Abstract

We report the discovery of a planet --- OGLE-2014-BLG-0676Lb --- via gravitational microlensing. Observations for the lensing event were made by the MOA, OGLE, Wise, RoboNET/LCOGT, MiNDSTEp and $\mu$FUN groups. All analyses of the light curve data favour a lens system comprising a planetary mass orbiting a host star. The most favoured binary lens model has a mass ratio between the two lens masses of $(4.78 \pm 0.13)\times 10^{-3}$. Subject to some important assumptions, a Bayesian probability density analysis suggests the lens system comprises a $3.09_{-1.12}^{+1.02}$ M_jup planet orbiting a $0.62_{-0.22}^{+0.20}$ M_sun host star at a deprojected orbital separation of $4.40_{-1.46}^{+2.16}$ AU. The distance to the lens system is $2.22_{-0.83}^{+0.96}$ kpc. Planet OGLE-2014-BLG-0676Lb provides additional data to the growing number of cool planets discovered using gravitational microlensing against which planetary formation theories may be tested. Abstract

We present V and R photometry of the gravitationally lensed quasars WFI2033-4723 and HE0047-1756. The data were taken by the MiNDSTEp collaboration with the 1.54 m Danish telescope at the ESO La Silla observatory from 2008 to 2012. Differential photometry has been carried out using the image subtraction method as implemented in the HOTPAnTS package, additionally using GALFIT for quasar photometry. The quasar WFI2033-4723 showed brightness variations of order 0.5 mag in V and R during the campaign. The two lensed components of quasar HE0047-1756 varied by 0.2-0.3 mag within five years. We provide, for the first time, an estimate of the time delay of component B with respect to A of $\Delta t= 7.6\pm1.8$ days for this object. We also find evidence for a secular evolution of the magnitude difference between components A and B in both filters, which we explain as due to a long-duration microlensing event. Finally we find that both quasars WFI2033-4723 and HE0047-1756 become bluer when brighter, which is consistent with previous studies., Comment: 16 pages, 15 figures, 10 tables. Abstract

We present 13 high-precision and four additional light curves of four bright southern-hemisphere transiting planetary systems: WASP-22, WASP-41, WASP-42 and WASP-55. In the cases of WASP-42 and WASP-55, these are the first follow-up observations since their discovery papers. We present refined measurements of the physical properties and orbital ephemerides of all four systems. No indications of transit timing variations were seen. All four planets have radii inflated above those expected from theoretical models of gas-giant planets; WASP-55b is the most discrepant with a mass of 0.63 Mjup and a radius of 1.34 Rjup. WASP-41 shows brightness anomalies during transit due to the planet occulting spots on the stellar surface. Two anomalies observed 3.1 d apart are very likely due to the same spot. We measure its change in position and determine a rotation period for the host star of 18.6 +/- 1.5 d, in good agreement with a published measurement from spot-induced brightness modulation, and a sky-projected orbital obliquity of lambda = 6 +/- 11 degrees. We conclude with a compilation of obliquity measurements from spot-tracking analyses and a discussion of this technique in the study of the orbital configurations of hot Jupiters., Comment: 13 pages, 8 tables, 11 figures. Abstract

We report the discovery of a Jupiter-mass planet orbiting an M-dwarf star that gave rise to the microlensing event OGLE-2011-BLG-0265. Such a system is very rare among known planetary systems and thus the discovery is important for theoretical studies of planetary formation and evolution. High-cadence temporal coverage of the planetary signal combined with extended observations throughout the event allows us to accurately model the observed light curve. The final microlensing solution remains, however, degenerate yielding two possible configurations of the planet and the host star. In the case of the preferred solution, the mass of the planet is $M_{\rm p} = 0.9\pm 0.3\ M_{\rm J}$, and the planet is orbiting a star with a mass $M = 0.22\pm 0.06\ M_\odot$. The second possible configuration (2$\sigma$ away) consists of a planet with $M_{\rm p}=0.6\pm 0.3\ M_{\rm J}$ and host star with $M=0.14\pm 0.06\ M_\odot$. The system is located in the Galactic disk 3 -- 4 kpc towards the Galactic bulge. In both cases, with an orbit size of 1.5 -- 2.0 AU, the planet is a "cold Jupiter" -- located well beyond the "snow line" of the host star. Currently available data make the secure selection of the correct solution difficult, but there are prospects for lifting the degeneracy with additional follow-up observations in the future, when the lens and source star separate., Comment: 10 pages, 2 tables, 5 figures. Abstract

Little is known about the development of the human entorhinal cortex (EC), a major hub in a widespread network for learning and memory, spatial navigation, high-order processing of object information, multimodal integration, attention and awareness, emotion, motivation, and perception of time. We analyzed a series of 20 fetal and two adult human brains using Nissl stain, acetylcholinesterase (AChE) histochemistry, and immunocytochemistry for myelin basic protein (MBP), neuronal nuclei antigen (NeuN), a pan-axonal neurofilament marker, and synaptophysin, as well as postmortem 3T MRI. In comparison with other parts of the cerebral cortex, the cytoarchitectural differentiation of the EC begins remarkably early, in the 10th week of gestation (w.g.). The differentiation occurs in a superficial magnocellular layer in the deep part of the marginal zone, accompanied by cortical plate (CP) condensation and multilayering of the deep part of CP. These processes last until the 13-14th w.g. At 14 w.g., the superficial lamina dissecans (LD) is visible, which divides the CP into the lamina principalis externa (LPE) and interna (LPI). Simultaneously, the rostral LPE separates into vertical cell-dense islands, whereas in the LPI, the deep LD emerges as a clear acellular layer. In the 16th w.g., the LPE remodels into vertical cell-dense and cell-sparse zones with a caudorostral gradient. At 20 w.g., NeuN immunoreactivity is most pronounced in the islands of layer II cells, whereas migration and differentiation inside-out gradients are seen simultaneously in both the upper (LPE) and the lower (LPI) pyramidal layers. At this stage, the EC adopts for the first time an adult-like cytoarchitectural organization, the superficial LD becomes discernible by 3T MRI, MBP-expressing oligodendrocytes first appear in the fimbria and the perforant path (PP) penetrates the subiculum to reach its molecular layer and travels along through the Cornu Ammonis fields to reach the suprapyramidal blade of the dentate gyrus, whereas the entorhinal-dentate branch perforates the hippocampal sulcus about 2-3 weeks later. The first AChE reactivity appears as longitudinal stripes at 23 w.g. in layers I and II of the rostrolateral EC and then also as AChE-positive in-growing fibers in islands of superficial layer III and layer II neurons. At 40 w.g., myelination of the PP starts as patchy MBP-immunoreactive oligodendrocytes and their processes. Our results refute the possibility of an inside-out pattern of the EC development and support the key role of layer II prospective stellate cells in the EC lamination. As the early cytoarchitectural differentiation of the EC is paralleled by the neurochemical development, these developmental milestones in EC structure and connectivity have implications for understanding its normal function, including its puzzling modular organization and potential contribution to consciousness content (awareness), as well as for its insufficiently explored deficits in developmental, psychiatric, and degenerative brain disorders.

Observations for the lensing event were made by the following groups: Microlensing Observations in Astrophysics; Optical Gravitational Lensing Experiment; Wise Observatory; RoboNET/Las Cumbres Observatory Global Telescope; Microlensing Network for the Detection of Small Terrestrial Exoplanets; and µ-FUN. All analyses of the light-curve data favour a lens system comprising a planetary mass orbiting a host star. The most-favoured binary lens model has a mass ratio between the two lens masses of (4.78 ± 0.13) × 10-3. Subject to some important assumptions, a Bayesian probability density analysis suggests the lens system comprises a 3.09-1.12+1.02 MJ planet orbiting a 0.62-0.22+0.20 M? host star at a deprojected orbital separation of 4.40-1.46+2.16 au. The distance to the lens system is 2.22-0.83+0.96 kpc. Planet OGLE- 2014-BLG-0676Lb provides additional data to the growing number of cool planets discovered using gravitational microlensing against which planetary formation theories may be tested. Most of the light in the baseline of this event is expected to come from the lens and thus high-resolution imaging observations could confirm our planetary model interpretation.

Abstract

Aims. We present the analysis of 26 nights of V and I time-series observations from 2011 and 2012 of the globular cluster M 30 (NGC 7099). We\ud used our data to search for variable stars in this cluster and refine the periods of known variables; we then used our variable star light curves to\ud derive values for the cluster’s parameters.\ud Methods. We used difference image analysis to reduce our data to obtain high-precision light curves of variable stars. We then estimated the cluster\ud parameters by performing a Fourier decomposition of the light curves of RR Lyrae stars for which a good period estimate was possible. We also\ud derived an estimate for the age of the cluster by fitting theoretical isochrones to our colour-magnitude diagram (CMD).\ud Results. Out of 13 stars previously catalogued as variables, we find that only 4 are bona fide variables. We detect two new RR Lyrae variables,\ud and confirm two additional RR Lyrae candidates from the literature. We also detect four other new variables, including an eclipsing blue straggler\ud system, and an SX Phoenicis star. This amounts to a total number of confirmed variable stars in M 30 of 12. We perform Fourier decomposition of\ud the light curves of the RR Lyrae stars to derive cluster parameters using empirical relations. We find a cluster metallicity [Fe/H]ZW = −2.01±0.04,\ud or [Fe/H]UVES = −2.11 ± 0.06, and a distance of 8.32 ± 0.20 kpc (using RR0 variables), 8.10 kpc (using one RR1 variable), and 8.35 ± 0.42 kpc\ud (using our SX Phoenicis star detection in M 30). Fitting isochrones to the CMD, we estimate an age of 13.0 ± 1.0 Gyr for M 30.

Abstract

We report the discovery of a Jupiter-mass planet orbiting an M-dwarf star that gave rise to the microlensing event OGLE-2011-BLG-0265. Such a system is very rare among known planetary systems and thus the discovery is important for theoretical studies of planetary formation and evolution. High-cadence temporal coverage of the planetary signal combined with extended observations throughout the event allows us to accurately model the observed light curve. The final microlensing solution remains, however, degenerate yielding two possible configurations of the planet and the host star. In the case of the preferred solution, the mass of the planet is $M_{\rm p} = 0.9\pm 0.3\ M_{\rm J}$, and the planet is orbiting a star with a mass $M = 0.22\pm 0.06\ M_\odot$. The second possible configuration (2$\sigma$ away) consists of a planet with $M_{\rm p}=0.6\pm 0.3\ M_{\rm J}$ and host star with $M=0.14\pm 0.06\ M_\odot$. The system is located in the Galactic disk 3 -- 4 kpc towards the Galactic bulge. In both cases, with an orbit size of 1.5 -- 2.0 AU, the planet is a "cold Jupiter" -- located well beyond the "snow line" of the host star. Currently available data make the secure selection of the correct solution difficult, but there are prospects for lifting the degeneracy with additional follow-up observations in the future, when the lens and source star separate., Comment: 10 pages, 2 tables, 5 figures. 22. Impact of surgery on fertility among patients with deep infiltrating endometriosis.

Abstract

Objective: The aim of the study was to investigate pregnancy and live birth rate after surgical resection of rectosigmoid deep infiltrating endometriosis (DIE), and study if complications affect these rates., Study Design: Historical case series. 193 patients with rectosigmoid DIE and pregnancy intention undergoing a rectosigmoid resection for DIE from January 2009 to May 2019. All operations were performed at the Department of Obstetrics and Gynecology, Aarhus University Hospital, Denmark. Surgical and fertility outcome data were obtained through patient files. Anonymized data was analyzed statistically. Normally distributed continuous variables are stated as means, categorical data as percentages and time to pregnancy as Kaplan-Meier failure function. Live birth rates stratified on complications were tested with chi2 test., Results: 117 patients became pregnant postoperatively with a pregnancy and live birth rate of 60.6% and 53.9%, respectively. 39 patients (20.2%) became pregnant spontaneously and 78 patients (40.4%) by intrauterine insemination or assisted reproductive technologies. Median time to pregnancy after surgery was 12.4 months (range: 0.4-58). Clavien-Dindo complication grade III (none grade IV) was registered among 16.6%. These patients had pregnancy and live birth rates of 50%, not statistically significantly different from those without complications., Conclusions: Postoperative pregnancy and live birth rates after resection of rectosigmoid endometriosis in this study are in line with conservative treatment, when comparing with the literature. Abstract

Background: Health care services are increasingly being digitized, but extant literature shows that digital technologies and applications are often developed without careful consideration of user needs. Research is needed to identify and investigate best-in-class methods to support user-centered design of mHealth applications., Objectives: The article investigates how the Kano model can be adapted and used for the purpose of eliciting child patients' information needs during the design phase of mHealth application development. The aim is to demonstrate its applicability for collecting and analyzing patient-centered data that are key to designing technology-supported solutions for health management., Methods: The article is based on a mixed-methods case study, which includes interviews with 21 patients aged 6 to 18. Structured interviews are analyzed based on prescriptions of the Kano model. Semi-structured interviews about child patients' information needs are analyzed thematically., Results: The results demonstrate several improvements to the Kano model that take into account the difficulties of effectively communicating with child patients. The combination of two types of interviews offers unique insights into the what , how , and why of patients' needs. Adaptation of the Kano model, simplification of response options, and participation of child patients' parents in interviews facilitate data collection., Conclusion: The article shows how the Kano model can be adapted to provide an effective means of eliciting child patients' needs. 24. Mobile App Support for Cardiopulmonary Resuscitation: Development and Usability Study.

Abstract

Background: The user requirements for in-hospital cardiopulmonary resuscitation (CPR) support apps are understudied. To study usability, functionality, and design based on user requirements, we applied a mixed methods research design using interviews, observations, and a Kano questionnaire to survey perspectives of both physicians and nurses., Objective: This study aims to identify what an in-hospital CPR support app should include to meet the requirements and expectations of health care professionals by evaluating the CprPrototype app., Methods: We used a mixed methods research design. The qualitative methods consisted of semistructured interviews and observations from an advanced life support (ALS) course; both provided input to the subsequent questionnaire development. The quantitative method is a questionnaire based on the Kano model classifying user requirements as must-be, one-dimensional (attributes causing satisfaction when present and dissatisfaction when absent), attractive, indifferent, and reverse (attributes causing dissatisfaction when present and satisfaction when absent). The questionnaire was supplemented with comment fields. All respondents were physicians and nurses providing ALS at hospitals in the Central Denmark Region., Results: A total of 83 physicians and nurses responded to the questionnaire, 15 physicians and nurses were observed during ALS training, and 5 physicians were interviewed. On the basis of the Kano questionnaire, 53% (9/17) of requirements were classified as indifferent, 29% (5/17) as attractive, and 18% (3/17) as one-dimensional. The comments revealed 7 different categories of user requirements with noticeable differences between those of physicians and nurses: technological challenges, keep track of time, documentation and history, disturbing element, improvement areas: functions, improvement areas: design, and better guidance., Conclusions: The study provides recommendations to developers on the user requirements that need to be addressed when developing CPR support apps. Three features (one-dimensional attributes) must be incorporated in an in-hospital CPR support app: reminder of rhythm check, reminder of resuscitation drugs, and differentiate between adults and children. Abstract

The aim of the study is to investigate how time and uncertainties of clinical action and decision-making plays out in the practical work of early neurorehabilitation in order to present new analytical ways to understand the underlying logics and dynamic social processes that take place during professional treatment of patients with severe acquired brain injury. Drawing on ethnographic fieldwork in a Danish neuro-intensive step-down unit (NISU) specialising in early neurorehabilitation, we found that negotiation of futures takes place in the modern ICU in the present by strategically building upon past experiences. We have argued that the clinical programme therefore cannot be understood only from a "here and now perspective", since the early neurorehabilitation practice is embedded in overlapping temporalities of the past, the present, and desired futures. The study discusses the underlying logics-often hidden or unnoticed-that impact clinical practice of early neurorehabilitation, in what we have termed a logic of clinical reenactment, a logic of future negotiation and a logic of paradox., Competing Interests: The authors have declared that no competing interests exist. 26. Biochemical markers after the Norseman Extreme Triathlon.

Abstract

Prolonged exercise is known to cause changes in common biomarkers. Occasionally, competition athletes need medical assistance and hospitalisation during prolonged exercise events. To aid clinicians treating patients and medical teams in such events we have studied common biomarkers after at The Norseman Xtreme Triathlon (Norseman), an Ironman distance triathlon with an accumulated climb of 5200 m, and an Olympic triathlon for comparison. Blood samples were collected before, immediately after, and the day following the Norseman Xtreme Triatlon (n = 98) and Oslo Olympic Triathlon (n = 15). Increased levels of clinical significance were seen at the finish line of the Norseman in white blood cells count (WBC) (14.2 [13.5-14.9] 109/L, p < 0.001), creatinine kinase (CK) (2450 [1620-3950] U/L, p < 0.001) and NT-proBNP (576 [331-856] ng/L, p < 0.001). The following day there were clinically significant changes in CRP (39 [27-56] mg/L, p < 0.001) and Aspartate Aminotransferase (AST) (142 [99-191] U/L, p < 0.001). In comparison, after the Olympic triathlon distance, there were statistically significant, but less clinically important, changes in WBC (7.8 [6.7-9.6] 109/L, p < 0.001), CK (303 [182-393] U/L, p < 0.001) and NT-proBNP (77 [49-88] ng/L, p < 0.01) immediately after the race, and in CRP (2 [1-3] mg/L, p < 0.001) and AST (31 [26-41] U/L, p < 0.01) the following day. Subclinical changes were also observed in Hemoglobin, Thrombocytes, K+, Ca2+, Mg2+, Creatinine, Alanine Aminotransferase and Thyroxine after the Norseman. In conclusion, there were significant changes in biomarkers used in a clinical setting after the Norseman. Of largest clinical importance were clinically significant increased WBC, CRP, AST, CK and NT-proBNP after the Norseman. Abstract

Introduction: Altogether 10% of all women of fertile age suffer from endometriosis, and up to 25% of these women require assisted reproductive technology (ART) to conceive. During ART the process of controlled ovarian stimulation causes high levels of estrogen, which in theory increases the risk of the progression of symptoms related to this estrogen-dependent disorder. Because several case reports have described the worsening of endometriosis during ART we carried out this study to investigate whether controlled ovarian stimulation during ART aggravates symptoms in women with endometriosis in terms of pain and quality of life., Material and Methods: This prospective cohort study was based on questionnaires containing the Endometriosis Health Profile (EHP-30) and pain evaluated on the numerical rating scale (NRS). Women aged below 40 years were recruited and divided into three groups according to their endometriosis and ART status. Questionnaires were administered before and after controlled ovarian stimulation in one ART cycle. Change in EHP-30 and NRS scores from the 1st to 2nd questionnaire was analyzed., Results: In total 52 women with endometriosis undergoing ART, 50 not undergoing ART, and 52 without endometriosis undergoing ART completed two questionnaires each. Both groups with endometriosis experienced a small increase in their quality of life, while women without endometriosis experienced a decrease. Pelvic pain worsened among women undergoing ART, but no greater worsening was detected among women with endometriosis compared with women without., Conclusions: This study showed no worsening in quality of life and a slight worsening in pelvic pain during ART regardless of endometriosis status., (© 2018 Nordic Federation of Societies of Obstetrics and Gynecology.)

Introduction: The aim of the study was to assess the intra- and interobserver variability of two- and three-dimensional rectosigmoid nodule size measurements by transvaginal sonography in patients with rectosigmoid endometriosis., Material and Methods: Intra- and interobserver variability was assessed in 10 and 30 patients, respectively. Measurements in two dimensions were performed in real-time during the scan, and three-dimensional measurements of volume were done on a computer. Differences within and between observers were expressed in absolute units (mm) and percentage (%) of average nodule size. Coefficient of repeatability and Bland-Altman plots with limits of agreement were used to evaluate the intra- and interobserver variability., Results: Intra- and interobserver variability in two-dimensional sonography ranged from 11 to 14 mm (46-51%) for length, 3 to 6 mm (32-57%) for depth and 5 to 9 mm (33-58%) for width of the nodule. Results of three-dimensional sonography, with assessment of nodule volume, showed intra- and interobserver variability 0.4 to 2.5 times the average nodule size., Conclusions: Measurements of rectosigmoid endometriosis nodule size with two- and three- dimensional transvaginal sonography were associated with large intra- and interobserver variability.
