Abstract

In het kader van KRW, ontstaat de wens om inzichten te verkrijgen in de trends van fysisch-chemische parameters, alsook inzichten in de effectiviteit van het waterbeheer. Het waterschap Aa en Maas beschikt over een uitgebreide database over fysisch-chemische en biologische gegevens (macrofauna, vissen, diatomeeën) die het mogelijk maken om de effectiviteit van beheersmaatregelen in kaart te brengen. In reeds uitgebrachte analyses in 2021-2022 zijn de effecten van beheermaatregelen op, alsook trends van, macrofauna en fysische-chemische parameters geanalyseerd. In de huidige notitie wordt een nadere analyse uitgevoerd onder een tweedelig projectvoorstel.

Abstract

De effecten van klimaatverandering worden steeds beter voelbaar: extreme wateroverlast in 2016 en 2021 en extreem droge zomers in de tussenliggende jaren. Klimaatverandering heeft mede daardoor ook een groot effect op het leven in en onder water. Het realiseren van de ecologische doelen is sterk afhankelijk van het op orde krijgen van hydrologische randvoorwaarden in de zomerperiode: geen of beperkte droogval, voldoende doorstroming, voldoende grote stroomsnelheid en passende stromingsdynamiek. In de SGBP-plannen die Aa en Maas heeft opgesteld, is tot nu toe geen rekening gehouden met effecten van klimaatverandering, en ook is de realiseerbaarheid van de hydrologische randboorwaarden niet getoetst. Daarom is voor het beheergebied van Aa en Maas per KRW-waterlichaam verkend of de voor de ecologische doelen noodzakelijke hydrologische randvoorwaarden gerealiseerd kunnen worden bij zowel het huidig als toekomstig klimaat. Onderzocht is hoe het effect van klimaat via neerslag en verdamping leidt tot andere beekafvoeren en daarmee tot veranderingen in de hydrologische randvoorwaarden stroomsnelheid, watervoerendheid en fluctuatie in debieten. Er is geen rekening gehouden met het klimaateffect van een hogere watertemperatuur op het leven in de beek en ook is geen rekening gehouden met een wijziging in de beschibare hoeveelheid aanvoerwater als gevolg van klimaatverandering.

Abstract

"Nasal hyperreactivity" is a key feature in various phenotypes of upper airway diseases, whereby reactions of the nasal epithelium to diverse chemical and physical stimuli are exacerbated. In this review, we illustrate how nasal hyperreactivity can result from at least three types of mechanisms: (1) impaired barrier function, (2) hypersensitivity to external and endogenous stimuli, and (3) potentiation of efferent systems. We describe the known molecular basis of hyperreactivity related to the functional impairment of epithelial cells and somatosensory innervation, and indicate that the thermal, chemical, and mechanical sensors determining hyperreactivity in humans remain to be identified. We delineate research directions that may provide new insights into nasal hyperreactivity associated with rhinitis/rhinosinusitis pathophysiology and therapeutics. The elucidation of the molecular mechanisms underlying nasal hyperreactivity is essential for the treatment of rhinitis according to the precepts of precision medicine.

Abstract

On March 11th 2020, the World Health Organization (WHO) declared COVID-19 pandemic, with subsequent profound impact on the entire health care system. During the COVID-19 outbreak, activities in the rhinology outpatient clinic and operation rooms are limited to emergency care only. Health care practitioners are faced with the need to perform rhinological and skull base emergency procedures in patients with a positive or unknown COVID-19 status. This article aims to provide recommendations and relevant information for rhinologists, based on the limited amount of (anecdotal) data, to guarantee high-quality patient care and adequate levels of infection prevention in the rhinology clinic.

It aims to provide the most up to date scientifically robust information on the topic published in the literature which has been critically analysed by an international group of clinicians drawn from all disciplines dealing with these problems together with patients. The guidelines offer evidence-based recommendations and care pathways for acute and chronic rhinosinusitis in both adults and children. Management of these diseases from the patients' perspective is an important part of EPOS2020. Not only is this included in the main document but, for the first time, we have produced a separate supplement dedicated to and in collaboration with patients, EPOS4Patients, which aims to provide information in an accessible format, to answer frequently asked questions about these diseases and their treatment options as well as including useful patient resources and websites. It has never been more important for patients to be actively involved in their care. Being well informed helps you to make the best decisions together with your doctor.

Abstract

Omdat de waterkwaliteit in veel gebieden nog niet op orde is, wordt van agrariërs verwacht dat ze ook een bijdrage leveren aan de verbetering ervan. Een eerder ontwikkelde versie van ‘Maatregel op de Kaart’ is verbeterd en in de praktijk getoetst. De toetsing bevestigt dat de kaart een basis biedt voor een gericht en perceel-specifiek advies voor kansrijke maatregelen. Bijgaand artikel is het vervolg op het vakartikel ‘Maatregel op de Kaart: Kansrijke landbouwmaatregelen per perceel voor schoner grond- en oppervlaktewater’ dat is bekroond tot het beste H2O-vakartikel van 2020.

Abstract

Dutch society makes use of a variety of goods and services provided by ecosystems – generally referred to as ecosystem services. A visible record of the status and trends in ecosystem services can help government and industry to incorporate the interests of nature and the environment in the Netherlands into decisionmaking processes. The results show that although Dutch ecosystems make a valuable contribution towards the need for the sustainable provision of goods and services, they do not satisfy the entire demand. Moreover, over the past 20 years the demand for most ecosystem services has risen faster than the supply. De Nederlandse samenleving gebruikt verschillende goederen en diensten die ecosystemen leveren – de zogeheten ecosysteemdiensten. Het zichtbaar maken van de toestand en trends van ecosysteemdiensten in Nederland kan helpen bij de oordeelsvorming en besluitvorming van natuur in Nederland door de overheid en het bedrijfsleven. Resultaten laten zien dat, alhoewel Nederlandse ecosystemen een belangrijke bijdrage leveren aan de behoefte aan een duurzame voorziening van goederen en diensten, ze niet voorzien in de totale vraag. Bovendien geldt dat voor de meerderheid van de ecosysteemdiensten de vraag ernaar sneller toeneemt dan het aanbod in de afgelopen twintig jaar.

Abstract

‘Maatregel op de Kaart’ biedt inzicht in kansrijke maatregelen op landbouwperceelsniveau om uitstoot van fosfor en stikstof naar het water te verminderen. De kaart moet zich nog in de praktijk bewijzen en wordt verder doorontwikkeld. Maar belanghebbenden geven aan dat de kaart nu al helpt bij het kiezen van de beste maatregelen voor schonere grond- en oppervlaktewater. Daarmee faciliteert ‘Maatregel op de Kaart’ onder meer agrariërs bij het nemen van de juiste maatregelen om de doelen van de Kaderrichtlijn Water te halen.

Abstract

De kwaliteit van het grond- en oppervlaktewater in Nederland is de afgelopen decennia weliswaar verbeterd, maar is op veel plekken nog niet op orde. De landbouwsector lanceerde daarom in 2017 een lijst met vrijwillige maatregelen om uitspoeling van meststoffen tegen te gaan. Om het nemen van maatregelen makkelijker te maken is een kaart ontwikkeld die voor elk landbouwperceel in Nederland aangeeft welke vrijwillige maatregelen kansrijk zijn. Deze maatregelenkaart is voor iedereen beschikbaar en wordt in 2020 verder doorontwikkeld. De kaart biedt een basis om met verschillende partijen te werken aan een betere waterkwaliteit.

Abstract

The aim of this paper is to review intranasal trigeminal system and associated reflexes. The literature survey was performed on PubMed, ProQuest Central database of Kirikkale University and Google Scholar. The intranasal trigeminal system and associated reflexes play an important role in humans in both health and disease, including in rhinitis of non-allergic and mixed type. The intranasal trigeminal nerve provides sensory perception to the lining of the nose, supplying information on how patent the nasal airway is and responding to various chemical signals. The reflexes known to exist within the intranasal trigeminal system are nasobronchial reflex, trigemino-cardiac reflex, nasogastric reflex, and nasal cycle. The intranasal trigeminal system and its reflexes play a vital role in normal human physiology. Alterations in how this system operates may underlie multiple forms of rhinitis and more research is needed to fully understand the mechanisms involved. [ABSTRACT FROM AUTHOR]

Abstract

This EAACI position paper aims at providing a state-of-the-art overview on nonallergic rhinitis (NAR). A significant number of patients suffering from persistent rhinitis are defined as nonallergic noninfectious rhinitis (NANIR) patients, often denominated in short as having NAR. NAR is defined as a symptomatic inflammation of the nasal mucosa with the presence of a minimum of two nasal symptoms such as nasal obstruction, rhinorrhea, sneezing, and/or itchy nose, without clinical evidence of endonasal infection and without systemic signs of sensitization to inhalant allergens. Symptoms of NAR may have a wide range of severity and be either continuously present and/or induced by exposure to unspecific triggers, also called nasal hyperresponsiveness (NHR). NHR represents a clinical feature of both AR and NAR patients. NAR involves different subgroups: drug-induced rhinitis, (nonallergic) occupational rhinitis, hormonal rhinitis (including pregnancy rhinitis), gustatory rhinitis, senile rhinitis, and idiopathic rhinitis (IR). NAR should be distinguished from those rhinitis patients with an allergic reaction confined to the nasal mucosa, also called “entopy” or local allergic rhinitis (LAR). We here provide an overview of the current consensus on phenotypes of NAR, recommendations for diagnosis, a treatment algorithm, and defining the unmet needs in this neglected area of research. © 2017 EAACI and John Wiley and Sons A/S. Abstract

The first European Rhinology Research Forum organized by the European Forum for Research and Education in Allergy and Airway Diseases (EUFOREA) was held in the Royal Academy of Medicine in Brussels on 17th and 18th November 2016, in collaboration with the European Rhinologic Society (ERS) and the Global Allergy and Asthma European Network (GA2LEN). One hundred and thirty participants (medical doctors from different specialties, researchers, as well as patients and industry representatives) from 27 countries took part in the multiple perspective discussions including brainstorming sessions on care pathways and research needs in rhinitis and rhinosinusitis. The debates started with an overview of the current state of the art, including weaknesses and strengths of the current practices, followed by the identification of essential research needs, thoroughly integrated in the context of Precision Medicine (PM), with personalized care, prediction of success of treatment, participation of the patient and prevention of disease as key principles for improving current clinical practices. This report provides a concise summary of the outcomes of the brainstorming sessions of the European Rhinology Research Forum 2016.
