Objective: C-reactive protein (CRP) is an acute-phase protein that is widely used and can be elevated in many conditions that lead to acute inflammation, including infectious diseases. In this study, we aimed to determine the rate of presence of an infectious disease in patients who were consulted because of a high CRP level. Methods: Inpatients and outpatients consulted to the Infectious Diseases and Clinical Microbiology (IHCM) Clinic between January and March 2022 because of high CRP levels were included in the study. The anamnesis, physical examination, and laboratory findings of the patients were recorded, and the existence of an infectious disease was investigated. Results: 124 patients were referred for consultation during the study period. CRP levels of 6 (4.9%) patients were within average reference values; the remaining 118 patients were included in the study. Of the 118 patients, 54 (45%) had clinical complaints consistent with an infectious disease. A focus of infection was detected in only 49 (42%) patients. We found that there was a significant relationship between the presence of symptoms consistent with an infectious disease and the presence of a positive physical examination finding with the presence of an infectious focus (p<0.05) Conclusion: In this study, we determined that more than half of the patients for whom consultation was requested due to high CRP levels did not have an infectious disease. We think that unnecessary consultation burden can be avoided by detailed clinical evaluation of the patients before referring them for consultation by only looking at the CRP results. Introduction: Otolaryngologists are required to possess the knowledge, skills, and confidence necessary to manage consultations requested by palliative care services. However, the inclusion of palliative care needs in otolaryngology residency training is limited. The goal of this study was to enhance awareness of otolaryngologists' palliative care knowledge and skills by evaluating otolaryngology consultations requested by tertiary palliative care services between 2016 and 2021.Methods: This retrospective cross-sectional study included patients who consulted the otolaryngology department from the palliative care service of Kutahya Health Sciences University between January 1, 2016 and September 31, 2021. A total of 80 otolaryngology consultations were requested between 2016 and 2021. Patients were evaluated for age, gender, diagnosis of palliative admission, reason for consultation, date of consultation, interventions, and additional investigations. Results: There were 27 female patients (33.7%) and 53 male patients (66.2%). The mean age was 65.0 (range: 22-97) years. It was found that the volume of consultations did not increase significantly over a five-year period (p=0.502) and 78.8 % of consultations were requested during working hours (08:00-16:00). In addition, the response time (minutes) for consultations during working hours (08:00-16:00) was significantly lower than outside working hours (16:01-07:59) (p lt;0.001). The reasons for requesting a consultation and the procedures performed were significantly different between patients under and over 65 years of age (p=0.019 and p=0.008, respectively). Conclusions: This is the first study to evaluate otolaryngology consultations requested by a tertiary palliative care service. Awareness of these consultations may provide guidance for the assessment and management of patients in palliative care services. Keywords: consultation, otolaryngology, tracheotomy, palliative care

Objective: To reveal the characteristics of patients in need of neurosurgery by examining neurosurgery consultations within the emergency department (ED) admissions of a tertiary academic hospital. Methods: This is a retrospective, single-centre observational study. Patients admitted to the ED between 01.01.2022 - 31.12.2022 and consulted to the neurosurgery department were identified through the hospital computer system and included in the study. The demographic information of the patients, mode of admission to the ED, the reasons for admission, the time of admission, the number of brain computed tomography (CT) scans, whether they underwent surgery or not, and the mortality results were recorded. Results: A total of 441 neurosurgery consultations were examined. Fall 35.6% (n=157) and traffic accident 16.6% (n=73) were the most common reasons for consultation. It was observed that 92.5% (n=408) of the patients had a brain CT scan, and 19.5% (n=86) had two or more brain CTs. It was determined that 12.7% (n=56) of the patients consulted to neurosurgery were operated on, and 4.1% (n=18) of the patients' hospital admissions resulted in death. Only 53.7% (n=237) of the patients who underwent neurosurgery consultation were discharged from the ED. It was determined that significantly more Neurosurgery consultations were requested during working hours (p = 0.013). Conclusions: Most consultation calls from the ED to neurosurgery are for trauma patients. Brain CT examination is frequently used in neurosurgical patient evaluation. As a result of the consultations, almost half of the patients are hospitalized. Emergency physicians can select patients who need neurosurgery well. [ABSTRACT FROM AUTHOR]

Objective: A limited set of common skin problems leads to a disproportionate percentage of consultations that, in addition to the economic burden and cost, will lead to a loss of physician time and confidence in the necessity and importance of counseling, but so far, written documentation. There is no information about the frequency of various skin diseases in hospitalized patients. Therefore, our goal is to investigate the frequency of skin diseases in dermatological consultations requested from other departments of teaching hospitals in Mashhad, Iran. Material and Methods: This cross-sectional was done over six months. We used basic information, including demographic information, cause of hospitalization and primary underlying disease, the reason for consultation, description of skin lesion, diagnosis of the requesting physician, the importance of counseling or the impact of counseling on the treatment process, the preclinical results and the final diagnosis by dermatologist and entered into the checklist. Results: There were 200 patients in this study. The most common skin diseases in the requested consultations were a drug reaction, psoriasis, vasculitis, and dermatitis. Most specialists seeking counseling were internal medicine, rheumatology, and neurology. The consulting physician’s diagnosis was correct in 52 patients. According to the dermatologist, 40 consultations were very important, 129 consultations were important and 31 consultations were insignificant. Conclusion: Our study showed that dermatology consultation was effective in 84.5% of cases in the diagnosis and treatment of patients, and only in 26% of cases, the diagnosis of primary care specialists was correct.

PEDIATRIC neurology , *PEDIATRIC emergency services , *NEUROLOGICAL emergencies , *QUALITY of service , *EPILEPSY ,CENTRAL nervous system infections - Abstract
Objective: Pediatric neurology opinion is one of the most frequently requested consultations in emergency service practice. Symptoms and/or signs such as headache, altered consciousness, seizures, and focal neurological deficits are the most common consultation reasons. We aimed to retrospectively evaluate patients who were consulted with the pediatric neurology department in the pediatric emergency service. Method: This is a descriptive cross-sectional study in which the consultation notes of patients who presented to the pediatric emergency service and required a consultation with the pediatric neurology department between June 2016 and November 2019 were analyzed retrospectively. Results: The number of the consulted patients was 1,265. Sixteen patients left the hospital after their parents signed treatment and examination rejection form, 1,249 consultations were included. The most common reasons for consultation were seizure, routine follow-up of patients that receive home mechanical ventilator support, and headache. The rate of emergency neurological pathologies detected in brain computed tomography and magnetic resonance imaging was 1.7%. The most common electroencephalographic abnormality was focal epileptic discharges. Consultation rate requiring emergent intervention was 14.8% and status epilepticus, central nervous system infections, intracranial masses were the most common causes. Conclusion: The most common reason for consultation was seizure. The rate of consultations requiring acute intervention was low. We think that the emergency service admissions of patients that need examination and treatment in the outpatient clinic may harm the routine functioning of the emergency service. Community education for the use of the emergency room only when necessary is essential. [ABSTRACT FROM AUTHOR]

Objective: In hospitalized children and adolescents recognition of psychiatric symptoms, timely intervention and treatment are quite important. In this study, it was aimed to examine the characteristics of child and adolescent psychiatry consultations in a training and research hospital. Material and Methods: The psychiatric consultations of the child and adolescent inpatients between May 2016- October 2018 were assessed retrospectively. Demographic and clinical characteristics were analyzed using SPSS (Statistical Package Program for Social Sciences) 17.0. The compliance of continuous variables to normal distribution was tested using Kolmogorov-Simirnov test, Mann Whitney U test was used for comparisons. Categorical variables were expressed in terms of frequency (n) and percentage (%) and compared using the Pearson chi-square test. p values < 0.05 were regarded as statistically significant. Results: 66.9% (n = 85) of 127 children and adolescents evaluated within the scope of the study were females. 78.7% of the cases were in adolescent age group (13-17). Among the clinics that requested psychiatric evaluation, it was found that the highest rate of consultation was requested from the pediatric department with 93.7% (n = 119). Considering the reasons for requesting a consultation from the cases, it was found that the most requested consultation reason was the evaluation of suicide attempts (55.1%, n = 70). In the cross section evaluated, 19.7% (n = 25) of the cases were found to have at least one psychiatric disorder. It was found that at least one psychotropic drug was recommended to 18.1% of the cases. Conclusion: The importance of the Consultation-Liaison Psychiatry unit was highlighted in our study. Increasing awareness in the field of Consultation-Liaison is considered as a positive development, but it is thought that the services in this area should be improved more. Considering the different results found in the studies conducted in our country, to create a common language and algorithm in the field of consultation-liaison is thought to be important. HOSPITAL emergency services , *ADULTS , *LENGTH of stay in hospitals , *EMERGENCY physicians , *HOSPITAL mortality - Abstract
Objective: The many authors claim that poor communication may lead to bad consultations and patient results, but there is not enough evidence to support this argument. In this study, the aim was to determine whether consultation training makes a difference in the consultation process and patient outcomes. Method: This prospective, pre-/post-training study was conducted from June 1, 2019 to August 12, 2019 in an adult emergency department (ED). A feedback form with 9 questions related to the communication styles of emergency physicians (EPs) and their consultation processes was filled out by consultants. ED and hospital length of stay, in-hospital mortality, and additional requests were also noted. The validated 5Cs consultation model was used for training. Results: In the study, 724 of 1,295 eligible consultations (55.9%) were evaluated by consultants. The median scores of 7 questions related to the consultation processes and communication styles of the EPs increased after training, while there was no difference in patient outcomes or additional requests. In subgroup analyses, the median score of only 1 question increased in the internal consultations after training, while increases were noted for scores on 6 questions relating to surgical consultations. Similarly, there was no significant difference related to patients' outcomes and additional requests in these subgroup analyses. Conclusion: Consultation training had positive effects on the consultation processes and communication styles of the EPs. However, there was no difference in patient outcomes. Nevertheless, we believe that there should be communication skills training in countries that don't already have this in place. [ABSTRACT FROM AUTHOR]

Objective: Patients who come to the emergency department with dermatological complaints are generally referred to polyclinics. This practice increases the rate of correct diagnosis and treatment. It also reduces the workload of the emergency service. In this article, we wanted to examine the patients who applied to the emergency department and referred to our outpatient clinic but had any dermatological emergencies. Material and Methods: Patients between August 2018 and February 2020 were retrospectively evaluated from computer records and outpatient doctor records. In diagnoses were divided groups like urticaria, drug eruption, viral rash diseases, dermatitis, insect bites, infective conditions and pruritus. Results: 61.1% of the patients referred from the emergency service to our outpatient clinic were female and 38.9% male. Total number of patients was 72. The mean age was 39.94±14.10 (18-68) years. Twenty-six (36.1%) patients urticaria, 12 (16.7%) patients dermatitis, 12 (16.7%) patients pruritus, 7 (9.7%) patients infective conditions, 6 (8.3%) patients drug eruption, 5 (6.9%) patients had viral eruptions diseases and 4 (5.6%) patients were diagnosed with insect bites. The youngest group was the patients diagnosed with insect bites with an average of 32.75±15.34 years and the oldest group was the group diagnosed with viral rash with an average of 52.00±14.68 years. The average complaint duration of all patients was 2.47±1.85 (1-10) days. Objective: To evaluate the efficiency of electroencephalography (EEG) requested at the consultation. Methods: EEGs for which consultation was requested in one year were analyzed retrospectively. Neurology consultation notes, EEG forms, compliance with EEG findings were examined for each patient. Demographic data, requested unit, pre-diagnosis, final diagnosis were recorded. EEG results were divided into 3 groups:1)normal, 2)EEG with epileptiform anomaly (EEG-EA),3)EEG with nonepileptiform anomaly (EEG-NE). EEGs were also evaluated for their indications and contribution to diagnosis. Results: A total of 261 EEG recordings of 239 patients (133 men, average age 56.44 (18-90) ) were examined. 30.5% of the registered patients had a history of neurological diseases. EEGs were requested from 36.4% intensive care units and emergency departments, 51.5% internal branches, 12.3% surgical branches. Preliminaries were seizures in 42.5%, alertness/encephalopathy in 17.2%, syncope in 5.7%, nonconvulsive status epilepticus (NCSE) in 10.3%. 55% of EEGs were considered normal. 37.5% were in EEG-NE, 8.4% in EEG-EA. 91.6% of patients were imaged, 52.5% were pathological. Focal imaging pathology and EEG findings were compatible in 63% of patients with pathological EEG findings. EEG was pathological in 64.5% of the 127 patients requested with appropriate indications. 80% of EEG was thought not to contribute to the definitive diagnosis. Discussion: Male and advanced elderly people with neurological disease were preferred for EEG. Appropriate indication with a desired high probability of detection of pathological EEG. It is important that the doctor who evaluates the inpatient patient makes more careful decisions in terms of unnecessary process, resource and time wasting. [ABSTRACT FROM AUTHOR]

HOSPITAL emergency services , *HIGH technology , *AUGMENTED reality , *LENGTH of stay in hospitals , *MEDICAL consultants - Abstract
Emergency department (ED) consultation is an important part of acute patient care and one of the major determinants of ED length of stay (LOS). ED consultations between physicians who may not be in the same location frequently, require a tool to reach out one another and communicate. This paper reviews the historical change and development of consultation tools. The literature on ED consultation methods have been accumulating for the last two decades as the diversity of these methods has increased with technology. Most of the studies demonstrate that high tech systems are useful and safe to reach the consultant and share medical information flow. Moreover, the future of the area offers new developments regarding consultation tools that are hard to imagine yet like augmented reality. [ABSTRACT FROM AUTHOR]

Background: Medical consultation is defined as the exchange of views by more than one physician in order to diagnose or treat a disease. Counselor physician is called 'consultant' and the person executed the consultation is called a 'primary treating' physician. The medical genetics department is one of the departments that require the largest number of consultations from patients. In this context, we aimed to evaluate the consultations performed by medical genetic physicians both as consultant and primary treating physician in the light of additional parameters. Material and Methods: Between January 2017 and December 2017, 851 people evaluated in the outpatient clinic were included in the study. The department on consultation and the number of consultations requested are noted. There was no indication limitation in our study. Misplaced and repeated consultations were excluded from the study and only written consultations were included in the study. The results were evaluated using the SPSS 25.0 program. Results: The mean age of the participants was 11.27 ± 8.54. The age range of the individuals was 0-68. It was observed that more frequent consultations were requested from the internal medicine and the patients evaluated after the consultation. It has been observed that the pre-and post-diagnosis consultation request numbers are close to each other. Conclusion: It has been emphasized that the medical geneticists should work more closely with the which departments. The importance of the contributions of teachers and dentists in terms of medical geneticists has been pointed out. [ABSTRACT FROM AUTHOR]

Aim: With the aging population, chronic diseases are increasing, and many patients with high comorbidities are admitted to palliative care services. Consultations and opinions from various branches facilitate the multidisciplinary approach. In this study, the multidisciplinary approaches provided by the consultations requested to the patients hospitalized in the palliative care service were evaluated.Material and Methods: Patients treated in Eskişehir City Hospital Palliative Care Service between January and April 2021 were analyzed retrospectively. Results: In the study, 70 patients included, 48.6% (n=34) were female, and the mean age was 73.4 (27-97). 30% of the patients had malignancies. 25.7% applied directly from home (n=18), 18.6% came from different services (n=13) and 55.7% came from intensive care units (n=39). 72.9% were discharged (n=51), 25.7% were exitus (n=18) and 1.43% were transferred to intensive care (n=1). It was found that 72.9% (n=51) of the participants were requested consultation. It has been determined that the most frequent consultations belonged to the infectious diseases department and physical therapy and rehabilitation department. In contrast, the number of internal medicine and its subspecialties branches and algology consultations was low. It was found that consultation was not requested from 27.1% (n=19) of the patients. Conclusions: Many patient groups, from cancer patients to dementia, are hospitalized in the palliative care service. As the comorbid disease of the patient increases, the number of consultations increases. In our study, it was determined that the most frequently consulted branch was infectious diseases. It was thought that the low number of consultations of algology and internal diseases and sub-branches in the study was related to the fact that the relevant branches are following the patient with a broad perspective in the palliative service. The study evaluated the lack of algology consultation with internal diseases and subspecialties because internal diseases and anesthesia specialists were observing the patient with a broad perspective. The high rate of patients discharged to home, and the fact that they can be followed at home may result from the multidisciplinary approach in palliative care. The establishment of interdisciplinary communication, opinions, and suggestions requested from related branches in palliative care will contribute to the professional and healthy management of the process., Amaç: Yaşlanan nüfus ile kronik hastalıklar artmakta ve palyatif bakım servislerine komorbiditesi yüksek pek çok hasta yatırılmaktadır.

Effective management of the consultation process is critical for the patient who is waiting for a doctor in the emergency room to live a healthy future life. The purpose of this study is to look into the consultation services provided in the emergency departments of 36 hospitals in Ankara, in terms of the average time taken by consultant physicians to reach the emergency department and the factors that affect this time. In 2018, 36 hospitals (public, private, and academic) in the province of Ankara entered data into the Turkish Health Quality Indicators System. This study is a descriptive and retrospective research project. The type of institution, the number of beds, the number of inpatients, the number of emergency service examinations, the number of deaths in the emergency department, the average observation time in the emergency department, the patient satisfaction rate in the emergency department, and the number of consultant physician requests were all evaluated in the study, as well as the effects of these variables on the time the consultant physician arrived at the emergency department. The data was subjected to descriptive findings for the variables, as well as correlation and stepwise regression analysis. The number of emergency service examinations, the kind of institution, the number of consultant physician requests, and the number of beds in the hospital were found to be the factors determining the average time to reach the consultant physician to the emergency room, according to the analysis. The consulting physician determined that these variables explained 77 percent of the factors affecting the average time to reach the emergency department. In light of the study's findings, policy measures have been advocated to lower patient density in emergency rooms, increase primary health care services, and remove supply and demand-side impediments to health care access. It has also been determined that the successful execution of health quality assessments will help to improve the process' improvement., Konsültasyon sürecinin etkin yönetimi acil serviste hekimi bekleyen bir hastanın gelecekteki yaşantısının sağlıklı bir şekilde devam etmesi açısından oldukça önemlidir.

A multidisciplinary approach may be necessary in the diagnosis and management of certain patients; therefore, interdisciplinary consultations have an important place in the medical practice. Object: This study was conducted to evaluate child psychiatry consultations of patients which were stayed in different clinics in a university hospital. Methods: The records of the patients who were consulted in between 01.07.2016 and 31.12.2016 and was made diagnosis according to DSM5 was scanned restrospectively. The sociodemographic information of the patients, the diagnosis of physical illness, the clinic requested for consultation, the reason for the consultation, the psychiatric diagnoses determined in the consultation result and the psychiatric treatment process were analyzed by using the SPSS 15.0 program. Results: Psychiatric consultation was requested for 55 inpatient children within a six-month period. %63,6 of consulted cases was a girl, %36,4 of them was a boy and the average age was found as 13,2 ± 3,6. When the consultations were evaluated, it was determined that the 23,6% of the patients were consulted for suicide attempt and 83,6% of them had at least one mental disorder. The most common diagnosis was major depression in the rate of 36,4% and psychotropic treatment was recommended to 38,2% of evaluated cases because of the psychiatric disorder. Discussion: In our study, it was found that the consultation rates were low despite the rate of psychopathology (83,4%) was rather high in consulated children and adolescents. This suggests that the doctors who are working in non-psychiatric areas are having difficulty understanding and evaluating the psychiatric symptoms in children and that the cooperation among physicians should be increased so that psychiatric illnesses can be better recognized.
