Background: The radial artery is commonly accessed for arterial blood sampling, invasive blood pressure monitoring, and vascular access for cardiac catheterization. Iatrogenic radial artery injury is a rare complication with potentially devastating outcomes. The purpose of our study was to identify the timing of these injuries and define a treatment algorithm. Methods: A retrospective chart review of all patients with iatrogenic radial artery injuries were identified between the years 2008 and 2018. Patient demographics, mechanism of injury, interventions, and outcomes were recorded. Results: A total of 18 patients were identified with iatrogenic radial artery injury over a 10-year period. Fifty percent of these resulted from arterial line cannulation, and 50% occurred after transradial cardiac catheterization. Thirty-three percent resulted in radial artery pseudoaneurysm (RAP), and 66% had acute radial artery thrombosis (RAT). Eleven of the 18 patients underwent operative intervention. Of the 12 patients with RAT, 4 were treated with systemic anticoagulation for 3 months. All patients with RAP who were surgically treated had resolution of symptoms on follow-up evaluation. Of the patients with RAT, 2 had persistent sensorimotor deficits after treatment, and 1 patient had multiple necrotic fingers requiring amputation. Conclusion: Radial artery injuries are an uncommon but potentially devastating complication of common invasive procedures resulting in thrombosis, pseudoaneurysm, or overt hand ischemia. In the current study, we sought to examine whether patients incorporate the identity of a patient receiving autologous stem cell transplant (ASCT) for multiple myeloma (MM) into their daily lives. Multiple myeloma patients receive education before initiating the ASCT treatment process. In this ethnographic study using interpretative phenomenological analysis, we observed pretransplant education visits with 30 MM patients, followed by semistructured interviews in their hospital rooms during transplant. The experience of receiving ASCT for MM required effort by patients to not only maintain their past identity but also establish a new patient identity. Reconciling these 2 identities required deliberate and emotionally draining effort from the patient. Results were organized into 2 overarching themes of social relationships and aesthetics with subthemes for each. Understanding methods MM patients who are receiving ASCT use to negotiate normalcy during treatment may be helpful for developing interventions for alleviating distress during this difficult time.

Alzheimer's disease (AD) is the most common cause of dementia worldwide, but the molecular and cellular mechanisms underlying cognitive impairment remain poorly understood. To address this, we generated a single-cell transcriptomic atlas of the aged human prefrontal cortex covering 2.3 million cells from postmortem human brain samples of 427 individuals with varying degrees of AD pathology and cognitive impairment. Our analyses identified AD-pathology-associated alterations shared between excitatory neuron subtypes, revealed a coordinated increase of the cohesin complex and DNA damage response factors in excitatory neurons and in oligodendrocytes, and uncovered genes and pathways associated with high cognitive function, dementia, and resilience to AD pathology. Although murine models of coronary atherosclerotic disease have been used extensively to determine mechanisms, limited new therapeutic options have emerged. Pigs with familial hypercholesterolemia (FH pigs) develop complex coronary atheromas that are almost identical to human lesions. We reported previously that insulin-like growth factor 1 (IGF-1) reduced aortic atherosclerosis and promoted features of stable plaque in a murine model. We administered human recombinant IGF-1 or saline (control) in atherosclerotic FH pigs for 6 months. IGF-1 decreased relative coronary atheroma in vivo (intravascular ultrasound) and reduced lesion cross-sectional area (postmortem histology). IGF-1 increased plaque's fibrous cap thickness, and reduced necrotic core, macrophage content, and cell apoptosis, consistent with promotion of a stable plaque phenotype. IGF-1 reduced circulating triglycerides, markers of systemic oxidative stress, and CXCL12 chemokine levels. We used spatial transcriptomics (ST) to identify global transcriptome changes in advanced plaque compartments and to obtain mechanistic insights into IGF-1 effects. ST analysis showed that IGF-1 suppressed FOS/FOSB factors and gene expression of MMP9 and CXCL14 in plaque macrophages, suggesting possible involvement of these molecules in IGF-1's effect on atherosclerosis. Thus, IGF-1 reduced coronary plaque burden and promoted features of stable plaque in a pig model, providing support for consideration of clinical trials.

Composite polymers have become widely used in industries such as the aerospace, automobile, and civil construction industries. Continuous monitoring is essential to optimize the composite components' performance and durability. This paper describes the concept of a distributed fiber optic smart textile (DFOST) embedded into a composite panel that can be implemented during the fabrication process of bridges, planes, or vehicles without damaging the integrity of the composite. The smart textile used an embroidery method to create DFOST for easy installation between composite laminates. It also allows different layout patterns to provide two- or three-dimensional measurements. The DFOST system can then measure strain, temperature, and displacement changes, providing critical information for structural assessment. The DFOST was interrogated by using an optical frequency domain reflectometry (OFDR). It could measure strain variation during the dynamic and static test with a spatial resolution of 2 mm and a minimum strain resolution of 10 μϵ. This paper focuses on the study of strain measurement.

Each year in the United States, unwanted horses may become neglected, starved, or abandoned. Recovery therapies include refeeding and veterinary care, often requiring substantial time and financial investments. To better understand the likelihood for starved horses to successfully survive the first 100 days after initial evaluation, a retrospective case series was performed using hospital records of starved horses at the University of Tennessee College of Veterinary Medicine. A body condition score (BCS) of 3 or less and a malnourished diagnosis were utilized to select case records from an 11-year period (n = 82). Animal descriptors, vital signs, complete blood count data, blood chemistry panels, parasite load, and refeeding diet information were analyzed as available within the case record. Mixed model analyses of variance (PROC GLIMMIX, SAS 9.4) were utilized to test if response variables differed in horses that lived or died. Generalized linear mixed models were used to test factors associated with survival probability 100 days postinitial hospitalization. Body temperature (P = .05) and BCS (P = .0002) were significantly lower in horses that died compared with horses that survived. In addition, white blood cells were increased in horses that died (11.6 ± 1.3 cells/μL) compared with horses that survived (8.6 ± 0.5 cells/μL; P = .03). The initial BCS was associated with probability for survival, where survival likelihood increased 14.6 times for each 1 whole unit increase in the BCS. The health and condition of the world's reefs are in steep decline. This has triggered the development of fledgling micro-scale coral reef restoration projects along many reef coastlines. However, it is increasingly recognised that the scale and productivity of micro-scale coral gardening projects will be insufficient to meet the growing global threats to reefs. More recently, efforts to develop and implement restoration techniques for application at regional scales have been pursued by research organisations. Coral reefs are mostly located in the unindustrialised world. Yet, most of the funding, and scientific and engineering method development for larger-scale methods will likely be sourced and created in the industrialised world. Therefore, the development of the emerging at-scale global reef restoration sector will inevitably involve the transfer of methods, approaches, finances, labour and skills from the industrialised world to the unindustrialised world. This opens the door to the industrialised world negatively impacting the unindustrialised world and, in some cases, First Nations peoples. In Western scientific parlance, ecological imperialism occurs when people from industrialised nations seek to recreate environments and ecosystems in unindustrialised nations that are familiar and comfortable to them. How a coral reef 'should' look depends on one's background and perspective. While predominately Western scientific approaches provide guidance on the ecological principles for reef restoration, these methods might not be applicable in every scenario in unindustrialised nations. Imposing such views on Indigenous coastal communities without the local technical and leadership resources to scale-up restoration of their reefs can lead to unwanted consequences. Case: This case report describes a 46-year-old woman undergoing right-sided L5 to S1 decompression who received liposomal bupivacaine (LB) for postoperative analgesia and developed unintentional epidural anesthesia with symptoms mimicking cauda equina syndrome. The patient's symptoms resolved 72 hours postoperatively, approximately the length that LB typically lasts. At the 16-month follow-up, the patient demonstrated complete neurological function with no lower extremity strength or sensation deficits., Conclusions: Tracking of LB into the epidural space after lumbar surgery may cause transient epidural anesthesia with symptoms that mimic cauda equina syndrome.

Human studies demonstrate that physical activity reduces both morbidity and mortality of coronary heart disease (CHD) including decreased progression and/or regression of CHD with life-style modification which includes exercise. However, evidence supporting an intrinsic, direct effect of exercise in attenuating the development of CHD is equivocal. One limitation has been the lack of a large animal model with clinically evident CHD disease. Thus, we examined the role of endurance exercise in CHD development in a swine model of familial hypercholesterolemia (FH) that exhibits robust, complex atherosclerosis. FH swine were randomly assigned to either sedentary (Sed) or exercise trained (Ex) groups. At 10 months of age, Ex pigs began a 10 months, moderate-intensity treadmill-training intervention. At 14 months, all pigs were switched to a high-fat, high-cholesterol diet. CHD was assessed by intravascular ultrasound (IVUS) both prior to and after completion of 6 months on the HFC diet. Prior to HFC diet, Ex resulted in a greater coronary artery size in the proximal and mid sections of the LCX compared to SED, with no effect in the LAD. After 6 months on HFC diet, there was a 5-6 fold increase in absolute plaque volume in all segments of the LCX and LAD in both groups. At 20 months, there was no difference in vessel volume, lumen volume, absolute or relative plaque volume in either the LCX or LAD between Sed and Ex animals. A new low-flow-rate (0.5L/min) single-nozzle impactor for the concentration of dilute aerosol particles with selected pharmaceutical applications is described in this paper. The impactor can be configured up to 11 stages with a wide range of cutoff diameters from 0.6μm to 21.1μm, enabling convenient sampling of inhalable drug particles from inhalation devices and drug production processes. Its unique single-nozzle design and removable impaction plate allow direct sample transfer for subsequent compositional, morphological, solid-state, and other analysis. Agreement between the measured size distribution of fluticasone propionate particles actuated from commercial pMDI Flixotide ® 250 Evohaler ® and reported data in the literature verified that the impactor stages have accurate cutoff diameters as designed. The multi-stage configuration of the impactor allows rapid separation of polydisperse aerosol particles into different size classes for further characterization. Overlapping of the Raman spectra of the double-component powders from the Seretide ® 250 pMDI collected using two different methods demonstrated the applicability of the impactor for a representative sampling of multi-component aerosol particles for bulk composition analysis. A time-dependent and size-dependent stability study was conducted consuming only a single sample canister with 80mg of amorphous indomethacin particles suspended in HFA-134a. It was found that amorphous indomethacin particles converted to the γ crystalline polymorph upon storage at 45°C and that the crystallization rate is strongly size dependent. With its highly effective aerosol collection capability and accurate cutoff diameters for aerosol classification, the impactor will have various applications in the pharmaceutical industry., (Copyright © 2017 Elsevier B.V. Currently vascular repairs are treated using synthetic or biologic patches, however these patches have an array of complications, including calcification, rupture, re-stenosis, and intimal hyperplasia. An active patch material composed of decellularized tissue conjugated to gold nanoparticles (AuNPs) was developed and the long term biocompatibility and cellular integration was investigated. Porcine abdominal aortic tissue was decellularized and conjugated with 100 nm gold nanoparticles (AuNP). These patches were placed over a longitudinal arteriotomy of the thoracic aorta in six pigs. The animals were monitored for six months. Gross, histological, and immunohistochemical analyses of the patches were performed after euthanasia. Grossly there was minimal scar tissue with the patches still visible on the outer surface of the vessel. The inner lumen was smooth with a seamless transition from patch to native tissue. Histology demonstrated infiltration of host cells into the patch material. The immunohistochemical results demonstrated an endothelial cell layer forming over the patch within the vessel. Smooth muscle cells were repopulating the biomaterial in all animals. These results demonstrated that the AuNP biomaterial patch integrated well with the host tissue and did not failed over the six month implantation time., (Copyright © 2015 Elsevier Ltd.
