Le présent rapport national passe en revue les résultats de l’enquête SPOTnat (Coordination et qualité de l’aide et des soins à domicile – une étude national) menée de janvier à septembre 2021 dans 88 organisations d’aide et de soins à domicile (ASaD) en Suisse. Elle a été menée auprès de 3 223 collaborateurs de l'aide et des soins à domicile, 1 509 clients et 1 165 proches. L'enquête s'est concentrée sur la coordination et la qualité des soins, la qualité de l’environnement de travail et la situation du personnel, par exemple la satisfaction au travail ou l’intention de démissionner. English: In in this national report, we present the descriptive results of the SPOTnat survey (home care coordination and quality - a national study), which was conducted from January 2021 to September 2021 in 88 home care organizations in Switzerland. 3.223 home care employees, 1.509 clients and 1.165 relatives were surveyed. The focus of this survey was on coordination and quality of care, the quality of the work environment and personnel outcomes, e.g., job satisfaction or the intention to quit., This report is available in German, French and Italian.

Registration is an essential prerequisite for many applications when a multiple-camera setup is used. Due to the noise in depth images, registration procedures for depth sensors frequently rely on the detection of a target object in color or infrared images. However, this prohibits use cases where color and infrared images are not available or where there is no mapping between the pixels of different image types, e.g., due to separate sensors or different projections. We present our novel registration method that requires only the point cloud resulting from the depth image of each camera. For feature detection, we propose a combination of a custom-designed 3D registration target and an algorithm that is able to reliably detect that target and its features in noisy point clouds. Our evaluation indicates that our lattice detection is very robust (with a precision of more than 0.99) and very fast (on average about 20 ms with a single core). We have also compared our registration method with known methods: Our registration method achieves an accuracy of 1.6 mm at a distance of 2 m using only the noisy depth image, while the most accurate registration method achieves an accuracy of 0.7 mm requiring both the infrared and depth image.

Abstract. We estimated the incidence of COVID-19-compatible symptoms, COVID-19 testing, and GP consultations during the first lockdown period in Switzerland (March–April 2020). A representative sample of the adult Swiss population completed an online survey. Of the 1022 respondents, 74 % reported at least one symptom, 5 % were tested for COVID-19, and 8 % contacted their GP regarding COVID-19. Testing was significantly associated with reduced sense of taste/smell, fever, cough with expectoration, and fatigue. Respondents at risk for severe COVID-19 were less likely to be tested but were five times more likely to contact their GP than those not at risk. We highlight the need to involve the appropriate health care setting to reach the vulnerable population when testing capacities are restricted (e.g. GPs).

Purpose Anterior pituitary iron overload and volume shrinkage is common in patients with transfusion-dependent anemia and associated with growth retardation and hypogonadotropic hypogonadism. We investigated the accuracy of different MRI-based pituitary volumetric approaches and the relationship between pituitary volume and MRI-R2, particularly with respect to growth and hypogonadism. Methods In 43 patients with transfusion-dependent anemia (12–38 years) and 32 healthy controls (12–72 years), anterior pituitary volume was measured by a sagittal T1 GRE 3D sequence at 1.5T and analyzed by 3D semi-automated threshold volumetry (3D-volumetry). This reference method was compared with planimetric 2D-volumetry, approximate volume calculations, and pituitary height. Using a multiple SE sequence, pituitary iron as MRI-R2 was assessed by fitting proton signal intensities to echo times. Growth and hypogonadism were obtained from height percentile tables and patients’ medical charts. From body surface area and age adjusted anterior pituitary volumes of controls, Z‑scores were calculated for all subjects. Separation of controls and patients with respect to Z and pituitary R2 was performed by bivariate linear discriminant analysis. Results Tuned 2D volumes showed highest agreement with reference 3D-volumes (bias −4.8%; 95% CI:−8.8%|−0.7%). A linear discriminant equation of Z = −17.8 + 1.45 · R2 revealed optimum threshold sensitivity and specificity of 65% and 100% for discrimination of patients from controls, respectively. Of correctly classified patients 71% and 75% showed hypogonadism and growth retardation, respectively. Conclusion Accurate assessment of anterior pituitary size requires 3D or precise 2D volumetry, with shorter analysis time for the latter. Anterior pituitary volume Z‑scores and R2 allow for the identification of patients at risk of pituitary dysfunction.

We have synthesized the first isolable geminal bisenolates L2K2Ge[(CO)R]2 (R=2,4,6‐trimethylphenyl (2 a,b), L=THF for (2 a) or [18]‐crown‐6 for (2 b)), a new synthon for the synthesis of organometallic reagents. The formation of these derivatives was confirmed by NMR spectroscopy and X‐ray crystallographic analysis. The UV/Vis spectra of these anions show three distinct bands, which were assigned by DFT calculations. The efficiency of 2 a,b to serve as new building block in macromolecular chemistry is demonstrated by the reactions with two different types of electrophiles (acid chlorides and alkyl halides). In all cases the salt metathesis reaction gave rise to novel Ge‐based photoinitiators in good yields., A convenient method to obtain geminal bisenolates L2K2Ge[(CO)R]2 (R=2,4,6‐trimethylphenyl (1 a,b), L=THF for (1 a) or [18]‐crown‐6 for (1 b)), a new synthon for the synthesis of organometallic reagents, is reported. Selected examples of electrophiles were used to display the efficiency of 1 a,b as new building blocks in organometallic chemistry.

To investigate the influence of myocardial fibrosis on left ventricular (LV) diastolic filling patterns in triathletes compared to sedentary controls by cardiac magnetic resonance (CMR) imaging.101 male triathletes (43 ± 11 years) and 28 controls (41 ± 10 years) were recruited and underwent 1.5 T CMR including cine SSFP series, late gadolinium enhancement (LGE) imaging and T1 mapping. Functional and morphological parameters were obtained, and CMR-based LV diastolic filling parameters such as the early peak-filling rate (EPFR), atrial peak-filling rate (APFR) and peak-filling rate ratio (PFRR = EPFR/APFR) were determined by time-volume analysis of the cine series.Non-ischemic LGE was detected in 20 triathletes (20 %) and in none of the controls. Compared to controls LGE-negative (LGE-) triathletes showed similar EPFR (216 ± 58 ml/s/mAthletic activity leads to "supernormal" LV diastolic filling pattern in LGE- triathletes, which may be attributable to increased LV myocardial flexibility and elasticity. However, LGE + triathletes demonstrate a pseudo-normalization characterized by compensatory increase of atrial contraction. Possibly, due to reduced passive elasticity associated myocardial fibrosis.

Acylgermanes are known as highly efficient photoinitiators. In this contribution, we present the synthesis of new diacylgermanes 4a–evia a multiple silyl abstraction methodology. The method outperforms the state-of-the-art approach (Corey–Seebach reaction) towards diacylgermanes in terms of group tolerance and toxicity of reagents. Moreover, these compounds are decorated with bulky mesityl groups in order to improve their storage stability. The isolated diacylgermanes were characterized by multinuclear NMR-, UV-Vis spectroscopy and X-ray crystallography, as well as photolysis experiments (photobleaching) and photo-DSC measurements (photopolymerization behavior). Upon irradiation with an LED emitting at 385 nm, all compounds except for 4a and 4c bleach efficiently with quantum yields above 0.6. Due to their broad absorption bands, the compounds can be also bleached with blue light (470 nm), where especially 4e bleaches more efficiently than Ivocerin®., A series of novel diacylgermanes with bulky mesityl groups by the multiple silyl abstraction methodology, avoiding the Corey–Seebach reaction were synthesized.

Objectives To quantify the proportion of fat within the skeletal muscle as a measure of muscle quality using dual-energy CT (DECT) and to validate this methodology with MRI. Methods Twenty-one patients with abdominal contrast-enhanced DECT scans (100 kV/Sn 150 kV) underwent abdominal 3-T MRI. The fat fraction (DECT-FF), determined by material decomposition, and HU values on virtual non-contrast-enhanced (VNC) DECT images were measured in 126 regions of interest (≥ 6 cm2) within the posterior paraspinal muscle. For validation, the MR-based fat fraction (MR-FF) was assessed by chemical shift relaxometry. Patients were categorized into groups of high or low skeletal muscle mean radiation attenuation (SMRA) and classified as either sarcopenic or non-sarcopenic, according to the skeletal muscle index (SMI) and cut-off values from non-contrast-enhanced single-energy CT. Spearman’s and intraclass correlation, Bland-Altman analysis, and mixed linear models were employed. Results The correlation was excellent between DECT-FF and MR-FF (r = 0.91), DECT VNC HU and MR-FF (r = - 0.90), and DECT-FF and DECT VNC HU (r = − 0.98). Intraclass correlation between DECT-FF and MR-FF was good (r = 0.83 [95% CI 0.71–0.90]), with a mean difference of - 0.15% (SD 3.32 [95% CI 6.35 to − 6.66]). Categorization using the SMRA yielded an eightfold difference in DECT VNC HU values between both groups (5 HU [95% CI 23–11], 42 HU [95% CI 33–56], p = 0.05). No significant relationship between DECT-FF and SMI-based classifications was observed. Conclusions Fat quantification within the skeletal muscle using DECT is both feasible and reliable. DECT muscle analysis offers a new approach to determine muscle quality, which is important for the diagnosis and therapeutic monitoring of sarcopenia, as a comorbidity associated with poor clinical outcome. Key Points • Dual-energy CT (DECT) material decomposition and virtual non-contrast-enhanced DECT HU values assess muscle fat reliably. • Virtual non-contrast-enhanced dual-energy CT HU values allow to differentiate between high and low native skeletal muscle mean radiation attenuation in contrast-enhanced DECT scans. • Measuring muscle fat by dual-energy computed tomography is a new approach for the determination of muscle quality, an important parameter for the diagnostic confirmation of sarcopenia as a comorbidity associated with poor clinical outcome.

A series of heteroleptic copper(II) complexes of the composition [Cu(L1–5)Cl]X, where X = ClO4 and/or PF6 and [bis(2-ethyl-di(3,5-dimethyl-1H-pyrazol-1-yl))-(6-methyl-(2-pyridylmethyl))]amine (L1), [bis(2-ethyl-di(3,5-dimethyl-1H-pyrazol-1-yl))-(3,4-dimethoxy-(2-pyridylmethyl))]amine (L2), [bis(2-ethyl-di(3,5-dimethyl-1H-pyrazol-1-yl)-(2-quinolymethyl)]amine (L3), [bis(2-ethyl-di(3,5-dimethyl-1H-pyrazolyl)-(di(3,5-dimethyl-1H-pyrazol-1-yl-methyl))]amine (L4) and [bis(2-ethyl-di(3,5-dimethyl-1H-pyrazol-1-yl)-(5-methyl-3-phenyl-1H-pyrazol-1-yl-methyl)]amine (L5), were prepared and thoroughly characterized including single-crystal X-ray diffraction technique. The in vitro cytotoxicity of complexes against A2780, A2780R, HOS and MCF-7 human cancer cell lines was evaluated using the MTT test. The results revealed that complexes [Cu(L1)Cl]PF6 (1-PF6), [Cu(L2)Cl]ClO4 (2-ClO4) and [Cu(L3)Cl]PF6 (3-PF6) are the most effective, with IC50 values ranging from 1.4 to 6.3 μM, thus exceeding the cytotoxic potential of metallodrug cisplatin (IC50 values ranging from 29.9 to 82.0 μM). The complexes [Cu(L4)Cl]PF6 (4-PF6) and [Cu(L5)Cl]PF6 (5-PF6) showed only moderate cytotoxicity against A2780, with IC50 = 53.6 μM, and 33.8 μM, respectively. The cell cycle profile, time-resolved cellular uptake, interactions with small sulfur-containing biomolecules (cysteine and glutathione), intracellular ROS production, induction of apoptosis and activation of caspases 3/7 were also evaluated in the case of the selected complexes. It has been found that the best performing complexes 1 and 2 cause cell arrest in the G2/M phase and induce apoptosis via the increase in production of ROS, dominantly due to the overproduction of superoxide.

Five new eight-coordinated Nd(III) mononuclear complexes with the formulae [Nd(ntfa)3(EtOH)2] (1), [Nd(ntfa)3(phen)] (2), [Nd(ntfa)3(bipy)] (3), [Nd(ntfa)3(5,5′-Me2bipy)] (4) and [Nd(ntfa)3(4,4′-Mt2bipy)] (5), where ntfa = 4,4,4-trifluoro-1-(naphthalen-2-yl)butane-1,3-dionate, phen = phenanthroline, bipy = 2,2′-dipyridine, 5,5′-Me2bipy = 5,5′-dimethyl-2,2′-dipyridine, and 4,4′-Mt2bipy = 4,4′-dimethoxy-2,2′-dipyridine, were synthesized and structurally characterized. Magnetic measurements were carried out on complexes 2–5. Dynamic magnetic studies revealed single-molecule magnet (SMM) behavior for complexes 2, 4 and 5 with anisotropy energy barriers and preexponential factors of Ueff = 18 cm−1, τ0 = 2.2 × 10−7 s; Ueff = 31 cm−1, τ0 = 1.0 × 10−9 s and Ueff = 19 cm−1, τ0 = 8.7 × 10−8 s, respectively. The ancillary chelating bipyridyl ligands in complexes 2–5 greatly enhance the solid state luminescence emission in the visible and NIR regions through efficient energy transfer from the ligands to the central Nd3+ ion. This study highlights the bifunctionality of the Nd(III) complexes.

Tetraacylgermanes are known as highly efficient photoinitiators. Herein, the synthesis of mixed tetraacylgermanes 4 a–c and 6 a–e with a nonsymmetric substitution pattern is presented. Germenolates are crucial intermediates of these new synthetic protocols. The synthesized compounds show increased solubility compared with symmetrically substituted tetraacylgermanes 1 a–d. Moreover, these mixed derivatives reveal broadened n–π* absorption bands, which enhance their photoactivity. Higher absorption of these new compounds at wavelengths above 450 nm causes efficient photobleaching when using an LED emitting at 470 nm. The quantum yields are in the range of 0.15–0.57, depending on the nature of the aroyl substituents. On the basis of these properties, mixed‐functionalized tetraacylgermanes serve as ideal photoinitiators in various applications, especially in those requiring high penetration depth. The synthesized compounds were characterized by elemental analysis, IR spectroscopy, NMR and CIDNP spectroscopy, UV/Vis spectroscopy, photolysis experiments, and X‐ray crystallography. The CIDNP data suggest that the germyl radicals generated from the new tetraacylgermanes preferentially add to the tail of the monomer butyl acrylate. In the case of 6 a–e only the mesitoyl groups are cleaved off, whereas for 4 a–c both the mesitoyl and the aroyl group are subject to α‐cleavage., Just mix it! Mixed‐functionalized tetraacylgermanes were synthesized by two innovative pathways. These new derivatives outperform the state‐of‐the‐art germanium based photoinitiators in terms of both solubility and absorption properties.

Abstract Six mixed metal complexes with 3-aminopyridine (3-ampy) as a co-ligand have been synthesized: catena-{[M(μ2-3-ampy)(H2O)4]SO4·H2O} (M=Ni (1) and Co (2)), [Co(3-ampy)4(NCS)2] (3), [Co(3-ampy)2(NCS)2] (4), [Co(3-ampy)4(N3)2] (5) and mer-[Co(3-ampy)3(N3)3] (6), (NCS−=isothiocyanate ion, N3− azide ion), and characterized by physio-chemical and spectroscopic methods as well as single crystal X-ray and powder diffraction. In the isostructural complexes 1 and 2 single μ2-3-ampy links the Ni(II) and Co(II) centers into polymeric chains. The mononuclear Co(II) and Co(III) pseudohalide complexes 3–6 reveal only terminal 3-ampy ligands. The 3-ampy ligands form supramolecular hydrogen bonded systems via their NH2-groups and non-covalent π-π ring-ring interactions via their pyridine moieties. Attention-deficit hyperactivity disorder (ADHD) is associated with substantial personal and social impairments. Besides psychosocial interventions, current guidelines recommend a therapy with methylphenidate (MPH). This prospective, non-interventional study aims to investigate the efficacy and tolerability of MPH treatment of adult ADHD patients in a real-world setting. 468 adult patients with newly diagnosed ADHD were observed for 12–14 weeks. Primary efficacy endpoint was the clinical global impression (CGI) by the physician. Secondary endpoints comprise patient evaluation (Wender–Reimherr self-report, WR-SR), safety, tolerability, and dosage of MPH. With a mean daily dose of 35.8 (±17.0) mg MPH, the population of patients being severely/most extremely ill or markedly ill decreased by 64% and 61%, respectively. According to physicians’ assessment (CGI), 74.5% of patients were identified as treatment responders. The total score of patient-based assessment (WR-SR) improved by 23.5% (50.1 ± 40.3 points) with the most profound improvement in attention deficit (−30.0%), disorganization (−26.6%), and hyperactivity / unrest (−23.3%). Self-evaluation revealed a responder rate of 35.4%. In summary, MPH treatment improves the degree of ADHD severity under routine conditions. In addition, activities of daily living were facilitated when taking MPH. The rather poor responder rates determined by patient assessment as well as the comparatively low applied mean daily dose of 35.8 mg (median 40 mg) indicate sub-optimal dosing under routine conditions, not exploiting the full beneficial therapeutic potential of MPH.

Advances in computer technology and increasing usage of computer graphics in a broad field of applications lead to rapidly rising demands regarding size and detail of virtual landscapes. Manually creating huge, realistic looking terrains and populating them densely with assets is an expensive and laborious task. In consequence, (semi-)automatic procedural terrain generation is a popular method to reduce the amount of manual work. However, such methods are usually highly specialized for certain terrain types and especially the procedural generation of landscapes composed of different biomes is a scarcely explored topic. We present a novel system, called AutoBiomes, which is capable of efficiently creating vast terrains with plausible biome distributions and therefore different spatial characteristics. The main idea is to combine several synthetic procedural terrain generation techniques with digital elevation models (DEMs) and a simplified climate simulation. Moreover, we include an easy-to-use asset placement component which creates complex multi-object distributions. Our system relies on a pipeline approach with a major focus on usability. Our results show that our system allows the fast creation of realistic looking terrains.

In this contribution a convenient synthetic method to obtain the previously unknown dianionic cyclic silenolates and germenolates is described. These dianions 2a,b and 4a,b are easily accessible via a one-pot synthetic protocol in high yields. Their structural properties were analyzed by a combination of NMR, single-crystal X-ray crystallography, and DFT quantum mechanical calculations. Moreover, the reactivity of 2a,b and 4a,b with selected examples of electrophiles was investigated. 2a and 4a were reacted with ClSiiPr3 to give new examples of polysilanes and polygermanes with exocyclic double bonds. The reaction of 2b with ClSiMe2SiMe2Cl led to the formation of the acyl bicyclo[2.2.2]octasilane 6. Moreover, the reaction of 2a,b and 4a,b with MeI, as an example of a carbon-centered electrophile, led to selective alkylation reactions at the negatively charged silicon and germanium atoms. The corresponding methylated structures 9a,b and 10a,b were formed in nearly quantitative yields. The competitive reactivity of the silyl and silenolate anion toward 1 equiv of ClSiMe3 showed that the outcome of the reaction was strongly influenced by the substituent at the carbonyl moiety. 2a reacted with 1 equiv of ClSiMe3 to give the corresponding cyclic silenolate S1a, which demonstrated that the silyl anion is more nucleophilic than the silenolate with attached aromatic groups. 2b, on the other hand, reacted with 1 equiv of ClSiMe3 to give the bicyclic compound 11via an intramolecular sila-Peterson alkenation reaction. These findings clearly showed that the alkyl-substituted silenolate is more nucleophilic than the silyl anion. This paper demonstrates that 2a,b and 4a,b have the potential to be used as unique building blocks for complex polysilane and polygermane frameworks.

The reaction of diphenyltin dihydride with LiAlH4 gives access to a set of charged tin cages as their lithium salts. Variation in the ratio of reactants provides a perstannabicyclooctane dianion and a perstannanorbornane as the di- and monoanions. These compounds can be synthesised selectively by careful stoichiometric control and have been characterised by single crystal X-ray diffractometry, NMR and UV-vis spectroscopy. Computational exploration of the electronic structures of these compounds was undertaken and, in agreement with structural and spectroscopic features, indicated significant σ-delocalisation in the tin skeletons.

Two penta-coordinate complexes of the general formula [Co(Ln)(NCS)]ClO4, where L1 = {bis[(3,5-dimethyl-1H-pyrazol-1-yl)ethyl]-[(3,4-dimethoxypyridin-2-yl)methyl]}amine and L2 = {bis[(3,5-dimethyl-1H-pyrazol-1-yl)ethyl]-[(4-methoxy-3,5-dimethylpyridin-2-yl)methyl]}amine, have been synthesized and thoroughly characterized. Each of the cobalt(ii) atoms is penta-coordinated in the {CoN5} donor set with a distorted square-pyramidal geometry in [Co(L1)(NCS)]ClO4·MeOH (1), while the vicinity of the central atom can be described as a distorted trigonal-bipyramidal geometry in [Co(L2)(NCS)]ClO4 (2) as revealed using the SHAPE analysis. Differences in interatomic parameters among the cobalt(ii) and donor atoms in 1 and 2 have definite impact on the magnetic features of both compounds. The complexes show an easy-axis magnetic anisotropy (D = -38.5 cm-1 for 1 and D = -8.5 for 2), and both complexes reveal a large rhombicity with E/D = 0.21 for 1 and E/D = 0.29 for 2. The ZFS parameters (g, D and E) were also calculated using CASSCF/NEVPT2 methods and they are in good agreement with those determined from experimental data. A frequency dependent out-of-phase susceptibility has been observed in external magnetic field (Bdc = 0.1 T) revealing the following parameters of slow relaxation of magnetization for 1: energy of the spin reversal barrier, Ueff = 16.0 cm-1 (Ueff/kB = 23.0 K) and the relaxation time, τ0 = 1.28 × 10-6 s. In the case of complex 2, no maxima of frequency dependent out-of-phase susceptibility have been observed and thus, the value of Ueff = 17 cm-1 has been estimated using the expression Ueff = |D| × (S2 - 1/4). It has been demonstrated that the degree of substitution and the type of substituents on the pyridyl moieties of the tripodal ligands (L1 and L2) used in these penta-coordinate cobalt(ii) complexes have significant impact on structural and magnetic features.

Metal xanthates are versatile single source precursors for the preparation of various metal sulfides. In this study, we present the synthesis of the two novel zinc xanthate complexes bis(O-2,2-dimethylpentan-3-yl-dithiocarbonato)(N,N,N′,N′-tetramethylethylenediamine)zinc(II) and bis(O-2,2-dimethylpentan-3-yl-dithiocarbonato)(pyridine)zinc(II). A thorough investigation of these compounds revealed distinct differences in their structural and thermal properties. While in the complex containing the chelating tetramethylethylenediamine, the xanthate groups coordinate in a monodentate way, they are bidentally coordinated to the zinc atom in the pyridine containing complex. Both compounds show a two-step thermal decomposition with an onset temperature of 151 °C and 156 °C for the tetramethylethylenediamine and pyridine containing complex, respectively. Moreover, different mechanisms are revealed for the two phases of the decomposition based on high resolution mass spectrometry investigations. By the thermal conversion process nanocrystalline zinc sulfide is produced and the coligand significantly influences its primary crystallite size, which is 4.4 nm using the tetramethylethylenediamine and 11.4 nm using the pyridine containing complex for samples prepared at a temperature of 400 °C.
