Background and aimsThe DE-PLAN was a European multicenter study, with the primary objective of testing whether a community-based lifestyle modification programme could serve as a means of primary prevention for type 2 diabetes (T2D) in high-risk individuals (based on the FINDRISC questionnaire). The aim of this study was to examine the impact of a 1-year community-based lifestyle intervention on health-related quality of life (HRQOL) in individuals from four participating European centers (Athens, Barcelona, Krakow, Kaunas), through a post-hoc analysis.Materials and methodsEach center was allowed to implement different intervention strategies specifically tailored to the needs of their corresponding population sample. Before and after the intervention, participants underwent clinical evaluation, anthropometric measurements, an oral glucose tolerance test and lipid profile measurements. Health-related quality of life was assessed using the validated HRQOL-15D questionnaire. A difference of ±0.015 in the 15D questionnaire score was set as the threshold of clinically meaningful change.ResultsData from 786 participants (67% females, mean age 59.7±9.4 years, BMI 31.5±4.5 kg/m2) with complete data regarding the HRQOL were analyzed (Athens: 104, Barcelona: 434, Krakow: 175, Kaunas: 70). After 1 year, a significant overall improvement in HRQOL was shown, as depicted by a change of 15D score from baseline value (0.88±0.9) to post-intervention (0.90±0.87, PConclusionA community-based lifestyle intervention programme aiming at T2D prevention, applied on a heterogeneous population and with varied methods, was shown to improve overall health-related quality of life to a clinically meaningful degree.

The DE-PLAN-CAT project (Diabetes in Europe-Prevention using lifestyle, physical activity and nutritional intervention-Catalonia) has shown that an intensive lifestyle intervention is feasible in the primary care setting and substantially reduces the incidence of diabetes among high-risk Mediterranean participants. The DP-TRANSFERS project (Diabetes Prevention-Transferring findings from European research to society) is a large-scale national programme aimed at implementing this intervention in primary care centres whenever feasible.A multidisciplinary committee first evaluated the programme in health professionals and then participants without diabetes aged 45-75 years identified as being at risk of developing diabetes: FINDRISC (Finnish Diabetes Risk Score)>11 and/or pre-diabetes diagnosis. Implementation was supported by a 4-channel transfer approach (institutional relationships, facilitator workshops, collaborative groupware, programme website) and built upon a 3-step (screening, intervention, follow-up) real-life strategy. The 2-year lifestyle intervention included a 9-hour basic module (6 sessions) and a subsequent 15-hour continuity module (10 sessions) delivered by trained primary healthcare professionals. A 3-level (centre, professionals and participants) descriptive analysis was conducted using cluster sampling to assess results and barriers identified one year after implementation.The programme was started in June-2016 and evaluated in July-2017. In all, 103 centres covering all the primary care services for 1.4 million inhabitants (27.9% of all centres in Catalonia) and 506 professionals agreed to develop the programme. At the end of the first year, 83 centres (80.6%) remained active and 305 professionals (60.3%) maintained regular web-based activities. Implementation was not feasible in 20 centres (19.4%), and 5 main barriers were prioritized: lack of healthcare manager commitment; discontinuity of the initial effort; substantial increase in staff workload; shift in professional status and lack of acceptance. Overall, 1819 people were screened and 1458 (80.1%) followed the lifestyle intervention, with 1190 (81.6% or 65.4% of those screened) participating in the basic module and 912 in the continuity module (62.5% or 50.1%, respectively).A large-scale lifestyle intervention in primary care can be properly implemented within a reasonably short time using existing public healthcare resources. Regrettably, one fifth of the centres and more than one third of the professionals showed substantial resistance to performing these additional activities.

Rationale and Objective: Latinx individuals are at a higher risk for kidney failure than non-Latinx White individuals; however, they are less likely to receive pre-kidney failure medical care. The objective of this study was to determine the feasibility and acceptability of a community health worker (CHW) intervention that facilitated access to medical care for Latinx individuals. Study Design: Single-arm prospective study. Setting and Participants: Latinx adults were found to have albuminuria or risk factors for kidney disease at community screening events in Austin, Texas. Intervention: A 6-month CHW intervention that facilitated the following: (1) obtaining medical insurance; (2) medical care coordination with primary and nephrology care; (3) kidney disease education; and (4) connection with local resources to address health-related social needs. Outcomes: Recruitment, retention, medical care linkage, and participant and CHW-reported satisfaction with the intervention. Results: Of the 173 individuals who attended the 2 community screening events, 49 agreed to participate in the study, of whom, 51% were men with a mean ± standard deviation (SD) age of 45 ± 14 years, and all self-identified as Mexican or Chicano. The mean ± SD estimated glomerular filtration rate (eGFR) was 110 ± 21 mL/min/1.73 m2 and 41% of the participants reported a urine albumin-creatinine ratio of ≥30 mg/g. Among those enrolled, 28 of the 49 (57%) completed at least 1 CHW visit, and 20 of 49 (41%) completed the intervention. 7 individuals who needed assistance with insurance obtained insurance, and 15 of 20 (75%) scheduled an appointment with a primary care physician within 180 days. Participants reported that the US health care previously seemed inaccessible but gained insurance, the ability to navigate the system, and the ability to help others in their community to access medical care because of the program. Limitations: Small sample size and a single community may limit generalizability. Conclusions: We reported the acceptability of a CHW intervention. We encountered challenges with feasibility and identified strategies to overcome them. Studies are needed to test the effect of CHW interventions on outcomes and kidney health disparities. En este artículo se publican una tegula romana y diez piezas inéditas de material latericio, la mayoría de ellas de la tardo-antigüedad, que se conservan en el Museo de Jerez.

Las provincias del centro/norte de Argentina, como Entre Ríos, Corrientes, Santa Fe, Catamarca, Salta y Jujuy, juegan un papel muy importante en la historia de la paleoherpetología de nuestro país. Han sido escenario de numerosas exploraciones geológico-paleontológicas y territorio donde crecieron parte de las instituciones que, a través de sus colecciones, protagonistas y líneas de investigación, construyeron y construyen el conocimiento de esta disciplina en América del Sur. En este artículo se presentan los principales hechos y protagonistas que dejaron su impronta en esta historia, algunos de los cuales hoy trabajan en los distintos museos, institutos y universidades nacionales de estas provincias del Noreste y Noroeste Argentino.

Introducción: el consumo de sustancias ilícitas y la ingesta compulsiva de alcohol son situaciones que causan efectos negativos para el individuo y la sociedad. La asociación de trastornos psiquiátricos al abuso o dependencia de drogas, ha alcanzado importancia en los últimos años, hasta proponerse el término de diagnóstico dual, para referirse a esta asociación y verla como una entidad. Objetivo: establecer la prevalencia del consumo de alcohol y drogas ilícitas en pacientes hospitalizados en instituciones psiquiátricas. Metodología: estudio descriptivo, transversal, retrospectivo. Se recolectó información desde las historias clínicas de pacientes hospitalizados en los años 2008 y 2009 en tres instituciones psiquiátricas de los municipios de Cartagena y Turbaco en Colombia. Se calculó la prevalencia porcentual de consumo compulsivo de alcohol, consumo de sustancias ilícitas y de ambas con respecto a la totalidad de pacientes hospitalizados. Resultados: 2526 pacientes fueron ingresados en los dos años en las tres instituciones, 1086 pacientes en el 2008 y 1440 en el 2009. La prevalencia de consumo compulsivo de alcohol y sustancias ilícitas fue de 19,5%, la prevalencia de consumo compulsivo de alcohol fue de 2,3% y de consumo de sustancias ilícitas fue de 18,6%. La sumatoria no es exacta debido a que es común encontrar policonsumo. No se observó diferencia entre los dos años en ninguna de las tres evaluaciones. La sustancia ilícita de mayor consumo fue la marihuana. Conclusión: la prevalencia encontrada de consumo de sustancias ilícitas y consumo compulsivo de alcohol en enfermos mentales es una situación que debe ser adecuadamente abordada, para ofrecer una correcta intervención sanitaria. Introducción: las crisis sicógenas se definen como cambios en la conducta, en la sensopercepción o en la actividad cognitiva, simulando trastornos epilépticos generalizados o parciales, sin la presencia de descargas neuronales anómalas. Objetivo: determinar los trastornos psiquiátricos más frecuentes que se presentan en pacientes con el diagnóstico de crisis psicógenas. Métodos: estudio realizado en la Fundación Centro Colombiano contra la Epilepsia y Enfermedades Neurológicas, FIRE, Cartagena, Colombia. Se incluyeron 39 pacientes que resultaron con crisis de origen psicógeno de 250 video telemetrías realizadas entre los años 2008 - 2009, a los cuales se les aplicó una entrevista estructurada, conocida como mini entrevista neuropsiquiatríca internacional (MINI), la cual evalúa los principales diagnósticos psiquiátricos del eje I. Se les realizó además, una encuesta con datos socio demográficos tales como, edad, sexo, estado civil, nivel de escolaridad; además de datos acerca del tipo de crisis, el tiempo de evolución, la presencia de diagnósticos psiquiátricos conocidos y antecedentes de abuso sexual. Los datos obtenidos fueron analizados en el programa estadístico STATA 10.1, obteniendo proporciones para las variables categóricas, mediana y media para las variables continuas. Resultados: al 85% de los pacientes con crisis psicógenas se les identificó una patología psiquiátrica asociada. La enfermedad más prevalente fue trastorno depresivo mayor, seguido de trastorno de ansiedad y distimia. El 30% de los pacientes tenía antecedente de abuso sexual y el 61% de abuso físico. Conclusiones: la población diagnosticada con crisis psicógena evaluada presentó una alta prevalencia de trastornos psiquiátricos. El reconocimiento y manejo de estas patologías asociadas, contribuyen a mejorar el curso de las entidades. Rev.cienc. biomed. 2011; 2 (2): 270-275

Introducción: la depresión es la principal causa de discapacidad y de suicidio a nivel mundial, se ha relacionado con diferentes patologías. Al menos el 65% de los pacientes con infarto agudo al miocardio (IAM) experimentan síntomas depresivos y 15 a 20% son diagnosticados con depresión mayor. No se conoce en el medio la prevalencia de depresión luego de IAM. Objetivos: estimar la prevalencia de depresión posterior a un episodio de IAM en tres hospitales de la ciudad de Cartagena, Colombia y describir la severidad de la depresión, el riesgo de complicaciones del IAM por medio del score GRACE y la morbimortalidad intrahospitalaria. Metodología: estudio descriptivo donde se evaluaron 76 pacientes que ingresaron a las siguientes instituciones: Clínica Universitaria San Juan de Dios, Hospital Universitario del Caribe y Nuevo Hospital Bocagrande, con diagnóstico de IAM desde octubre 2011 hasta junio 2012. A los participantes se les aplicó la escala HADS y el test de Zung para evaluar presencia de depresión. De las historias clínicas se obtuvieron los datos sociodemográficos, el examen físico y de laboratorio al igual que información sobre complicaciones intrahospitalarias. Se calculó la puntuación del score de riesgo de GRACE. Resultados: con el Test de Zung se encontró prevalencia del 79.0% de depresión. 18.4% de los pacientes sin depresión, 48.7% con depresión leve, 25.0% depresión moderada y 5.3% depresión grave. Con la escala HADS se estableció 10.5% de presencia de depresión. 19.7% sintomatología probable de depresión y 69.7% sin depresión. Entre los paciente que tuvieron score de riesgo de GRACE alto, el 84.4% presentó depresión según escala de Zung y el 12.5% según escala de HADS. Conclusión: la alta prevalencia encontrada de depresión, puede obedecer a que los pacientes con IAM reúnen factores de riesgo para depresión: edad avanzada, inactividad laboral y deterioro de la salud. The fossil record of Alligatoroidea in Argentina is mainly represented by Caimaninae alligatorids. This lineage recorded two impor- tant moments in its natural history, one at the beginning of the Paleogene (Late Paleocene–Middle Eocene) and the other in the Neogene (Late Miocene). The most ancient record of alligatoroids in South America comes from the Early Paleocene of Patagonia. It includes basal forms of caimanines such as Necrosuchus ionensis, Eocaiman palaeocenicus, Eocaiman cavernensis, and probably a new species of alligatoroid, which provide key morphological information on the evolutionary and biogeographic history of these crocodylians. Another important moment of the evolutionary history of caimanines is the diversification of the lineage observed during the Miocene. Although there is some isolated cranial material of caimanines from the Late Miocene of northwestern Argentina, the most abundant and diverse fossil Miocene material comes from the northeast, from a level informally called “Conglomerado Osífero”. Two genera of caimanines are known from this stratigraphic level (Caiman and Mourasuchus) with at least five valid species. Here we present an update of the knowledge of Cenozoic alligatorids of Argentina, as a tribute to Zulma Gasparini for her invaluable contribution to the understanding of the crocodilian evolution in South America.
