Hypereosinophilic syndrome (HES) encompasses numerous diverse conditions resulting in peripheral hypereosinophilia that cannot be explained by hypersensitivity, infection, or atopy and that is not associated with known systemic diseases with specific organ involvement. HES is often attributed to neoplastic or reactive causes, such as chronic eosinophilic leukemia, although a majority of cases remains unexplained and are considered idiopathic. Here, we review the current diagnosis and management of HES and present a unique case of profound hypereosinophilia associated with warm autoimmune hemolytic anemia requiring intensive management. This case clearly illustrates the limitations of current knowledge with respect to hypereosinophilia syndrome as well as the challenges associated with its classification and management.

This study aims to provide a critical assessment on the trajectory of the extensionist activity of the Center for Extension in Territorial Development Center Serra (NEDET CS, Brazilian abbreviation), from 2014 to 2017. The NEDETs were created through the Public Call CNPq / MDA / SPM-PR No. 11/2014 in order to set up advisory teams to the Territorial Development Colleges (CODETERs, Brazilian abbreviation). At the Federal University of Santa Maria, the NEDET CS was set up to operate in the Serra do Rio Grande do Sul Center Territory. From 2014 to 2017, it was develop activities for social management and productive inclusion. The analysis of the actions point out that the NEDET has organized and promoted social participation in CODETER, carried out 115 activities and mobilized 2.6 million reais. It was concluded that the NEDET had a positive role in the articulation and development of the CS Territory besides being a privileged space for academic formation since it provides opportunity to develop research and extension actions.

This paper presents an improvement in the long-term hydrothermal system operation planning by including peak generation as a decision variable in the simulations, in order to better operate the hydro plants. Although this has been widely considered in the short-term, the objective is to correctly set the power system operation planning to avoid instantaneous shortages and blackouts. Thus, the instantaneous generation capacity is included in the objective function in order to better consider this variable in long-term operation planning. The results show that it is possible to significantly increase peak power by simply modifying the operation of hydro plants. Moreover, this approach does not present a significant increase in the operation costs.

This article presents an alternative approach to solve hydrothermal systems operation planning based on stochastic dynamic programming. Under the presented approach, the hydroelectric power plants are grouped into equivalent subsystems of energy and the expected cost functions are modeled by a piecewise linear approximation, by means of the Convex Hull algorithm. Also, under this methodology, the problem is solved independently for each subsystem such that the state variables to be considered are the energy storage and energy net interchange of the subsystem. The presented results have shown that this subsystems separation technique reduces significantly the computation time when compared with the traditional techniques of stochastic dynamic programming, demonstrating the effectiveness of the proposed methodology.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES From the recognition of the extreme ethnic and racial inequality in Brazil, the study aimed at discussing the integration of ethnic-racial “agenda” in the Rural Development Policy with a territorial approach. Using Sociology of Public Action as theoretical frame, the implementation of “Programa de Desenvolvimento Sustentavel de Territorios Rurais (PRONAT) in the “Centro Serra” territory and its impact to the “Comunidade Quilombola Linha Fão” (municipality of Arroio do Tigre) was analyzed for 2009/2017. Suggested analytical categories of the Public Policies Pentagon (actors, representations, institutions, processes and results) were used to guide the analysis of the policy implementation. The analyses were carried out based on a review of the scientific literature produced on the theme (for contextualization of ethnic-racial “agenda”, characterizes territorial development policy and the historical trajectory of “Centro Serra” social configuration), analysis of PRONAT normative documents, the mitnutes of CODETER meetings, participant observation and semi-structured interviews with “quilombola” community members. As research results, it was observed that the territorial development public policy, by itself, does not guarantee the productive inclusion of “Quilombola” communities and that, even with the resource being accessed, did not generate much expectation in the community regarding its execution. It is considered that due to brazilian territorial policy origins (with a family farm orientation) and given the high bureaucratic requirement for the submission of proposals, there is limited opportunity to Quilombola community agency in this process. The territory history, characterized by a continuous expropriation of lands of the most vulnerable groups, made them search and adopt livelihoods other than those recommended by the territorial policy. Partindo do reconhecimento da extrema desigualdade étnico-racial no Brasil, o estudo visou discutir a inserção da agenda étnico-racial na Política de Desenvolvimento Rural com enfoque territorial, implementada pelo então Ministério do Desenvolvimento Agrário. Usando como referencial teórico a Sociologia da Ação Pública, foi analisada a implementação do Programa de Desenvolvimento Sustentável de Territórios Rurais (PRONAT) no Território Centro Serra (RS) no período 2009/2017 e suas repercussões para a comunidade quilombola Linha Fão (município de Arroio do Tigre). As categorias analíticas sugeridas no Pentágono das Políticas Públicas (atores, das representações, das instituições, dos processos e dos resultados) foram utilizadas para orientar a análise da implementação. As análises foram realizadas com base na revisão da literatura científica produzida sobre o tema (para contextualização da agenda étnico-racial, da política de desenvolvimento territorial e da trajetória histórica da configuração social do território Centro Serra), análise de documentos normativos do PRONAT, analise das atas do Colegiado de Desenvolvimento Territorial (CODETER) Centro Serra, observação participante e entrevistas semi-estruturadas a membros da comunidade quilombola. Como resultados, observou-se que a política pública de desenvolvimento territorial, por si só, não garante a inclusão produtiva de comunidades quilombolas e que, mesmo com o recurso sendo acessado, não gerou muita expectativa na comunidade quanto a sua execução. Considera-se que devido à política territorial brasileira ter sua origem em uma política voltada para agricultura familiar e dada a alta exigência burocrática para a submissão de propostas, há limitada possibilidade de participação da comunidade quilombola neste processo. O histórico de ocupação do território analisado, caracterizado por uma contínua expropriação de terras dos grupos mais vulneráveis, fez com que eles adotassem modos de vida distintos daqueles recomendados pela política territorial.

Os NEDETs foram criados através da Chamada Pública CNPq/MDA/SPM-PR Nº 11/2014 com o objetivo de estruturar equipes de assessoramento aos Colegiados de Desenvolvimento Territorial (CODETERs). Na Universidade Federal de Santa Maria, foi constituído o NEDET CS para atuação no Território Centro Serra do Rio Grande do Sul. Entre 2014 e 2017, foram desenvolvidas atividades voltadas à gestão social e inclusão produtiva. A análise das ações realizadas aponta que o NEDET deu organicidade e promoveu a participação social no CODETER, realizou 115 atividades e mobilizou a execução de 2,6 milhões de reais. Conclui-se, assim, que o NEDET teve um papel positivo na articulação e no desenvolvimento do Território CS, além de ser um espaço privilegiado de formação acadêmica através das ações de pesquisa e extensão desenvolvidas.

HES is often attributed to neoplastic or reactive causes, such as chronic eosinophilic leukemia, although a majority of cases remains unexplained and are considered idiopathic. Here, we review the current diagnosis and management of HES and present a unique case of profound hypereosinophilia associated with warm autoimmune hemolytic anemia requiring intensive management. This case clearly illustrates the limitations of current knowledge with respect to hypereosinophilia syndrome as well as the challenges associated with its classification and management. PONTIFÍCIA UNIVERSIDADE CATÓLICA DO RIO DE JANEIRO COORDENAÇÃO DE APERFEIÇOAMENTO DO PESSOAL DE ENSINO SUPERIOR PROGRAMA DE EXCELENCIA ACADEMICA As condições hidrológicas desfavoráveis vivenciadas entre 2014 e 2019 levaram ao esgotamento dos principais sistemas de reservatórios no Brasil, causando um aumento na geração de energia proveniente de usinas térmicas. Todavia, uma parte relevante da geração térmica verificada foi comandada por entidades governamentais de forma heterodoxa (fora do mérito econômico calculado pelos modelos de otimização), baseada principalmente na percepção de risco tácita. Apesar do senso comum de que o armazenamento dos reservatórios está intrinsecamente ligado à segurança do sistema, as métricas utilizadas até o momento não conseguiram computar as reais necessidades do sistema em termos de energia armazenada mínima nas usinas hidrelétricas. Ao final de 2019, o ONS propôs um novo método para avaliar a necessidade de despacho térmico adicional, chamado Curva Referencial de Armazenamento (CREF). No entanto, este método considera hipóteses muito específicas de afluências e geração térmica, e com base em seu processo iterativo de tentativa e erro, pode resultar em resultados sub-ótimos para o cálculo dos armazenamentos mínimos necessários. Este trabalho propõe um novo método para avaliar a segurança do fornecimento de energia em sistemas predominantemente hidroelétricos. Este método é uma evolução do método CREF, e é baseado no desenvolvimento de um modelo de otimização que calcula os níveis mínimos de segurança para operação de usinas hidrelétricas em cada mês, a partir de uma simulação recursiva de séries históricas de afluências de 1931 a 2018. Além disso, com base nos resultados da simulação, foram sugeridas curvas de referência para o monitoramento contínuo da operação dos reservatórios, com o objetivo de subsidiar decisões de órgãos do Governo Brasileiro sobre o despacho heterodoxo de geração térmica. Espera-se que o monitoramento das curvas de referência propostas represente um critério mais robusto para decisões sobre geração térmica fora-do-mérito no Sistema Elétrico Brasileiro. Unfavorable hydrological conditions experienced from 2014 to 2019 led to the depletion of main reservoir systems in Brazil, causing an increase of thermal energy dispatch. However, an important share of the observed thermal generation was out of economic merit, commanded by government entities which risk perception relies mainly on experts tacit knowledge. Despite the common sense that storage in reservoirs is intrinsically linked to system security, the metrics employed so far failed to compute the system s real needs in terms of required stored energy in hydroelectric plants. By the end of 2019, ONS proposed a new method to assess the need for additional thermal dispatch the Referential Storage Curve (CREF - Curva Referencial de Armazenamento). However, it fails as a reference for the security of energy supply since it considers very specific assumptions of rivers inflows and thermal generation. Besides, based on its iterative trial and error process, it can result in sub-optimal results of minimum storage levels. This work proposes a new method to evaluate the security of power supply in systems with predominance of hydroelectricity. This method is intended to be an evolution to the CREF method, and it is based on the development of an optimization model that computes the minimum secure levels for hydroelectric plants operation in each month, from a recursive simulation of historical inflow series from 1931 to 2018. In addition, based on the simulation results, reference curves were suggested for the continuous monitoring of the reservoirs operation, with the purpose of subsidizing Brazilian government entities decisions on unorthodox thermal generation dispatch. The monitoring of the proposed reference curves is expected to represent a more robust criterion for decisions on out-of-merit thermal generation in Brazilian power system.

32. Lateral Meniscal Tears Can be Safely Repaired "All-Inside" Without Peroneal Nerve Injury: Case Series and Literature Review.

Introduction: The menisci are a pair of fibrocartilaginous structures important for the normal biomechanical function of the knees. Tears are common, and multiple approaches have been used to repair meniscal tears. Of the approaches, the all-inside approach has historically been avoided for tears to the posterior aspect of the lateral meniscus (LM) due to the risk of popliteal and common peroneal neurovascular injury., Case Report: We reviewed the electronic medical records of all patients who underwent all-inside LM repair by our senior author from August 2022 through August 2023. Demographic, injury, and surgical data were collected to characterize surgical success and any complications attributed to the surgical approach. A literature review of MEDLINE was performed to investigate the current data regarding the all-inside approach and identify methods for improving safety and efficacy. Two cases were identified (one male and one female) who underwent all inside repair of the LM. No surgical complications were attributed to suture selection or placement, and no damage to the popliteal neurovascular structures was identified., Conclusion: The all-inside approach is a safe arthroscopic approach for the repair of the LM. Introduction: The pectoralis major (PM) muscle is the largest and most superior muscle of the anterior chest wall. The PM plays an important role in flexion, adduction, and internal rotation of the arm. The pectoralis major's size, strength, and anatomical location make it an excellent candidate in transfer surgeries due to its ability to restore balancing forces that may be lost in scapular winging and subscapularis tears. Each of these injuries and pathologies involves the PM muscle in some way, and careful consideration of its anatomy and physiology is necessary. This review article aims to provide a comprehensive overview of the anatomy, physiology, and surgical considerations of the pectoralis muscle with a specific focus on the surgical techniques involving the muscle., Methods: A comprehensive literature search using a combination of the following terms: pectoralis major, rupture, transfer, chronic subscapularis tear, pectoralis surgery, anatomy, scapular winging, and long thoracic nerve anatomy. There were no limitations with regards to article type or publishing date, but article language was limited to only English., Conclusion: The pectoralis muscle is an important muscle when it comes to function of the upper extremity primarily or through transfer procedure. Injuries and impairments of the pectoralis major or other muscles of the upper extremity can significantly impact an individual's quality of life, limiting their ability to perform activities of daily living. 34. Novel Meniscus Transfer Technique: A Case Report.

- Abstract
The meniscus is an essential component of the knee joint, acting as a shock absorber as well as assisting in the transmission of forces. Due to the meniscus importance of the knee, much of the current literature focuses on treatment techniques that can spare and repair the meniscus when it is torn. The unique vasculature of the meniscus often makes repair difficult or, in many cases, impossible. A current focus within orthopedics has been on meniscal allograft transplantation to fill this gap. The lack of a universal surgical technique for graft fixation, along with the current failure rates, demonstrates the need for further improvements. The senior author proposes a novel technique for meniscal allograft transplantation that has shown decreased blood loss and surgical time, while also reducing intra-operative trauma to the knee. This case reports a 16-year-old patient who underwent a right lateral meniscal allograft transplant following a large segmental defect tear of the lateral meniscus. The patient initially underwent arthroscopy and meniscectomy with screw fixation of the lateral femoral condyle lesion. After physical therapy, the patient experienced increased pain and swelling, with magnetic resonance imaging (MRI) demonstrating a meniscal defect unamenable to repair. The patient met indications for meniscal allograft transplantation given the failed meniscectomy, absence of cartilage loss and significant osteoarthritis, and the patient's age of less than 50 years old., Competing Interests: Human subjects: Consent was obtained or waived by all participants in this study. 35. Tibial Eminence Fracture and Pediatric ACL Avulsion Diagnosis and Treatment: A Case Report.

- Abstract
Introduction: Tibial eminence fractures resulting from the avulsion of the anterior cruciate ligament (ACL) insertion are a common knee injury in the pediatric population. This injury is traditionally corrected through internal fixation using a guide pin and a cannulated screw. Due to the reported consequences of cannulated screws impinging on the joint space, we explored an alternative procedure to address this injury without the use of cannulated screws. We believe this is the first report of an alternate and improved procedure for repairing a tibial eminence fracture caused by ACL avulsion using non-absorbable sutures to secure the bony fragment back onto the tibial eminence., Case Report: We report the diagnosis, treatment, and follow-up of a 15-year-old Caucasian male who suffered a tibial eminence fracture associated with an ACL avulsion following a tackle football injury. We utilized a new all-arthroscopic procedure that involved securing the bony fragment of the tibial eminence using sutures that were passed through the ACL and proximal tibial using interosseous bone tunnels., Conclusion: This novel procedure successfully eliminates the need for intra-articular screws in the adolescent knee while treating tibial avulsion fractures - as supported by the patient's marked radiographical and symptomatic improvement at the 2, 6, and 12-week follow-up visits., Competing Interests: Conflict of Interest: Nil, (Copyright: © Indian Orthopaedic Research Group.)

36. Knee Extension Lag Treated with Posterior Capsulotomy of the Knee: A Case Report and Review of the Literature.

Introduction: The posterior capsule of the knee plays an important role in maintaining the integrity of the knee joint. Pathologies involving the posterior capsule have been implicated in developing flexion contractures. Posterior capsulotomy is a well-studied modality for this issue, performed through either an open or arthroscopic approach., Case Report: A 74-year-old Caucasian male presented with a complex extensor lag of his right knee and an inability to gain full extension either actively or passively. Through the use of standard knee arthroscopy, the authors were able to not only correct his posterior capsular pathology but also address anterior knee lesions. Only the combination of both allowed a full extension of the joint., Conclusion: Posterior capsulotomy of the knee may be carried out successfully and safely through either an open or arthroscopic approach. 37. Pain Relief and the Opioid Crisis in the United States and Canada.

- Abstract
Opioid analgesics hijack the body's innate wellness machinery (eg, naloxone blocks the placebo effect) and alleviate both physical and emotional pain. Starting in the 1980s, marketing and advocacy created an opioid-centric pain relief strategy based on the idea that physicians undermanage pain and worry too much about addiction. The increase of prescription opioids in the ecosystem (along with a resurgence in heroin use) contributed to dependence, misuse, overdoses, and overdose deaths. Laws punishing undermanagement of pain from the opioid crisis combined with more recent laws punishing overprescription of opioids add to the difficulties orthopaedic surgeons have in managing the pain of surgery and acute injury. The substantial variation in pain intensity for nociception (actual or potential tissue damage) and the persistent use of opioids after healing is well established are both accounted for largely by psychosocial factors (stress, distress, and less effective coping strategies). When a patient has more pain than expected, surgeons should first rule out compartment syndrome and infection and then focus on a comprehensive team- and strategy-based approach that addresses these psychosocial factors.
