, phototaxis, photoperiodism, and synchronization of circadian clocks. These aspects are treated in depth, as their perturbation underlies much of the disruptive effects of ALAN. The review goes into detail on circadian networks in living organisms, since these fundamental features are of critical importance in regulating the interface between environment and body. Especially, hormonal synthesis and secretion are often under circadian and circannual control, hence perturbation of the clock will lead to hormonal imbalance. The review addresses how the ubiquitous introduction of light-emitting diode technology may exacerbate, or in some cases reduce, the generalized ever-increasing light pollution. Numerous examples are given of how widespread exposure to ALAN is perturbing many aspects of plant and animal behaviour and survival: foraging, orientation, migration, seasonal reproduction, colonization and more. We examine the potential problems at the level of individual species and populations and extend the debate to the consequences for ecosystems. We stress, through a few examples, the synergistic harmful effects resulting from the impacts of ALAN combined with other anthropogenic pressures, which often impact the neuroendocrine loops in vertebrates. The article concludes by debating how these anthropogenic changes could be mitigated by more reasonable use of available technology – for example by restricting illumination to more essential areas and hours, directing lighting to avoid wasteful radiation and selecting spectral emissions, to reduce impact on circadian clocks. We end by discussing how society should take into account the potentially major consequences that ALAN has on the natural world and the repercussions for ongoing human health and welfare.

Abstract

This conceptual theoretical text discusses, based on an ontological criticism, knowledge production, the processes of knowing, teaching and learning, and the ontological foundations that consolidate these processes. We affirm the pre-existence of an objective world, independent from our consciousness, which considers that it has a material basis that sustains it and allows - through this procedural and historical materiality – this consciousness to establish objective knowledge, of itself that is, an intelligibility of this world. The work is based on studies of the dialectical-materialist perspective of history, found in the work of Lukács, Rubinstein Sheptulin and Heller, and the work of education that we have conducted with the Group of Research and Study in Critical Ontology. We first approach some questions about knowledge and its appropriation from the above mentioned point of view. We then deepen the understanding of the concepts of daily school life and everyday life, creating a tension between their contents, their forms of transmission and appropriation, and their relationship with didactics. We conclude by presenting questions for further investigations.

Abstract

O artigo discute as possíveis inspirações do romance goetheano Os anos de aprendizado de Wilhelm Meister na estética literária lukácsiana. Evidenciamos a presença do romance goetheano como uma das referências que compõem a crítica da modernidade feita por Lukács em sua Teoria do Romance. Essa referência aponta para o problema da reconciliação indivíduo-sociedade, que o autor também perseguiu em sua guinada à perspectiva materialista dialética, juntamente com outros temas que lhe foram caros, tais como o debate sobre o realismo e a questão da dimensão estética na formação da individualidade humana.

Abstract

Nas últimas décadas, o conhecimento e sua produção se tornaram expressivos nos discursos da ciência e, em especial, no campo educacional. É nesse panorama que surge o significado da “sociedade do conhecimento”, cuja premissa principal é a “centralidade do conhecimento”. Assim, torna-se instigante perguntar que tipo de conhecimento se está priorizando? Para quem este conhecimento está pensado? Interessa-nos neste artigo aprofundar alguns aspectos relevantes em relação à formação humana a partir da perspectiva da ontologia crítica – defendida por Lukács – que procura compreender de forma mais apurada o papel da ciência na produção do conhecimento. Nessa direção, apoiamos uma formação humana em que o ser social é um sujeito ativo e não meramente passivo diante dos acontecimentos sociais com os quais se articula e se relaciona.
