Abstract

Central nervous system infections are among the most severe infectious conditions in the neonatal period and are still burdened by significant mortality, especially in preterm infants and those with a low birth weight or other comorbidities. In this study, we examined the role of fosfomycin-containing antibiotic regimens in neonates with central nervous system infections. We included six neonates over a period of five years: four with meningitis and two with cerebral abscesses. All patients underwent fosfomycin therapy after failing first-line antibiotic regimens. Of the six neonates, two died; two developed neurological and psychomotor deficits and two recovered uneventfully. None of the neonates experienced adverse reactions to fosfomycin, confirming the safety of the molecule in this population. In conclusion, the deep penetration in the central nervous system, the unique mechanism of action, the synergy with other antibiotic therapies, and the excellent safety profile all make fosfomycin an attractive drug for the treatment of neonatal central nervous system infections. Abstract

Bronchiolitis is a common cause of hospitalization in infants. The long-lasting impact of hygiene and social behavior changes during the pandemic on this disease is debated. We investigated the prevalence of hospitalized cases, clinical severity, and underlying risk factors before and during pandemic. The study was conducted in 27 hospitals in Italy and included infants hospitalized for bronchiolitis during the following four periods: July 2018-March 2019, July 2020-March 2021, July 2021-March 2022, and July 2022-March 2023. Data on demographics, neonatal gestational age, breastfeeding history, underlying chronic diseases, presence of older siblings, etiologic agents, clinical course and outcome were collected. A total of 5330 patients were included in the study. Compared to 2018–19 (n = 1618), the number of hospitalizations decreased in 2020–21 (n = 121). A gradual increase was observed in 2021–22 (n = 1577) and 2022–23 (n = 2014). A higher disease severity (need and length of O2-supplementation, need for non-invasive ventilation, hospital stay) occurred in the 2021–22 and, especially, the 2022–23 periods compared to 2018–19. This tendency persisted after adjusting for risk factors associated with bronchiolitis severity. Conclusions: Compared to adults, COVID-19 in infants is often asymptomatic or mildly symptomatic and rarely results in hospitalization. This study indicates that the pandemic has indirectly induced an increased burden of bronchiolitis among hospitalized infants. This shift, which is not explained by the recognized risk factors, suggests the existence of higher infant vulnerability during the last two seasons. 18. Association between body composition at term equivalent age and Bayley scores at 2 years in preterm infants

Abstract

Objective: To evaluate whether in a historical cohort of preterm infants, body composition at term equivalent age (TEA) correlated with Bayley scores at 2 years of corrected age. Study design: Ninety-five preterm babies were admitted to our neonatal intensive unit and underwent air-displacement plethysmography assessment at TEA. Of these, 74 completed Bayley tests at 2 years. We used multiple linear regression analysis to assess the association of body composition with Bayley scores. Results: Mean gestational age and birthweight of our population were respectively 29.8 (±2.2) weeks and 1150 (±330) grams. Higher fat-free mass (FFM) z-score was associated with higher language (adjusted r= 0.28, p= 0.03) and motor composite scores (adjusted r= 0.33, p= 0.03) in both univariate and multiple regression analysis including birth weight, sex, maternal university degree, mechanical ventilation, and bilingualism. Conclusions: In our study FFM at term equivalent age was associated with higher Bayley composite motor and language scores at 2 years.

Abstract

Objective: To evaluate the pattern of cortical activation during a painful procedure, such as a venipuncture, in children with intellectual disability and compare it with that of cognitively healthy children.Study Design and Setting: A cohort study was conducted and cortical activation was assessed by multichannel cerebral near-infrared spectroscopy to monitor variations in oxyhaemoglobin and deoxyhaemoglobin (Hbb) in children with and without intellectual disability during a venipuncture for blood sampling with topical anaesthesia. Pain and distress were assessed as well using different validated pain scales (visual analogue scale and Non-Communicating Children's Pain Checklist-Postoperative Version for children with intellectual disability), and compared between groups.Participants: 16 children with severe to profound intellectual disability and 20 cognitively healthy peers (age range: 4-17 years).Results: When Hbb was analysed, children with intellectual disability exhibited a bilateral activation of the somatosensory (p<0.006) and right motor cortex (p=0.0045), whereas cognitively healthy peers never showed a cortical activation. Children with intellectual disability also showed more pain than controls (p=0.001).Conclusions: When subjected to a painful procedure, only children with intellectual disability show an activation of the cerebral cortex, even if topical anaesthesia is applied, and express more pain than cognitively healthy peers. 48. Training for ultrasound-guided implantation of central venous catheters eliminates venolysis in pediatric patients

Abstract

Background: In the pediatric setting, the procurement of vascular access can be particularly difficult. Surgical venolysis was the first technique described but, in recent years, the literature has shown that ultrasound-guided implantation has fewer complications. The principal aim of this paper is to state how after a structured training to place ultrasound-guided central lines, venolysis was definitively abandoned in our hospital. We also analyzed the impact of training on the duration of procedures. Finally, the number of procedures performed per operator to maintain the quality standard achieved.Method: In this observational retrospective study, we analyzed data from 2013 to 2020 of 1497 pediatric and neonatal central venous access placements in a single center during a training path.Results: Venolysis performed by the pediatric surgeons was used in 11% of total placements, the most in the smallest patients (76.1%, <1 year of age). With the implementation of training, this invasive technique became obsolete to the point where it was no longer practiced (24.6% of vascular placement in 2013 to 0% in 2020). In the placement performed by the pediatric anesthesiologists, time progressively decreased from 51.5 min in 2013 to 29.4 min in 2022. From the analysis of the number of procedures performed by pediatric anesthesiologists, the first consultant involved performed averaging 48 procedures per year, while the other consultants had an overall average of 16.7–25 placements per year.Conclusion: Echo-guided vascular cannulation was found to be safe, reliable, and reproducible in pediatric patients and neonates and permits to deletion definitively venolysis. Well-structured training is essential and should be standardized in the future.
