Abstract

Climate change may have spatially variable impacts on growth of trees in topographically diverse environments, making generalizing across broad spatial and temporal extents inappropriate. Therefore, topography must be considered when analyzing growth response to climate. We address these topo-climatic relationships in the Canadian Rocky Mountains, focusing on lodgepole pine (Pinus contorta Douglas ex Louden) and interior spruce (Picea glauca (Moench) Voss * Picea engelmannii hybrid Parry) growth response to climate, Palmer drought severity index (PDSI), aspect, and slope angle. Climate variables correlate with older lodgepole pine growth on south- and west-facing slopes, including previous August temperature, winter and spring precipitation, and previous late-summer and current spring PDSI, but younger lodgepole pine were generally less sensitive to climate. Climate variables correlate with interior spruce growth on all slope aspects, with winter temperature and PDSI important for young and old individuals. Numerous monthly growth-climate correlations are not temporally stable, with shifts over the past century, and response differs by slope aspect and angle. Both species are likely to be negatively affected by moisture stress in the future in some, but not all, topographic environments. Results suggest species-specific and site-specific spatiotemporally diverse climate-growth responses, indicating that climate change is likely to have spatially variable impacts on radial growth response in mountainous environments. Le changement climatique pourrait avoir des impacts sur la croissance des arbres qui varient dans l'espace dans les environnements topographiquement heterogenes, de telles sorte qu'il serait inapproprie de generaliser sur de vastes etendues spatiales et temporelles. Par consequent, on doit tenir compte de la topographie lorsqu'on analyse la reaction de la croissance au climat. Nous abordons ces relations entre la topographie et le climat dans les montagnes Rocheuses canadiennes, en mettant l'accent sur la reaction de la croissance du pin tordu (Pinus contorta Douglas ex Louden) et de l'epicea de l'interieur (un hybride entre Picea glauca (Moench) Voss et P. engelmannii Parry) au climat, a l'indice Palmer de gravite de secheresse (IPGS), a l'orientation et a l'inclinaison de la pente. Les variables du climat sont correlees avec la croissance des pins tordus plus vieux sur les pentes orientees au sud et a l'ouest, incluant la temperature du mois d'aout de l'annee precedente, la precipitation hivernale et printaniere ainsi que l'IPGS a la fin de l'ete de l'annee precedente et du printemps de l'annee en cours, mais les pins tordus plus jeunes etaient generalement moins sensibles au climat. Les variations du climat sont correlees avec la croissance de l'epicea de l'interieur peu importe l'orientation de la pente; la temperature et l'IPGS durant la saison hivernale sont importants pour tous les individus, jeunes ou vieux. De nombreuses correlations entre le climat et la croissance mensuelle ne sont pas stables dans le temps et ont varie au cours du siecle passe; la reaction differe selon l'orientation et l'inclinaison de la pente. Les deux especes ont des chances d'etre negativement affectees dans l'avenir par le stress hydrique dans certaines, mais pas toutes les situations geographiques. Les resultats suggerent des reactions de la croissance au climat variees du point de vue spatiotemporel et propres a chaque espece et a chaque site, ce qui indique que le changement climatique aura vraisemblablement des impacts variables selon l'endroit sur la reaction de la croissance radiale dans les environnements montagneux. Abstract

Background Retrocorneal membranes (RCMs) may result from epithelial ingrowth, stromal keratocytic downgrowth, fibrous metaplasia of the corneal endothelium, or a combination of these processes. In an institutional case series, the clinical history, ocular findings, and immunohistochemical staining results of RCMs were analysed in patients with unilateral corneal decompensation after complicated intraocular surgery. Methods and Patients Between January 2021 and September 2022, six retrocorneal membranes were excised during Descemetʼs stripping automated endothelial keratoplasty (DSAEK) and Descemet membrane endothelial keratoplasty (DMEK) procedures and classified after screening with haematoxylin and eosin, periodic acid-Schiff, elastic van Gieson staining, and immunohistochemical screening with cytokeratin 7 (CK7), anti-cytokeratin (CAM5.2 and AE1/3), cell surface glycoprotein CD34, smooth muscle actin (α-SMA), and vimentin. Results On the basis of the immunohistochemical screening, the majority of excised RCMs (5 of 6) could histopathologically be classified as membranes originating from fibrous metaplasia of the corneal endothelium. All these RCMs were positive for CK7, α-SMA, and vimentin and negative for CAM5.2 and CD34. In one patient, an RCM had developed after 18 days of corneal contact to a free-floating dexamethasone implant in the anterior chamber and was classified as originating from stromal keratocyte downgrowth (α-SMA- and vimentin-positive, all others negative). All eyes in this series had a previous history of complicated cataract surgery, partially with subsequent intraocular lens exchange. No eyes after previous penetrating keratoplasty were in this series. Conclusions In this series of eyes with previous complicated intraocular interventions (in most cases cataract surgery and revisions), the dominating RCM belonged to the type originating from fibrous metaplasia of the corneal endothelium.

Abstract

Purpose: To describe a patient who developed major topographical changes mimicking post-laser in situ keratomileusis (LASIK) ectasia during breast cancer treatment with tamoxifen. Methods: Case report and literature review. Results: A 36-year-old woman who had had 10 years of stable vision after LASIK surgery was diagnosed as having breast cancer 6 months before presentation. As therapy, the patient received tamoxifen, a selective estrogen receptor modulator. After the initiation of therapy, the patient experienced rapidly decreasing vision in her left eye. Assessment of corneal tomography showed marked steepening with irregular astigmatism, leading to the diagnosis of postoperative ectasia in the left eye. After 3 months of treatment with tamoxifen, the anterior cornea showed marked flattening again and improvement of corrected distance visual acuity, both of which were maintained at the 6-month follow-up visit. Conclusions: Serum estrogen level changes during breast cancer treatment may induce major topographical changes, mimicking postoperative ectasia after LASIK. 23. Challenges and concepts in the diagnosis and management of ocular graft-versus-host disease

Abstract

Graft-versus-host disease (GVHD) is characterized by tissue inflammation in the host following an allogeneic hematopoietic cell transplantation (HCT). The pathophysiology is complex and only incompletely understood yet. Donor lymphocyte interaction with the histocompatibility antigens of the host plays a crucial role in the pathogenesis of the disease. Inflammation may affect multiple organs and tissues, e.g., the gastrointestinal tract, liver, lung, fasciae, vaginal mucosa, and the eye. Subsequently, alloreactive donor-derived T and B lymphocytes may lead to severe inflammation of the ocular surface (i.e., cornea and conjunctiva) and the eyelids. Furthermore, fibrosis of the lacrimal gland may lead to severe dry eye. This review focuses on ocular GVHD (oGVHD) and provides an overview of current challenges and concepts in the diagnosis and management of oGVHD. Ophthalmic manifestations, diagnostic procedures, grading of severity and recommendations for ophthalmic examination intervals are provided. Management of ocular surface disease with lubricants, autologous serum eye drops, topical anti-inflammatory agents and systemic treatment options are described based on the current evidence. Ocular surface scarring and corneal perforation are severe complications of oGVHD. Therefore, ophthalmic screening and interdisciplinary treatment approaches are highly relevant to improve the quality of life of patients and to prevent potentially irreversible visual loss. 26. Cross-sectional study of intraocular cataract lens replacement, circadian rest-activity rhythms, and sleep quality in older adults

Abstract

Study Objectives Age-related cataract decreases light transmission at the most sensitive spectrum for circadian photoentrainment, with negative ramifications for human health. Here, we assessed whether intraocular lens replacement (IOL) in older patients with previous cataract was associated with increased stability and amplitude of circadian rest-activity rhythms, and improved sleep quality. Methods Our cross-sectional study included sixteen healthy older individuals without ocular diseases (controls; 55-80 years; 63.6 +/- 5.6y; 8 women) and 13 patients with previous cataract and bilateral IOL (eight with blue-blocking [BB] lens and five with ultraviolet-only [UV] blocking lens; 55-80 years; 69.9 +/- 5.2y; 9 women). The study comprised three weeks of at home rest-activity assessments using wrist-worn actigraphs, and each week preceded a laboratory protocol. Primary outcomes were actigraphy-derived interdaily stability, intradaily variability, and relative amplitude of circadian rest-activity rhythms. Secondary outcomes were actigraphy-assessed sleep quality (i.e. time in bed, sleep duration, sleep efficiency, mean wake bout time and fragmentation index). Results Patients with IOL had significantly higher interdaily stability (Group effect: pFDR =.001), but not intradaily variability (Group effect: pFDR = n.s.), and significantly higher relative amplitude of rest-activity rhythms (Group effect: pFDR < .001). Moreover, patients with IOL had significantly higher activity levels during the day and lower levels during the evening, as compared to healthy older controls (Group effect: pFDR = .03). Analyses of actigraphy-derived sleep parameters yielded no significant differences across groups (Group effect: all pFDR > .1). Conclusions Our cross-sectional study suggests that enhancing spectral lens transmission in patients with cataract may benefit their circadian health.

33. Impact of examinees' stereopsis and near visual acuity on laparoscopic virtual reality performance

Abstract

Purpose: Laparoscopic surgery represents specific challenges, such as the reduction of a three-dimensional anatomic environment to two dimensions. The aim of this study was to investigate the impact of the loss of the third dimension on laparoscopic virtual reality (VR) performance. Methods: We compared a group of examinees with impaired stereopsis (group 1, n=28) to a group with accurate stereopsis (group 2, n=29). The primary outcome was the difference between the mean total score (MTS) of all tasks taken together and the performance in task 3 (eye-hand coordination), which was a priori considered to be the most dependent on intact stereopsis. Results: The MTS and performance in task 3 tended to be slightly, but not significantly, better in group 2 than in group 1 [MTS: −0.12 (95% CI −0.32, 0.08; p=0.234); task 3: −0.09 (95% CI −0.29, 0.11; p=0.385)]. The difference of MTS between simulated impaired stereopsis between group 2 (by attaching an eye patch on the adominant eye in the 2nd run) and the first run of group 1 was not significant (MTS: p=0.981; task 3: p=0.527). Conclusion: We were unable to demonstrate an impact of impaired examinees' stereopsis on laparoscopic VR performance. Individuals with accurate stereopsis seem to be able to compensate for the loss of the third dimension in laparoscopic VR simulations.

39. Intraocular cataract lens replacement and light exposure potentially impact procedural learning in older adults

Abstract

Procedural learning declines with age and appropriately timed light exposure can improve cognitive performance in older individuals. Because cataract reduces light transmission and is associated with cognitive decline in older adults, we explored whether lens replacement (intraocular blue-blocking [BB] or UV-only blocking) in older patients with cataracts enhances the beneficial effects of light on procedural learning. Healthy older participants (n = 16) and older patients with post-cataract surgery (n = 13 with BB or UV lens replacement) underwent a randomized within-subject crossover laboratory design with three protocols. In each protocol, 3.5 hr dim-dark adaptation was followed by 2 hr evening blue-enriched (6,500K) or non-blue-enriched light exposure (3,000K or 2,500K), 30 min dim post-light, ~8 hr sleep and 2 hr morning dim light. Procedural learning was assessed by the alternating serial reaction time task (ASRT), as part of a larger test battery. Here, ASRT performance was indexed by type of trial (random or sequence) and sequence-specific (high or low probability) measures. During evening light exposure, we observed a significant effect of the interaction of "group" versus "light condition" on the type of trial (p = .04; p = .16; unadjusted and adjusted p-values, respectively) and sequence-specific learning (p = .04; p = .16; unadjusted and adjusted p-values, respectively), whereby patients with UV lens replacement performed better than patients with BB lens or non-cataract controls, during blue-enriched light exposure. Lens replacement in patients with cataracts may potentially be associated with beneficial effects of blue light on procedural learning. Thus, optimizing spectral lens transmission in patients with cataracts may help improve specific aspects of cognitive function, such as procedural learning.
