Abstract

Cardiovascular disease (CVD) is the most common non-communicable disease occurring globally. Although previous literature has provided useful insights into the important role that diet plays in CVD prevention and treatment, understanding the causal role of diets is a difficult task considering inherent and introduced weaknesses of observational (e.g. not properly addressing confounders and mediators) and experimental research designs (e.g. not appropriate or well designed). In this narrative review, we organised current evidence linking diet, as well as conventional and emerging physiological risk factors, with CVD risk, incidence and mortality in a series of diagrams. The diagrams presented can aid causal inference studies as they provide a visual representation of the types of studies underlying the associations between potential risk markers/factors for CVD. This may facilitate the selection of variables to be considered and the creation of analytical models. Evidence depicted in the diagrams was systematically collected from studies included in the British Nutrition Task Force report on diet and CVD and database searches, including Medline and Embase. Although several markers and disorders linked to conventional and emerging risk factors for CVD were identified, the causal link between many remains unknown. There is a need to address the multifactorial nature of CVD and the complex interplay between conventional and emerging risk factors with natural and built environments, while bringing the life course into the spotlight.

3. Multiwavelength aerosol lidars at the Maïdo supersite, Réunion Island, France: instrument description, data processing chain, and quality assessment

Abstract

Understanding optical and radiative properties of aerosols and clouds is critical to reducing uncertainties in climate models. For over 10 years, the Observatory of Atmospheric Physics in Reunion (OPAR; 21.079° S, 55.383° E) has been operating three active lidar instruments, named lidar 1200 (Li1200), stratospheric ozone lidar (LiO3S), and tropospheric ozone lidar (LiO3T), providing time series of vertical profiles from 3 to 45 km of the aerosol extinction and backscatter coefficients at 355 and 532 nm as well as the linear depolarization ratio at 532 nm. This work provides a full technical description of the three systems, the details about the methods chosen for the signal preprocessing and processing, and an uncertainty analysis. About 1737 nighttime averaged profiles were manually screened to provide cloud-free and artifact-free profiles. Data processing consisted of Klett inversion to retrieve aerosol optical products from preprocessed files. The measurement frequency was lower during the wet season and the holiday periods. There is a good correlation between the Li1200 and LiO3S instruments in terms of stratospheric aerosol optical depth (AOD) at 355 nm (0.001–0.107; R=0.92±0.01) and with LiO3T in terms of Ångström exponent 355/532 (0.079–1.288; R=0.90±0.13). The lowest values of the averaged uncertainty in the aerosol backscatter coefficient for the three time series are 64.4 ± 31.6 % for LiO3S, 50.3 ± 29.0 % for Li1200, and 69.1 ± 42.7 % for LiO3T. These relative uncertainties are high for the three instruments because of the very low values of extinction and backscatter coefficients for background aerosols above Maïdo observatory. Uncertainty increases due to the signal-to-noise ratio (SNR) decrease above 25 km for LIO3S and Li1200 and above 20 km for LiO3T. The lidar ratio (LR) is responsible for an uncertainty increase below 18 km (10 km) for LiO3S and Li1200 (LiO3T). LiO3S is the most stable instrument at 355 nm due to fewer technical modifications and fewer misalignments. Li1200 is a valuable addition meant to fill in the gaps in the LiO3S time series at 355 nm or for specific case studies about the middle and low troposphere. Abstract

Let $H_{n}^{+}(\mathbb{R})$ be the cone of all positive semidefinite $n\times n$ real matrices. Two of the best known partial orders that were mostly studied on subsets of square complex matrices are the L\"owner and the minus partial orders. Motivated by applications in statistics we study these partial orders on $H_{n}^{+}(\mathbb{R})$. We describe the form of all surjective maps on $H_{n}^{+}(\mathbb{R})$, $n>1$, that preserve the L\"owner partial order in both directions. We present an equivalent definition of the minus partial order on $H_{n}^{+}(\mathbb{R})$ and also characterize all surjective, additive maps on $H_{n}^{+}(\mathbb{R})$, $n\geq3$, that preserve the minus partial order in both directions., Comment: 17 pages

Abstract

Abstract Background An estimated 20% of allogeneic blood transfusions in the United States are associated with cardiac surgery. It is estimated that 11% of red cell resources were used for transfusion support of patients undergoing coronary artery bypass grafting (CABG) with a documented wide variability in transfusion rate (7.8 to 92.8%). To address the issue of unnecessary transfusions within the CABG population, we developed a model to predict which patients are at low risk of bleeding for whom transfusion treatment might be considered unnecessary. Herein we present our “SHOULD-NOT-BLEED-SCORE” application developed for the Windows® software platform which is based on our previous research. Methods This study is aimed to develop a user-friendly application that stratifies patients with respect to bleeding risk. The statistical model we used in our previous research was focused on detection of CABG patients at low risk of bleeding. The rationale behind such an approach was to identify a CABG patient subgroup at low risk of bleeding. By identifying patients at low risk of bleeding we can define a subgroup of patients for whom transfusion treatment might be considered unnecessary. We developed a Windows platform application based on risk modelling which we previously calculated for 1426 patients undergoing elective CABG from January 2010 to January 2018. Results The SHOULD-NOT-BLEED-SCORE risk score is developed for the Windows software platform. A mathematical model that is based on multivariate analysis was used for app development. The variables that entered the scoring system were: Age; Body Mass Index; Chronic Renal Failure; Preoperative Clopidogrel Exposure; Preoperative Red Blood Cells Count; Preoperative Fibrinogen Level; Preoperative Multiplate ASPI test area under the curve (AUC) units. Abstract

AIM: Evidence from mouse models suggests that brain serotonergic pathways control blood glucose. We hypothesized that sumatriptan (5HT1B -receptor agonist) would alter glucose homeostasis in humans. MATERIALS AND METHODS: We conducted a two-visit random-order double-blinded placebo-controlled cross-over trial in 10 overweight adults that were otherwise healthy. Participants received sumatriptan (single dose, 100 mg) or placebo before undergoing a 60-min intravenous glucose tolerance test, followed by a 120-min hyperinsulinaemic euglycaemic clamp. RESULTS: Glucose excursion was greater during intravenous glucose tolerance test with sumatriptan compared with placebo [iAUC0-60 min 316 (268-333) vs. 251 (197-319) min/mmol/L p = .047]. This was probably explained by a combination of reduced circulating insulin levels [iAUC0-10 min 1626 (1103-2733) vs. 2336 (1702-3269) min/pmol/L, p = .005], reduced insulin sensitivity [M/I-value 2.11 (1.15, 4.05) vs. 3.03 (1.14, 4.90) mg/kg/min per pmol/L, p = .010] and glucose effectiveness [SG 0.17 (0.12, 0.21) vs. 0.22 (0.18, 0.65)/min, p = .027]. CONCLUSIONS: 5HT1B receptors have a glucoregulatory role in humans, probably acting on insulin secretion, insulin sensitivity and glucose effectiveness.

49. Cryopreserved human heart valve allografts: a ten-year single centre experience

Abstract

This study provides an overview of tissue banking activities at the Croatian Cardiovascular Tissue Bank (CTB) during past ten years and presents the outcomes of cryopreserved heart valve allografts (CHAs) use in different patient groups. From June 2011 until December 2021, 75 heart donations were referred to CTB: 41 recipient of heart transplant (RHT), 32 donors after brain death (DBD) and 2 donors after circulatory death (DCD) donations. Processing resulted in 103 valves of which 65 met quality requirements for clinical use. Overall tissue discard rate was 37%. The most frequent reasons for discard were inadequate morphology (12%) in RHT donations and microbiological contamination (19%) in DBD donations. Altogether, 38 CHAs were transplanted to 36 patients. Recipients were divided in three groups ; infective endocarditis (IE), noninfectious heart disease and congenital heart disease group. In the IE group, the 30-day, 1-year and 3-year survival was 71%, 53% and 47%, respectively. Freedom from re- operation due to all graft-related causes was 76% and due to structural valve deterioration 88%. There were no cases of graft reinfection. In the congenital heart disease group CHAs were predominantly (94%) used for right ventricular outflow tract reconstruction and 88% of patients recovered without graft-related complications. At present, the number of demands for CHAs at CTB considerably outweighs their availability.

50. Time for nutrition in medical education

Abstract

Aim To synthesise a selection of UK medical students’ and doctors’ views surrounding nutrition in medical education and practice.Methods Information was gathered from surveys of medical students and doctors identified between 2015 and 2018 and an evaluation of nutrition teaching in a single UK medical school. Comparative analysis of the findings was undertaken to answer three questions: the perceived importance of nutrition in medical education and practice, adequacy of nutrition training, and confidence in current nutrition knowledge and skills.Results We pooled five heterogeneous sources of information, representing 853 participants. Most agreed on the importance of nutrition in health (>90%) and in a doctor’s role in nutritional care (>95%). However, there was less desire for more nutrition education in doctors (85%) and in medical students (68%). Most felt their nutrition training was inadequate, with >70% reporting less than 2 hours. There was a preference for face-to-face rather than online training. At one medical school, nutrition was included in only one module, but this increased to eight modules following an increased nutrition focus. When medical students were asked about confidence in their nutrition knowledge and on advising patients, there was an even split between agree and disagree (p=0.869 and p=0.167, respectively), yet few were confident in the UK dietary guidelines. Only 26% of doctors were confident in their nutrition knowledge and 74% gave nutritional advice less than once a month, citing lack of knowledge (75%), time (64%) and confidence (62%) as the main barriers. There was some recognition of the importance of a collaborative approach, yet 28% of doctors preferred to get specialist advice rather than address nutrition themselves.Conclusion There is a desire and a need for more nutrition within medical education, as well as a need for greater clarity of a doctor’s role in nutritional care and when to refer for specialist advice. Despite potential selection bias and limitations in the sampling frame, this synthesis provides a multifaceted snapshot via a large number of insights from different levels of training through medical students to doctors from which further research can be developed.
