3. Kodály's Sonata for Solo Cello Op.8 and Hungarian nationalism

Abstract

Zoltán Kodály's Sonata for Solo Cello Op.8 (1915), a landmark of the cello repertoire, grew out of an environment that was artistically progressive and culturally nationalistic. It is a remarkably innovative work in several respects. While not an explicitly nationalistic work (title, programme, etc.), an extensive web of connections links it deeply to Kodály's activities in folksong research and folk instrument performance and his particular brand of Hungarian nationalism. Most writings on the Solo Sonata neglect this aspect of the composition, but this thesis argues that it is essential for its understanding and, above all, for its performance. The Hungarian nationalist dimension of the work may be realised in performance more or less clearly, or not at all. A Hungarian tradition of playing, represented principally by the cellists Janos Starker (1924-2013) and Miklós Perényi (born 1948), do realise this dimension, although many other performances and recordings do not, reducing it to a bland, 'international modernism'. The thesis reconstructs the Solo Sonata's links to its progressive/nationalistic world and the recital provides a 'Hungarian' interpretation.

4. An investigation into the relationship between uncertainty and negative affect in individuals with Autism Spectrum Disorder

People who find it especially hard to cope with the unexpected or unknown are said to have an intolerance of uncertainty (IoU). Individuals with autism often report a preference for certainty and experience levels of anxiety that can interfere with their daily life. Understanding more about the association between IoU and anxiety in people with autism might lead to better treatments being developed. Therefore, a systematic review and meta-analysis was conducted of research examining the correlation between IoU and anxiety in people with autism. Twelve studies were found and their results compared and contrasted. In general, people with autism reported very high levels of anxiety and IoU. Of the ten studies that used relevant statistics, nine found a statistically-significant correlation between anxiety and IoU. The strength of the association was broadly equivalent to that reported by research conducted with people who do not have autism. A person's age and gender did not change the strength of the association, but it appeared slightly stronger in people with autism who scored higher on intelligence tests. There were limitations with this part of the thesis and these are discussed, together with the implications of this work. Some adults report being dissatisfied with the assessment process they went through when they were diagnosed with autism; particularly because they felt stressed and anxious due to not knowing what to expect beforehand. Therefore, in the second part of this thesis, an intervention was created to address this. The intervention was essentially a short story (with accompanying photographs), describing what it was like to attend an assessment. It was written in a way that research has suggested is helpful for people with autism. Interventions of this type are known as Social Stories. The people who took part were adults awaiting an assessment at one of two NHS services in the UK. They were divided at random into two groups; those in the first group read a standard leaflet about what to expect, whereas those in the second group read both the Social Story and the leaflet. People in both groups completed questionnaires at home and on arrival at their assessment. It was expected most would experience an increase in unpleasant emotions (such as anxiety, fear and frustration) on arrival at their assessment, compared to how they felt at home. The results showed that, on average, people who read the Social Story reported significantly less of an increase in unpleasant emotions than those who only read the leaflet. This suggested the intervention was effective. However, the results from a different questionnaire suggested that, in general, the Social Story and the leaflet were equally effective at helping people know what to expect. People in both groups were also equally satisfied with the assessment. The limitations and implications associated with this study are discussed.

5. The Relationship between Intolerance of Uncertainty and Anxiety in Autism: A Systematic Literature Review and Meta-Analysis

Abstract

The association between intolerance of uncertainty and anxiety has proved robust in neurotypical populations and has led to effective interventions targeting intolerance of uncertainty. The aim of this systematic review and meta-analysis was to investigate this association in autistic people, given the high prevalence of anxiety in this population and the limited effectiveness of therapies used currently to treat anxiety in autism. A protocol was published on the Prospero database (CRD42019125315), and electronic databases were searched using terms related to intolerance of uncertainty, anxiety and autism. Included in the systematic review were 12 studies, of which 10 were included in a meta-analysis. Results showed that anxiety and intolerance of uncertainty were consistently elevated in autistic participants. Examining the correlation between these two constructs, the meta-analysis revealed a large sample-weighted effect size, r=0.62, 95% confidence interval=[0.52, 0.71], p<0.001. The strength of this association was comparable to meta-analyses conducted on neurotypical populations, and therefore, it was concluded intolerance of uncertainty may be an appropriate target for intervention for autistic individuals. However, conclusions were limited due to the small number of relevant studies that were available and due to issues with methodological quality.

Abstract

To summarize the research on the effectiveness of virtual reality (VR) therapy for the management of phantom limb pain (PLP). Moreover, combining VR with tactile stimulation had a larger beneficial effect on PLP compared with VR alone. Based on the current literature, VR therapy has the potential to be an effective treatment modality for the management of PLP. However, the low quality of studies, heterogeneity in subject population and intervention type, and lack of data on long-term relief make it difficult to draw definitive conclusions. Abstract

BACKGROUND: Although a delay of 24 hours for hip fracture repair is associated with medical complications and costs, it is unknown how long patients wait for surgery for hip fracture. We describe novel methods for measuring exact urgent and emergent surgical wait times (in hours) and the factors that influence them. METHODS: Adults aged 45 years and older who underwent surgery for hip fracture (the most common urgently performed procedure) in Ontario, Canada, between 2009 and 2014 were eligible. Validated data from linked health administrative databases were used. The primary outcome was the time elapsed from hospital arrival recorded in the National Ambulatory Care Reporting System until the time of surgery recorded in the Discharge Abstract Database (in hours). The influence of patient, physician and hospital factors on wait times was investigated using 3-level, hierarchical linear regression models. RESULTS: Among 42 230 patients with hip fracture, the mean (SD) wait time for surgery was 38.76 (28.84) hours, and 14174 (33.5%) patients underwent surgery within 24 hours. Variables strongly associated with delay included time for hospital transfer (adjusted increase of 26.23 h, 95% CI 25.38 to 27.01) and time for preoperative echocardiography (adjusted increase of 18.56 h, 95% CI 17.73 to 19.38). More than half of the hospitals (37 of 72, 51.4%), compared with 4.8% of surgeons and 0.2% of anesthesiologists, showed significant differences in the risk-adjusted likelihood of delayed surgery. INTERPRETATION: Exact wait times for urgent and emergent surgery can be measured using Canada's administrative data. Only one-third of patients received surgery within the safe time frame (24 h). 49. Timing of femoral shaft fracture fixation following major trauma: A retrospective cohort study of United States trauma centers

Abstract

Background Femoral shaft fractures are common in major trauma. Early definitive fixation, within 24 hours, is feasible in most patients and is associated with improved outcomes. Nonetheless, variability might exist between trauma centers in timeliness of fixation. Such variability could impact outcomes and would therefore represent a target for quality improvement. We evaluated variability in delayed fixation ([greater than or equal to]24 hours) between trauma centers participating in the American College of Surgeons (ACS) Trauma Quality Improvement Program (TQIP) and measured the resultant association with important clinical outcomes at the hospital level. Methods and findings A retrospective cohort study was performed using data derived from the ACS TQIP database. Adults with severe injury who underwent definitive fixation of a femoral shaft fracture at a level I or II trauma center participating in ACS TQIP (2012-2015) were included. Patient baseline and injury characteristics that might affect timing of fixation were considered. A hierarchical logistic regression model was used to identify predictors of delayed fixation. Hospital variability in delayed fixation was measured using 2 approaches. First, the random effects output of the hierarchical model was used to identify outlier hospitals where the odds of delayed fixation were significantly higher or lower than average. Second, the median odds ratio (MOR) was calculated to quantify heterogeneity in delayed fixation between hospitals. Finally, complications (pulmonary embolism, deep vein thrombosis, acute respiratory distress syndrome, pneumonia, decubitus ulcer, and death) and hospital length of stay were compared across quartiles of risk-adjusted delayed fixation. We identified 17,993 patients who underwent definitive fixation at 216 trauma centers. The median injury severity score (ISS) was 13 (interquartile range [IQR] 9-22). Median time to fixation was 15 hours (IQR 7-24 hours) and delayed fixation was performed in 26% of patients. After adjusting for patient characteristics, 57 hospitals (26%) were identified as outliers, reflecting significant practice variation unexplained by patient case mix. The MOR was 1.84, reflecting heterogeneity in delayed fixation across centers. Compared to hospitals in the lowest quartile of delayed fixation, patients treated at hospitals in the highest quartile of delayed fixation suffered 2-fold higher rates of pulmonary embolism (2.6% versus 1.3%; rate ratio [RR] 2.0; 95% CI 1.2-3.2; P = 0.005) and required greater length of stay (7 versus 6 days; RR 1.15; 95% CI 1.1-1.19; P < 0.001). There was no significant difference with respect to mortality (1.3% versus 0.8%; RR 1.6; 95% CI 1.0-2.8; P = 0.066). The main limitations of this study include the inability to classify fractures by severity, challenges related to the heterogeneity of the study population, and the potential for residual confounding due to unmeasured factors. Conclusions In this large cohort study of 216 trauma centers, significant practice variability was observed in delayed fixation of femoral shaft fractures, which could not be explained by differences in patient case mix. Patients treated at centers where delayed fixation was most common were at significantly greater risk of pulmonary embolism and required longer hospital stay. Trauma centers should strive to minimize delays in fixation, and quality improvement initiatives should emphasize this recommendation in best practice guidelines., Author(s): James P.
