In this work we evaluated the possibility of carrying out classifications of the outcome of patients with COVID19 disease through machine learning (ML) techniques working on small datasets of computed tomography (CT) images. In fact, one of the most common problems for medical artificial intelligence (AI) applications is the limited availability of annotated clinical data for model training. In the framework of the artificial intelligence in medicine (AIM) project funded by INFN, we analyzed datasets of CT scans of 79 subjects combined with clinical data containing information relating to positive outcome (no need for intensive care) or poor prognosis (admission into intensive care unit and/or death). After segmentation of ground glass opacities related to this pathology, the radiomic features were subsequently extracted from the CTs, selected through various algorithms of dimension reduction or fea ture selection and used for the training various classifiers. Values of the area under the ROC curve (AUC) of 0.84 were obtained with Gradient Boosting after BORUTA feature selection. Features selected are related to disease characteristics of poor prognosis patients.

In this work we report the analyses of the energy released per unit mass in organic compounds used for EPR dosimetry exposed to neutron beams in order to predict the increase in dose achievable by addition of gadolinium (Gd) inside the pellets. In particular, Monte Carlo (MC) simulations were carried out for alanine, ammonium tartrate and phenolic compounds irradiated with neutron beams with different energy spectra at various depths inside a water phantom. The addition of gadolinium increases sensitivity of these dosimeters to neutrons thanks to the high gadolinium cross section for neutron capture and to the large number of secondary particles (mainly Auger and internal conversion electrons) which are able to release energy inside the sensitive material layers. For small depths in water phantom and low energy neutron spectra the increase in dose due to gadolinium is large. The enhancement is smaller in case of epithermal neutron beam, whereas the increase in dose for fast neutrons is less than 50%. The sensitivity improvements for the various compounds considered were compared. In particular, the sensitivity enhancements were calculated for each neutron energy range (therrmal, epithermal and fast neutrons). The results are explained and discussed on the basis of the compositions of the compounds considered. All results here found suggest that considered organic compounds added with ggadolinium could be fruitfully used for dosimetric applications in Neutron Capture Therapy.

Experimental evidence is growing, supporting the evidence of a considerable normal tissue sparing effect when treatments are delivered with dose rates much larger with respect to the conventional ones [1]. In particular, an increasing of the therapeutic window has been demonstrated for dose rates over 50 Gy/s, over a large variety of in-vivo experiments [2]. If confirmed, the ‘FLASH effect’ has the potential to re-shape the future of radiation treatments, with a significant impact on many oncology patients [3]. Ultra-high dose rate (UHDR) beams for FLASH radiotherapy present significant dosimetric challenges [4]. Ionization chambers are affected by ion recombination effects, although novel approaches for decreasing or correcting for this effect are being proposed [5]. Passive dosimeters, as radiochromic films and alanine [6], could be used for UHDR measurements, although dose determination is typically time consuming. Solid state detectors, such as diamond or Silicon Carbide (SiC) detectors, have been also recently investigated, as a valuable alternative for real-time measurements. In this work we analysed the response of alanine pellets to UHDR electron beams. Irradiations of alanine pellets with electron beams at 7 and 9 MeV, accelerated by a SIT-Sordina ElectronFlash Linac, at conventional and UHDR regimes have been carried out. Average dose rates up several hundreds of Gy/s were used for the experimental campaign, with instantaneous dose rate even more two orders of magnitudes larger. Indeed, pulse structure of the used accelerator is characterized by a pulse duration between 1-4 us and a frequency up to hundreds of Hz. The analysis of the depth dose profile performed by stacking alanine pellets along the electron beam direction allowed to evaluate whether a dependence on the dose rate is present for these UHDR beams. The experimental results were aided by computational analyses. The results will be presented and discussed in details.

Scopo Negli ultimi anni la terapia transcranica ad ultrasuoni focalizzati guidati da risonanza magnetica (tcMRgFUS) per il trattamento di disordini neurologici quali il tremore essenziale (ET) e il Parkinson tremorigeno (PD) è stata effettuata utilizzando esclusivamente scanner RM da 3T. Presentiamo i risultati ottenuti con il primo sistema tcMRgFUS mai integrato con uno scanner RM da 1.5T. Materiali e metodi 23 pazienti (19 ET; 4 PD) sono stati selezionati per essere sottoposti a tcMRgFUS (talamotomia monolaterale; target: nucleo ventrale intermedio, VIM). I pazienti arruolati sono stati valutati prima del trattamento e 2 giorni dopo. L'efficacia del trattamento è stata valutata utilizzando la scala Fahn-Tolosa-Marin (FTM). Una rivalutazione globale è stata eseguita 3 mesi (18/23 pz.) e 6 mesi (11/23 px.) dopo il trattamento. In tutti i trattamenti è stato utilizzato uno scanner RM a 1.5T per la pianificazione ed il monitoraggio della procedura che è stata effettuata con un sistema tcMRgFUS da 1024 elementi phased-array operante a 650kHz. Risultati Tutti i pazienti con PD sono stati sottoposti a tcMRgFUS e hanno completato la procedura. Tra i 19 pazienti con ET: 15 sono stati sottoposti con successo a tcMRgFUS, 3 hanno interrotto la procedura e 1 non ha subito la procedura dopo il posizionamento del frame stereotassico. Tutti i pazienti trattati con ET e PD che hanno completato la procedura hanno mostrato un miglioramento immediato della gravità del tremore. Sebbene dopo il trattamento siano stati osservati effetti collaterali da lievi a moderati, nessun evento avverso è durato oltre i 2-3 mesi. Conclusioni La procedura tcMRgFUS effettuata utilizzando uno scanner RM da 1.5T è risultata essere una un'opzione terapeutica sicura ed efficace per il trattamento del tremore sia nei pazienti con ET e che nei pazienti con PD.

Introduzione La tcMRgFUS consente di effettuare una vera e propria esplorazione neurofunzionale per confermare ed ottimizzare il target lesionale prima di indurre una lesione cerebrale permanete. La scelta del target di trattamento si basa tuttavia su coordinate stereotassiche che non tengono conto delle diversità anatomiche di ciascun individuo. L'ottimizzazione del target di trattamento viene effettuata sulla base del feedback del paziente durante la procedura stessa. Materiali e metodi È’ stato utilizzato uno scanner RM da 1.5T per acquisire sequenze morfologiche 3D e ultrastrutturali (DTI) in pazienti successivamente sottoposti a tcMRgFUS. È stata effettuata una segmentazione delle aree eloquenti cerebrali poi utilizzate come seed per un algoritmo di trattografia probabilistica mirato a rappresentare le proiezioni corticali da e verso il talamo con particolare riferimento alle aree motoria primaria e supplementaria e alla corteccia premotoria. Le mappe di parcellizzazione talamica ottenute sono state confrontate con le lesioni indotte mediante tcMRgFUS. Obiettivi Scopo di questo lavoro è valutare retrospettivamente il possibile ruolo della parcellizzazione talamica per l'identificazione del nucleo ventrale intermedio (VIM) in pazienti sottoposti a tcMRgFUS. Risultati In tutti i casi è stato possibile rappresentare i maggiori gruppi di nuclei talamici che ricevono da o proiettano verso la corteccia. È stato possibile identificare un’ottima sovrapposizione tra le mappe di parcellizzazione talamica così ottenute (con particolare riferimento al nucleo VIM) e le lesioni indotte mediante tcMRgFUS. Conclusioni Questi risultati preliminari lasciano ipotizzare la possibilità di utilizzare questa tecnica durante la pianificazione di trattamenti tcMRgFUS e ottimizzare i tempi di trattamento specie nei casi in cui il target di trattamento si discosti dalle coordinate stereotassiche.
