Abstract

The demand for integrated photonic chips combining the generation and manipulation of quantum states of light is steadily increasing, driven by the need for compact and scalable platforms for quantum information technologies. While photonic circuits with diverse functionalities are being developed in different single material platforms, it has become crucial to realize hybrid photonic circuits that harness the advantages of multiple materials while mitigating their respective weaknesses, resulting in enhanced capabilities. Here, we demonstrate a hybrid III-V/Silicon quantum photonic device combining the strong second-order nonlinearity and compliance with electrical pumping of the III-V semiconductor platform with the high maturity and CMOS compatibility of the silicon photonic platform. Our device embeds the spontaneous parametric down-conversion (SPDC) of photon pairs into an AlGaAs source and their subsequent routing to a silicon-on-insulator circuitry, within an evanescent coupling scheme managing both polarization states. This enables the on-chip generation of broadband telecom photons by type 0 and type 2 SPDC from the hybrid device, at room temperature and with internal pair generation rates exceeding $10^5$ $s^{-1}$ for both types, while the pump beam is strongly rejected. Two-photon interference with 92% visibility (and up to 99% upon 5 nm spectral filtering) proves the high energy-time entanglement quality characterizing the produced quantum state, thereby enabling a wide range of quantum information applications on-chip, within an hybrid architecture merging the assets of two mature and highly complementary platforms in view of out-of-the-lab deployment of quantum technologies.

The recent category of advanced therapy medicinal products has been created to comprise cellular, gene therapy, and tissue engineered products, as a new class of drugs. Their manufacture requires the same pharmaceutical framework as for conventional drugs and this means that industrial, large-scale manufacturing process has to be adapted to the peculiar characteristics of cell-containing products. Our hospital took up the challenge of this new path in the early 2000s; and herein we describe the approach we followed to set up a pharmaceutical-grade facility in a public hospital context, with the aim to share the solutions we found to make cell therapy compliant with the requirements for the production and the quality control of a high-standard medicinal product.Keywords: advanced therapy medicinal product, good manufacturing practices, stem cells

3. Electrically charged supermassive twin stars

Abstract

By assuming that ultra dense hybrid neutron stars are endowed with a distribution of electric charge, we study the corresponding twin star solutions and their properties resulting from a sharp first order transition from confined hadronic to a deconfined quark phase. Two distinct quark matter equations of state with increasing stiffness are considered and the values for the maximum gravitational masses of the hadronic and hybrid twin configurations are obtained for different values of the total electric charge. Interestingly, our calculations indicate that sharp transitions make charged twin hybrid stars more massive than their neutral counterparts, and that the {$\SI{2}{M_{\odot}}$ constraint from PSR J0740+6620} is surpassed for standard values of electric charge and can be considered stable only satisfying $\partial M/\partial \epsilon_0 > 0$. In particular, our charged stellar models reach masses even higher than the unknown compact object measured in the GW190814 event., Comment: 8 pages, 4 figures, 2 tables. Abstract

The description of the heaviest neutron stars observed in Nature depends on the understanding of the physical concepts present in General Relativity and Quantum Chromodynamics. In this work, we review the basic concepts need to describe strange stars, constituted by up, down and strange quarks, which is one of the possible alternatives to describe the general properties of the most dense neutron stars. We will review the Bodmer-Witten-Terazawa hypothesis, which states that strange matter is absolutely stable in relation to ordinary nuclear matter, and discuss the basic properties that characterize strange stars. Abstract

The study of the general properties and stability of charm stars with a net electric charge is performed within the MIT bag model framework. We consider two different models for the electric charge distribution and demostrate that both imply stellar configurations with larger masses and that satisfy the equilibrium condition. The dynamical stability against radial oscillations is investigated. Our results demonstrate that the eigenfrequencies are modified by the presence of a net electric charge, but the instability, previously demonstrated for the electrically neutral case, is also present in charged charm stars., Comment: 5 pages, 5 figures

6. Electrically charged strange stars with an interacting quark matter equation of state

Abstract

The properties of electrically charged strange quark stars predicted by an interacting quark matter equation of state (EoS) based on cold and dense perturbative quantum chromodynamics (pQCD) are investigated. The stability of strange stars is analyzed considering different models for the electric charge distribution inside the star as well as for distinct values for the total electric charge. A comparison with the predictions derived using the MIT bag model is also presented. We show that the presence of a net electric charge inside strange stars implies in a larger maximum mass in comparison to their neutral counterparts. Moreover, we demonstrate that the pQCD EoS implies larger values for the maximum mass of charged strange stars, with very heavy charged stars being stable systems against radial oscillations. For an electric charge distribution given by $q(r) = \beta r^3$, the pQCD EoS implies unstable configurations for large values of the renormalization scale as well as for large values of $\beta$, in contrast to the MIT bag model predictions., Comment: 9 pages, 4 figures. Abstract

Aims A previous randomized study demonstrated that the subcutaneous implantable cardioverter defibrillator (S-ICD) was noninferior to transvenous ICD with respect to device-related complications and inappropriate shocks. However, that was performed prior to the widespread adoption of pulse generator implantation in the intermuscular (IM) space instead of the traditional subcutaneous (SC) pocket. The aim of this analysis was to compare survival from device-related complications and inappropriate shocks between patients who underwent S-ICD implantation with the generator positioned in an IM position in comparison with an SC pocket. Methods and results We analysed 1577 consecutive patients who had undergone S-ICD implantation from 2013 to 2021 and were followed up until December 2021. Subcutaneous patients (n = 290) were propensity matched with patients of the IM group (n = 290), and their outcomes were compared. : During a median follow-up of 28 months, device-related complications were reported in 28 (4.8%) patients and inappropriate shocks were reported in 37 (6.4%) patients. The risk of complication was lower in the matched IM group than in the SC group [hazard ratio 0.41, 95% confidence interval (CI) 0.17-0.99, P = 0.041], as well as the composite of complications and inappropriate shocks (hazard ratio 0.50, 95% CI 0.30-0.86, P = 0.013). The risk of appropriate shocks was similar between groups (hazard ratio 0.90, 95% CI 0.50-1.61, P = 0.721). There was no significant interaction between generator positioning and variables such as gender, age, body mass index, and ejection fraction. Conclusion Our data showed the superiority of the IM S-ICD generator positioning in reducing device-related complications and inappropriate shocks.

Abstract

Background aims: Advanced therapy medicinal products (ATMPs) are novel drugs based on genes, cells or tissues developed to treat many different diseases. Stability studies of each new ATMP need to be performed to define its shelf life and guarantee efficacy and safety upon infusion, and these are presently based on guidelines originally drafted for standard pharmaceutical drugs, which have properties and are stored in conditions quite different from cell products. The aim of this report is to provide evidence-based information for stability studies on ATMPs that will facilitate the interlaboratory harmonization of practices in this area. Methods: We have collected and analyzed the results of stability studies on 19 different cell-based experimental ATMPs, produced by five authorized cell factories forming the Lombardy “Plagencell network” for use in 36 approved phase I/II clinical trials; most were cryopreserved and stored in liquid nitrogen vapors for 1 to 13 years. Results: The cell attributes collected in stability studies included cell viability, immunophenotype and potency assays, in particular immunosuppression, cytotoxicity, cytokine release and proliferation/differentiation capacity. Microbiological attributes including sterility, endotoxin levels and mycoplasma contamination were also analyzed. All drug products (DPs), cryopreserved in various excipients containing 10% DMSO and in different primary containers, were very stable long term at <–150°C and did not show any tendency for diminished viability or efficacy for up to 13.5 years. Conclusions: Our data indicate that new guidelines for stability studies, specific for ATMPs and based on risk analyses, should be drafted to harmonize practices, significantly reduce the costs of stability studies without diminishing safety. Some specific suggestions are presented in the discussion.

Abstract

By assuming that ultra dense hybrid neutron stars are endowed with a distribution of electric charge, we study the corresponding twin star solutions and their properties resulting from a sharp first order transition from confined hadronic to a deconfined quark phase. Two distinct quark matter equations of state with increasing stiffness are considered and the values for the maximum gravitational masses of the hadronic and hybrid twin configurations are obtained for different values of the total electric charge. Interestingly, our calculations indicate that sharp transitions make charged twin hybrid stars more massive than their neutral counterparts, and that the {$\SI{2}{M_{\odot}}$ constraint from PSR J0740+6620} is surpassed for standard values of electric charge and can be considered stable only satisfying $\partial M/\partial \epsilon_0 > 0$. In particular, our charged stellar models reach masses even higher than the unknown compact object measured in the GW190814 event., Comment: 8 pages, 4 figures, 2 tables. Abstract

Background: Obesity is a well-established risk factor for venous thromboembolic events, such as pulmonary emoblism and deep vein thrombosis. The rising prevalence of obesity and its associated co-morbidities, including VTE, represent a growing public health issue. Bariatric surgery has been widely validated as an effective method of achieving weight reduction in patients with obesity. However, the impact of this weight loss on the long-term risk of thromboembolic events has yet to be explored. The aim of the present study is to evaluate the effect of bariatric surgery on long-term risk of venous thromboembolic events in a large cohort of patients with obesity. Methods: A nested cohort study was designed; including the 4,056 patients of the Clinical Practice Research Datalink database who had undergone bariatric surgery, and equal numbers of age, gender and BMI matched controls. The primary endpoint of the study was the occurrence of any venous thromboembolic event; with secondary endpoints of deep vein thrombosis alone, pulmonary embolism alone and all-cause mortality. A landmark analysis was carried out to describe and quantify the time-dependent pattern of thromboembolic risk in the initial three years following bariatric surgery. Data was analysed using a Cox proportional hazards model accounting for multiple covariates. Results: Patients were followed up for a median of 10·7 years. The bariatric surgery cohort had a significantly lower occurrence of the primary outcome (HR 0·601; 95%CI 0·430-0·841, p=0·003); mainly driven by a reduction in deep vein thrombosis (HR 0·523; 95% CI 0·349-0·783, p=0·002) and not in pumonary embolism (HR 0·882; 95% CI 0·511-1.521, p=0·651). Conclusion: The results of this nation-wide investigation set out to characterise the impact of bariatric surgery on long-term risk of thromboembolic events in patients with obesity outline a significant reduction in thromboembolic events, driven by a reduction in deep vein thrombosis only.

Abstract

Electrocardiographic algorithms are particularly useful to differentiate, in the presence of a wide complex tachycardia, between supraventricular aberrancy and ventricular tachycardias (VT). There are numerous limitations to the sensitivity and specificity of these algorithms including the presence of accessory pathways, use of antiarrhythmic drugs, congenital heart diseases, electrolytes impairments, and artificial pacing. Once the diagnosis of VT has been reached, other algorithms can help in localizing the origin of the ventricular arrhythmia. These approaches are also limited by the anatomic structure of where the arrhythmia originates. This article illustrates the difficulties in applying common algorithms in many clinical circumstances.

Abstract

Background: To report health-related quality of life outcomes as assessed by validated patient-reported outcome measures (PROMs) after radical prostatectomy (RP). ­Methods: This study analyzed patients treated with RP within The PROState cancer monitoring in Italy, from the National Research Council (Pros-IT CNR). Italian versions of Short-Form Heath Survey and university of California los Angeles-prostate cancer index questionnaires were administered. PROMs were physical composite scores, mental composite scores and urinary, bowel, sexual functions and bothers (UF/B, BF/B, SF/B). Baseline unbalances were controlled with propensity scores and stabilized inverse weights; differences in PROMs between different RP approaches were estimated by mixed models. Results: Of 541 patients treated with RP, 115 (21%) received open RP (ORP), 90 (17%) laparoscopic RP (LRP) and 336 (61%) robot-assisted RP (RARP). At head-to-head ­comparisons, RARP showed higher 12-month UF vs. LRP (interaction treatment * time p = 0.03) and 6-month SF vs. ORP (p < 0.001). At 12-month from surgery, 67, 73 and 79% of patients used no pad for urinary loss in ORP, LRP and RARP respectively (no differences for each comparison). Conversely, 16, 27 and 40% of patients declared erections firm enough for sexual intercourse in ORP, LRP and RARP respectively (only significant difference for ORP vs. RARP, p = 0.0004). Conclusions: Different RP approaches lead to significant variations in urinary and sexual PROMs, with a general trend in favour of RARP. However, their clinical significance seems limited.

Abstract

Metabolism and mitochondrial biology have gained a prominent role as determinants of stem cell fate and function. In the context of regenerative medicine, innovative parameters predictive of therapeutic efficacy could be drawn from the association of metabolic or mitochondrial parameters to different degrees of stemness and differentiation potentials. Herein, this possibility was addressed in human mesenchymal stromal/stem cells (hMSC) previously shown to differ in lifespan and telomere length. First, these hMSC were shown to possess significantly distinct proliferation rate, senescence status and differentiation capacity. More potential hMSC were associated to higher mitochondrial (mt) DNA copy number and lower mtDNA methylation. In addition, they showed higher expression levels of oxidative phosphorylation subunits. Consistently, they exhibited higher coupled oxygen consumption rate and lower transcription of glycolysis-related genes, glucose consumption and lactate production. All these data pointed at oxidative phosphorylation-based central metabolism as a feature of higher stemness-associated hMSC phenotypes. Consistently, reduction of mitochondrial activity by complex I and III inhibitors in higher stemness-associated hMSC triggered senescence. Finally, functionally higher stemness-associated hMSC showed metabolic plasticity when challenged by glucose or glutamine shortage, which mimic bioenergetics switches that hMSC must undergo after transplantation or during self-renewal and differentiation. Altogether, these results hint at metabolic and mitochondrial parameters that could be implemented to identify stem cells endowed with superior growth and differentiation potential.

31. Bridge therapy or standard treatment for urgent surgery after coronary stent implantation: Analysis of 314 patients

Abstract

Intravenous administration of a short acting glycoprotein IIb/IIIa inhibitor has been proposed as a bridge to surgery in patients on dual antiplatelet treatment, but data in comparison with other treatment options are not available. We conducted a retrospective analysis of consecutive patients who underwent un-deferrable, nonemergency surgery after coronary stenting. The bridge therapy was performed after discontinuation of the oral P2Y12 inhibitor by using i.v. tirofiban infusion. Net Adverse Clinical Events (NACE) was the primary outcome. We analyzed 314 consecutive patients: the bridge strategy was performed in 87 patients, whereas 227 were treatedwith other treatment options and represent the control group. Thirty-day NACE occurred in 8% of patients in the bridge group and in 22.5% in the control group (p < 0.01). Bridge therapy was associated with decreased 30-day NACE rate [Odds ratio (OR) 0.30; 95% confidence interval (CI) 0.13-0.39; p < 0.01], particularly when the time interval between stenting and surgery was ≤ 60 days (OR 0.09, 95% CI 0.01-0.72; p = 0.02). There were no cases of stent thrombosis in the bridge group and 3 (1.3%) in the control group. Bridge therapy was associated with decreased events rates as compared to both patients with and without P2Y12 inhibitors discontinuation in the control group. After adjustment for themost relevant covariates, the favorable effect of the bridge therapy was not materially modified. In conclusion, perioperative bridge therapy using tirofiban was associated with reduced 30-day NACE rate, particularly when surgery was performed within 60 days after stent implantation.

Abstract

Progressive Supranuclear Palsy (PSP) is a rare neurodegenerative disease whose etiopathogenesis remains elusive. The intraneuronal accumulation of hyperphosphorylated Tau, a pivotal protein in regulating microtubules (MT), leads to include PSP into tauopathies. Pathological hallmarks are well known in neural cells but no word yet if PSP‐linked dysfunctions occur also in other cell types. We focused on bone marrow mesenchymal stromal cells (MSCs) that have recently gained attention for therapeutic interventions due to their anti‐inflammatory, antiapoptotic and trophic properties. Here, we aimed to investigate MSCs biology and to disclose if any disease‐linked defect occurs in this non‐neuronal compartment. First, we found that cells obtained from patients showed altered morphology and growth. Next, Western blotting analysis unravelled the imbalance in α‐tubulin post‐translational modifications and in MT stability. Interestingly, MT mass is significantly decreased in patient cells at baseline and differently changes overtime compared to controls, suggesting their inability to efficiently remodel MT cytoskeleton during ageing in culture. Thus, our results provide the first evidence that defects in MT regulation and stability occur and are detectable in a non‐neuronal compartment in patients with PSP. We suggest that MSCs could be a novel model system for unravelling cellular processes implicated in this neurodegenerative disorder.

Abstract

Transplantation of human bone marrow mesenchymal stromal cells (hBM-MSC) promotes functional recovery after stroke in animal models, but the mechanisms underlying these effects remain incompletely understood. We tested the efficacy of Good Manufacturing Practices (GMP) compliant hBM-MSC, injected intravenously 3.5 hours after injury in mice subjected to transient middle cerebral artery occlusion (tMCAo). We addressed whether hBM-MSC are efficacious and if this efficacy is associated with cortical circuit reorganization using neuroanatomical analysis of GABAergic neurons (parvalbumin; PV-positive cells) and perineuronal nets (PNN), a specialized extracellular matrix structure which acts as an inhibitor of neural plasticity. tMCAo mice receiving hBM-MSC, showed early and lasting improvement of sensorimotor and cognitive functions compared to control tMCAo mice. Furthermore, 5 weeks post-tMCAo, hBM-MSC induced a significant rescue of ipsilateral cortical neurons; an increased proportion of PV-positive neurons in the perilesional cortex, suggesting GABAergic interneurons preservation; and a lower percentage of PV-positive cells surrounded by PNN, indicating an enhanced plastic potential of the perilesional cortex. These results show that hBM-MSC improve functional recovery and stimulate neuroprotection after stroke. Moreover, the downregulation of "plasticity brakes" such as PNN suggests that hBM-MSC treatment stimulates plasticity and formation of new connections in the perilesional cortex.

Abstract

Hybrid photonic devices, harnessing the advantages of multiple materials while mitigating their respective weaknesses, represent a promising solution to the effective on-chip integration of generation and manipulation of non-classical states of light encoding quantum information. We demonstrate a hybrid III-V/Silicon quantum photonic device combining the strong second-order nonlinearity and compliance with electrical pumping of the III-V semiconductor platform with the high maturity and CMOS compatibility of the silicon photonic platform. Our device embeds the spontaneous parametric down-conversion (SPDC) of photon pairs into an Al-GaAs source and their subsequent routing to a silicon-on-insulator circuitry. This enables the on-chip generation of broadband telecom photon pairs by type 0 and type 2 SPDC from the hybrid device, at room temperature and with strong rejection of the pump beam. Two-photon interference with 92% visibility proves the high energy-time entanglement quality characterizing the produced quantum state, thereby enabling a wide range of quantum information applications.

Abstract

The purpose of this registry is to report the immediate and long-term safety and efficacy of the Biotronik Orsiro stent in an unselected population during everyday practice.Between May 2012 and June 2013, 246 consecutive coronary angioplasty procedures were performed using at least one Orsiro drug-eluting stent, in 225 patients and 303 lesions.Diabetes was present in 34.7% of patients. Procedures were non-elective percutaneous coronary intervention (PCI) in 17.1% of cases and acute coronary syndromes were 55.1%. Radial vascular access was used in 78% of cases, multivessel PCI was performed in 19.5% of the procedures. In 81.6% of cases lesions were B2/C type, 20.7% of procedures had bifurcation lesions. Procedural success was 99.6%. No acute thrombosis occurred. Clinical follow-up median period was 24.3 (±8.28) months and FU was available in 93.7% of patients. Death for any cause occurred in 11 patients (5.2%), 6 of them were non cardiac-related. Cardiac-related death rate was 2.4%. Two patients had in-stent restenosis. The overall target lesion failure rate was 3.3%.This observational data regards our experience with Biotronik Orsiro stent in an unselected population. This initial data, although limited by a mainly clinical follow-up and restricted number of patients, confirms the good clinical performance of this sirolimus-eluting stent with a biodegradable polymer in everyday practice, including complex lesions, according to current larger studies in the literature.

Abstract

To assess cathodic protection criteria in the presence of alternating current (AC) interference, weight loss testson cathodically protected carbon steel specimens were performed in soil-simulated conditions in the presenceof AC densities ranging from 10 to about 500 A/m2. Tests last for four months. Carbon steel potential wasperiodically monitored by means of a Luggin capillary in order to eliminate the ohmic drop contribution and torecord the true polarization level of the polarised carbon steel samples. At the end of the test weight loss wasevaluated. On the basis of the results, a corrosion risk map is proposed. Main conclusion is that overprotectionis the most dangerous condition in the presence of AC interference.

La corrosione delle armature è la principale causa di degrado delle strutture in calcestruzzo armato. La suaprevenzione si attua in primo luogo in fase di progettazione mediante l’utilizzo di calcestruzzo di adeguataqualità e spessore di copriferro opportuno. In presenza di ambienti particolarmente aggressivi o per struttureche richiedano una vita di servizio particolarmente lunga, è possibile adottare metodi di prevenzione aggiuntivatra i quali gli inibitori di corrosione offrono una soluzione semplice ed a costi contenuti. Gli inibitori di corrosionepossono essere utilizzati sia come tecnica preventiva, qualora siano aggiunti durante l’impasto del calcestruzzo,sia come sistema di ripristino se applicati sul calcestruzzo indurito.
