1. Is a fan coil unit (FCU) an efficient cooling system for net-zero energy buildings (NZEBs) in tropical regions? An experimental study on thermal comfort and energy performance of an FCU

Abstract

In tropical regions, the selection of an efficient cooling system is a prerequisite for energy-efficient and net-zero energy buildings (NZEBs), while fan coil unit (FCU) systems are commonly used to provide thermal comfort (TC) of the interior environment throughout the entire year. In this study, to find a cooling solution for Malaysia's first net-zero building, the performance of a case study FCU-classroom was evaluated based on a one-month experimental assessment and a one-year simulation function. The precision of the measurements was computed using uncertainty analysis, and the simulation model was validated using measurement data. The impact of mechanical dehumidification on FCU performance was evaluated by analyzing the collected data in terms of cooling load allocation, cooling coil capacity, TC, and energy consumption. In order to guide the selection of a cooling system, a comparative analysis has been conducted between the performance of the FCU and two alternative systems, namely the redesigned FCU and the desiccant cooling system. This analysis was focused on evaluating the TC and energy use of these systems. The findings from the FCU performance analysis indicated that, in order to handle the cooling load in a classroom with an equal distribution of latent and sensible load (50:50 ratio), the FCU utilized 56 % of its total cooling coil capacity (13.06 kW) and 37 % of its energy consumption for the dehumidification process. The FCU utilized an average of 1360.3 kWh per month to maintain a comfortable room temperature of 24.3 °C and an uncomfortable relative humidity (RH) of 77.2 %. However, it was observed that the amount of moisture removal was insufficient to achieve a humidity comfort level in the room. Comparing alternative solutions to the FCU revealed that while a redesigned could provide TC, it consumed approximately 1.6 times more energy per month than the FCU. The desiccant cooling system provided TC with an average monthly energy savings of 37.8 % compared to the case study FCU.

Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). 15. Morphology and Late Pleistocene–Holocene sedimentation of the Strait of Istanbul (Bosphorus): a review

Abstract

The Bosphorus (Istanbul) Strait is a natural strait that connects the Black Sea with the Aegean Sea via the Sea of Marmara and the Dardanelles Strait. It is a 31 km-long and 3.5 km-wide winding channel, with an irregular bottom morphology. It has depressions up to 110 m deep, and two sills with depths of 35 and 58 m in the south and north, respectively. Presently, a two-layer water exchange exists through the strait, with the Mediterranean and Black Sea waters forming the lower and upper layers, respectively. The Bosphorus channel extends as shelf valleys on the Black Sea and Sea of Marmara shelves. However, it operated as a river valley or an estuary during the stadial lowstand periods. The infill sedimentary succession of the Bosphorus channel is up to 100 m thick above the Paleozoic–Cretaceous basement with an irregular topography. The oldest sediments are sandy to muddy fluvial–lacustrine facies of late Pleistocene age, which are preserved only in up to 160 m-deep scoured depressions of the basement. They are overlain by mid–late Holocene estuarine–marine shelly sandy to muddy sediments with patches of bioherms and shelly lag deposits. The Bosphorus outlet areas of the Black Sea and Sea of Marmara are characterized by a submarine fan and a shelf valley, respectively. The fan system in the Black Sea started depositing c. 900 years after the initial vigorous marine water incursion at c. 8.4 14 C ka BP. On the Marmara shelf, extension of the Bosphorus channel is a sinuous shelf valley with a channel–levee complex that was deposited by the Black Sea outflow during 11–10 14 C ka BP. Catastrophic floodings of the Sea of Marmara by torrential Black Sea outflows during the Greenland Interstadial melt-water pulses, as well as the strong Mediterranean current towards the Black Sea during the interglacial periods, were responsible for carving out the Bosphorus channel and the shelf valleys, as well as removing the sediments belonging to the earlier periods.

Abstract

The rapid spread of the global pandemic COVID-19 had challenged the Malaysian Healthcare and Education Systems. Teaching and learning at dental schools across the country were drastically changed to online approach as the nation enforced Movement Control Order (MCO). However, translation of psychomotor skills still require hands-on approach to produce competent dentists upon graduation, but most dental schools were not prepared for the prevention of COVID-19 transmission that require more strict cross infection protocol. The aim of this paper was to describe the response by a dental school in Malaysia to COVID-19 in modifying the clinical teaching and learning, infrastructure, facilities, and clinical services, and enhancing the infection control protocol through risk assessment of COVID-19 transmission and prevention strategies. The paper also discusses the limitations of our approach, challenges in providing clinical services and supervision as well as implications of changes to our practice. In conclusion, COVID-19 has modified the delivery and implementation of clinical teaching and learning in Malaysia. However, within the limited fund and resources, dental teaching institutions need to adapt to the new normal clinical environment in order to ensure that dentals students graduate on time with sufficient clinical training. © 2021 Penerbit Universiti Kebangsaan Malaysia. Abstract

This research concentrates on optimizing hydrogen production via a 1kW electrolyzer powered by renewable solar energy, specifically targeting efficiency enhancement through adjusting membrane area. The rising global demand for clean energy solutions has positioned hydrogen as a pivotal element in the shift toward sustainable energy systems. Nonetheless, the efficacy of hydrogen generation via electrolysis continues to pose a challenge, particularly at reduced scales. This research involved the creation of a comprehensive simulation model utilizing MATLAB Simulink to investigate the impact of membrane area variations on hydrogen production and overall system efficiency. The simulation results indicate that optimizing the membrane area can substantially enhance hydrogen production rates. The system was capable of producing up to 2.5 kg/h of hydrogen, signifying a substantial enhancement over traditional methods. Moreover, incorporating solar energy as the principal power source diminishes environmental repercussions while guaranteeing a sustainable and clean approach to hydrogen production. This research introduces an innovative method for optimizing small-scale electrolyzers, offering insights applicable to industrial hydrogen production and the integration of renewable energy. The findings facilitate the advancement of sustainable hydrogen production technologies, consistent with international initiatives to diminish dependence on fossil fuels.

Abstract

International audience; Submerged fault ruptures generate earthquake-triggered mass flow deposits, which are extensively used as a tool in subaqueous paleoseismology. In tectonically active deep sedimentary basins, such as the Sea of Marmara (SoM), these mass flow deposits are defined as turbidite-homogenite units (THUs), consisting essentially of a coarse basal part and an overlying homogeneous mud (homogenite). Detailed characterization of THUs is crucial in order to establish meaningful criteria to link these units with earthquakes events and to identify their transport routes and depositional mechanisms. Here, we combine μ-X-ray Fluorescence (μ-XRF), Anisotropy of Magnetic Susceptibility (AMS) and additional rock magnetism analyses of a 21-m long piston core from the Kumburgaz Basin of SoM to define the upper stratigraphic boundary of THUs with hemipelagic sediments and investigate the controls of hydrological changes on turbidite frequency and thickness over the last 15 kyrs BP. The sedimentary succession of this period includes a lower lacustrine and an upper marine unit with two Holocene sapropel intervals. The sequence is interrupted by a total of 70 THUs, characterized by a significant magnetic foliation related to the depositional setting rather than the magnetic signature. Magnetic mineralogy of the coarse basal parts of THUs have more ferromagnetic particles than the overlying homogenites and background sediments. While the homogenite parts have a more constant mineralogy than the basal parts, they do not differ the background sediments.Based on an event-free chronostratigraphic model derived from radiocarbon ages and the published age of lacustrine-marine (L-M) transition, the average THU occurrence intervals in the lacustrine (14.8–12.6 kyrs BP), lower sapropel (11.2–5.7 kyrs BP), upper sapropel (5.4–2.7 kyrs BP) and non-sapropelic part of the marine unit (2.7 kyrs BP-present) are 235 yrs., 287 yrs., 114 yrs. and 160 yrs. respectively. The average thickness of the THUs in the same units are 20.8 cm, 15.7 cm, 6.1 cm and 6.1 cm. The variability of average THU occurrence intervals and THU thicknesses are controlled by the sea level rise and salinity increase following the full marine connection of the SoM at 12.6 kyrs BP, which caused changes in slope stability, sediment composition and sediment deposition in different parts of the basin. Geomechanical properties of the lower sapropel appears to have been important in resulting long THU recurrence intervals and relatively high THU thicknesses.

Abstract

Immunotherapy has seen tremendous strides in the last decade, acquiring a prominent position at the forefront of cancer treatment since it has been proven to be efficacious for a wide variety of tumors. Nevertheless, while immunotherapy has changed the paradigm of adult tumor treatment, this progress has not yet been translated to the pediatric solid tumor population. For this reason, alternative curative therapies are urgently needed for the most aggressive pediatric tumors. In recent years, oncolytic virotherapy has consolidated as a feasible strategy for cancer treatment, not only for its tumor-specific effects and safety profile but also for its capacity to trigger an antitumor immune response. This review will summarize the current status of immunovirotherapy to treat cancer, focusing on pediatric solid malignancies. We will revisit previous basic, translational, and clinical research and discuss advances in overcoming the existing barriers and limitations to translate this promising therapeutic as an every-day cancer treatment for the pediatric and young adult populations.

Abstract

Background and Objective: Nowadays, the use of fiber post in weak teeth that had root canal treatment has gained acceptance worldwide. New materials and shapes of fiber posts were introduced. This study was conducted to compare in vitro bond strength among Bundle glass fiber posts, taper glass fiber post and short fiberreinforced composite as post material. Methods: Thirty Mandibular premolars were divided into three groups according to the post material that used: 1. Taper fiber post cemented with resin core10 samples. 2. Bundle fiber post cemented with resin core ten samples. 3. Short fiber reinforcement composite used instead of post and cement ten samples. Each root specimen cut into three slices of 2 mm thickness from each cervical, middle, and apical part of the posts. Push-out tests were performed using a universal testing machine at three sites in each root at a crosshead speed of 0.5 mm/min. Results: Micro push-out bond strength of the posts to dentin was not significantly affected by the type of post material (p > 0.05). Push-out bond strength values of cervical segments were significantly higher than the middle and apical segments in all groups (p < 0.05). Conclusion: The Bundle fiber post exhibited higher bond strength than other posts, but it was not significant, the highest bond strength was observed in the cervical third of all post spaces. Short fiber reinforced composite represented comparable bonding performance with taper Post and bundle fiber post when it was used instead of post material.

Abstract

Rapid changes in global shipping transportation introduce numerous technologies, among them known as “Automated Identification System (AIS)”. AIS is a transponder system that continuously transmits vessel information data between nearby vessels and shore stations via “Very High Frequency (VHF)” signals channels. In the recent past, a majority of the ports in the world use AIS to monitor and handle vessel activities within their jurisdiction. Nevertheless, there are integration issues with other information systems such as Geographic Information System (GIS) for marine and maritime environments. In regards to these issues, this study highlights the study of vessel charges estimation by using a geospatial approach for Port Kuantan, Pahang. The methodology involves tasks such as reviewing the current practice of vessel port charges estimation, undertaking geospatial analysis for vessel movement charges, estimating and validating charges for selected vessels. The result shows the information of vessel movement that is used to estimate specific vessel charges where finally this result is validated using Kuantan Port Consortium (KPC) charges system known as “Estimation Charges”. To conclude, the vessel charges estimation enables the integration between AIS and GIS in solving the problem of maritime transportation at port industries.

Abstract

Cardiac allograft vasculopathy (CAV) is an accelerated form of coronary artery disease that affects long-term outcomes in heart transplant (HTx) patients. We prospectively evaluated the feasibility of coronary computed tomography angiography (CCTA) for the detection of CAV during clinical implementation at our center. All consecutive HTx patients >4 years post-transplant were actively converted from myocardial perfusion imaging to CCTA for the annual assessment of CAV. Between February 2018 and May 2019, 129/172 (75%) HTx patients underwent a CCTA. Renal impairment (n = 21/43) was the most frequent reason for patients could not undergo CCTA. CCTA image quality was good–excellent in 118/129 (92%) patients, and the radiation dose was 2.1 (1.6–2.8) mSv. CCTA showed obstructive CAV in 19/129 (15%) patients. Thirteen (10%) patients underwent additional tests, of which 8 patients underwent coronary revascularization within 90 days of CCTA. After 1 year, 3 additional coronary angiograms were performed, resulting in one revascularization in a patient with known severe CAV who developed ventricular tachycardia. One myocardial infarction after coronary stenting and 2 non-cardiac deaths were observed. CCTA can be successfully implemented for routine detection of CAV with good image quality and low radiation dose. CCTA allows CAV evaluation with the limited need for additional invasive testing.

Abstract

Background: There is a clinical need for additional remote tools to improve left ventricular assist device (LVAD) patient management. The aim of this pilot concept study was to assess the safety and feasibility of optimizing patient management with add-on remote hemodynamic monitoring using the CardioMEMS in LVAD patients during different treatment stages. Methods: Ten consecutive patients accepted and clinically ready for (semi-) elective HeartMate 3 LVAD surgery were included. All patients received a CardioMEMS to optimize filling pressure before surgery. Patients were categorized into those with normal mean pulmonary artery pressure (mPAP) (≤25 mmHg, n = 4) or elevated mPAP (>25 mmHg, n = 6), and compared to a historical cohort (n = 20). Endpoints were CardioMEMS device safety and a combined endpoint of all-cause mortality, acute kidney injury, renal replacement therapy and/or right ventricular failure at 1-year follow-up. Additionally, we investigated hospital-free survival and improvement in quality of life (QoL) and exercise tolerance. Results: No safety issues or signal interferences were observed. The combined endpoint occurred in 60% of historical controls, 0% in normal and 83% in elevated mPAP group. Post-discharge, the hospital-free survival was significantly better, and the QoL improved more in the normal compared to the elevated mPAP group. Conclusion: Remote hemodynamic monitoring in LVAD patients is safe and feasible with the CardioMEMS, which could be used to identify patients at elevated risk of complications as well as optimize patient management remotely during the out-patient phase with less frequent hospitalizations. Larger pivotal studies are warranted to test the hypothesis generated from this concept study.

Abstract

Background: We aimed to assess differences in clinical characteristics, prognosis, and the temporal evolution of circulating biomarkers in male and female patients with HFrEF. Methods: We included 250 patients (66 women) with chronic heart failure (CHF) between 2011 and 2013 and performed trimonthly blood sampling during a median follow-up of 2.2 years [median (IQR) of 8 (5–10) urine and 9 (5–10) plasma samples per patient]. After completion of follow-up we measured 8 biomarkers. The primary endpoint (PE) was the composite of cardiac death, cardiac transplantation, left ventricular assist device implantation, and hospitalization due to acute or worsened CHF. Joint models were used to determine whether there were differences in the temporal patterns of the biomarkers between men and women as the PE approached. Results: A total of 66 patients reached the PE of which 52 (78.8%) were male and 14 (21.2%) were female. The temporal patterns of all studied biomarkers were associated with the PE, and overall showed disadvantageous changes as the PE approached. For NT-proBNP, HsTnT, and CRP, women showed higher levels over the entire follow-up duration and concomitant numerically higher hazard ratios [NT-proBNP: women: HR(95%CI) 7.57 (3.17–21.93), men: HR(95%CI) 3.14 (2.09–4.79), p for interaction = 0.104, HsTnT: women: HR(95%CI) 6.38 (2.18–22.46), men: HR(95%CI) 4.91 (2.58–9.39), p for interaction = 0.704, CRP: women: HR(95%CI) 7.48 (3.43–19.53), men: HR(95%CI) 3.29 [2.27–5.44], p for interaction = 0.106). In contrast, temporal patterns of glomerular and tubular renal markers showed similar associations with the PE in men and women. Conclusion: Although interaction terms are not statistically significant, the associations of temporal patterns of NT-proBNP, HsTnT, and CRP appear more outspoken in women than in men with HFrEF, whereas associations seem similar for temporal patterns of creatinine, eGFR, Cystatin C, KIM-1 and NAG. Abstract

Researchers are turning to nanofluids in PV/T hybrid systems for enhanced efficiency due to nanoparticle dispersion, improving thermal and optical properties over conventional fluids. Three different concentrations of formulated soybean oil based MXene nanofluids are considered 0.025, 0.075 and 0.125 wt.%. Maximum specific heat capacity nanofluids ( $c_{pNF}$ ) augmentation is 24.49% at 0.125 wt.% loading of Ti3C2 in the base oil. The calculation of the $c_{pNF}$ based on the temperature and nanoflakes concentration is very expensive and time-consuming as it should be calculated via the practical test investigation. This study employs a long short-term memory (LSTM) as an efficient machine learning method to extract the surrogate model for calculating the $c_{pNF}$ based on the temperature and nanoflakes concentration. In addition, a couple of other machines learning methods, including support vector regression (SVR), group method of data handling (GMDH), and multi-layer perceptron (MLP), are developed to prove the higher efficiency of the recently proposed LSTM model in the calculation of the $c_{pNF}$ . In addition, the Bayesian optimization technique is employed to calculate the optimal hyperparameters of the developed SVR, GMDH, MLP and LSTM to reach the highest efficiency of the system in predicting the $c_{pNF}$ based on temperature and nanoflakes concentration. Notably, 95% of the recorded data via differential scanning calorimetry (DSC) is used for training machine learning techniques. In comparison, 5% is used for testing and validation purposes of the developed algorithm. The newly proposed optimized SVR, GMDH, MLP, and LSTM are modelled in MATLAB software. The results show that the newly proposed optimized LSTM model can reduce the mean square error in calculating the $c_{pNF}$ by 99%, 99% and 91% compared with SVM, GMDH and MLP, respectively. The proposed methodology can be used to calculate other thermophysical properties of nanofluids.

Abstract

Patients with pelvic and abdominal cancers usually present with bowel obstruction, especially peritoneal, colorectal, and pancreatic carcinomatosis. A poor prognosis has been reported for patients that suffer from gastrointestinal bowel obstruction secondary to later stage carcinomatosis, although maximal treatment approaches might have been administered. In this context, these patients are suggested to survive for only a few weeks to months, and parenteral nutrition did not enhance the outcomes in these situations. Medical treatment includes the administration of corticosteroids, opioids, anticholinergics, octreotide, and anti-emetics, while surgical outcomes might be more efficacious with more favorable clinical outcomes. However, these operations have been reported with multiple complications that might worsen the prognosis. Stent application is another non-surgical modality with fewer adverse events. Nevertheless, evidence regarding its superiority over the surgical approaches is conflicting among the different studies in the literature. Accordingly, further investigations are still needed for adequate validation.

The low understanding of students on fractional topic has hampered the development of mathematics learning in Indonesia. This study was conducted to investigate in depth the ability of prospective teachers in developing the problem. Thirty four mathematics prospective teachers were asked to ask about contextual word problem whose solution could be obtained from counting 4 5 and 2 3 . Data consists of a verbal protocol together with written notes made by the subjects. Qualitative analysis of the obtained data is focused on identifying the resulting problem structure and the type of context commonly used. The results showed that the most widely used contexts in the proposed story problem were the distribution of the cake, the weight of the object, the lots of the object, and the length of measurement. The structure of contextual problem produced by mathematicis prospective teacher has weaknesses in two components, namely: 1) in proposing appropriate contexts; and 2) in providing sufficient information so that the solution of the proposed problem can be obtained by applying the arithmetic concept of 4 5 plus 2 3 , These results suggest new findings about prospective teacher difficulties in conceptualizing fractions.

Mineralogical and geochemical analyses of samples collected from piston core HZ11-P01 at the western margin of Lake Hazar were studied to determine the provenance and weathering conditions on the source area together with tectonic setting of the source rocks, paleoclimate of the region, and paleo-redox conditions of the lake sediments. Nonclay minerals in the sediment core are represented by feldspar, quartz, dolomite, and calcite. The clay mineral assemblage consists of smectite/chlorite mixed-layer clay, chlorite, and illite. Most major, trace, and rare earth element contents of the lake samples are generally similar to those of the catchment area. Chondrite-normalized rare earth element patterns of the lake samples are characterized by enrichment of light rare earth elements, a relatively flat heavy rare earth element pattern, and no Eu negative anomaly. The chemical index of alteration and index of compositional variability of the core sediment suggested that the intensity of weathering in the source area was low to moderate. The paleoclimatic indicator (C-values) showed that a paleoclimate changing from semiarid to semimoist prevailed during the last similar to 2 ka BP. Authigenic U and element ratios such as Th/U, Ni/Co, Cu/Zn, V/Cr, Eu/Eu*, and Ce/Ce* indicate that the lake sediments were deposited under oxic conditions. La/Sc, Co/ th, Cr/Th, Zr/Sc, and Th/Sc element ratios in the core sediments are consistent with values of sediments derived from mainly mafic and intermediate source rocks. According to discriminant-function diagrams, lake samples are plotted within the arc setting and arc-continent collision, which is in accordance with the geology of the study area.

Abstract

Objectives: To quantify the availability of authorised, age-appropriate paediatric medicines in clinical practice in the Netherlands and to identify gaps by assessing dispensing practice in a paediatric hospital. Methods: The availability of age-appropriate formulations was assessed by conducting a survey on the use of pharmacy compounded medicines among the paediatric hospitals in the Netherlands, and by analysing dispensing data of oral medication from the inpatient pharmacy of the largest paediatric hospital in the Netherlands. The age-appropriateness of the dispensed formulations was assessed on two aspects: dose-capability and acceptability. Liquid drug products that are unsuitable due to the presence of potentially harmful excipients, were identified based on the dosage in clinical practice. Results: For 129 out of 139 drug substances included in the survey (93%), at least one of the eight respondents stated to use a pharmacy compounded product to meet the needs of their paediatric patients. The age-appropriateness of medicines dispensed from the inpatient pharmacy increased with age, and was higher for non-intensive care unit (ICU) patients than for ICU patients. Abstract

Feature extraction is a technique to extract derived value which is informative and non-redundant. There are many techniques have been reported in the literature on its usage application on signal and image. Some of the common techniques of feature extraction are Fast Fourier Transform, Wavelet Transform, Histogram of Oriented Gradients (HOG), Speeded Up Robust Features (SURF), Local Binary Patterns (LBP), Haar Wavelets, and Color Histograms. Recently, a new feature extraction technique based on Pulse Width Modulation (PWM) has been proposed. PWM is widely used for speed control technique and for the application in feature extraction. PDT is a feature extraction technique that derived from PWM. Initially, PDT applies on electrocardiogram (ECG) signal and draw out a non-invertible output. This output is presented as pulse waveform which is generated by the amplitude’s comparison of ECG signal with a periodic triangular waveform to get the intersection points. The time location of the intersection points are then used to be the transition state for output pulse to raise, fall or preserve the status quo. Since the PDT concept is only applied to 1D signal, it is interesting if the technique can be extending to 2D signal. This work presents a 2D PDT feature extraction technique. The concept of 2D PDT also based on the principle of PWM which is to process the incoming image (2D signal) with the aid of a 2D triangular waveform to form an output of 2D pulse waveform. Furthermore, the 2D triangular waveform is set up based on the desired threshold for its frequency and amplitude, namely, these thresholds are modulated in accordance with the size of the image. Afterward, the superposition process is proceeding between the processed image and modulated 2D triangular waveform to generate the intersection points for further forming of the output of 2D pulse waveform.
