This is the second of two articles presenting findings from a wider study that aimed to explore and better understand the personal story of cancer beyond the label of 'patient' and the healthcare context. Part 1 focused on the importance of relationships with other people with cancer, family members and the healthcare team in the search to make sense of life. This article focuses on the importance of relationships with people who have died, with God or a higher being, and the self in the search to make sense of life. Using an interpretative phenomenological approach, data were gathered through semi-structured interviews with 15 people living with cancer. It was identified that the ability to relate to those who have died, with God or a higher being, and the self was an important component of the sense-making process. For some participants, important relationships continued to exist with people who had died, which provided support and consolation. Their relationship with God or a higher being guided many participants in their sense-making process. Several participants also reported that their relationship with themselves evolved and was redefined through their illness experience. Understanding the importance of these relationships may reveal some of the less visible components of the illness experience. 4. Casuistry: On a Method of Ethical Judgement in Patient Care.

The article is dedicated to the application questions of a case study method known as casuistry. In its long tradition, it focuses on an influential variant of the early modern period and reconstructs its functionality. In the course of reading recent receptions, it is noted that some studies speak of a "casuistic revival" in moral case deliberation in health care. As a result of this revival, casuistry has been modified in such a way that it guides case discussions in practice with the help of a tripartite methodology (morphology, taxonomy, and kinetics). However, as it turns out, casuistry, a case comparison method of ethical judgement based on reasoning logic, is less suitable for moral case deliberations in direct patient care. This stems from the fact that casuistry is a detailed procedure of ethical learning beneficial to institutionalized ethics committees or similar forms of ethics consultation in health care. The thrust of narrative medicine is that patients and communities construct stories that guide their lives and give meaning to both health and illness. The responsibility of health care practitioners, therefore, is to learn how to read these local narratives to provide care that is relevant to an individual or community. Given the recognition that interaction must be tailored to the subjective experiences of patients and their communities, can there be universal ethical standards in the treatment of illness? We argue that the constructed nature of patient experiences does not pose a threat to making ethical clinical judgments. The solution to this dilemma requires that the traditional distinction between objectivity and subjectivity be eschewed by clinicians in favor of establishing dialogue with patients. Narratives are never created alone and are therefore not esoteric. Listening carefully to patients' stories is an ethical practice that can be fostered in health care settings. Subjectivity can be embraced by clinicians without jeopardizing ethical or evidence-based patient care. [ABSTRACT FROM AUTHOR]

8. Irregularly emitting air pollution sources: acute health risk assessment using AERMOD and the Monte Carlo approach to emission rate.

- Abstract
Intermittent irregular sources of air pollution, when used in dispersion modeling, can exaggerate the acute health risk due to improbable coincidence of release with the worst-case meteorological conditions. This problem is alleviated by randomizing the moments of emission and applying the Monte Carlo method to obtain the realistic expected yearly maxima of hourly concentrations/risks. Emissions are modeled as irregular 'pulses,' possibly with additional constraints on timing. Such are major emission sources in important industries: oil refineries, gas extraction, cement production, etc. We have tested the approach in ~100 projects for industrial plants in Russia and obtained considerable reductions in estimated acute health risks: up to two orders of magnitude, depending on the level of intermittency of sources. These corrections to unrealistically high worst-case concentration values at nearby populated areas are, in many cases, a key to obtaining reasonable exclusion/protection zones for plants. To our knowledge, such a body of results on intermittent irregular sources is unique, and it can be useful especially for developing countries where the exact timeline of emissions is often unknown. Taking intermittency into account is also known to be an important step toward compliance with 1-h US National Ambient Air Quality Standard. We provide a detailed description of the Monte Carlo algorithm used. We compare Monte Carlo with the usual quantile-based approach to peak values; they agree when quantile is dependent on intermittency. The non-linearity of maximum function gives rise to some counterintuitive phenomena, which call for refinement of risk definition for intermittent irregular sources. [ABSTRACT FROM AUTHOR]

CONTEXT Catholic hospitals control a growing share of health care in the United States and prohibit many common reproductive services, including ones related to sterilization, contraception, abortion and fertility. Professional ethics guidelines recommend that clinicians who deny patients reproductive services for moral or religious reasons provide a timely referral to prevent patient harm. Referral practices in Catholic hospitals, however, have not been explored. METHODS Twenty-seven obstetrician-gynecologists who were currently working or had worked in Catholic facilities participated in semistructured interviews in 2011-2012. Interviews explored their experiences with and perspectives on referral practices at Catholic hospitals. The sample was religiously and geographically diverse. Referral-related themes were identified in interview transcripts using qualitative analysis. RESULTS Obstetrician-gynecologists reported a range of practices and attitudes in regard to referrals for prohibited services. In some Catholic hospitals, physicians reported that administrators and ethicists encouraged or tolerated the provision of referrals. In others, hospital authorities actively discouraged referrals, or physicians kept referrals hidden. Patients in need of referrals for abortion were given less support than those seeking referrals for other prohibited services. Physicians received mixed messages when hospital leaders wished to retain services for financial reasons, rather than have staff refer patients elsewhere. Respondents felt referrals were not always sufficient to meet the needs of low-income patients or those with urgent medical conditions. CONCLUSIONS Some Catholic hospitals make it difficult for obstetrician-gynecologists to provide referrals for comprehensive reproductive services. [ABSTRACT FROM AUTHOR]

10. Pediatric euthanasia in Belgium: some ethical considerations.

On February 2014, the Belgian Parliament approved the first law on children euthanasia stating that also young children can make the autonomous choice to have their lives terminated. This article reviews the ethical objections to the law. These include the impossibility to gauge the competence of children and their vulnerability, and the fact that euthanasia itself implies that some lives are not worthy of living. Person Centered Medicine, as a concept and global programmatic movement developed in collaboration with the World Medical Association, World Health Organization, International Council of Nurses and 30 other institutions over a decade of annual Geneva Conferences, places the whole person as the center of health and as the goal and protagonist of health actions. Seeking the person at the center of medicine, has meant a medicine of the person, for the person, by the person and with the person. Articulating science and humanism, it strives for a medicine informed by evidence, experience and values and aimed at the restoration and promotion of health for all. The textbook on Person Centered Medicine reviews this perspective as it has evolved to date and its resultingknowledge base. The book structure encompasses an Introduction to the field and four sections on Principles, Methods, Specific Health Fields, and Empowerment Perspectives. Its 42 chapters are authored by 105 clinician-scholars from 25 different countries across world regions (North America, Latin America, Europe, Africa, the Middle East, Asia and Oceania). Its vision and goals involve total health for a total person.
