This article presents an integrated analysis of three case studies on the promotion of research networks by the Brazilian Decit - Departamento de Ciência e Tecnologia do Ministério da Saúde (Department of Science and Technology of the Ministry of Health), involving three major areas of national strategic interest: clinical research, neglected diseases and collective health. With the focus on the collective health field, the study seeks to justify the conditions that hinder or promote the consolidation of research networks, and it was stimulated by the failure on the promotion by the RNPPS - Rede Nacional de Pesquisas sobre Política de Saúde (National health policy research network). The capacity for the construction of tjhe formal requests related to the degree of maturity of the study fild, the complex network management model, and the leadership profie are among the main conditions observed. With knowledge of common challenges in the consolidation of research networks, the collective health may learn to balance more appropriately the political and scientifi weights in the scale of its study field. Este artículo presenta un análisis integrado de tres estudios de casos sobre el fomento de redes de investigación instituidas por el Decit – Departamento de Ciência e Tecnologia do Ministério da Saúde (Departamento de Ciencia y Tecnología del Ministerio de Salud brasileño), abarcando tres grandes áreas de interés estratégico nacional: investigación clínica, enfermedades olvidadas y salud colectiva. En el caso de la salud colectiva, el estudio fue impulsado por el fracaso del fomento de la RNPPS – Rede Nacional de Pesquisas sobre Política de Saúde (Red nacional de investigaciones sobre políticas de salud) y busca fundamentar los condicionantes que difiultan o promueven la consolidación de redes de investigación. La capacidad de construcción de las convocatorias relacionada al grado de madurez del campo de estudio, modelo de gestión de redes complejas y perfi de liderazgo están entre los principales condicionantes observados. Con el conocimiento de desafíos comunes en la consolidación de redes de investigación, la salud colectiva debe aprender a equilibrar con mayor propiedad los pesos político y científio en la balanza de su campo de estudio. Este artigo apresenta uma análise integrada de três estudos de caso sobre fomentos de redes de pesquisa instituídas pelo Departamento de Ciência e Tecnologia do Ministério da Saúde (Decit), envolvendo três grandes áreas de interesse estratégico nacional: pesquisa clínica, doenças negligenciadas e saúde coletiva. Com foco na saúde coletiva, o estudo desenvolvido, estimulado pelo insucesso do fomento da Rede Nacional de Pesquisas sobre Política de Saúde (RNPPS), busca fundamentar condicionantes que difiultam ou promovem a consolidação de redes de pesquisa nessa área. A pesquisa clínica no Brasil precisa rever a parceria entre a indústria farmacêutica, a academia científica e o governo. A Rede Nacional de Pesquisa Clínica em Hospitais Docentes (RNPC) foi criada em 2005, com o objetivo de estabelecer uma rede de ensaios brasileiros. E com o objetivo de destacar as características da RNPC que dificultam ou promovem esforços de pesquisa clínica bem-sucedidos, este estudo descritivo exploratório foi desenvolvido com uma abordagem de estudo de caso qualitativo. Na pesquisa, o software Gephi foi usado para visualizar a rede usando o algoritmo de layout Force Atlas 2. Os resultados bem-sucedidos estão relacionados ao treinamento de profissionais, a promoção de pesquisa com foco no SUS em intercâmbio acadêmico e técnico entre as instituições. more...

Due to the longer survival of critically ill children, venous thromboembolism is a problem which is becoming increasingly recognized in pediatric practice. In the last decades, several international studies have been published, shedding a light upon the epidemiology of this disease during childhood. These data show peculiarities in the clinical presentation and the significant morbidity and mortality. The new “epidemic of thrombosis” in pediatric hospitals points toward the urgent need for specific treatment and prevention protocols targeting this population. In Brazil, knowledge regarding this disease remains scarce. The lack of epidemiological data impacts both the clinical care and the design of specific public policies in the field. Thus, a national registry of pediatric venous thromboembolism is relevant to the proposal of an appropriate plan of action to create a qualified net of assistance. The improvement in educational initiatives related to the field of Pediatric Hemostasis is also very important. In this review, we have updated the epidemiological, clinical and therapeutic aspects of the disease, as well as the prevention strategies. Resumo Faz uma breve revisão da trajetória profissional do médico sanitarista Walter Sidney Pereira Leser, falecido em 2004 aos 94 anos. Autodidata, a partir de sua tese de doutoramento de 1933, torna-se referência no campo da estatística e epidemiologia no país, com dezenas de estudos e orientações. No campo clínico é um dos fundadores do Laboratório Fleury, e participa da criação do CREMESP. Como acadêmico, foi catedrático da Escola de Sociologia e Política de São Paulo, Escola Paulista de Medicina e Faculdade de Farmácia e Odontologia da USP. Introduziu os testes objetivos no exame vestibular, e esteve à frente da criação do CESCEM e Fundação Carlos Chagas. Na Escola Paulista criou o primeiro Departamento de Medicina Preventiva do país. Como administrador público, foi secretário de Estado da Saúde entre 1967 e 1971 e entre 1975 e 1979, responsável por extensas reformas e inovações. Entre as mais lembradas, a criação da carreira de médico sanitarista. Por todo esse legado, emprestou seu nome à “Medalha de Honra e Mérito da Gestão Pública em Saúde” do governo paulista. Self-taught, from his 1933 doctoral thesis he became a country reference in the field of statistics and epidemiology, with dozens of studies and supervisions. In the clinical field he is one of the founders of Fleury Laboratory, and participates in the creation of CREMESP. As an academic, Leser was a professor at the Escola de Sociologia e Política de São Paulo, Escola Paulista de Medicina e Faculdade de Farmácia e Odontologia da USP. Also, Leser introduced objective tests in the college entrance examination, and led the creation of CESCEM and Carlos Chagas Foundation. In the Escola Paulista de Medicina he created the first Preventive Medicine Department of the country. As a public official, he was secretary of the State Department of Health of São Paulo between 1967 and 1971 and between 1975 and 1979, responsible for extensive reforms and innovations. Among the most remembered, the creation of sanitary medical career. This essay assumes that from a simple administrative guideline, regionalization has become the main concept for enabling the doctrine of equitable and universal coverage behind the Brazilian Sistema Único de Saúde. At the interface between studies of the historicity of the concepts and extensive fieldwork, it is argued that the traditional concept of the regionalization of the Sistema Único de Saúde has reached the historical limits of its productive capacity. However, updating this term challenges novel perspectives for a clinical system integrated in networks within a broader intersectoral strategy of regional production clusters.Este ensaio assume que, de simples diretriz administrativa, a regionalização se tornou hoje o principal conceito de viabilização doutrinária da universalidade equitativa e integral do Sistema Único de Saúde. Na interface de estudos sobre a historicidade dos conceitos com extensas experiências de campo, argumenta-se que o conceito de regionalização tradicional do Sistema Único de Saúde alcançou limites históricos de capacidade produtiva. Resumo Nesta revisão foram incluídos apenas estudos específicos sobre o processo de regionalização do SUS, baseados em resultados empíricos e publicados a partir de 2006, já sob o referencial do Pacto pela Saúde. Foi evidenciado que o processo de regionalização é hoje uma realidade em todas as esferas de governo, sujeito a um conjunto de desafios comuns às diversas realidades do país. Entre os principais, os colegiados são valorizados com espaços de inovação, mas ainda em busca da superação da cultura política burocrática e clientelista. A governança regional é ainda prejudicada pela fragmentação do sistema e, em particular, pela histórica deficiência com planejamento, desde o nível local às políticas estratégicas de incorporação tecnológica. As análises permitiram implicar a cultura de amplo privilégio para negociação política em detrimento do planejamento como uma das principais responsáveis por um ciclo vicioso que sustenta a deficiência técnica da gestão. Trata-se de ensaio conceitual sobre a ideia de ruptura paradigmática e sua implicação na leitura histórica da saúde pública/coletiva, campo em que se confundem as dimensões política e científica. Um argumento inicial serve para esclarecer o caráter polissêmico e pré-conceitual de “paradigma”, atento às implicações conceituais, mas reafirmando sua utilidade semântica. Segue com a discussão de rupturas essenciais e cumulativas, aplicada ao confronto da ruptura epistêmica promovida pelos centros de saúde distritais e idealização do movimento de reforma sanitária. Conclui pela dificuldade do paradigma “saúde coletiva” em sustentar sua independência discursiva, de modo que a difusão planetária da matriz discursiva dos centros de saúde pela Fundação Rockefeller ainda se configura como a última ruptura paradigmática holística da saúde pública brasileira. This conceptual essay investigates the idea of paradigmatic rupture and its implications in historical interpretations of public/collective health, where the dimensions of politics and science intermingle. The polysemic and pre-conceptual nature of "paradigm" is clarified, taking account of the conceptual implications, while reaffirming their semantic usefulness. The purpose of this article is to highlight a number of underlying issues that may be useful for a comprehensive review of the management of Health-Related Science, Technology and Innovation policies (STI/H), and its strategies and priorities. It is an analytical study supported by an extensive review of the technical and journalistic literature, clippings, legislation and federal government directives. The results show that the Healthcare Production Complex undeniably and increasingly needs science to maintain itself. One may infer that a framework of institutional milestones is being built in Brazil, to strengthen, guide and encourage Research and Development, and that clinical research creates scientific knowledge to address public healthcare issues by generating new inputs or enhancing existing techniques, processes and technologies that will be produced, marketed and used in the different segments, thus feeding the Healthcare Productive Complex. more...

Sob o referencial da história das idéias, este estudo revisa o pensamento sanitarista brasileiro, norteado pela figura do Centro de Saúde. A reforma do Serviço Sanitário paulista de 1925 e a instituição das Unidades de Saúde da Família em 1994 definem os limites gerais da revisão. Considera-se a possibilidade de que o discurso sobre o Centro de Saúde inaugurou uma nova era epistemológica na saúde pública, definida pela dimensão individual da educação sanitária, em detrimento da medicina das cidades ou era bacteriana. As novas concepções surgiram, sustentaram e foram sustentadas pelo deslocamento da influência política e intelectual francesa para os Estados Unidos, materializada nas ações da Fundação Rockefeller, tendo por base a Universidade Johns Hopkins. São Paulo foi uma das cidades escolhidas para investimento maciço, como efeito demonstração do novo ideário. Embora a atuação institucional da Fundação tenha tido escala planetária, a apropriação discursiva e adaptação local se mostrou mais precoce e coesa na América Latina, dando ensejo a criação e fortalecimento de um verdadeiro pan-americanismo na saúde pública, com a participação do Brasil. No país, o discurso da era sanitária e seus corolários medicina preventiva, medicina integral, medicina comunitária e atenção básica/primária, foi clivado por três grandes matrizes políticas, que se sucederam e predominaram em momentos distintos: pensamento clássico, de matriz liberal e que dá significação cultural local às idéias que prevalecem no debate internacional a partir dos anos 1920; o pensamento radical, de matriz marxista, que aflora na segunda metade dos anos 1970, principalmente no seio da intelectualidade acadêmica; e o pensamento comunitário em saúde pública, de raízes no pensamento político da assistência social, estimulado pela estratégia da Saúde da Família nos anos 1990. O pensamento radical em saúde pública nasce sob incisiva oposição ao regime militar e ao pensamento clássico. Em sua matriz discursiva a saúde coletiva substitui as expressões anteriores e se concentra no determinantes sociais da saúde, relegando o debate setorial ao segundo plano. Embora sem a devida clareza, o pensamento comunitário recupera e busca atualizar muitas das concepções clássicas, sem fugir da composição com o discurso radical. Alguns conceitos originalmente ligados ao ideário dos Centros de Saúde se destacam ao longo do período analisado: integração (integralidade); descentralização (desconcentração); e socialização (universalidade). Num plano histórico mais abrangente, reconhece-se atualmente que a saúde pública brasileira esteve na essência de um projeto de construção nacional iniciado na Primeira República, mas efetivamente posto em ação na era Vargas. Os Centros de Saúde, hoje representados no ideário da Atenção Básica, não só participaram desta história, como, na realidade, lhe foram em grande parte seus protagonistas This study reviews the history of ideas in the public health of Brazil. The reform of the Health Service of Sao Paulo in 1925 and the establishment of the Family Health Program in 1994 set the general limits of the review. It considers the possibility that the discourse on the Health Centre inaugurated a new epistemological era in public health, defined by the individual dimension of \"health education\", to the detriment of the \"medicine of the cities\" or \"bacterial era\". New concepts emerged, sustained and were sustained by the displacement of French intellectual and political influence for the United States, embodied in the actions of the Rockefeller Foundation, based on the Johns Hopkins University. São Paulo was one of the cities chosen for massive investment, as \"demonstration effect\" of those new ideas. Although the institutional performance of the Foundation has had a planetary scale, the discursive appropriation and local adaptation was earlier and more cohesive in Latin America, giving rise to creation and strengthening of a true Pan-Americanism in public health, with the participation of Brazil. Nationwide, the \"health education\" ideas and its corollaries \"preventive medicine\", \"comprehensive medicine\", \"community medicine and primary care, was cleaved by three major political perspectives that ensued and prevailed at different times. The \"classical thought\", in a liberal perspective, gave local significance to the ideas present in the international debate since the 1920s. The \"radical thought\", under a Marxist matrix, arouse in the second half of the 1970s, especially within academic intellectuals, and \"community thought\" in public health, stimulated by the Family Health Program in the 1990s. The radical thought in public health movement was strengthened in opposition to the military regime and the classical thought. \"Collective health\" replaced the previous expressions and focuses on social determinants of health, relegating the services organization in the background. Although lacking the necessary clarity, community thought recovers and seeks to update many of the classical concepts, in composition with the radical discourse. Some concepts originally linked to the ideals of the Health Centers are highlighted throughout the studied period: integration (comprehensiveness), decentralization and socialization (universality). In a broader historical view, it is current recognized the role of the Brazilian public health in the national identity construction started in the First Republic, but actually put into action in the Vargas era. O saneamento é uma questão essencial para a qualidade de vida, para condições ambientais salubres, entre outros aspectos. O reconhecimento do saneamento como direito social diante da essencialidade à vida humana e à proteção ambiental ainda não foi contemplado dignamente. A pesquisa tem como objetivo investigar a viabilidade da escola tornar-se Promotora da Saúde implementando práticas sócioeducativas nos ambientes escolar e comunitário visando alcançar melhores condições de saneamento básico com mobilização social. A proposta adota a metodologia da pesquisa-ação com a estratégia da pesquisa social, atendendo os aspectos de interação e de investigação não se limitando a uma forma de ação, mas aumentando o conhecimento do pesquisador e dos atores envolvidos. Para tratar os dados de uma das atividades se utiliza o processo de análise de conteúdo. A pesquisa trata da Promoção da Saúde, da temática escola, do complexo saneamento e recursos hídricos; da mobilização, controle social e aprendizagem social. Definida a bacia hidrográfica do ribeirão da Vargem do Salto em Ibiúna/SP, para desenvolver as atividades previstas na pesquisa onde estão localizadas escolas municipais dos bairros Vargem do Salto, Lageado, Saltinho, Salto e Samano. Há um diálogo estabelecido entre a pesquisa e documentos oficiais legais para fundamentação teórica; e também sustentado pelo pensamento de Paulo Freire, John Dewey, Pedro Roberto Jacobi, Michael Apple, Moacir Gadotti, Leonardo Boff, Henry Giroux, Edgar Morin e Michel Thiollent. Promoção da Saúde definida como o processo de capacitação da comunidade para atuar na melhoria da sua qualidade de vida e saúde, inclui uma maior participação no controle deste processo e a escola pode ser um espaço para se concretizar as ações da Promoção da Saúde, proporcionando a mobilização, participação, o controle social e aprendizagem social. A síntese analítica adota o percurso metodológico de forma cronológica para apresentação dos resultados. Constata-se a função social da escola que adotando os princípios da Escola Promotora da Saúde adquire potencial para gerar processos que fortalecem a convivência, a participação, o diálogo permanente das pessoas e a compreensão consciente da sociedade em que vivem e a possibilidade de efetiva transformação. Sanitation is essential to quality of life, healthy environmental conditions, among other things. The recognition of sanitation as a social right in light of its essentiality to human life and to environmental protection has not been contemplated in a dignified manner. This research aims to investigate the viability for the school to become a Promoter of Health by implementing socio-educational practices in the school and community environments in order to achieve better conditions of sanitation with social mobilization. The proposal adopts the methodology of research-action with the strategyof social research, taking into account the aspects of interaction and research, not limited to one form of action, but increasing the knowledge of the researcher and all involved. To process the data of one of the activities the content analysis process was used. The research deals with the Promotion of Health, the topic of the school, the sanitation and water resources complex; the mobilization, social control and social learning. The basin of the stream in Vargem do Salto in Ibiúna, SP, was established for the activities set in the survey to be developed, locality where municipal schools of Vargem do Salto, Lageado, Saltinho, Salto and Samanoare located. There is a dialogue between the research and the legal official documents for theoretical foundation; sustained by the points of view from Paulo Freire, John Dewey, Pedro Roberto Jacobi, Michael Apple, MoacirGadotti, Leonardo Boff, Henry Giroux, Edgar Morin and Michel Thiollent. Health promotion, defined as the process of empowerment of the community to work on improving their quality of life and health, includes a greater participation in the control of this process. The school can be a place to achieve the actions of Health Promotion, providing the mobilization, participation, social control and social learning. The analytical overview adopts a methodological path in a chronological way to present the results. It is noted that the schools social function that, adopting the principles of the School as a Health Promoter, gains potential to generate processes that strengthen coexistence, participation, permanent dialogue and the conscious understanding of the community in which they live and the possibility of effective transformation. more...

O presente trabalho teve como objetivo geral compreender as especificidades da organização, práticas assistenciais e participação popular na saúde das comunidades rurais, remanescentes de quilombos e comunidades caiçaras do Vale do Ribeira - SP. Teve como objetivos específicos: descrever a organização do trabalho e as práticas assistenciais existentes nas comunidades rurais em comparação às práticas e organizações nas equipes urbanas; discutir os conceitos de rural e de ruralidades e sua implicação na gestão e ações de saúde locais; descrever a participação social na saúde dentro das diferentes comunidades e sua relação com a gestão local e municipal. Método: trata-se de um estudo de Casos Múltiplos envolvendo atores dos três segmentos da saúde do Vale do Ribeira (gestores, trabalhadores da saúde, usuários de áreas rurais). A pesquisa teve como loci áreas rurais de atuação das Equipes de Saúde da Família de 12 dos 15 municípios do Vale do Ribeira sob a responsabilidade do Departamento Regional de Saúde XII (DRS XII). O método de coleta de dados constou de entrevistas semiestruturadas e de grupos focais, cujos roteiros seguiram uma matriz descritiva composta por duas linhas básicas que também guiaram a análise de conteúdo: 1. Organização do serviço em torno da Atenção Primária/ESF (histórico da organização das equipes rurais, organização e gestão da atenção primária e saúde da família no contexto rural e urbano, integração e integralidade do serviço municipal e a regional de saúde, envolvimento da gestão com a comunidade rural); 2. Práticas assistenciais na Atenção Primária e participação popular (diferença das práticas da atenção primária e ESF rurais e urbanas, perfil da equipe e competência cultural, conceitos de rural e ruralidades aplicados, práticas não oficiais e complementares, participação da comunidade no processo saúde e gestão da saúde em contexto rural). Resultados: foram realizadas 22 entrevistas e 15 grupos focais em que foram ouvidas 84 pessoas. Destas, 13 eram gestores, 46 trabalhadores e 25 usuários de 12 dos 13 municípios escolhidos (por considerarem possuir equipes rurais). Em termos de organização dos serviços, observou-se que as equipes rurais em sua totalidade não possuem gestão específica, em sua maioria se desdobram em vários locais de atendimento com distâncias variadas entre eles, têm estrutura de atendimento ainda aguardando melhorias nas bases oficiais e complementares (pior nas bases complementares, unidades de saúde sem cadastro), também em sua maioria não possuem dispensação de medicamentos ou realizam atendimentos de urgência e emergência. O envolvimento com a comunidade para fins de sua participação na formulação de políticas públicas está parcialmente ligado à característica dela, porém completamente relacionado ao perfil e compreensão da equipe na promoção e manutenção dessas políticas. Em relação às práticas assistenciais, essa pulverização das ações dificulta o seguimento adequado dos usuários, diminuindo a efetividade da rede de serviços de saúde, as equipes rurais são distribuídas sem levar em conta as tipificações de conceitos de rurais, os trabalhadores e usuários relataram poucos problemas em relação ao alcance de competência cultural para o trabalho nas regiões, as práticas de saúde não oficiais e complementares são incipientes e a participação comunitária na saúde nas áreas das comunidades tradicionais é mais marcante que nas áreas não tradicionais, porém são dependentes do perfil da equipe e da gestão This study has the general objective of understanding the specifics of the organization, care practices and popular participation in the health of rural communities, quilombolas and caiçara communities of Vale do Ribeira - SP. Its specific objectives were: to describe the organization of work and existing care practices in rural communities compared to practices and organizations in urban teams; to discuss the concepts of rural and ruralities and their involvement in the management and local health actions; to describe the social participation in health within the different communities and their relationship with the local and municipal management. Method: This is a Multiple Case study involving actors in the three segments of the health of Vale do Ribeira (managers, health workers, rural users). The research loci were rural areas in which acted the Family Health Teams of 12 out of the 15 municipalities of Vale do Ribeira under the responsibility of the Regional Department of Health XII (DRS XII). The data collection method consisted of semi-structured interviews and focus groups whose screenplays followed a descriptive matrix of two basic lines that have also guided the content analysis: 1. Service organization around the primary care/FHS (organization history of rural teams, organization and management of primary care and family health in rural and urban context, integration and comprehensiveness of municipal service and the regional health, involvement of management with the rural community); 2. care practices in Primary Care and community participation (difference of primary care practices and rural and urban FHS, team profile and cultural competence, concepts of rural and ruralities applied, non-official and complementary practices, community participation in health and health management process in rural context). Results: were conducted 22 interviews and 15 focus groups in which were heard 84 people. Out of these, 13 were managers, 46 employees and 25 users of 12 of the 13 selected municipalities (for considering having rural teams). In terms of organization of services, it was observed that rural teams in its entirety have no specific management, most of them commute in various service areas with varying distances between them, have service structure still awaiting improvements in official and complementary bases (worst in complementary bases, health units without registration), also most of them do not have medication dispensing or perform emergency urgent care. The involvement with the community for the purpose of participating in the formulation of public policies is partly linked to the community feature, but completely related to the profile and understanding of team in the promotion and maintenance of such policies.
