Abstract

Zusammenfassung: Die chronische Urtikaria (CU) gehört zu den häufigsten Krankheitsbildern in der Dermatologie. Aktuelle Studien zeigen eine Punktprävalenz zwischen 0,5 und 1% in der Gesamtbevölkerung mit einem Übergewicht an Frauen. Prinzipiell können alle Altersgruppen und Bevölkerungsschichten betroffen sein. Allerdings gibt es eine Häufung in der 3. und 4. Lebensdekade. Nach Definition der aktuellen Leitlinien besteht eine chronische Urtikaria dann, wenn Quaddelbildung und/oderAngioödeme spontan und für länger als 6 Wochen auftreten. Epidemiologische Studien belegen, dass die Mehrheit der Betroffenen über Monate, oft Jahre, unter den Beschwerden leidet. Negativ auf die Dauer der Erkrankung scheinen sich das Vorkommen von Angioödemen, das zeitgleiche Bestehen einer physikalischen Urtikaria, Positivität im autologen Serumtest und eine hohe Krankheitsschwere auszuwirken. Studien zur Lebensqualität zeigen, dass CU-Patienten ähnlich stark unter der Erkrankung leiden wie Menschen mit einer ausgeprägten koronaren Herzkrankheit. Neben den rein körperlichen Symptomen erleben die Patienten auch Einschränkungen bei alltäglichen Tätigkeiten und im Sozialleben. Zudem ist der Schlaf oft stark beeinträchtigt, und die Patienten leiden überdurchschnittlich häufig an psychiatrischen Begleiterkrankungen. Um Missverständnisse und Frustration bei der Betreuung zu vermeiden, ist es wichtig, die Patienten mit ihrer Erkrankung in all ihren Dimensionen wahr- und ernst zu nehmen. Das Ziel der Therapie von CU-Patienten sollte es sein, komplette Beschwerdefreiheit zu erreichen.

Abstract

Objective: The present paper describes the design, development, and implementation of the AutoInflammatory Disease Alliance (AIDA) International Registry specifically dedicated to patients with Schnitzler syndrome. Methods: This is a clinical physician-driven, population- and electronic-based registry implemented for the retrospective and prospective collection of real-life data from patients with Schnitzler’s syndrome; the registry is based on the Research Electronic Data Capture (REDCap) tool, which is thought to collect standardised information for clinical research, and has been realised to change over time according to future scientific acquisitions and potentially communicate with other existing or future similar registries. Results: Since its launch, 112 centres from 23 countries in 4 continents have been involved. Fifty-four have already obtained the approval from their local Ethics Committees. The platform counts 303 users (113 Principal Investigators, 186 Site Investigators, 2 Lead Investigators, and 2 data managers) at current (April 12th 2022). The registry collects baseline and follow-up data using 3922 fields organised into 25 instruments, including patient’s demographics, history, clinical manifestations and symptoms, trigger/risk factors, laboratory, instrumental exams, therapies, socioeconomic information, and healthcare access. Conclusions: This International Registry for patients with Schnitzler syndrome facilitates standardised data collection, enabling international collaborative projects through data sharing and dissemination of knowledge; in turn, it will shed light into many blind spots characterizing this complex autoinflammatory disorder

Abstract

The accumulation of reactive oxygen species (ROS) in cells and tissues contributes to the development and the progression of numerous diseases such as cancers, metabolic syndromes, or sensorineural disorders. NAPDH oxidases, a family of enzymes which sole function is to produce reactive oxygen species (ROS), appeared as relevant therapeutic targets in the treatment of oxidant-mediated pathologies. More specifically, the NOX3 isoform is only expressed in the inner ear and, although its physiological role in the cochlea is not known, there is increasing evidence that NOX3 is involved in different forms of acquired hearing loss. Thus, the inhibition of NOX3 would provide an efficient otoprotec-tive strategy, notably by preventing ROS-induced damages to the auditory synapse. This project aims at discovering NOX3 small molecule inhibitors for the prevention of acquired sensorineural hearing loss. We developed a cell-based high-throughput screen using an inducible system allowing the expression of NOX3 upon 24 h treatment with tetracy-cline. NOX3 activity was assessed through the detection of generated extracellular superoxide radical anion (O2*-) using the colorimetric assay WST-1. The non-specific NOX inhibitor diphenyleneiodonium chloride (DPI) was used as a reference compound for maximal inhibition. Among the 15,511 compounds screened, 115 showed an inhibitory activity on NOX3 equal to or higher than 50% and were considered as hits. These hits were further tested in dose-response using WST-1 and validated using orthogonal assays detecting hydrogen peroxide (Amplex Red/HRP and CBA fluorometric assays) and cytotoxicity. The specificity of the validated hits for NOX3 over the 6 other isoforms was also assessed. This critical early drug discovery step paves the way to the development of new small molecule therapeutics for the prevention of sensorineural hearing loss. [ABSTRACT FROM AUTHOR]

Abstract

Background Healthcare-associated infection (HAI) is a major challenge for patient safety worldwide, and is further complicated by antimicrobial resistance (AMR) due to excessive antimicrobial use in both humans and animals. Existing infection prevention and control (IPC) networks must be strengthened and adapted to better address the global challenges presented by emerging AMR. Methods In June 2017, 42 international experts convened in Geneva, Switzerland, to discuss two key areas for strengthening the global IPC network: 1) broadening collaboration in IPC; and 2) how to bring the fields IPC and AMR control together. Results The US Centers for Disease Prevention and Control, the European Centre for Disease Prevention and Control, and the World Health Organization (WHO) convened together with international experts to discuss collaboration and networks, demonstrating the participating organizations’ commitment to close collaboration in IPC. The challenge of emerging AMR can only be addressed by strengthening this collaboration across international organisations and between public health and academia. The WHO SAVE LIVES: Clean Your Hands initiative is an example of a successful collaboration between multiple global stakeholders including academia and international public health organisations; it can be used as a model. IPC-strategies are included within the four pillars to combat AMR: surveillance, IPC, antimicrobial and diagnostic stewardship, research and development. The prevention of transmission of multidrug-resistant microorganisms is a patient safety issue, and must be strengthened in the fight against AMR. Conclusions The working group determined that international organisations should take the lead in creating new networks, which will in turn attract academia and other stakeholders to join. At the same time, they should invest in bringing existing IPC and AMR networks under one umbrella. Transmission of multidrug-resistant microorganisms in hospitals and in the community threatens the success of antimicrobial stewardship programmes, and thus, research and development in IPC should be addressed as an enhanced global priority.

Background: Autoimmune chronic spontaneous urticaria (aiCSU) comes with high disease activity and poor response to treatment. Recently, elevated levels of IgG anti-thyroid peroxidase (aTPO) and low levels of total IgE were reported to be common in aiCSU. Objective: To investigate how high aTPO and low IgE individually and combined are linked to features of aiCSU, including treatment responses. Methods: We analyzed records of patients with CSU from 2 independent cohorts (n = 1120) for demographic, clinical, and laboratory parameters and treatment responses. Total IgE and aTPO were measured, and 4 markers of aiCSU were analyzed: autologous serum skin test, basophil activation test (BAT), and blood eosinophil and basophil counts. Cutoff values were greater than or equal to 34 kU/L (high aTPO) and less than 40 IU/mL (low total IgE). Results: One of 10 patients with CSU had both high aTPO and low IgE (aTPOUPIgEDN, 11%, n = 123). aTPOUPIgEDN was linked to higher age at CSU onset, being female, angioedema, and shorter CSU duration. aTPOUPIgEDN was associated with markers of aiCSU, that is, BAT and autologous serum skin test positivity, basopenia, and eosinopenia (P < .01 for all). Almost half the patients with aTPOUPIgEDN (44%, 19 of 43) had a positive BAT result, the best single marker for aiCSU, versus 12% (43 of 344) of patients without aTPOUPIgEDN (P < .001). Relative risk of showing BAT positivity for a patient with aTPOUPIgEDN is 3.636 (95% CI, 2.382-5.551). Patients with aTPOUPIgEDN showed low response rates to antihistamine treatment as compared with patients without aTPOUPIgEDN (30% vs 47%; P = .01). Conclusions: Our findings suggest that aTPOUPIgEDN is a useful diagnostic marker for aiCSU in everyday clinical practice.

Abstract

Background: The ongoing global SARS-CoV-2 pandemic has caused over 4.7 million infections greatly challenging healthcare workers (HCW) and medical institutions worldwide. The SARS-CoV-2 pandemic has shown to significantly impact mental and physical health of HCW. Thus, implementation of testing facilities supporting HCW are urgently needed. Methods: A low-threshold SARS-CoV-2 testing facility was introduced at the University Hospital Bonn, Germany, in March 2020. Irrespective of clinical symptoms employees were offered a voluntary and free SARS-CoV-2 test. Furthermore, employees returning from SARS-CoV-2 risk regions and employees after risk contact with SARSCoV-2 infected patients or employees were tested for SARS-CoV-2 infection. Pharyngeal swabs were taken and reverse transcription polymerase chain reaction for detection of SARS-CoV-2 was performed, test results being available within 24 h. Profession, symptoms and reason for SARS-CoV-2 testing of employees were recorded. Results: Between 9th March and April 30, 2020, a total of 1510 employees were tested for SARS-CoV-2 infection. 1185 employees took advantage of the low-threshold testing facility. One percent (n = 11) were tested positive for SARS-CoV-2 infection, 18% being asymptomatic, 36% showing mild and 36% moderate/severe symptoms (missing 10%). Furthermore, of 56 employees returning from SARS-CoV-2 risk regions, 18% (10/56) were tested SARS-CoV-2 positive. After risk contact tracking by the hospital hygiene 6 patient-to-employee transmissions were identified in 163 employees with contact to 55 SARS-CoV-2 positive patients. Conclusion: In the absence of easily accessible public SARS-CoV-2 testing facilities low-threshold SARS-CoV-2 testing facilities in hospitals with rapid testing resources help to identify SARS-CoV-2 infected employees with absent or mild symptoms, thus stopping the spread of infection in vulnerable hospital environments. Abstract

Objective: Thyroid hormones (TH) are essential for the homeostatic control of energy metabolism and the regulation of bodytemperature. The hypothalamic–pituitary–thyroid (HPT) axis is regulated by negative feedback mechanisms, ensuring that TH levels are maintained at a constant level. However, the feedback mechanisms underlying the resetting of the HPT axis regulation in the control of body temperature are still not fully understood. Here, we aimed to determine the thermoregulatory response in hypothyroid mice to different environmental temperatures and the underlying mechanisms. Methods: Distinct thermogenic challenges were induced in hypothyroid female C57BL/6N and leptin-deficient ob/ob mice through housing at either room temperature or thermoneutrality. The thermogenic and metabolic effects were analyzed through metabolic chambers, 18F-FDG-PET/MRI, infrared thermography, metabolic profiling, histology, gene expression and Western blot analysis. Results: In hypothyroid mice maintained at room temperature, high leptin serum levels induce a pyrexic effect leading to the stabilization of body temperature through brown adipose tissue thermogenesis and white adipose tissue browning. Housing at thermoneutrality leads to the normalization of leptin levels and a reduction of the central temperature set point, resulting in decreased thermogenesis in brown and white adipose tissue and skeletal muscle and a significant decline in body temperature. Furthermore, anapyrexia in hypothyroid leptin-deficient ob/ob mice indicates that besides its pyrexic actions, leptin exerts a stimulatory effect on the HPT axis to stabilize the remaining TH serum levels in hypothyroid mice. Conclusion: This study led to the identification of a previously unknown endocrine loop in which leptin acts in concert with the HPT axis to stabilize body temperature in hypothyroid mice.

Abstract

Increased plasma and adipose tissue protease activity is observed in patients with type 2 diabetes and obesity. It has been proposed that specific proteases contribute to the link between obesity, adipose tissue inflammation and metabolic diseases. We have recently shown that ablation of the serine protease kallikrein-related peptidase 7 (Klk7) specifically in adipose tissue preserves systemic insulin sensitivity and protects mice from obesity-related AT inflammation. Here, we investigated whether whole body Klk7 knockout (Klk7−/−) mice develop a phenotype distinct from that caused by reduced Klk7 expression in adipose tissue. Compared to littermate controls, Klk7−/− mice gain less body weight and fat mass both under chow and high fat diet (HFD) feeding, are hyper-responsive to exogenous insulin and exhibit preserved adipose tissue function due to adipocyte hyperplasia and lower inflammation. Klk7−/− mice exhibit increased adipose tissue thermogenesis, which is not related to altered thyroid function. These data strengthen our recently proposed role of Klk7 in the regulation of body weight, energy metabolism, and obesity-associated adipose tissue dysfunction. The protective effects of Klk7 deficiency in obesity are likely linked to a significant limitation of adipocyte hypertrophy. In conclusion, our data indicate potential application of specific KLK7 inhibitors to regulate KLK7 activity in the development of obesity and counteract obesity-associated inflammation and metabolic diseases.
