1. In-vivo blood pressure sensing with bi-filler nanocomposite

Abstract

Conductive elastomers present desirable qualities for sensing pressure in-vivo, such as high piezoresistance in tiny volumes, conformability and, biocompatibility. Many PDMS-based electrically conductive nanocomposites however, are susceptible to electrical drift following repeated stress cycles and chemical aging. Innovative approaches are needed to stabilize their percolation network against deformation to improve reproducibility and facilitate sensor calibration. One approach we propose here is to decouple the tunnelling-percolation network of HOPG nanoparticles from the incomplete viscoelastic recovery of the PDMS matrix by inserting minute amounts of insulating SiO$_2$ nanospheres. SiO$_2$ nanospheres effectively reduce the number of nearest neighbours at each percolation node switching off the parallel electrical pathways that might become activated by incomplete viscoelastic relaxation. We varied the size of SiO$_2$ nanospheres and their filling fraction to demonstrate nearly complete piezoresistance recovery when SiO$_2$ and HOPG nanoparticles have equal diameters ($\approx$400nm) and SiO$_2$ and HOPG volume fractions are 1% and 29.5% respectively. We demonstrate an in-vivo blood pressure sensor based on this bi-filler composite.

Cardiovascular diseases represent a major cause of morbidity and mortality, necessitating research to improve diagnostics, and to discover and test novel preventive and curative therapies. All of which warrant experimental models that recapitulate human disease. The translation of basic science results to clinical practice is a challenging task. In particular for complex conditions such as cardiovascular diseases, which often result from multiple risk factors and co-morbidities. This difficulty might lead some individuals to question the value of animal research, citing the translational 'valley of death', which largely reflects the fact that studies in rodents are difficult to translate to humans. This is also influenced by the fact that new, human-derived in vitro models can recapitulate aspects of disease processes. However, it would be a mistake to think that animal models cannot provide a vital step in the translational pathway as they do provide important pathophysiological insights into disease mechanisms particularly on a organ and systemic level. While stem cell-derived human models have the potential to become key in testing toxicity and effectiveness of new drugs, we need to be realistic, and carefully validate all new human-like disease models. In this position paper, we highlight recent advances in trying to reduce the number of animals for cardiovascular research ranging from stem cell-derived models to in situ modelling of heart properties, bioinformatic models based on large datasets, and improved current animal models, which show clinically relevant characteristics observed in patients with a cardiovascular disease. We aim to provide a guide to help researchers in their experimental design to translate bench findings to clinical routine taking the replacement, reduction and refinement (3R) as a guiding concept. Abstract

Cardioprotection; Drug development; Infarction Cardioprotección; Desarrollo de fármacos; Infarto Cardioprotecció; Desenvolupament de fàrmacs; Infart Acute myocardial infarction (AMI) and the heart failure (HF) which may follow are among the leading causes of death and disability worldwide. As such, new therapeutic interventions are still needed to protect the heart against acute ischemia/reperfusion injury to reduce myocardial infarct size and prevent the onset of HF in patients presenting with AMI. However, the clinical translation of cardioprotective interventions that have proven to be beneficial in preclinical animal studies, has been challenging. One likely major reason for this failure to translate cardioprotection into patient benefit is the lack of rigorous and systematic in vivo preclinical assessment of the efficacy of promising cardioprotective interventions prior to their clinical evaluation. To address this, we propose an in vivo set of step-by-step criteria for IMproving Preclinical Assessment of Cardioprotective Therapies (‘IMPACT’), for investigators to consider adopting before embarking on clinical studies, the aim of which is to improve the likelihood of translating novel cardioprotective interventions into the clinical setting for patient benefit. Abstract

Ischemic heart disease including myocardial infarction develops on the basis of several risk-factors and comorbidities such as obesity, diabetes, hypertension, and hypercholesterolemia. Ischemic heart disease is the leading cause of mortality worldwide, therefore, identification of novel drug targets for cardioprotection is of great importance. Ischemic preconditioning, postconditioning, and remote conditioning trigger endogenous cardioprotective mechanisms that render the heart more resistant to lethal ischemic-reperfusion injury. However, major cardiovascular co-morbidities such as hyperlipidemia, diabetes, and their co-medications interfere with these cardioprotective mechanisms thereby limiting the efficacy of cardioprotective ischemic conditioning maneuvers. Ischemia reperfusion injury and cardioprotection by conditioning have been shown to affect global myocardial gene expression profile at the transcript level. Further understanding and the comprehensive analysis of the cardioprotective gene expression fingerprint in normal, protected, and in comorbid conditions may lead to identification of novel molecular targets for cardioprotection.

25. Abstract 14701: Preconditioning of Cardiosphere Derived Cells With Secretome of Apoptotic Peripheral Blood Mononuclear Cells Enhances Cardiac Repair Efficacy

Abstract

Introduction:Cardiosphere-derived cardiac progenitor cells (CDCs) have shown promising effect on preclinical and initial clinical cardiac regeneration studies. Intravenous and percutaneous intramyocardial application of APOSEC, the secretome of irradiated peripheral blood mononuclear cells (PBMCs) demonstrated beneficial influence on acute and chronic myocardial infarction (MI) in small and large animal models.Hypothesis:We aimed to enhance the reparative effect of CDCs by pre-conditioning with APOSEC in a translational pig acute MI and ischemic left ventricular dysfunction model by assessing the myocardial metabolism and function by cardiac 18F-FDG-PET-MRI with late enhancement.Methods:Porcine closed-chest reperfused acute MI was induced by percutaneous balloon occlusion of the mid LAD for 90 min followed by reperfusion. CDCs with (Group apoCDC, n=8) and without APOSEC -preconditioning (Group CDC, n=6) were infused intracoronarily 15 min after start of the reperfusion. Animals in Group AMI (n=7) received physiologic saline, while Group control (n=5) pigs underwent sham procedure.Results:Proteome analyses of secretome of the pre-conditioned CDCs with APOSEC revealed reduced production of collagens and other extracellular matrix (ECM) proteins as compared to CDCs alone. In contrast, VEGF and CXCL-12 expressions were increased in the presence of APOSEC. Cardiac 18F-FDG-PET-MRI after one month showed a reduction of scar volume, lower transmurality of infarcted area in both CDC and apoCDC treated animals compared to AMI pigs (p<0.05), while quantitative PET images displayed the highest 18F-FDG uptake in the infarcted area, and less number of non-viable segments in apoCDC group, suggesting more residual viability. LVEF was improved to 45.8 % for apoCDC compared to 38.5 % for controls (p=0.13).Conclusions:Incubation with APOSEC beneficially modifies the paracrine factor production of CDCs in vitro. Intracoronary CDCs, with or without APOSEC-preconditioning improve metabolic and functional recovery, while Aposec-preconditioned CDCs led to more preserved viability of the infarcted area after AMI in a clinically-realistic large-animal model.

26. Abstract 16359: Pacing Threshold Predicts Survival in Patients With Pacemaker and Chemotherapy: A Retrospective Trial

Abstract

Introduction:Anticancer therapy includes agents with cardiotoxic side effects. In patients with pacemakers (PM), pacing threshold (PT) reflects conditions of myocardial tissue.Hypothesis:The aim of this study was to investigate PT, rates of device and lead replacements and survival in patients undergoing anticancer treatment.Methods:In total, 5070 PM patients of the Medical University of Vienna were included retrospectively. ICD-10 Codes of the category C (malignant neoplasms) and I42 (cardiomyopathy) and mortality data were retrieved.Results:Cancer was diagnosed between first PM implantation and the last documented follow-up of the PM in 321 patients. 4749 non-cancer patients represented the control group. Fist implantation age and rates of dual or single chamber PM was not different.Cardiomyopathy occurred in 16.4% of the cancer patients, and in 12.1% of the non-cancer patients (p=0.03).Device replacement was more likely in patients with cancer (10-year follow-up: 61.1% vs. 36.6%, HR 2.0, 95%CI 1.6-2.5, p<0.001). Lead replacement was increased in cancer-patients (17.6% vs. 5.3%, HR 3.5, 95%CI 1.9-6.5, p<0.001).Ventricular PT showed no difference at the time of PM implantation and increased in the cancer-group by 0.2V (IQR -0.25 - +0.5). There was no change in the non-cancer group (0V IQR 0-0.25, p<0.001).Mean survival was 14.3 years (95%CI 13.5-15.1) in non-cancer-patients and 12.2 years (95%CI 11.1-13.3) in cancer-patients. In a univariate COX regression of the cancer-group, a stronger increase of ventricular PT during the follow-up period was associated with worse 10-years survival (HR 1.4, 95CI 1.3-1.6, p<0.001).Conclusions:In patients with cancer, PT increases significantly stronger, an increased rate of device and lead-replacement was found, and an increase of the PT was a predictor for worse survival. This might be closely related to the cardiotoxic effects of chemotherapy.

Abstract

WOS: 000474931400001, PubMed ID: 30934986, Cardiovascular disease (CVD) remains the leading cause of death worldwide and, despite continuous advances, better diagnostic and prognostic tools, as well as therapy, are needed. The human transcriptome, which is the set of all RNA produced in a cell, is much more complex than previously thought and the lack of dialogue between researchers and industrials and consensus on guidelines to generate data make it harder to compare and reproduce results. This European Cooperation in Science and Technology (COST) Action aims to accelerate the understanding of transcriptomics in CVD and further the translation of experimental data into usable applications to improve personalized medicine in this field by creating an interdisciplinary network. It aims to provide opportunities for collaboration between stakeholders from complementary backgrounds, allowing the functions of different RNAs and their interactions to be more rapidly deciphered in the cardiovascular context for translation into the clinic, thus fostering personalized medicine and meeting a current public health challenge. Thus, this Action will advance studies on cardiovascular transcriptomics, generate innovative projects, and consolidate the leadership of European research groups in the field.
