1. Irregular breakfast eating in type 2 diabetes mellitus is associated with greater social jetlag and poorer metabolic health.

Abstract

Summary Circadian disruption, arising from conflict between internal circadian time and behavioural sleep–wake and fasting–feeding rhythms, may contribute to the prevalence of type 2 diabetes mellitus and disease severity. Previous studies have demonstrated a link between irregular breakfast eating and poorer metabolic health. We aimed to further explore the relationships between breakfast habits, circadian misalignment (social jetlag), and metabolic parameters in a cohort of adult participants with type 2 diabetes mellitus. A total of 330 adult participants with type 2 diabetes mellitus attending for routine clinical review completed structured questionnaires to assess habitual sleep timing, chronotype, and social jetlag. Statistical analysis was via inferential groupwise approaches and path analysis to establish interdependencies of effects of social jetlag, chronotype, and breakfast eating regularity on HbA1c. 22.7% of the participants reported eating breakfast five times or fewer a week, and were categorised as irregular breakfast eaters. Compared with those who ate breakfast six or seven times a week, irregular breakfast eaters had significantly higher HbA1c and diastolic blood pressure, were younger and had greater social jetlag. In the path analysis, irregular breakfast eating exerted a direct effect on HbA1c, whilst social jetlag exerted only an indirect effect on HbA1c through breakfast eating regularity. Chronotype did not exert any effect on HbA1c, but did exert an indirect effect on breakfast eating regularity via social jetlag. Our results showed that adult participants with type 2 diabetes mellitus, who ate breakfast irregularly had poorer metabolic health and greater social jetlag. 6. A Case Control Study Exploring the Association Between Type 2 Diabetes Mellitus and Social Jetlag

Abstract

The circadian system, a hierarchical network of oscillating molecular clocks which align behaviours to the external solar clock and synchronise complimentary physiological processes between disparate tissues, plays an important role in regulating metabolic processes relevant the pathogenesis of Type 2 Diabetes Mellitus. Disruption to the circadian system, in the form of rotating shift work, is a risk factor for developing metabolic disease including Type 2 Diabetes Mellitus and obesity. Superseding circadian preferred sleep-wake timing to meet social obligations such as employment or education represents a more subtle, but much more prevalent, form of circadian disruption. Such behaviour results in significantly different sleep-wake times on free days compared to non-free days; a phenomenon termed social jetlag. We hypothesise that social jetlag is a risk factor for Type 2 Diabetes Mellitus and as such will be more prevalent in subjects with Type 2 Diabetes Mellitus compared to matched normoglycaemic controls. To test this hypothesis, we completed a case control study exploring the association between diabetes and social jetlag/sleep timing variability. Metabolic parameters were examined by means of a 75g oral glucose tolerance test, social jetlag was determined by means of the Munich Chronotype Questionnaire, and objective sleep parameters were obtained by means of wrist-worn actigraphy. Here we report that subjects with Type 2 Diabetes Mellitus demonstrate an excess of social jetlag compared to age- and gender-matched normoglycaemic controls (0.96 hours vs 0.61 hours, p = 0.035), which remains significant after adjusting for BMI and other covariates. However, this observation was based on self-reported sleep times and was not associated with any difference in objective actigraphy measurements. Further studies will be needed to confirm this association, to assess for causality, and to determine if treatment strategies to mitigate circadian disruption are effective in treating or preventing metabolic disease.

9. Greater Social Jetlag Associates with Higher HbA1c in Adults with Type 2 Diabetes: A Cross Sectional Study

Abstract

Background/Objectives: Later chronotype has been associated with poorer glycemic control in type 2 diabetes. It is unclear whether this is a direct relationship, or if personality factors or social jetlag ([SJL], ie, chronic circadian misalignment reflecting the discrepancy between the entrained phase of the circadian clock and socially-determined behavioural cycles) play a role. This study aimed to determine the relationships among chronotype, SJL, personality factors and glycemic control in type 2 diabetes, independently of sleep disturbances and daily caloric distribution. Methods: In sum, 252 type 2 diabetes patients attending an annual review outpatients' clinic completed questionnaires, including the Munich Chronotype Questionnaire to assess chronotype and SJL, the Pittsburgh Sleep Quality index (PSQI), the Big Five Personality Inventory and the Center for Epidemiologic Studies Depression Scale. Chart review provided information on diabetes duration, Hemoglobin A1c (HbA1c), body mass index (BMI) and other clinical variables. Caloric intake was assessed via 24-h dietary recall. Results: Hierarchical linear regression revealed that SJL, but not chronotype or personality factors, was a significant predictor of HbA1c levels (b ¼ 0.16, p < 0.05). There was a significant relationship between later chronotype and HbA1c levels, but only in patients who had more than 90 min SJL (r ¼ 0.51, p ¼ 0.002). Younger age was associated with a higher HbA1c (r ¼ 0.23, p < 0.001), and this effect was partially mediated through SJL (Pm ¼ 0.22). Conclusions: We identify SJL as a novel factor that may impact on glycemic control in type 2 diabetes. Further study is needed to determine whether interventions aimed at reducing SJL may lead to improvements in glycemic control.

Abstract

Circadian rhythms are endogenously generated recurring patterns of around 24 hours with well-established roles in physiology and behaviour. These circadian clocks are important in both the aetiology and treatment of various psychiatric and metabolic diseases. To maintain physiological homeostasis and optimal functioning, living life synchronised to these clocks is desirable; modern society, however, promotes a '24/7' lifestyle where activity often occurs during the body’s 'biological night', resulting in mistimed sleep and circadian misalignment. This circadian desynchrony can increase the risk of disease and can also influence treatment response. Clinicians should be aware of the influence that circadian desynchrony can have on health and disease, in order to potentially develop new therapeutic strategies and to incorporate chronotherapeutics into current treatment strategies to enhance their utility.
