Abstract

As communications are increasingly taking place virtually, the ability to present well online is becoming an indispensable skill. Online speakers are facing unique challenges in engaging with remote audiences. However, there has been a lack of evidence-based analytical systems for people to comprehensively evaluate online speeches and further discover possibilities for improvement. This paper introduces SpeechMirror, a visual analytics system facilitating reflection on a speech based on insights from a collection of online speeches. The system estimates the impact of different speech techniques on effectiveness and applies them to a speech to give users awareness of the performance of speech techniques. A similarity recommendation approach based on speech factors or script content supports guided exploration to expand knowledge of presentation evidence and accelerate the discovery of speech delivery possibilities. SpeechMirror provides intuitive visualizations and interactions for users to understand speech factors. Among them, SpeechTwin, a novel multimodal visual summary of speech, supports rapid understanding of critical speech factors and comparison of different speech samples, and SpeechPlayer augments the speech video by integrating visualization of the speaker's body language with interaction, for focused analysis. The system utilizes visualizations suited to the distinct nature of different speech factors for user comprehension. 4. Risk Factors for Extubation Failure After Pediatric Cardiac Surgery and Impact on Outcomes: A Multicenter Analysis.

Abstract

IMPORTANCE: Extubation failure (EF) after pediatric cardiac surgery is associated with increased morbidity and mortality. OBJECTIVES: We sought to describe the risk factors associated with early (< 48 hr) and late (48 hr ≤ 168 hr) EF after pediatric cardiac surgery and the clinical implications of these two types of EF. DESIGN SETTING AND PARTICIPANTS: Retrospective cohort study using prospectively collected clinical data for the Pediatric Cardiac Critical Care Consortium (PC4) Registry. Pediatric patients undergoing Society of Thoracic Surgeons benchmark operation or heart transplant between 2013 and 2018 available in the PC4 Registry were included. MAIN OUTCOMES AND MEASURES: We analyzed demographics and risk factors associated with EFs (primary outcome) including by type of surgery. We identified potentially modifiable risk factors. Clinical outcomes of mortality and length of stay (LOS) were reported. RESULTS: Overall 18,278 extubations were analyzed. Unplanned extubations were excluded from the analysis. The rate of early EF was 5.2% (948) and late EF was 2.5% (461). Cardiopulmonary bypass time, ventilator duration, airway anomaly, genetic abnormalities, pleural effusion, and diaphragm paralysis contributed to both early and late EF. Extubation during day remote from shift change and nasotracheal route of initial intubation was associated with decreased risk of early EF. Extubation in the operating room was associated with an increased risk of early EF but with decreased risk of late EF. Across all operations except arterial switch, EF portrayed an increased burden of LOS and mortality. CONCLUSION AND RELEVANCE: Both early and late EF are associated with significant increase in LOS and mortality. Study provides potential benchmarking data by type of surgery. Modifiable risk factors such as route of intubation, time of extubation as well as treatment of potential contributors such as diaphragm paralysis or pleural effusion can serve as focus areas for reducing EFs.

Abstract

What makes speeches effective has long been a subject for debate, and until today there is broad controversy among public speaking experts about what factors make a speech effective as well as the roles of these factors in speeches. Moreover, there is a lack of quantitative analysis methods to help understand effective speaking strategies. In this paper, we propose E-ffective, a visual analytic system allowing speaking experts and novices to analyze both the role of speech factors and their contribution in effective speeches. From interviews with domain experts and investigating existing literature, we identified important factors to consider in inspirational speeches. We obtained the generated factors from multi-modal data that were then related to effectiveness data. Our system supports rapid understanding of critical factors in inspirational speeches, including the influence of emotions by means of novel visualization methods and interaction. Two novel visualizations include E-spiral (that shows the emotional shifts in speeches in a visually compact way) and E-script (that connects speech content with key speech delivery information). In our evaluation we studied the influence of our system on experts' domain knowledge about speech factors. 10. HOLMES: Health OnLine Model Ensemble Serving for Deep Learning Models in Intensive Care Units

Abstract

Deep learning models have achieved expert-level performance in healthcare with an exclusive focus on training accurate models. However, in many clinical environments such as intensive care unit (ICU), real-time model serving is equally if not more important than accuracy, because in ICU patient care is simultaneously more urgent and more expensive. Clinical decisions and their timeliness, therefore, directly affect both the patient outcome and the cost of care. To make timely decisions, we argue the underlying serving system must be latency-aware. To compound the challenge, health analytic applications often require a combination of models instead of a single model, to better specialize individual models for different targets, multi-modal data, different prediction windows, and potentially personalized predictions. To address these challenges, we propose HOLMES-an online model ensemble serving framework for healthcare applications. HOLMES dynamically identifies the best performing set of models to ensemble for highest accuracy, while also satisfying sub-second latency constraints on end-to-end prediction. We demonstrate that HOLMES is able to navigate the accuracy/latency tradeoff efficiently, compose the ensemble, and serve the model ensemble pipeline, scaling to simultaneously streaming data from 100 patients, each producing waveform data at 250~Hz. HOLMES outperforms the conventional offline batch-processed inference for the same clinical task in terms of accuracy and latency (by order of magnitude). HOLMES is tested on risk prediction task on pediatric cardio ICU data with above 95% prediction accuracy and sub-second latency on 64-bed simulation.

Abstract

ObjectiveThe purpose of this report is to provide insight from pediatric stakeholders with a shared desire to facilitate a revision of the current United States regulatory pathways for the development of pediatric healthcare devices.MethodsOn August 5, 2020, a group of innovators, engineers, professors and clinicians met to discuss challenges and opportunities for the development of new medical devices for pediatric health and the importance of creating a regulatory environment that encourages and accelerates the research and development of such devices. On January 6, 2021, this group joined regulatory experts at a follow-up meeting.ResultsOne of the primary issues identified was the need to present decision-makers with opportunities that change the return-on-investment balance between adult and pediatric devices to promote investment in pediatric devices.Discussion/conclusionSeveral proposed strategies were discussed, and these strategies can be divided into two broad categories: 1. Removal of real and perceived barriers to pediatric device innovation; 2. Increasing incentives for pediatric device innovation.

17. RAIM: Recurrent Attentive and Intensive Model of Multimodal Patient Monitoring Data

Abstract

With the improvement of medical data capturing, vast amount of continuous patient monitoring data, e.g., electrocardiogram (ECG), real-time vital signs and medications, become available for clinical decision support at intensive care units (ICUs). However, it becomes increasingly challenging to model such data, due to high density of the monitoring data, heterogeneous data types and the requirement for interpretable models. Integration of these high-density monitoring data with the discrete clinical events (including diagnosis, medications, labs) is challenging but potentially rewarding since richness and granularity in such multimodal data increase the possibilities for accurate detection of complex problems and predicting outcomes (e.g., length of stay and mortality). We propose Recurrent Attentive and Intensive Model (RAIM) for jointly analyzing continuous monitoring data and discrete clinical events. RAIM introduces an efficient attention mechanism for continuous monitoring data (e.g., ECG), which is guided by discrete clinical events (e.g, medication usage). We apply RAIM in predicting physiological decompensation and length of stay in those critically ill patients at ICU. With evaluations on MIMIC- III Waveform Database Matched Subset, we obtain an AUC-ROC score of 90.18% for predicting decompensation and an accuracy of 86.82% for forecasting length of stay with our final model, which outperforms our six baseline models.

Abstract

Induced pluripotent stem cells (iPSCs) offer an unprece-dented opportunity to study human physiology and disease at the cellular level. They also have the potential to be leveraged in the practice of precision medicine, for example, personalized drug testing. This statement comprehensively describes the provenance of iPSC lines, their use for cardiovascular disease modeling, their use for precision medicine, and strategies through which to promote their wider use for biomedical applications. Human iPSCs exhibit properties that render them uniquely qualified as model systems for studying human diseases: they are of human origin, which means they carry human genomes; they are pluripotent, which means that in principle, they can be differentiated into any of the human body's somatic cell types; and they are stem cells, which means they can be expanded from a single cell into millions or even billions of cell progeny. iPSCs offer the opportunity to study cells that are genetically matched to individual patients, and genome-editing tools allow introduction or correction of genetic variants. Initial progress has been made in using iPSCs to better understand cardiomyopathies, rhythm disorders, valvular and vascular disorders, and metabolic risk factors for ischemic heart disease. This promising work is still in its infancy. Similarly, iPSCs are only just starting to be used to identify the optimal medications to be used in patients from whom the cells were derived. This statement is intended to (1) summarize the state of the science with respect to the use of iPSCs for modeling of cardiovascular traits and disorders and for therapeutic screening; (2) identify opportunities and challenges in the use of iPSCs for disease modeling and precision medicine; and (3) outline strategies that will facilitate the use of iPSCs for biomedical applications. This statement is not intended to address the use of stem cells as regenerative therapy, such as transplantation into the body to treat ischemic heart disease or heart failure.

50. Historical Escape Rooms: Unlocking the potential for communication and collaboration in History lessons.

Abstract

The purpose of this Living Theory action research project was to identify how I can change my practice in History lessons to live closer to my values of social-constructivism and democracy. I explored ways in which Educational Escape Rooms could be used during History lessons to create opportunities for communication and collaboration between students in a mixed gender, urban, disadvantaged school with a 5th class group. This research documents my professional learning journey, captured from multiple perspectives to negate some of the shortcomings of the Living Theory approach. Data was collected from my reflective journal, lesson observations and discussions from Critical Friends, and through questionnaires, feedback loops and interviews with children. By navigating the ethical considerations carefully, placing the child’s voice at the centre of this research allowed me to live closer to my value of democracy. The research was conducted across two research cycles, each consisting of a different teacher-made, History-based, Educational Escape Room. Children completed History lessons on a topic over four weeks and entered the Escape Room on week five. Puzzles were based on History content and skills developed during class lessons and children worked collaboratively to solve these and escape before the time expired. This self-study found that the Educational Escape Room provided opportunities for collaboration, affected by the degree of choice children had in choosing their own teams. The Escape Room also created opportunities for meaningful communication between students within the game and for communication between the teacher, students and colleagues. Evidently, children applied historical content knowledge from lessons in the new situation of the Escape Room and children utilised higher order historical skills without even realising it.
