Abstract

In prostate cancer, androgen receptor (AR)-targeting agents are very effective in various disease stages. However, therapy resistance inevitably occurs, and little is known about how tumor cells adapt to bypass AR suppression. Here, we performed integrative multiomics analyses on tissues isolated before and after 3 months of AR-targeting enzalutamide monotherapy from patients with high-risk prostate cancer enrolled in a neoadjuvant clinical trial. Transcriptomic analyses demonstrated that AR inhibition drove tumors toward a neuroendocrine-like disease state. Additionally, epigenomic profiling revealed massive enzalutamide-induced reprogramming of pioneer factor FOXA1 from inactive chromatin sites toward active cis-regulatory elements that dictate prosurvival signals. Notably, treatment-induced FOXA1 sites were enriched for the circadian clock component ARNTL. Posttreatment ARNTL levels were associated with patients' clinical outcomes, and ARNTL knockout strongly decreased prostate cancer cell growth. Our data highlight a remarkable cistromic plasticity of FOXA1 following AR-targeted therapy and revealed an acquired dependency on the circadian regulator ARNTL, a novel candidate therapeutic target.SignificanceUnderstanding how prostate cancers adapt to AR-targeted interventions is critical for identifying novel drug targets to improve the clinical management of treatment-resistant disease. Our study revealed an enzalutamide-induced epigenomic plasticity toward prosurvival signaling and uncovered the circadian regulator ARNTL as an acquired vulnerability after AR inhibition, presenting a novel lead for therapeutic development. Abstract

BackgroundAlthough the prognosis of core-binding factor (CBF) acute myeloid leukemia (AML) is better than other subtypes of AML, 30% of patients still relapse and may require allogeneic hematopoietic cell transplantation (alloHCT). However, there is no validated widely accepted scoring system to predict patient subsets with higher risk of relapse.MethodsEleven centers in the US and Europe evaluated 247 patients with t(8;21)(q22;q22).ResultsComplete remission (CR) rate was high (92.7%), yet relapse occurred in 27.1% of patients. A total of 24.7% of patients received alloHCT. The median disease-free (DFS) and overall (OS) survival were 20.8 and 31.2 months, respectively. Age, KIT D816V mutated (11.3%) or nontested (36.4%) compared with KIT D816V wild type (52.5%), high white blood cell counts (WBC), and pseudodiploidy compared with hyper- or hypodiploidy were included in a scoring system (named I-CBFit). 21. Early Postoperative Changes in Von Willebrand Factor Activity Are Associated With Future Bleeding and Stroke in HeartMate 3 Patients

Abstract

Hemocompatibility-related adverse events (HRAEs), particularly gastrointestinal bleeding, remain a frequent complication after left ventricular assist device (LVAD) implantation. The current study sought to describe and analyze whether early (<60 days) postoperative von Willebrand factor (VWF) activity assays predict the risk of gastrointestinal bleeding and stroke. A prospective single-center study including 74 HeartMate 3 device recipients between 2016 and 2023 was undertaken. The postoperative trajectory of the VWF profile was analyzed using linear mixed-effect models and Cox models were used to quantify associations between an early postoperative dip (≤0.7) in VWF activity assay measurements and late outcomes. Preoperatively, the mean VWF:Activity (Act)/Antigen (Ag) and VWF:Collagen Binding (CB)/Ag ratios were 0.94 (95% confidence interval [CI] = 0.81–1.02) and 0.95 (95% CI = 0.80–1.03), respectively, decreasing to 0.66 (95% CI = 0.57–0.73) and 0.67 (95% CI = 0.58–0.74) within 40 days (p< 0.05). In patients with VWF:CB/Ag and VWF:Act/Ag ratios ≤0.7 significantly more gastrointestinal bleeding (hazard ratio [HR]: 2.53; 95% CI = 1.1–5.8, and HR: 3.7; 95% CI = 1.5–9.2, respectively) and hemorrhagic stroke events (HR: 3.5; 95% CI = 1.6–7.6 and HR: 4.9; 95% CI = 2.1–11.7, respectively) were observed throughout the entire late (>60 days) postoperative period. In patients with VWF:Act/Ag ratio ≤0.7 less ischemic stroke events were observed (HR: 0.11; 95% CI = 0.01–0.85). In conclusion, VWF:Act/Ag and VWF:CB/Ag ratios ≤0.7 in the early postoperative phase can be used as biomarkers to predict HRAEs during long-term LVAD support.

35. The future of valvular heart disease assessment and therapy

Abstract

Valvular heart disease (VHD) is becoming more prevalent in an ageing population, leading to challenges in diagnosis and management. This two-part Series offers a comprehensive review of changing concepts in VHD, covering diagnosis, intervention timing, novel management strategies, and the current state of research. The first paper highlights the remarkable progress made in imaging and transcatheter techniques, effectively addressing the treatment paradox wherein populations at the highest risk of VHD often receive the least treatment. These advances have attracted the attention of clinicians, researchers, engineers, device manufacturers, and investors, leading to the exploration and proposal of treatment approaches grounded in pathophysiology and multidisciplinary strategies for VHD management. This Series paper focuses on innovations involving computational, pharmacological, and bioengineering approaches that are transforming the diagnosis and management of patients with VHD. Artificial intelligence and digital methods are enhancing screening, diagnosis, and planning procedures, and the integration of imaging and clinical data is improving the classification of VHD severity. The emergence of artificial intelligence techniques, including so-called digital twins—eg, computer-generated replicas of the heart—is aiding the development of new strategies for enhanced risk stratification, prognostication, and individualised therapeutic targeting. Various new molecular targets and novel pharmacological strategies are being developed, including multiomics—ie, analytical methods used to integrate complex biological big data to find novel pathways to halt the progression of VHD. In addition, efforts have been undertaken to engineer heart valve tissue and provide a living valve conduit capable of growth and biological integration. Overall, these advances emphasise the importance of early detection, personalised management, and cutting-edge interventions to optimise outcomes amid the evolving landscape of VHD. Although several challenges must be overcome, these breakthroughs represent opportunities to advance patient-centred investigations.

Abstract

Background: A precise understanding of the neuroanatomical basis of clinical manifestation of dementia is an important step for timely diagnosis. Dementia due to proteinopathies, such as Alzheimer's disease (AD), dementia with Lewy bodies (DLB), and Frontotemporal dementia (FTD), have different yet overlapping atrophy patterns which could account for their overlapping clinical presentations. To disentangle these relationships and facilitate timely differential diagnosis, we estimated distinct and overlapping atrophy patterns and associated them with cognitive deficits. Method: We included 1702 AD (age = 66.1±8.3), 319 FTD (age = 63.3±7.7), 213 DLB (age = 69.7±7.0), and 1330 subjective cognitive decline (SCD) (age = 59.8±9.0) participants from the Amsterdam Dementia Cohort. MRIs were processed with Freesurfer and outputs were manually checked. Scanner‐related bias in outputs was corrected using NeuroHarmony pipeline. Participants' cognition was evaluated using standardized neuropsychological tests. Z‐scores for cognitive domains of memory, executive functioning, attention, language, and visuospatial functioning were derived using SCD group as reference. We estimated distinct and overlapping atrophy patterns in each disease by computing odds ratio in each region, using SCD group as reference. Furthermore, we computed group‐wise Pearson partial correlation between the (sub‐)cortical measures and cognition while correcting for confounding effects of age, sex, and education level. Result: Atrophy in bilateral inferior temporal, and right hippocampus was more prevalent in AD and FTD patients than in DLB patients (Figures 1‐3). Temporal pole atrophy was more prevalent in FTD than in AD or DLB patients. Superior parietal and supra marginal atrophy was more prevalent in AD than in FTD or DLB patients. Bilateral hippocampus and amygdala atrophy associated with memory impairment in AD, whereas left hemisphere atrophy associated with memory impairment in FTD. Frontal atrophy associated with worse executive functioning and attention in FTD, but not in AD. Language deficit associated with left temporal lobe atrophy in both FTD and AD. Visuospatial dysfunction associated with parietal lobe atrophy in both AD and FTD. In DLB, we observed no significant correlation between atrophy and cognitive deficits. Abstract

Background: Automated image segmentation methods together with increasing computer power allow quantifying brain alterations in great detail in large neuroimaging datasets. The increasing size of data complicates visual quality control (QC), which is the gold standard. Increasingly often, research relies on automated QC methods. Here, we aimed to investigate the performance of automated FreeSurfer segmentation through visual QC in a large memory clinic cohort with more than ten thousand images, and to compare an automated QC measure to the visual QC. Method: 10,400 T1‐weighted MRI scans from the Amsterdam Dementia Cohort were segmented with FreeSurfer(v7.1). Quality control was performed using an adapted version of the Enigma QC protocol[https://doi.org/10.1038/ng.2250]. Twenty‐six individuals visually assessed cortical segmentations, rating each as "fail", "moderate", or "pass", identifying failure reasons and affected lobes. We investigated the error occurrence in failed or moderate quality scans, and whether segmentation failures depend on clinical diagnosis in a subset of 4990 baseline scans with most common diagnoses (i.e., SCD, MCI, AD, FTD, VaD and DLB). We compared an automated QC measure (i.e. median ± 3*IQR thresholded SurfaceHoles) with the visual QC output. Result: The majority (78.3%) of 10,400 cortical segmentations were rated as having "pass"‐quality, 16.2% were "moderate"‐quality and 5.4% segmentations were rated as "fail". Concordance between Automated QC and Visual QC was high for pass ratings(84%), but low for fail ratings(22.5%) (Table‐1). Within failed segmentations, most common reasons were processing errors(51.7%), image artifacts(14.3%) and underestimation(13.3%); for moderate ratings reasons were inclusion of meninges (47.8%) and underestimation of cortical thickness(33.2%) (Figure‐1). Stratified per diagnosis, segmentation failed in 2.3% of the SCD scans, 3% of MCI, 4.7% of DLB, 5.4% of AD, 7% of FTD and 14.5% of VaD. Further, we observed variation in affected lobes among diagnostic groups (Figure‐2b). Conclusion: The majority of scans passed visual QC, in high concordance with the automated QC. Images with moderate or failed quality, occurred more often in VaD and FTD. The most common mis‐segmentations were overestimation and underestimation of cortical thickness. 46. The influence of cardiopulmonary bypass on pharmacokinetics and pharmacodynamics of anaesthetic drugs during cardiac surgery; a review with emphasis on sex differences.

Abstract

Cardiopulmonary bypass (CPB) can alter the pharmacokinetics and -dynamics of drugs, which can lead to altered plasma and effect site concentrations and consequently to incorrect dosing and thus adverse events. Females and males differ in their physiological body size and composition causing different pharmacokinetics and –dynamics of drugs. However, research in this field remains scarce, especially during CPB. The aim of this review is to summarise current knowledge on CPB-induced alterations in pharmacokinetics and -dynamics of anaesthetic drugs with an emphasis on sex differences. Nineteen articles were selected addressing pharmacokinetics and -dynamics: The studied drugs were propofol (8 articles), sevoflurane (3), fentanyl, remifentanil and sufentanil (2), alfentanil and lidocaine (1). Plasma concentrations fluctuated during CPB. Initiation of CPB resulted in haemodilution and hypothermia, leading to a decrease in total plasma concentration combined with an increase in unbound plasma concentrations. For propofol, sevoflurane and remifentanil, this implies that lower doses were required during CPB to achieve the same anaesthetic effect. For fentanyl, alfentanil and sufentanil, these fluctuations were clinically irrelevant. Standard pharmacokinetic models were unable to accurately predict plasma concentrations and showed over- or underprediction. Only five studies assessed sex as a potential confounder, and found that consideration of sex did not improve the ability of pharmacokinetic models to predict plasma concentrations during CPB. In conclusion, understanding the altered pharmacokinetics and -dynamics of drugs during CPB is crucial to ensure appropriate drug dosing in this high-risk population. Abstract

ObjectivesDifferences in indication and technique make a randomised comparison between valve-sparing root replacement (VSRR) and personalised external aortic root support (PEARS) challenging. We performed a propensity score (PS)-matched comparison of PEARS and VSRR for syndromic root aneurysm.MethodsPatients in the PEARS 200 Database and Aortic Valve Insufficiency and ascending aorta Aneurysm InternATiOnal Registry (undergoing VSRR) with connective tissue disease operated electively for root aneurysm <60 mm with aortic regurgitation (AR) <1/4 were included. Using a PS analysis, 80 patients in each cohort were matched. Survival, freedom from reintervention and from AR ≥2/4 were estimated using a Kaplan-Meier analysis.ResultsMedian follow-up was 25 and 55 months for 159 PEARS and 142 VSRR patients. Seven (4.4%) patients undergoing PEARS required an intervention for coronary injury or impingement, resulting in one death (0.6%). After VSRR, there were no early deaths, 10 (7%) reinterventions for bleeding and 1 coronary intervention. Survival for matched cohorts at 5 years was similar (PEARS 98% vs VSRR 99%, p=0.99). There was no difference in freedom from valve or ascending aortic/arch reintervention between matched groups. Freedom from AR ≥2/4 at 5 years in the matched cohorts was 97% for PEARS vs 92% for VSRR (p=0.55). There were no type A dissections.ConclusionsVSRR and PEARS offer favourable mid-term survival, freedom from reintervention and preservation of valve function. Both treatments deserve their place in the surgical repertoire, depending on a patient’s disease stage. This study is limited by its retrospective nature and different follow-ups in both cohorts.
