2. An operative case of gastric cancer with ectopic bile duct drainage in the lesser curvature of the stomach

Abstract Background Among the various anomalies of the biliary system, a double common bile duct with ectopic drainage in the stomach is rare. Furthermore, ectopic bile ducts are extremely rare in gastric cancers. Case presentation A 67-year-old man was admitted to our hospital with gastric cancer and ectopic left extrahepatic bile duct drainage in the stomach. Pre-operative testing revealed no communication between the intrahepatic bile ducts. Distal gastrectomy and bile duct jejunostomy were performed. The post-operative course was uneventful, and the patient did not exhibit recurrence for 39 mo. Conclusions Although it is uncertain whether sustained bile exposure from an ectopic bile duct is related to gastric cancer, short-term follow-up might be necessary because of the possibility of gastric cancer.

Introduction Despite the acknowledged interconnection between smoking and pain, research on the relationship between smoking and central sensitization (CS) is scarce; this pain mechanism has attracted recent research attention. Considering potential sex differences, this cross-sectional study aimed to investigate the association between smoking and CS. Material and Methods Overall, 415 adult participants from an outpatient clinic underwent evaluation. The analysis focused on determining the relationship between smoking status and CS by differentiating between sexes. Data were collected on smoking presence or absence (independent variable) and CS (dependent variable) for each sex, with age, education level, drinking history, depression, and anxiety as covariates. CS was evaluated using the Central Sensitization Inventory. Following a descriptive analysis of the study population’s characteristics, logistic regression analysis was employed to assess the relationships. Social isolation among older adults affects their physical and mental health. Ego-resilience is associated with flexible adaptation to various environments and acceptable behaviors. However, its association with social isolation among older people is unclear. Therefore, a health survey was conducted with 510 adults aged 65 years or older to assess social isolation, its associated factors, and ego resilience. The results showed that the social isolation group had a lower median ego-resiliency scale score (42.0) compared to the non-social isolation group (38.0). The social isolation group were mostly males without spouses, took time to move, had more depressive moods, and poorer subjective health status. The non-social isolated group had faster maximum walking speed. Low ego-resilience was newly identified as a factor associated with social isolation among older people, as were being male, having no spouse, and depressive symptoms. Mediation analysis revealed that ego-resilience affected social isolation and was partially mediated by depressive symptoms.

5. Characteristics of blood biomarkers and eating behavior among older community-dwelling adults with typical and atypical depression

Summary: Background & Aims: Depression is receiving increase attention for its relation to lifestyle-related diseases. We aim to assess differences in blood biomarkers and dietary habit between individuals with typical and atypical depression. Methods: We gathered data from 804 older Japanese individuals. Participants with diagnosed depression were further grouped into typical or atypical subtypes. Dietary habits were measured by a dietary variety score. Considering the small sample size, descriptive analysis and logistic regression analysis adjusted by propensity score were used to evaluate the associations of blood biomarkers and dietary habits with typical and atypical depression. Results: Via interviews, 777 participants were confirmed to not be depressed, with the remaining 27 individuals having either typical (n = 18) or atypical (n = 9) depression. Weight, BMI, HbA1c levels, and the proportion of never drinkers were higher in the depressed group than in the non-depressed group (P = 0.04, P = 0.02, P = 0.04, P < 0.01, respectively). The habit of fruit intake was lower in the depression group (P = 0.01). Atypical depression was positively associated with weight and BMI (odds ratio: OR 1.06, P = 0.02, OR 1.20, P = 0.01), while typical depression had a positive relation to HbA1c (OR 1.76, P = 0.01). As for eating habit, atypical depression was negatively associated with fruit intake (OR 0.38, P < 0.01). Conclusions: Depression in older individuals was positively associated with weight (atypical type), BMI (atypical type), HbA1c (typical type), and low fruit intake (atypical type).

Abstract Background Among congenital anomalies of the portal venous system, prepancreatic postduodenal portal vein (PPPV) is very rare and has only been reported to date. Herein, we report a case of PPPV identified in preoperative examinations for hepatocellular carcinoma and a literature review. Case presentation A 63-year-old man was admitted to our hospital for treatment of a liver tumor. After examination, he was diagnosed with hepatocellular carcinoma with a diameter of 40 mm in segment 8. Contrast-enhanced computed tomography scan showed a portal vein passing between the duodenum and pancreas, hence called PPPV. At the hepatic hilus, the portal vein branched off in a complicated course with some porto-portal communications. We determined that anatomical resection with manipulation of the hepatic hilum in this case resulted in major vascular injury. Therefore, we performed partial liver resection, and the patient was discharged uneventfully on postoperative day 14. Conclusions Although PPPV is an extremely rare congenital vascular variant, it is important to carefully identify vascular patterns preoperatively and to recognize the possibility of such an anomaly to avoid misidentification and inadvertent injuries during surgery.

7. The Association of Central Sensitisation with Depression, Anxiety, and Somatic Symptoms: A Cross-Sectional Study of a Mental Health Outpatient Clinic in Japan

For patients with chronic pain and persistent physical symptoms, understanding the mechanism of central sensitisation may help in understanding how symptoms persist. This cross-sectional study investigated the association of central sensitisation with depression, anxiety, and somatic symptoms. Four hundred and fifteen adults attending an outpatient psychosomatic clinic were evaluated. Participants completed the Hospital Anxiety and Depression Scale, Somatic Symptom Scale 8, and the Central Sensitisation Inventory. The relationships between these factors were examined using descriptive statistics and multiple logistic regression analyses. The mean age was 42.3 years, and 59% were female. The disorders included adjustment disorders (n = 70), anxiety disorders (n = 63), depressive disorders (n = 103), feeding and eating disorders (n = 30), sleep–wake disorders (n = 37), somatic symptoms and related disorders (n = 84), and others (n = 28). In multiple logistic regression analyses, higher central sensitisation was associated with more severe anxiety, depression, and somatic symptoms after controlling for potential confounders. In the disease-specific analysis, somatic symptoms correlated more positively with central sensitisation than with depression or anxiety. Central sensitisation and depression, anxiety, and somatic symptoms were associated with patients attending an outpatient clinic. These findings highlight the importance of evaluating depression, anxiety, and somatic symptoms when assessing central sensitisation.

Abstract Background Medically unexplained symptoms (MUS) are common conditions that cause various somatic complaints and are often avoided in primary care. Fatigue frequently occurs in patients with MUS. However, the somatic and psychiatric symptoms associated with fatigue in patients with MUS are unknown. This study aimed to clarify the intensity of fatigue and the related somatic and psychiatric symptoms in patients with MUS. Methods A total of 120 patients with MUS aged 20–64 years who visited the Department of Psychosomatic Medicine, Toho University Medical Center Omori Hospital, between January and March 2021 were considered. The participants' medical conditions were assessed using the Chalder Fatigue Scale (CFS), Somatic Symptom Scale‐8 (SSS‐8), and Hospital Anxiety and Depression Scale (HADS). 9. Crystal growth of HAp on plate-like ZnO particles using APTES as surface treatment agents

ABSTRACTWe successfully prepared Hydroxyapatite/Zinc oxide (HAp/ZnO) composite particles with hexagonal plate-like shapes. The surface of the ZnO particles was treated with (3-aminopropyl)triethoxysilane (APTES) as a scaffold for the crystallization of HAp from the precursor solution. From the results of XRD measurements and SEM images of composite particles, formation of HAp on the ZnO particles was revealed. In addition, diffraction peaks associated with the (0 0 2) and (0 0 4) planes of HAp were clearly observed. They also revealed that the HAp/ZnO composite particles had a c-face orientation. Furthermore, element mapping analysis by EPMA showed that the elemental distributions of Ca, P and Zn of the composites were almost coincident. These results suggested that the formation of HAp occurred on the ZnO particles. In contrast, the ZnO particles without APTES treatment readily dissolved in the precursor solution and the diffused Zn2+ ions reacted with PO43- ions and Ca2+ ions to form CaZn2(PO4)2. APTES treatment of the ZnO surfaces appeared to prevent the dissolution of ZnO in the solution and induce the adsorption of anions such as phosphate on ZnO.

Abstract Background Central sensitization syndrome (CSS) involves severe functional symptoms due to central sensitization. for patients with severe somatic symptoms and related disorders (SSRDs), central sensitization may be responsible for their functional symptoms. We hypothesized that screening for CSS in patients with SSRDs would identify those with severe disease. The Somatic Symptom Scale-8 (SSS-8) is a simple tool to assess medical conditions related to SSRDs, but the cut-off point to identify severe cases of comorbid CSS is unknown. This study aimed to determine the optimal cut-off point of SSS-8 for screening the CSS of patients with severe SSRDs. Methods In total, 143 patients with SSRDs attending outpatient clinics of a university hospital in Japan were included in the study. The participants were evaluated using the SSS-8 for somatic symptoms, Hospital Anxiety and Depression Scale (HADS) for anxiety and depressive symptoms, Pain Catastrophizing Scale (PCS) for catastrophic thoughts, and Central Sensitization Inventory (CSI-A, B) for CSS. Receiver operating characteristic (ROC) curve analysis was performed using the propensity score. The area under the curve (AUC) was calculated using a propensity score considering PCS, age, sex, HADS, and CSI-B as confounders of SSS-8 and CSS to evaluate differences in diagnostic accuracy between patients with and without SSS-8. The sensitivity and specificity of the ROC analysis were then used to determine the cut-off point for discriminating severe cases of SSS-8. Results Of the 143 participants, 126 responded (51 CSS group and 75 non-CSS group), with a valid response rate of 88.1 percent. In the ROC analysis, the propensity score including SSS-8 was statistically more accurate. The optimal cut-off point was 13, with an AUC of 0.88, sensitivity of 84.3 percent, and specificity of 77.3 percent. Conclusions The SSS-8 is a useful tool for discriminating severe cases of SSRDs comorbid with CSS.

Abstract Background Inflammatory indices and tumor-infiltrating lymphocytes (TILs) have prognostic value in many cancer types. This study aimed to assess the prognostic value of inflammatory indices and evaluate their correlation with survival and presence of TILs in patients with colorectal liver metastasis (CRLM). Methods Medical records of 117 patients who underwent hepatectomy for CRLM were retrospectively reviewed. We calculated inflammatory indices comprising the neutrophil/lymphocyte ratio, platelet/lymphocyte ratio, C-reactive protein/albumin ratio (CAR), and Glasgow prognostic score (GPS). Furthermore, we evaluated the relationship between these ratios and the GPS and survival rates and immunohistochemical results of tumor-infiltrating CD3+, CD8+, and Foxp3+ lymphocytes. Results The patients with low CAR values and low GPS had significantly better overall survival as per the log-rank test (p = 0.025 and p = 0.012, respectively). According to the multivariate analysis using the Cox proportional hazard model, the CAR (hazard ratio [HR], 0.57; 95% confidence interval [CI], 0.33–0.99; p = 0.048) and GPS (HR, 0.40; 95% CI, 0.19–0.83; p = 0.013) were independent prognostic factors. Additionally, Foxp3+ lymphocytes were more common in samples from the patients with a low CAR (p = 0.041). Moreover, the number of CD3+ TILs was significantly higher in the patients with a low GPS (p = 0.015). Conclusions The CAR and GPS are simple, inexpensive, and objective markers associated with predicting survival in patients with CRLM. Moreover, they can predict the presence of Foxp3+ and CD3+ lymphocytes in the invasive margin of a tumor. Abstract Background Persistent postural-perceptual dizziness (PPPD) is a chronic functional dizziness symptom triggered by psychological stress, but its pathophysiology is unknown. Central sensitization is considered the cause of functional diseases, such as medically unexplained symptoms, and is a psychosocially affected condition. However, the association between dizziness symptoms in PPPD and central sensitization remains unclear. Thus, we conducted a cross-sectional study on the relation between dizziness symptoms and central sensitization in PPPD. Methods We recruited 61 outpatients with dizziness who met the PPPD diagnostic criteria. In addition to the evaluation of dizziness symptoms using the Dizziness Handicap Inventory, the participants were evaluated using the Hospital Anxiety and Depression Scale, Pittsburgh Sleep Quality Index, and Central Sensitization Inventory (CSI). A CSI score of 40 or higher was defined as central sensitization syndrome (CSS), and the severity of each condition in CSS and non-CSS participants was compared. We also evaluated the association between dizziness symptoms and central sensitization and coexisting symptoms using linear multiple regression analysis. Results We analyzed the data of 50 valid responses (valid response rate of 82.0 percent). Compared with the non-CSS group, the CSS group had a higher degree of disability owing to dizziness and a higher rate of complications of anxiety and depression. The regression analysis results showed that the severity of central sensitization was a related factor that could enhance the dizziness symptoms of PPPD. Conclusions Central sensitization may affect the dizziness symptoms of PPPD as an exacerbating factor.

13. Nontraumatic perforation of the small intestine caused by true primary enteroliths associated with radiation enteritis: a case report

Abstract Background True primary enterolithiasis is an uncommon condition, and nontraumatic perforation of the small intestine (NTPSI) is also an unusual entity. Therefore, NTPSI due to true primary enteroliths is an exceptionally rare complication. Moreover, enterolithiasis and radiation enteritis are also unique combinations. Herein, we present an exceedingly rare case of NTPSI induced by multiple true primary enteroliths associated with radiation enteritis. Case presentation A 92-year-old woman with acute abdominal pain was transferred to our hospital because a computed tomography (CT) scan performed by her family doctor revealed free air and fluid collection within her abdomen. Our initial diagnosis was upper gastrointestinal perforation, and we selected nonoperative management (NOM) with adnominal drainage. Although her general condition was stable, jejunal juice was drained continuously. Given that the CT performed 10 days after onset demonstrated perforation of the small intestine and adjacent concretion, we performed an emergency partial resection of the small intestine and jejunostomy. The resected bowel was 1 m in length and had many strictures that contained multiple enteroliths in their proximal lumens. The patient’s postoperative course was uneventful. The enteroliths were composed of deoxycholic acid (DCA). She was diagnosed with peritonitis due to NTPSI derived from multiple true primary enteroliths associated with radiation enteritis, as she had previously undergone hysterectomy and subsequent internal radiation therapy. Conclusions Clinicians should consider the rare entity of true primary enteroliths associated with radiation enteritis in NTPSI cases with unknown etiologies.

Abstract Background The change in the benzodiazepine (BZD) use of patients with medically unexplained symptoms (MUS) following the application of relaxation therapy were examined. Methods Of the 221 outpatients with MUS using BZD, 42 received relaxation therapy. Change in BZD use was compared using a relaxation group (n = 42) and a control group that had 84 MUS patients whose baseline was matched by optimal matching algorithms. Logistic regression analysis was done to evaluate the effect of BZD-dependent factors on the BZD dose of the relaxation group. Abstract Background Somatization is produced due to the summation of psychological factors, irrespective of the presence or absence of physical factors. A group of diseases with severe pain and other disorders exhibit so-called Medically Unexplained Symptoms (MUS), and the characteristics of patients with MUS are largely unexplained. In this paper, the characteristics of a series of new patients with somatization treated in a Japanese university hospital are discussed. Method The subjects were 871 patients who newly visited the Department of Psychosomatic Medicine, Toho University Omori Medical Center between January and December of 2015. Under the assumption that the definition of somatization is same as that of MUS, the correlation between somatization and the age, sex, academic background, chief complaints, reasons for visiting the medical center, diagnosis, symptoms, presence or absence of a referral form, continued treatment after the first visit, and marital status of these patients at the time of their respective examinations were evaluated. Results Of the patients studied, 68% suffered from somatization. Among them, 11% met the definition of Functional Somatic Symptoms (FSS) and 74% had somatization associated with mood disorder or anxiety disorder. Digestive symptoms were reported by 33%, headaches by 24%, and unusual sensations by 21%. A histopathological growth pattern (HGP) occurs at the interface between tumor cells and the surrounding liver parenchyma. Desmoplastic HGP (dHGP) is associated with a favorable prognosis and shows denser infiltration of lymphocytes than other HGPs. Tumor-infiltrating lymphocytes (TILs) exert antitumor immunity, nonetheless, their prognostic significance in patients with dHGP is unknown. This study aimed to identify the prognostic significance of HGP and TILs in colorectal liver metastasis (CRLM).The study analyzed 140 patients who underwent hepatectomy for CRLM. Depending on the type of HGP and TIL, the patients were categorized into four groups (dHGP/high TIL, dHGP/low TIL, non-dHGP/high TIL, and non-dHGP/low TIL) for a comparison of their recurrence-free survival (RFS) and overall survival (OS). Uni- and multivariate analyses were performed using a Cox proportional hazards model.The RFS and OS curves differed significantly between the groups. Medically unexplained symptoms (MUS) are common conditions that cause various somatic complaints and are often avoided in primary care. Fatigue frequently occurs in patients with MUS. However, the somatic and psychiatric symptoms associated with fatigue in patients with MUS are unknown. This study aimed to clarify the intensity of fatigue and the related somatic and psychiatric symptoms in patients with MUS.A total of 120 patients with MUS aged 20-64 years who visited the Department of Psychosomatic Medicine, Toho University Medical Center Omori Hospital, between January and March 2021 were considered. The participants' medical conditions were assessed using the Chalder Fatigue Scale (CFS), Somatic Symptom Scale-8 (SSS-8), and Hospital Anxiety and Depression Scale (HADS). We estimated the relationship between CFS, SSS-8 and HADS by using Spearman's rank correlation. Additionally, linear multiple regression analysis with CFS as the objective variable was used to identify symptoms related to fatigue.Fatigue was significantly associated with all symptoms observed (The intensity of anxiety, depression, headache, and dizziness were all associated with the intensity of fatigue in MUS patients. On the contrary, sleeping medication was associated with lower levels of fatigue in MUS.

Background In central sensitization syndrome (CSS), functional physical symptoms are severe due to central sensitization. The Somatic Symptom Scale-8 (SSS-8) is a simple tool to assess medical conditions with somatic symptoms and related disorders, but the cut-off point to identify severe cases of comorbid CSS is unknown. This study aimed to determine the SSS-8 score cut-off point for discriminating severe somatic symptoms and related conditions. Methods In total, 143 patients with somatic symptoms and related disorders attending outpatient clinics of a university hospital in Japan were included in the study. The participants were evaluated using the SSS-8 for somatic symptoms, Hospital Anxiety and Depression Scale (HADS) for anxiety and depressive symptoms, Pain Catastrophizing Scale (PCS) for catastrophic thoughts, and Central Sensitization Inventory (CSI-A, B) for CSS. Receiver operating characteristic (ROC) curve analysis was performed using the propensity score. The area under the curve (AUC) was calculated using a propensity score considering PCS, age, sex, HADS, and CSI-B as confounders of SSS-8 and CSS to evaluate differences in diagnostic accuracy between patients with and without SSS-8. The sensitivity and specificity of the ROC analysis were then used to determine the cut-off point for discriminating severe cases of SSS-8. Results Of the 143 participants, 126 responded (51 in the CSS group and 75 in the non-CSS group), with a valid response rate of 88.1 percent. In the ROC analysis, the propensity score including SSS-8 was statistically more accurate. The optimal SSS-8 score for discriminating severe cases of somatic symptoms and related disorders was at 13 points, with an AUC of 0.88, sensitivity of 84.3 percent, and specificity of 77.3 percent. Conclusions The SSS-8 is a useful tool for discriminating severe cases of somatic symptomatology and related disorders comorbid with CSS.

Hexagonal plate-like ZnO particles with a high degree of c-face orientation have been synthesized using hydrothermal method in the presence of various anionic surfactants bearing different hydrocarbon chains. The c-face of the ZnO particles increased upon increasing the surfactant alkyl chain length. The photocatalytic activity of the as-obtained hexagonal plate-like ZnO particles was evaluated using the degradation of methylene blue (MB). Although the specific surface area of hexagonal rod-like particles is higher than that of hexagonal plate-like particles, the amount of MB adsorption on the ZnO particle surface was different for the hexagonal plate-like and rod-like particles. In addition, the hexagonal plate-like ZnO particles showed a significantly higher decrease in the MB concentration with the duration of ultraviolet light irradiation when compared to the hexagonal rod-like ZnO particles obtained in the absence of a surfactant. These results indicate that crystal-face-controlled ZnO with a high degree of c-face orientation exhibits high photocatalytic activity.

In this study, we investigated the direct detection of DNA, without pretreatment, using a quartz crystal microbalance (QCM) sensor. This sensor is modified by a self-assembled monolayer of a thiol derivative that has an amino group as the terminal functional group. Contact angle values and the attenuated total reflectance Fourier transform infrared (ATR/FT-IR) spectra of the QCM sensors after immersion into an ethanol solution of thiol derivatives clearly showed that self-assembled monolayers of the derivatives were formed on the QCM sensors. Although QCM measurements using unmodified and carboxylic group-modified sensors could not detect DNA-Na salt, the sensor modified with amino groups could detect the DNA. This system can be used for the analysis of the interaction between DNA and DNAbinding proteins.

A 71-year-old man who underwent laparoscopic partial liver resection for local recurrence hepatocellular carcinoma (HCC)in segment 4 one year after percutaneous radiofrequency ablation(RFA)for HCC. About 3 years after treatment, the patient showed elevation of serum level of tumor marker and a mass lesion in the round ligament on CT and EOB-MRI. We made a diagnosis of peritoneal dissemination of HCC. Laparoscopic extirpation of peritoneal dissemination using indocyanine green(ICG)imaging was performed and no other tumors were observed in the peritoneal cavity. A lesion was diagnosed as peritoneal dissemination of HCC, and postoperative course was uneventful. This patient underwent repeated RFA and partial resection for recurrence of HCC. The patient was died for intrahepatic multiple recurrence of HCC without peritoneal dissemination 25 months after extirpation of peritoneal dissemination. In the field of hepatobiliary surgery, ICG imaging can be used for the intraoperative real-time visualization of hepatic malignancies. The ICG imaging is restricted to detection of fluorescence for liver tumors 5-10 mm from the liver surface. In the detection of peritoneal dissemination, however, there are no such limitations. Laparoscopic extirpation using ICG imaging is useful for the detection of peritoneal dissemination of HCC and may improve the prognosis in selected patients.

Zinc oxide (ZnO) particles were synthesized in the presence of anionic surfactants (ASs). The effect of ASs on the morphology of the ZnO particles was investigated by using ASs with various alkyl chain lengths and changing the molar ratio of AS/ZnSO4. Hexagonal plate-like ZnO particles were formed in the presence of ASs. Adsorption of the AS on the c face of the ZnO crystals inhibited (promoted) crystal growth along the c-axis (ab-axis) direction. Increasing the molar ratio of AS/ZnSO4 decreased the particle thickness, owing to the resulting increase in the coverage of the c face with AS. The particle diameter of the hexagonal plate-like ZnO particles (the diagonal length of the hexagonal plate) increased with increasing alkyl chain length of AS as a result of the van der Waals interactions between the alkyl chains. The data indicate that the particle diameter and thickness can be controlled by fine-tuning the van der Waals interactions between the alkyl chains and the coverage of the c face of the ZnO particles with AS.

Background The change in the benzodiazepine (BZD) use of patients with medically unexplained symptoms (MUS) following the application of relaxation therapy were examined. Methods Of the 221 outpatients with MUS using BZD, 42 received relaxation therapy. Change in BZD use was compared using a relaxation group (n = 42) and a control group that had 84 MUS patients whose baseline was matched by optimal matching algorithms. Logistic regression analysis was done to evaluate the effect of BZD-dependent factors on the BZD dose of the relaxation group. In this study, we synthesized ZnO particles using anionic amphiphiles as an additive. While the single-crystal particles prepared in the absence of such amphiphiles had a hexagonal rod-like shape, those fabricated using anionic amphiphilic molecules had a hexagonal plate-like shape. The anionic amphiphiles inhibited crystal growth in the c-axis direction of ZnO. This demonstrated that the anionic surfactants served as crystal-growth-directing agents, controlling the shape of the ZnO particles.

Hydroxyapatite (HAp) plates were successfully synthesized using dodecanedioic acid (DDDA). Following the preparation of an ethanolic solution of DDDA, solutions of calcium and phosphate were added in succession to produce a white slurry upon mixing. Subsequent processing of this slurry under hydrothermal conditions provided HAp plates of various sizes. Indeed, the plate size could be changed by varying the quantity of DDDA employed in the synthesis, with larger particles being obtained in the presence of larger quantities of DDDA. It was confirmed that a calcium–DDDA complex (Ca-DDD, calcium dodecanedioate) was formed upon mixing of the DDDA and calcium solutions. The observed morphological control could potentially be accounted for by the formation of Ca-DDD. Background Somatization is produced due to the summation of psychological factors, irrespective of the presence or absence of physical factors. A group of diseases with severe pain and other disorders exhibit so-called Medically Unexplained Symptoms (MUS), and the characteristics of patients with MUS are largely unexplained. In this paper, the characteristics of a series of new patients with somatization treated in a Japanese university hospital are discussed. Method The subjects were 871 patients who newly visited the Department of Psychosomatic Medicine, Toho University Omori Medical Center between January and December of 2015. Under the assumption that the definition of somatization is same as that of MUS, the correlation between somatization and the age, sex, academic background, chief complaints, reasons for visiting the medical center, diagnosis, symptoms, presence or absence of a referral form, continued treatment after the first visit, and marital status of these patients at the time of their respective examinations were evaluated. Results Of the patients studied, 68% suffered from somatization. Among them, 11% met the definition of Functional Somatic Symptoms (FSS) and 74% had somatization associated with mood disorder or anxiety disorder. Digestive symptoms were reported by 33%, headaches by 24%, and unusual sensations by 21%. Synthetic hydroxyapatite (HAp) is used clinically as a material for bone prostheses owing to its good bone-bonding ability; however, it does not contribute to bone remodeling. Carbonate-substituted hydroxyapatite (CAp) has greater bioresorption capacity than HAp while having similar bone-bonding potential, and is therefore considered as a next promising material for bone prostheses. However, the effects of the CAp instability on inflammatory and immune responses are unknown in detail. Here, we show that the surface layer of CAp is more hydrated than that of HAp and induces changes in the shape and function of macrophage-like cells. HAp and CAp were synthesized by wet method and molded into disks. The carbonate content of CAp disks was 6.2% as determined by Fourier transform (FT) infrared spectral analysis. Diffuse reflectance infrared FT analysis confirmed that physisorbed water and surface hydroxyl groups (OH- ) were increased whereas structural OH- was decreased on the CAp as compared to the HAp surface. The degree of hydroxylation in CAp was comparable to that in bone-apatite structures, and the CAp surface exhibited greater hydrophilicity and solubility than HAp. We investigated immune responses to these materials by culturing RAW264 cells (macrophage precursors) on their surfaces. Cell spreading on the CAp disk was suppressed and the secretion level of inflammatory cytokines was reduced as compared to cells grown on HAp. These results indicate that the greater surface hydration of CAp surface can attenuate adverse inflammatory responses to implanted bone prostheses composed of this material. © 2017 Wiley Periodicals, Inc. We studied the precipitation of calcium phosphate (CaP) in the presence or absence of polyacrylic acid (PAA) under double diffusion conditions in agar hydrogels. Waiting times and first precipitation positions were measured to analyze the CaP crystallization behavior, with the equivalency rule governing the precipitation in gel for a double diffusion process. We speculated that amorphous dicalcium phosphate (ADCP, CaHPO4·nH2O) nucleates at the first precipitation position via the reaction that the Ca2+ ion interacts with the HPO42– ion, subsequently transforming into octacalcium phosphate (OCP) according to the Ostwald rule of stages and finally being converted to the thermodynamically more stable hydroxyapatite (HAp) via dissolution. The irregularly shaped aggregates formed at an early stage of crystallization were a mixture of OCP and HAp. We observed a critical phenomenon of ferroelectric transition in hydroxyapatite (HAp) through measuring the thermally stimulated depolarization current of polycrystalline monoclinic HAp. Two peaks attributable to hydroxide (OH−) ion dipole reorientations were observed. The one was very sharp, the other displayed a broad peak ranging from 350 to 550 K. While the broad peak can be attributed to the diffusive reorientation motions of OH− ion dipoles, the sharp peak, related to the ferroelectric phase transition, implies the presence of ferroelectricity in HAp. The sharp peak was found near 380 K, the phase-transition temperature from the monoclinic to hexagonal is ascribed to a critical phenomenon caused by the ferroelectric behavior of OH− ion dipole reorientations.

Zinc oxide (ZnO) particles were synthesized in the presence of anionic surfactants (ASs). The effect of ASs on the morphology of the ZnO particles was investigated by using ASs with various alkyl chain lengths and changing the molar ratio of AS/ZnSO4. Hexagonal plate-like ZnO particles were formed in the presence of ASs. Adsorption of the AS on the c face of the ZnO crystals inhibited (promoted) crystal growth along the c-axis (ab-axis) direction. Increasing the molar ratio of AS/ZnSO4 decreased the particle thickness, owing to the resulting increase in the coverage of the c face with AS. The particle diameter of the hexagonal plate-like ZnO particles (the diagonal length of the hexagonal plate) increased with increasing alkyl chain length of AS as a result of the van der Waals interactions between the alkyl chains. The data indicate that the particle diameter and thickness can be controlled by fine-tuning the van der Waals interactions between the alkyl chains and the coverage of the c face of the ZnO particles with AS. In this study, we synthesized ZnO particles using anionic amphiphiles as an additive. While the single-crystal particles prepared in the absence of such amphiphiles had a hexagonal rod-like shape, those fabricated using anionic amphiphilic molecules had a hexagonal plate-like shape. The anionic amphiphiles inhibited crystal growth in the c-axis direction of ZnO. In this study, the formation and structure of a single-particle layer of organo-zinc oxide are investigated using surface-pressure-area (π-A) isotherms, out-of-plane X-ray diffraction (XRD) analysis, and atomic force microscopy (AFM). Further, techniques for achieving the solubilization of inorganic fine particles in general solvents have been proposed, and a single-particle layer has been formed using such an inorganic solution as a "spreading solution" for an interfacial film. Surface modification of ZnO is performed using a long-chain carboxylic acid. Accordingly, a regular arrangement of ZnO can be easily achieved in order to overcome the relatively weak van der Walls interactions between inorganic materials. A condensed Langmuir monolayer of these particles is also formed. A multiparticle layered structure is constructed by the Langmuir-Blodgett (LB) technique. Out-of-plane XRD measurement results for a single-particle layer of organo-ZnO clearly show a sharp peak at 42 A. This peak is attributed to the distance between ZnO layers. The AFM image of this single-particle layer of organo-ZnO shows a particle assembly with a uniform height of 60 nm. These aggregated particles form large two-dimensional crystals. In other words, a regular periodic structure along the c-axis and a condensed single-particle layer had been fabricated using Langmuir and LB techniques.

For appropriate road maintenance and management of the road, detection and evaluation of the asphalt distress is very important. Standard test method for asphalt pavement in Japan have defined that crack ratio, amount of rutting, and flatness are main damage indices for the evaluation. Though the amount of rutting and the flatness can be obtained automatically from the pavement condition survey vehicles, the crack ratio is not automatically derived. Since this process requires to draw and count cracks manually from the photo of the road surface taken by the vehicle, enormous effort and time are required. In addition, the accuracy of the evaluation is doubt because the manual method does not evaluate the important information including the crack width. To solve these problems, this paper have developed automated asphalt pavement crack detection method using image processing technique and naive Bayes based machine learning approach. The developed method is tested using the photos of various types of asphalt pavements, and it is found that the method can detect cracks with very high accuracy. アスファルト舗装のひび割れ損傷を定量的に評価する指標としてひび割れ率が定められている.このひび割れ率の算出にあたっては路面のひび割れをスケッチした後に区画内のひび割れの本数を数える必要があるが,手作業となるため膨大な労力と時間が必要となり,さらにはひび割れ開口幅などの重要な情報を得ることができないという問題がある.そこで本研究ではナイーブベイズ法による機械学習と画像解析を組み合わせ,撮影画像からひび割れを自動的に検出する手法を構築した.本手法は画素単位でひび割れを検出できるため,上述のひび割れ開口幅や面積などについても計算が容易である.そして本手法を複数箇所の密粒度アスファルトおよびポーラスアスファルト舗装の路面から撮影された画像に適用した実験により,本手法の高いひび割れ検出性能を確認した.

With an aim to grasp the damage progress on porous asphalt pavement, those data has been measured every other month using the high-accuracy pavement-condition measuring vehicle at the sites. Afterwards, it is confirmed that the cracks have progressed to potholes in a short time at the concerned place. Such the damages could not be detected and forecasted at their early stage with the traditional evaluation indicators (cracks, rutting, and surface roughness) and the conventional frequency of investigations. This paper is to propose (i) the new evaluation factors, the partial settlement values specific to porous asphalt pavement, based on the analysis of the high-accuracy surface profile, and (ii) the forecasting method of potholes occurrence, introducing the growth curve models for the damage progresses specific to the damage factors. 排水性舗装の損傷進行の把握を目的として,高精度な路面性状車両で,同一箇所の隔月計測を実施した結果,この場所では短期間でひび割れ発生からポットホールへ進展する事象を確認した.この様な損傷は従来の評価(ひび割れ,わだち,平坦性)指標や調査頻度では早期に発見,予測できない.本論文は,高精度で取得した路面の高さ情報を解析することで,排水性舗装特有の局所沈下量という新たな評価指標を提案するとともに,損傷要因に応じた損傷進行における成長曲線モデルを導くことで,ポットホール発生予測を提案するものである.
