1. Analysis of dysautonomia in patients with Chagas Cardiomyopathy

Abstract

Chagas disease American trypanosomiasis is caused by a flagellated parasite: trypanosoma cruzi, transmitted by an insect of the genus Triatoma and also by blood transfusions. In Latin America the number of infected people is approximately 6 million, with a population exposed to the risk of infection of 550000. It is our interest to develop a non-invasive, low-cost methodology, capable of detecting any alteration early on cardiaca produced by T. cruzi. We analyzed the 24 hour RR records in patients with ECG abnormalities (CH2), patients without ECG alterations (CH1) who had positive serological findings for Chagas disease and healthy (Control) matched by sex and age. We found significant differences between the Control, CH1 and CH2 groups that show dysautonomy and enervation of the autonomic nervous system., Comment: 6 pages, 6 figures

Abstract

El síndrome de Brugada (SB) es una rara canalopatía cardiovascular hereditaria asociada con el riesgo de fibrilación ventricular y muerte súbita (MS). Su estratificación de riesgo es un desafío y la mayoría de los pacientes son diagnosticados en la fase asintomática siendo el único tratamiento contra la MS la implantación de un desfibrilador eléctrico. En este estudio, presentamos herramientas de clasificación supervisada para la estratificación del riesgo del SB basadas en el análisis automático de información electrocardiográfica de alta resolución. Para ello, se utilizaron señales de ECGs de 24 horas y 12 derivaciones y variables clínicas de 64 sujetos con SB. Las señales de ECG se preprocesaron promediando la señal para reducir el ruido y obtener latidos individuales para el calculo de 10 biomarcadores. Posteriormente, se probaron 4 algoritmos de clasificación supervisada diferentes basados en Random Forest (RF), Extreme Gradient Boosting (XGBoost), K-Nearest Neighbors (KNN) y Support Vector Machine (SVM). Los resultados del area bajo la curva fueron alrededor del 90%, sin embargo, los resultados de sensibilidad fueron alrededor del 50%. Los resultados no predicen de manera eficiente a los pacientes sintomáticos en riesgo de MS debido al reducido numero de pacientes sintomáticos. Estudios posteriores con sujetos y variables adicionales pueden mejorar este pronostico.

Abstract

This paper proposes the integration and analysis of a mathematical model representing the cardiovascular system and its short-term autonomic response to head-up tilt (HUT) testing. A Latin Hypercube Sampling method was applied to design an optimal experimental space, including 19 model parameters coming from the cardiovascular and baroreflex control systems. Then, a global, variance-based sensitivity analysis was applied to quantity the effects of these parameters on heart rate and systolic blood pressure. Results highlight the relevant influence of the intrinsic heart rate and the sympathetic and parasympathetic baroreflex gains on heart rate regulation, as well as the impact of left ventricle diastolic parameters on systolic blood pressure. Moreover, a significant effect of right ventricle dynamics on blood pressure was noted. These results provide valuable information for the application of such an integrated model for the analysis of the autonomic mechanisms regulating the cardiovascular response induced by postural changes. In particular, they suggest a convenient set of parameters to be identified in a subject-specific manner.

Abstract

This paper describes the integration of mathematical models of the cardiac electrical activity, the cardiovascular system and the baroreceptor reflex control of the autonomic nervous system, with a model representing a head-up tilt test. Sensitivity analyses are performed in order to determine those model parameters producing the greatest significant effects on heart rate and blood pressure. An optimization step is then applied to the most influential parameters in order to find the best model fit to real cardiac data obtained from a patient suffering from Brugada syndrome and a healthy subject, in supine and upright postures during a tilt test. Results show the feasibility of the model to generate realistic autonomic responses to head-up tilt testing in health and disease.
