Individual and combined attachment dimensions within the dyad may affect therapists’ experiences in ways that hinder mentalizing and threaten the alliance. Previous research on a particular therapists’ mentalizing ability named the elaboration of countertransference experience (ECE) suggests that attachment may affect therapists’ mental processes, which in turn are involved in alliance variations between dyads and over time. In this study, these findings are extended by examining therapist, client, and combined attachment dimensions as moderators of the association between ECE and working alliance. Forty-four assessments from the first 10 psychotherapy sessions of 12 independent dyads were examined. The ECE dimension of reflection/conjecture was positively associated with the alliance bond and total score in therapeutic dyads that differed more in attachment. Possible in-session dynamics at stake are discussed.

Objectives Healthy individuals from hereditary cancer families undergoing genetic testing for cancer susceptibility (GTC) report more distress when they perceive their social support as low and suppress their emotions. This study aimed to explore how suppressing emotions and perceiving others as unsupportive are related with cancer-risk distress. Methods We performed a regression-based mediation analysis to assess if expressive suppression mediates or is mediated by perceived social support in the relation with cancer-risk distress. Participants were 125 healthy adults aged over 18 (M = 36.07, SD = 12.86), mostly female (72,4%), who undergone GTC to assess the presence of hereditary breast and ovarian cancer or Lynch syndromes. Results Controlling for age and gender, we found a moderate size indirect effect of social support on cancer-risk distress through expressive suppression (β = −0.095) and a direct effect of expressive suppression on cancer-risk distress. Conclusions When healthy individuals from hereditary cancer families perceive their social network as less responsive, they tend to not express their emotions, which relates to increased distress facing GTC. Purpose: Screening instruments specifically developed to identify genetic testing applicants who may need professional psychosocial support are much needed. However, there are none validated for the Portuguese population. This paper presents the translation, adaptation, and validation process of the Genetic Psychosocial Risk Instrument (GPRI) in a sample of 207 Portuguese applicants to genetic testing in the context of inherited cancer risk. Methods: Participants were mainly female (84.06%), with a mean age of 40.08 (SD = 12.89) and were recruited from the Portuguese Oncology Institute of Porto (IPO-PORTO). Confirmatory Factor Analysis was conducted to study the GPRI factorial structure. Convergent validity was assessed with the Impact of Events Scale (IES), the Clinical Outcome Routine Evaluation – Outcome Measure (CORE-OM), and the Hospital Anxiety and Depression Scale (HADS). Results: It was confirmed a model composed by the factors Internal Impact of Genetic Testing, External Impact of Genetic Testing and History of Mental Health Concerns. These factors showed good internal consistency, convergent and discriminant validity. The factor Personal Loss to Cancer proposed in prior versions did not converge. We propose excluding this factor from the Portuguese Version of the scale. Conclusion: The Portuguese version of the GPRI is a reliable and valid instrument to use in routine clinical oncogenetic services as well as research settings.

Adolescence is one of the most curious and interesting periods of human development with complex neurobiological and social relational changes (Moretti, Psalich, Donnell, 2019). Transition to residential care, as well as experiencing maltreatment, non-responsive, and neglectful relationships with primary caregivers can enhance adolescent���s vulnerability to the development of behavioral and emotional maladjustment (e.g., Costa, Mota & Matos, 2019; Mota & Matos, 2014). Portugal, as a paradigmatic example of historic reliance on the use of residential care, has nowadays one of the highest rates of children and young people in residential care, even when comparing with other European countries such as Greece, Spain or Germany (Delgado, Pinto, Carvalho & Gilligan, 2018). According to the most recent national report, it is estimated that 6855 children and adolescents were under any type of residential care (ISSIP, 2020). The prevalence is higher for adolescents (54%), and 77% of behavior problems have been diagnosed between the ages of 12 and 17 years old (ISSIP, 2020). At the present time, there are important structural changes being conducted in child and youth welfare policies, namely in what concerns promotion and protection measures in residential and foster care. Legal frameworks adopted in 2019 highlight the need to develop tailored interventions adapted to the child and youth idiosyncrasies and elect their voice on decisions that affect their lives. Considering this, knowledge production on the characteristics and needs of the population under residential care measures, can contribute to the development of more comprehensive policies and interventions. Since 2007, the National Social Security sector has been producing, on an annual basis, a detailed report that characterizes, mainly through cross sectional analysis, out of home care in Portugal. These documents provide background and context, and act as an important mean to track the development and change that occurred over the years (Bowen, 2009). In this sense, in this paper we will present a comparative analysis of demographic indicators and psychosocial adjustment, and measures prior to placement, published in the reports of the last decade (2009 - 2019). This will include behavior and severity problems, mental health of young people and taking pedopsychiatric medication, as well as the existence of regular / irregular monitoring of mental health - pedopsychiatric and / or psychological. This study is part of a major project called CareMe (Efficacy of an attachment-based intervention in residential care: A randomized controlled trial on the effects on the caregivers��� relational skills and the adolescents��� psychosocial adaptation) developed with the aim of improving caregiving practices in residential care, and ultimately contribute to adolescents adaptive psychosocial development. Resumo Introdução/Objetivos: O Genetic Psychosocial Risk Instrument (GPRI) é um instrumento de rastreio utilizado por profissionais de saúde do ramo da genética para identificar candidatos a testes genéticos que possam precisar de apoio psicossocial adicional. Este artigo apresenta o processo de adaptação e validação do GPRI numa amostra de 207 portugueses candidatos a testes genéticos no contexto de risco de cancro hereditário. Materiais e Métodos: Os participantes são principalmente do sexo feminino (84,06%), com uma média de idades de 40,08 (DP = 12,89) e foram recrutados num hospital. Foi conduzida uma Análise Fatorial Confirmatória para estudar a estrutura fatorial do GPRI. A validade convergente foi avaliada com a Impact of Events Scale (IES), a Clinical Outcome Routine Evaluation – Outcome Measure (CORE-OM), e a Escala Hospitalar de Ansiedade e Depressão (HADS). Resultados: Os resultados confirmaram um modelo composto pelos fatores “Impacto Interno do Teste Genético”, “Impacto Externo do Teste Genético” e “História de Preocupações com a Saúde Mental”. Estes fatores mostraram boa consistência interna, validade convergente e discriminante. O fator “Perda Pessoal para o Cancro” proposto em versões anteriores não convergiu. Propomos excluir este fator da versão portuguesa da escala. Conclusão: Em conclusão, a versão portuguesa do GPRI é um instrumento confiável e válido para uso na prática clínica de rotina em serviços de oncogenética, bem como em investigações futuras.

Resumo Introdução/Objetivos: O teste de predisposição genética para o cancro (TPGC) constitui uma fonte de sofrimento psicológico para algumas pessoas. Investigação anterior tem mostrado que a supressão emocional (forma de regulação emocional caracterizada pelo evitamento e tentativa de controlo da expressão emocional) contribui para maior sofrimento psicológico. Por outro lado, o suporte social (perceção de ser cuidado, ter ajuda e fazer parte de uma rede de apoio) pode ajudar a reduzir o sofrimento psicológico face ao teste genético. Este estudo pretende compreender os mecanismos pelos quais a supressão emocional e o apoio social afetam o sofrimento psicológico face ao TPGC. Especificamente, pretendemos testar se a supressão emocional desempenha um papel mediador nesta relação. Métodos: Participaram 84 utentes (75% mulheres), com uma idade média de 34.47 (DP = 12.42), saudáveis, que realizaram TPGC para avaliar a presença de síndrome de cancro hereditário de mama e ovário, síndrome de cancro gástrico difuso hereditário, síndrome de Lynch e Síndrome hereditário de Polipose Adenomatosa Familiar. Após a primeira consulta de aconselhamento oncogenético, os candidatos preencheram a subescala de supressão emocional do Questionário de Regulação Emocional (ERQ), o Questionário Multidimensional de Perceção de Suporte Social (MSPSS) e a Escala de Impacto de Eventos (IES). Procedeu-se a uma análise de mediação utilizando o método de bootstrapping, considerando a supressão emocional (ERQ) como variável mediadora entre a perceção de suporte social (MSPSS) e o sofrimento psicológico (IES). Resultados: Os resultados mostram um efeito mediador da supressão emocional na relação entre a perceção de suporte social e o sofrimento psicológico (e = - 2,6863). Isto é, quando o participante perceciona que não pode contar com o apoio da sua família e dos seus amigos tende a inibir a sua expressão emocional, o que resulta num maior sofrimento psicológico. Conclusões: A supressão emocional é uma variável determinante, que explica os efeitos da perceção de suporte social no sofrimento psicológico após o teste de predisposição genética para cancro.
