Patients were divided into two groups: âwith RV dysfunctionâ (⥠1 classic parameter) and âwithout RV dysfunctionâ. We used speckle tracking to estimate myocardial deformation. For all analyses, results were considered significant if p < 0.05.Results: The mean age across all participants was 65 ± 9 years, with 53% (48/91) being male. Patients in the group with RV dysfunction were able to walk shorter distances and had higher estimated right ventricular systolic pressure (RVSP) and mean pulmonary arterial pressure (mPAP). The RV free wall longitudinal strain (RVFWLS) was the only deformation indices that showed a significant correlation with all classic measurements and indices in the diagnosis of RV dysfunction (Wald test, 10.24; p < 0.01; odds ratio, 1.61). Aims:\ud The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.\ud \ud Methods and Results:\ud Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTIC‐HF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressure

Aims The SHIFT trial showed that ivabradine reduced heart rate (HR) and the risk of cardiovascular outcomes. Concerns remain over the efficacy and safety of ivabradine on heart failure (HF) due to Chagas disease (ChD). We therefore conducted a post hoc analysis of the SHIFT trial to investigate the effect of ivabradine in these patients. Methods and results SHIFT was a randomized, double‐blind, placebo‐controlled trial in symptomatic systolic stable HF, HR ≥ 70 b.p.m., and in sinus rhythm. The ChD HF subgroup included 38 patients, 20 on ivabradine, and 18 on placebo. The ChD HF subgroup showed high prevalence of bundle branch right block and, compared with the overall SHIFT population, lower systolic blood pressure; higher use of diuretics, cardiac glycosides, and antialdosterone agents; and lower use of angiotensin‐converting enzyme inhibitor/angiotensin II receptor blocker or target daily dose of beta‐blocker. ChD HF presented a poor prognosis (all‐cause mortality at 2 years was ~60%). The mean twice‐daily dose of ivabradine was 6.26 ± 1.15 mg and placebo 6.43 ± 1.55 mg. Ivabradine reduced HR from 77.9 ± 3.8 to 62.3 ± 10.1 b.p.m. (P = 0.005) and improved functional class (P = 0.02). A trend towards reduction in all‐cause death was observed in ivabradine arm vs. placebo (P = 0.07). Ivabradine was not associated with serious bradycardia, atrioventricular block, hypotension, or syncope. Conclusions ChD HF is an advanced form of HF with poor prognosis. Ivabradine was effective in reducing HR in these patients and improving functional class. Although our results are based on a very limited sample and should be interpreted with caution, they suggest that ivabradine may have a favourable benefit–risk profile in ChD HF patients.

Background: Dobutamine stress echocardiography (DSE) is an important tool in the diagnosis of coronary artery disease. However, there is hesitation in clinical practice for using it in patients with Chagas disease (CD) due to the arrhythmogenic potential of this heart condition. This study aimed to evaluate the incidence and variables associated with arrhythmias during DSE in a population of patients with CD. Methods: A population of 205 consecutive patients with CD and suspected coronary heart disease was assessed through a retrospective database analysis. CD was confirmed in all patients by serological testing. Results: The mean age of the patients selected was 64 years, and 65.4% of the patients were female. Significant arrhythmias occurred as follows: nonsustained ventricular tachycardia in 7.3% of patients; supraventricular tachycardia and sustained ventricular tachycardia in 1%; and atrial fibrillation in 0.5%. Nonsignificant arrhythmias occurred as follows: premature ventricular contractions in 48% of patients and bigeminy in 4.4%. Values for the wall‐motion score index at rest greater than 1.12 and 1.18 were independently correlated with the occurrence of nonsignificant arrhythmias (odds ratio [OR] = 2.90, P < 0.001) and significant arrhythmias (OR = 4.23, P = 0.044), respectively. Conclusion: DSE should be considered a safe examination in patients with CD despite the known increased risk of arrhythmias in this group of patients. The occurrence of arrhythmias was low in this study. Abnormal wall‐motion score index values at rest were associated with the occurrence of significant and nonsignificant arrhythmias during the test. INTRODUÇÃO: A fragilidade é uma síndrome caracterizada por fraqueza, fadiga e aumento da vulnerabilidade a estressores físicos. Avaliar e entender fragilidade em pacientes com insuficiência cardíaca (IC) é essencial, pois traz novas pespectivas ao manejo e ao tratamento do paciente assistido. Por conseguir diferenciar fragilidade de incapacidade e de comorbidade, o fenótipo de fragilidade é o instrumento de medida mais aceito. Estudos de fragilidade em pacientes com insuficiência cardíaca de etiologia chagásica são inexistentes. OBJETIVOS: Avaliar fragilidade e sua prevalência em pacientes com IC com fração de ejeção reduzida de etiologia chagásica que frequentam o ambulatório de insuficiência cardíaca de um hospital terciário. METODOLOGIA: Avaliação de pacientes adultos com insuficiência cardíaca com FE< 40% e etiologia chagásica segundo os critérios do fenótipo de fragilidade: lentidão, exaustão, fraqueza, baixa atividade física e perda de peso não intencional. RESULTADOS: A amostra consistiu de 45 pacientes, com média de idade de 61,6 anos, com predominância do sexo masculino (71,1%) e de classe funcional II (93,3%), otimizados clinicamente, com 88,9% em uso de betabloqueador, 88,9% em uso de espironolactona e 77,8% em uso de Inibidor da Enzima Conversora de Angiotensina II (IECA) ou Bloqueador do Receptor de Angiotensina II (BRA) ou Inibidor da Neprilisina e do Receptor da Angiotensina (INRA). Não frágeis usavam mais IECA/BRA/INRA (p=0,03) e indivíduos frágeis usavam mais diuréticos de alça (p=0,04). Houve prevalência de 20% de fragilidade e 60% de pré-fragilidade na população estudada. O número de critérios positivos para fragilidade aumentou com a idade (p=0,006) e com a presença de comorbidades (p=0,03). CONCLUSÃO: Em uma população de indivíduos com insuficiência cardíaca de etiologia chagásica, a fragilidade e pré fragilidade são frequentes, juntas somando 80% da população estudada. O entendimento desse processo favorece a tomada de ações que visem evitar limitações, reduzam incapacidade e melhorem a qualidade de vida do paciente assistido. Assessing and understanding frailty in patients with heart failure (HF) is essential, as it brings new perspectives to the management and treatment of patients. The frailty phenotype is the most accepted measurement instrument because it can differentiate frailty from disability and comorbidity. There are no frailty studies in patients with heart failure due to Chagas disease. Introdução: A insuficiência cardíaca (IC) é uma síndrome clínica multifatorial e multissistêmica, que acarreta alterações físico-funcionais, musculares, respiratórias, metabólicas e emocionais, contribuindo para um declínio da capacidade funcional e piora da qualidade de vida. Objetivo: avaliar o poder preditivo de fatores sociodemográficos, físico-funcionais, clínicos e emocionais na qualidade de vida e na distância percorrida do teste de caminhada de seis minutos (DTC6), de portadores de IC crônica. Métodos: estudo transversal, realizado com portadores de IC, atendidos em ambulatório. Foram utilizados os seguintes instrumentos: ficha de perfil sociodemográfico e clínico, qualidade de vida relacionada à saúde (Minnesota Living With Heart Failure Questionare- MLHFQ), Inventário de Depressão e Ansiedade de Beck (BAI e BDI). Para avaliação físico-funcional, foram realizadas a dinamometria palmar, manovacumetria, espirometria, índice de massa corporal, escala de dispneia modificada do Medical Research Council (mMRC), teste de caminhada de seis minutos (TC6) e nível de atividade física (IPAQ curto). As variáveis desfecho consideradas foram a qualidade de vida relacionada à saúde (QVRS) e a distância percorrida no TC6 (DTC6). O tratamento medicamentoso da insuficiência cardíaca evoluiu nas últimas décadas. Os betabloqueadores (BB), inibidores da enzima conversora de angiotensina (IECA), bloqueadores dos receptores de angiotensina II (BRA) e antagonistas de aldosterona são classes fundamentais pois comprovadamente diminuem morbi/mortalidade na IC. A adesão ao tratamento vem sendo amplamente estudado em insuficiência cardíaca aguda como crônica. Vários são os fatores que podem influenciar a adesão. O objetivo deste estudo foi avaliar a adesão ao tratamento medicamentoso, em pacientes diagnosticados com Insuficiência Cardíaca em acompanhamento ambulatorial em um hospital universitário. Foram utilizados diferentes métodos para este fim, comparando-os entre si, assim como agrupou-se os pacientes em mais aderentes e menos aderentes, além de se descrever os fatores que influenciaram na adesão ao tratamento medicamentoso. Para isso, utillizou-se o teste de Morisky de quatro perguntas. Além dele, também foi utilizado um questionário confeccionado pela autora e um formulário de coleta de dados no prontuário. Descreveu-se a frequência dos dados e posteriormente, a análise hierárquica de clusters foi aplicada. A adesão medida por meio do teste de Morisky resultou em 71% dos pacientes mais aderentes e 26% menos aderentes. Um resultado diferente, e também com valores mais expressivos, foi encontrado no autorrelato de adesão ao tratamento medicamentoso pelo paciente, em que 90% dos pacientes se consideraram mais aderentes e 8% menos aderentes. Quando se compara os medicamentos prescritos e relatados, a maioria dos pacientes (78%) relata pelo menos um medicamento diferente da prescrição. Quando medida por métodos diferentes, a adesão pode apresentar resultados não uniformes. Os fatores clínicos e os relacionados à satisfação com o serviço não influenciam na adesão ao tratamento medicamentoso. Já alguns fatores sociodemográficos, como idade (p=0,0476), condições de emprego (p=0,0132), escolaridade (p=0,0167), estado marital (p=0,0110), renda familiar (p=0,0123) e fatores relacionados ao uso do medicamento, como ter ajuda da família para retirar os medicamentos (p=0,0060), utilizar os medicamentos sozinhos (p=0,0186) e ter orientação médica quanto aos medicamentos (p=0,0077), influenciam na adesão ao tratamento medicamentoso. Assim, observou-se que os pacientes atendidos no Ambulatório de Insuficiência Cardíaca do Hospital das Clínicas da Universidade Federal de Goiás (HC/UFG) possuem adesão satisfatória ao tratamento medicamentoso prescrito. Pharmacological treatment for heart failure evolved in the last decades. Betablockers, angiotensin converting enzyme inhibitors, angiotensin II receptor blockers and aldosterone antagonists have been widely used, as they were proven to reduce morbi/mortality in heart failure. Patient adherence has been studied all over the world in the last years in acute and chronic heart failure. Many factors knownly can influence the patient adherence to treatment. This study purpose was to evaluate pharmacological treatment in patients diagnosed with heart failure in outpatient follow up in a university hospital. Different methods were used to get to this objective, comparing each other and also clustering patients in more adherent and less adherent to pharmacological treatment, furthermore, we described the factors influencing patient adherence to pharmacological treatment. Thereunto, the Morisky questionnaire was used. Also a questionnaire made by the author and a form to collect data in patient’s chart were applied. Data frequency was described and then hierarchic clusters analysis was performed. Adherence measured by the Morisky questionnaire resulted in 71% of patients more adherent to pharmacological treatment and 26% less adherent. A different result and with more expressive values was found in the self-report adherence to pharmacological treatment by the patient, where 90% of the patients considered themselves as more adherent to pharmacological treatment and 8% considered themselves as less adherent. When comparing treatment prescribed and treatment reported by the patients, the majority of patients (78%) reports at least one drug different from the prescription. When measured by different methods, adherence can show not uniform results. The clinical factors and factors related to the satisfaction with the service did not influence the patient adherence to pharmacological treatment. In the other hand, some sociodemographics factors, like age (p = 0,0476), employment condition (p = 0,0132), schooling (p = 0,0167), marriage status (p = 0,011), family income (p = 0,0123), and factors related to the use of medications, as having family help to the the medications (p = 0,006), use medication alone (p = 0,0186) and have medical orientation bout the use of medication (p = 0,0077) had influence in patient adherence to pharmacological treatment. Thus, it was observed that patients being followed up in the Heart Failure Clinic of Hospital das Clínicas da Universidade Federal de Goiás are well medicated and have satisfactory adherence to pharmacological treatment.

Dentre as diversas patologias que acometem os idosos destacam-se as doenças cardiovasculares, entre elas a Insuficiência Cardíaca (IC). Recentemente, busca-se verificar a existência de prejuízos cognitivos associados a prejuízos físicos e psicológicos, comumente conhecidos na IC. Analisamos uma população de pacientes com insuficiência cardíaca com o objetivo de comparar seu desempenho cognitivo com o de indivíduos com envelhecimento normal em idade pareada. Verificou-se, ainda, se havia nesses pacientes com IC algum comprometimento mais específico de alguma função cognitiva. Foram utilizados onze testes neuropsicológicos abrangendo as funções cognitivas: atenção, linguagem, memória, humor e função executiva. A amostra foi composta de 78 idosos ao total. O grupo controle foi composto de 37 indivíduos (com idade média de 68,3 ± 6,3) e o grupo clínico de 41 indivíduos (com idade média de 68,6 ± 6,9). Os sujeitos foram pareados em relação ao nível de escolaridade, com predomínio de idosos com 0 a 4 anos de estudo (65,9% no Grupo Clínico e 59,5 % no Grupo Controle). Os resultados apontaram diferenças significativas entre os grupos, principalmente nas funções executivas, que englobam capacidade de planejamento, organização, alternância e evocação da informação anteriormente armazenada. O desempenho atentivo mostrou-se alterado no que se refere à atenção alternada, bem como no que diz respeito à velocidade de processamento, tendo o grupo clínico mostrado pior desempenho nessas habilidades. A memória de curto prazo também possui pior desempenho no grupo clínico, grupo que, ademais, apresentou dificuldades na memória de longo prazo, sofrendo interferência de prejuízos executivos nessa habilidade. Os dados colhidos ao apontarem prejuízos cognitivos nos indivíduos com IC sugerem que novos estudos sejam realizados com essa população, visto que os prejuízos nas habilidades cognitivas implicam em danos no cotidiano do indivíduo, na sua independência e na qualidade de vida, bem como na adesão ao tratamento. Among the various diseases that affect the elderly stand out from them cardiovascular disease among them heart failure (HF). Recently seeks to check for cognitive impairment associated with physical and psychological damage commonly known in the IC. We analyzed a population of patients with heart failure in order to compare their cognitive performance with that of individuals with normal aging in paired age. It was also verified whether there was any more specific impairment of some cognitive function in these HF patients. Through neuropsychological tests it was compared to 78 elderly and 37 in the control group (mean age 68.3 ± 6.3), 41 clinical group with mean age of 68.6 ± 6.9). The subjects were matched to the level of education with a predominance of elderly people with 0-4 years of education (65.9% in the Clinical Group and 59.5% in the control group). We used 11 neuropsychological tests covering the cognitive functions: attention, language, memory, mood and executive function. The results showed significant differences mainly in executive functions which include planning capacity, organization, alternation and evocation of Previously stored information. Cognitive performance in other functions was similar between the groups. The data collected when indicating cognitive impairment in individuals with HF, suggest that new studies be performed with this population since the impairments in cognitive abilities have implications in the daily life of the individual, in their independence and quality of life, as well as in adherence to treatment.

Introdução: A insuficiência cardíaca (IC) representa uma síndrome clínica complexa em que vários mecanismos estão envolvidos. O prognóstico dos pacientes é ruim e a doença de Chagas, dentre as várias etiologias, possui o pior prognóstico na IC. Há controvérsia em relação aos fatores que possivelmente identificam os pacientes com risco especialmente alto de morte na insuficiência cardíaca. Apesar de mais de 40 variáveis terem sido identificadas como preditoras de prognóstico na IC, não há consenso sobre a sua utilidade na prática clínica. Objetivos: Identificar a associação de fatores clínicos e laboratoriais com o prognóstico na IC chagásica com disfunção ventricular grave, bem como analisar a associação de fatores prognósticos com a taxa de mortalidade e a sobrevida global desta populção, em um seguimento de 7 anos e seis meses. Métodos: Este é um recorte do “Estudo Multicêntrico, Randomizado de Terapia Celular em Cardiopatias (EMRTCC) – Cardiopatia Chagásica”, com análise retrospectiva de dados colhidos prospectivamente. Analisamos as seguintes variáveis: idade, pressão arterial (PA), fração de ejeção (FE), sódio plasmático, creatinina, teste de caminhada de 6 minutos (TC6M), taquicardia ventricular não sustentada (TVNS), largura do QRS, volume do átrio esquerdo indexado e classe funcional. Resultados: Do total de 60 participantes, 53 (88,3%) foram à óbito durante o período total de seguimento (90 meses) e 7 (11.7%) permaneceram vivos no final do mesmo. A probabilidade de sobrevida geral acumulada dos pacientes portadores de insuficiência cardíaca grave de etilogia chagásica, identificados neste estudo, foi de aproximadamente 11%. A pressão arterial sistólica não se correlacionou de uma maneira significante em relação ao desfecho de mortalidade, no entanto a cada incremento de 10mmHg resultou numa redução de 4% no risco de morte. Não houve significância estatística para as variáveis idade, fração de ejeção, TC6M e largura do complexo QRS, em relação ao desfecho de mortalidade. Houve significância estatística em relação ao sódio plasmático e a creatinina com o desfecho de mortalidade, na análise univariada. Entretanto, após o ajuste do modelo na análise multivariada, perderam significância. Das variáveis utilizadas no modelo de regressão de Cox, apenas a TVNS e o volume do átrio esquerdo indexado permaneceram como preditores independentes de mortalidade. A taquicardia ventricular não sustentada apresentou valor de risco significativo – HR = 2,11 (IC 95% 1,04 – 4,31) p < 0,05. Valores de VAEi >72 ml/m2 estavam associados a um aumento significativo na mortalidade HR = 3,51 (IC 95% 1,63 – 7,52), p 72mL/m2 ao ecocardiograma conferem a eles um pior prognóstico. A probabilidade de sobrevida acumulada desta população é ruim, sendo de apenas 11% em 7.5 anos. Introduction: Heart failure represents a complex clinic syndrome in which several mechanisms are involved. Patient’s prognosis is bad and Chagas disease, between other etiologies, has the worse prognosis in heart failure. There are some controversies in relation to many factors that possibly identify the patients with especially high risk of death in heart failure. Although more than 40 variables have been identified as prognostic predictors in HF, there is no consensus about its applicability in clinical practice. Objectives: Identify the association between clinical and laboratory factors in chagasic heart failure with severe ventricular disfunction; analyze the association between prognostic factors and mortality rate; examine the global survival of this population in a 7 years and 6 months follow up. Methods: The present study is a cutting of the “Randomized, Multicenter Celular Therapy in Cardiopathies - Chagas Cardiopathy”, with retrospective analysis of the data collected prospectively. The following variables were analyzed: age, arterial pressure (AP) ejection fraction (EF), plasmatic sodium, creatinina, 6 minutes walking test (6MWT), nonsustained ventricular tachycardia (NSVT), electrocardiogram QRS width, echocardiogram indexed left atrium volume (iLAV) and functional class. Results: From the 60 participants, 53 (88,3%) died during the total follow up period (90 months) and 7 (11.7%) remained alive until the end of the period. The general accumulated survival probability of the severe Chagas etiology patients, identified in our study, was about 11%. The systolic arterial pressure did not correlate in a significant manner to the mortality outcome, although the increasing in 10 mmHg in systolic arterial pressure resulted in a reduction of 4% in the risk of death. There was no statistical significance to the variables age, ejection fraction, 6MWT and QRS width in relation to the outcome mortality. There was statistical significance in relation to plasmatic sodium and creatinina, with the outcome mortality, in the univariate analyses. However, after adjust to the multivariate analyses model, they lost significance. From the variables used in the Cox regression model, only NSVT and the indexed atrium volume remained as independent mortality predictors. Unsustained ventricular thachycardia presented a significant risk value – HR = 2,11 (95% CI, 1,04 – 4,31) p72 ml/m2 were associated to a significant risk in mortality – HR = 3,51 (95% CI, 1,63 – 7,52), p 72mL/m2 gives them a worse prognosis. This population prognosis is bad, with a cumulated survival probability of only 11% in 7.5 years.
