11. Assessing the overwintering habitat ecohydrology of an at-risk snake after wildfire

Abstract

Peatland ecosystems in the eastern Georgian Bay, Ontario, region often provide overwintering habitat for the eastern massasauga rattlesnake (Sistrurus c. catenatus), a species considered at-risk across its range. Suitable overwintering habitat requires a resilience zone with peat temperatures above 0°C and a water table position sufficient to provide moisture without risk of flooding and these ecohydrological conditions commonly occur in raised peatland microforms (hummocks). Due to a changing climate, these peatlands are at risk of increased wildfire frequency and burn severity which may threaten overwintering habitat availability and suitability. In 2018, a wildfire burned over 11,000 ha of the eastern Georgian Bay landscape which serves as critical habitat for the massasauga. We monitored water table position, snow depth, rainfall, and peat thermal dynamics in hummocks in three burned and three unburned peatlands to assess the potential impacts of wildfire on massasauga overwintering habitat. We found that hummocks were able to provide unfrozen and unflooded habitat regardless of peat burn severity and that surface complexity and peatland-scale characteristics provided the greatest control on microhabitat suitability. This research highlights the importance of conserving peatland ecosystems that provide resilient species at risk habitat. Thesis Master of Science (MSc) The eastern massasauga rattlesnake is a species at risk native to Ontario and parts of the USA. In the eastern Georgian Bay region, massasaugas overwinter in wetlands for up to half the year. This is a sensitive period because flooding or freezing within the hibernacula can be fatal. Due in part to climate change, wetlands in this region are at increased wildfire risk which may threaten the quality of massasauga overwintering habitat. In 2018, a wildfire burned over 11,000 ha of land along eastern Georgian Bay, some of which was massasauga habitat. We monitored the water table position and soil temperature in potential massasauga overwintering habitat to assess its quality after wildfire. We found that wetlands provide unflooded and unfrozen habitat even when burned, and that wetland surface complexity is likely an important regulator of overwintering habitat quality. This research highlights the importance of identifying and protecting wetland ecosystems that provide resilient habitat in the face of a disturbance.

Abstract

A retrospective review of hospital employees at a single employer institution who underwent ultrasound guided thread carpal tunnel release (TCTR) or open carpal tunnel release (OCTR) between January 2018 and August 2020 was performed to ascertain differences in return-to-work status. Patient age, sex, occupation, handedness, severity of carpal tunnel syndrome, prior treatments and surgical outcomes were reviewed. A total of 18 patients underwent TCTR and 17 patients underwent OCTR. The TCTR group averaged 12 days to return to work without restrictions, as opposed to 33 days for the OCTR group. Resolution of symptoms was afforded in all patients without any complications regardless of surgical technique. While both TCTR and OCTR were effective, our data indicates that TCTR resulted in a shorter return to work. Level of evidence: III [ABSTRACT FROM AUTHOR]

20. Skin Closure Technique and Postprocedural Pain after Spinal Cord Stimulator Implantation: A Retrospective Review

Abstract

Spinal cord and dorsal root ganglion stimulation are minimally invasive surgical techniques used to treat an array of chronic pain disorders. There is a paucity of data related to defining best practices in these specific patient populations, and historically, providers have relied on consensus committees to opine on the best techniques for patient safety and experience. The most efficacious mechanism of surgical closure—specifically a running suture closure compared to a surgical staple closure—is debated. A retrospective review of 155 patients implanted with either a spinal cord or dorsal root ganglion stimulator between 2017 and 2019 was undertaken to determine if the type of surgical closure was related to degree of postoperative surgical site discomfort. The primary outcome showed no statistically significant difference on postoperative pain scores between the suture (6.0 (IQR 5.0–8.0)) and staple (7.0 (IQR 5.0–8.0)) cohorts at postoperative day (POD) #1 (adjusted β 0.17 (95% CI −0.61 to 0.95), P=0.670). This finding held for postoperative pain scores at POD #10 as well (staples (1.0 (IQR 0.0–4.0)) and suture (2.0 (IQR 0.0–5.0), adjusted β −0.39 (95% CI −1.35 to 0.58), P=0.432)). A regression analysis was performed to identify secondary factors impacting postoperative pain scores. Higher preoperative pain score (β 0.50 (95% CI 0.09 to 0.92), P=0.019) and female gender (β 1.09 (95% CI 0.15 to 2.02), P=0.023) were predictive of higher incisional pain scores at POD#10. Increasing age was associated with decreased incisional pain scores at POD#10 (β −0.06 (95% CI −0.09 to −0.03), P<0.001). These findings are of interest to the pain practitioner and may be valuable in preoperative discussions with prospective patients.
