Abstract

The persistence of the DAMA/LIBRA (DAMA) modulation over the past two decades has been a source of great contention within the dark matter community. The DAMA collaboration reports a persistent, modulating event rate within their setup of NaI(Tl) scintillating crystals at the INFN Laboratori Nazionali del Gran Sasso (LNGS) underground laboratory. A recent work alluded that this signal could have arisen due to an analysis artefact, caused by DAMA not accounting for time variation of decaying background radioisotopes in their analysis procedure. In this work, we examine in detail this 'induced modulation' effect, arguing that a number of aspects of the DAMA signal are incompatible with an induced modulation arising from decays of background isotopes over the lifetime of the experiment. Using a toy model of the DAMA/LIBRA experiment, we explore the induced modulation effect under different variations of the activities of the relevant isotopes - namely, $^3$H and $^{210}$Pb - highlighting the various inconsistencies between the resultant toy datasets and the DAMA signal. We stress the importance of the SABRE experiment, whose goal is to unambiguously test for the presence of such a modulating signal in an experiment using the same target material and comparable levels of background.

Data from a recent meta-analysis indicated that whole-body electromyostimulation (WB-EMS), a time-effective, joint–friendly, and highly individualized training technology, demonstrated promising effects on LBP; however, methodologic limitations prevent definitive evidence for this result. Thus, the aim of this study was to conduct a randomized controlled WB-EMS trial to determine the corresponding effect on chronic, nonspecific LBP in people with chronic LBP. Patients and methods: Thirty LBP patients, 40–70 years old, were randomly assigned into two groups (WB-EMS: 15; control [CG]: 15). While the nonactive CG maintained their lifestyle, the WB-EMS group completed a 12-week WB-EMS protocol (1×20 min/week) with slight movements, specifically dedicated to LBP. Pain intensity and frequency were determined by a 4-week pain diary before and during the last 4 weeks of intervention. Primary study endpoint was average pain intensity at the lumbar spine. Results: At baseline, no group differences apart from nonregular exercise were observed. Mean intensity of LBP decreased significantly in the WB-EMS group (P=0.002) and remained unchanged in the CG (P=0.730), with a significant difference between both groups (P=0.027). Maximum isometric trunk extensors improved significantly in the WB-EMS group (P=0.005), while no significant difference was seen in the CG (P=0.683). In contrast to the significant difference between WB-EMS group and CG for the latter parameter (P=0.038), no intergroup difference was determined for maximum isometric trunk flexors (P=0.091). The WB-EMS group showed a significant increase of this parameter (P=0.003), while no significant change was determined in the CG (P=0.563). Conclusion: WB-EMS is a time-effective training method for reducing chronic nonspecific LBP and increasing maximum trunk strength in people with such complaints. After this promising comparison with a nonactive CG, research needs to be extended to include comparisons with active groups (WB-Vibration, conventional back strengthening). Abstract

Rare but regularly recurring complications are leading to ongoing regulation of the commercial, non-medical whole-body electromyostimulation (WB-EMS) market. In addition to the revised German Radiation Protection Statutes (NiSV), the Deutsche Industrie Norm (DIN) 33961-5 was recently published with safety policies for WB-EMS application, anchoring both relative and absolute contraindications for WB-EMS for the first time. The purpose of this article is to justify the rationale of contraindications in a commercial setting and to support their consistent application.While the relative contraindicationsappear plausible and uncritical, absolute contraindications for WB-EMS are much more debatable. In fact, some absolute contraindications (e.g. diabetes mellitus) could be safely addressed by WB-EMS at least after careful medical anamnesis and competent, close supervision. However, this requires sound knowledge of WB-EMS on the part of physicians and instructors, low user-trainer ratios and prompt medical care in an emergency.However, considering the multitude of different settings of commercial WB-EMS, in extreme cases with hardly supervised, only video-guided WB-EMS sessions, the necessary accurateness and expertise for safe WB-EMS is not always guaranteed. That there is no mandatory licensing of WB-EMS instructors, the key players in WB-EMS, underscores the concern. Whilst acknowledging the multitude of high-quality suppliers, it is advised thatthe commercial, non-medical WB-EMS sector as a whole to be wary of indications with significantly increased complication potential. Abstract

Hepatic tumors appear as stiff inclusions within the surrounding soft, healthy tissue. In open surgery they are searched for by manual palpation with the gloved fingertip. However, to exploit the benefits of MIS it is mandatory to implement a substitution for the human sense of touch. Therefore, a tactile instrument has been developed with the aim of enlarging the sensing area at the tool tip once it enters the abdominal cavity through the trocar. The provision of a large sensitive surface enables the detection of nearly all sizes of tumors and decreases the time needed for the performance of this task. A prototype was manufactured by laser sintering in PA serving as a carrier for an existing flexible silicone sensor. Automated as well as manual subject palpation tests have shown that a prototypical instrument with a laterally opening lid would be a suitable device for tumor detection in laparoscopic liver surgery.

Abstract

SABRE (Sodium iodide with Active Background REjection) is a direct detection dark matter experiment based on arrays of radio-pure NaI(Tl) crystals. The experiment aims at achieving an ultra-low background rate and its primary goal is to confirm or refute the results from the DAMA/LIBRA experiment. The SABRE Proof-of-Principle phase was carried out in 2020-2021 at the Gran Sasso National Laboratory (LNGS), in Italy. The next phase consists of two full-scale experiments: SABRE South at the Stawell Underground Physics Laboratory, in Australia, and SABRE North at LNGS. This paper focuses on SABRE South and presents a detailed simulation of the detector, which is used to characterise the background for dark matter searches including DAMA/LIBRA-like modulation. We estimate an overall background of 0.72 cpd/kg/keV$_{ee}$ in the energy range 1$-$6 keV$_{ee}$ primarily due to radioactive contamination in the crystals. Given this level of background and considering that the SABRE South has a target mass of 50 kg, we expect to exclude (confirm) DAMA/LIBRA modulation at $4~(5)\sigma$ within 2.5 years of data taking.

Abstract

Recoil imaging entails the detection of spatially resolved ionization tracks generated by particle interactions. This is a highly sought-after capability in many classes of detector, with broad applications across particle and astroparticle physics. However, at low energies, where ionization signatures are small in size, recoil imaging only seems to be a practical goal for micro-pattern gas detectors. This white paper outlines the physics case for recoil imaging, and puts forward a decadal plan to advance towards the directional detection of low-energy recoils with sensitivity and resolution close to fundamental performance limits. The science case covered includes: the discovery of dark matter into the neutrino fog, directional detection of sub-MeV solar neutrinos, the precision study of coherent-elastic neutrino-nucleus scattering, the detection of solar axions, the measurement of the Migdal effect, X-ray polarimetry, and several other applied physics goals. We also outline the R&D programs necessary to test concepts that are crucial to advance detector performance towards their fundamental limit: single primary electron sensitivity with full 3D spatial resolution at the $\sim$100 micron-scale. These advancements include: the use of negative ion drift, electron counting with high-definition electronic readout, time projection chambers with optical readout, and the possibility for nuclear recoil tracking in high-density gases such as argon. We also discuss the readout and electronics systems needed to scale-up such detectors to the ton-scale and beyond., Comment: 77 pages, 20 figures. Abstract

Background: Pseudomonas aeruginosa and other Gram-negative bacteria have the ability to persist in moist environments in healthcare settings, but their spread from these areas can result in outbreaks of healthcare-associated infections. Methods: This study reports the investigation and containment of a multi-drug-resistant P. aeruginosa outbreak in three intensive care units of a Swiss university hospital. In total, 255 patients and 276 environmental samples were screened for the multi-drug-resistant P. aeruginosa outbreak strain. The environmental sampling and molecular characterization of patient and environmental strains, and control strategies implemented, including waterless patient care, are described. Results: Between March and November 2019, the outbreak affected 29 patients. Environmental sampling detected the outbreak strain in nine samples of sink siphons of three different intensive care units with a common water sewage system, and on one gastroscope. Three weeks after replacement of the sink siphons, the outbreak strain re-grew in siphon-derived samples and newly affected patients were identified. The outbreak ceased after removal of all sinks in the proximity of patients and in medication preparation areas, and minimization of tap water use. Multi-locus sequence typing indicated clonality (sequence type 316) in 28/29 patient isolates and all 10 environmental samples. Conclusions: Sink removal combined with the introduction of waterless patient care terminated the multi-drug-resistant P. aeruginosa outbreak. Sinks in intensive care units may pose a risk for point source outbreaks with P. aeruginosa and other bacteria persisting in moist environments. 45. Implementation of daily chlorhexidine bathing in intensive care units for reduction of central line-associated bloodstream infections

Abstract

BACKGROUND Daily chlorhexidine bathing has been associated with a reduction in central line-associated bloodstream infections (CLABSI). In the setting of an already established CLABSI surveillance system and an implemented CLABSI prevention bundle, we analysed the effect of daily chlorhexidine bathing in ICU patients on CLABSI incidence and its causative pathogens. METHODS This was a before-and-after study in intensive care units (ICUs) at a tertiary-care centre in Switzerland. Prospective surveillance of CLABSIs and their aetiologies was established. The intervention consisted of daily chlorhexidine bathing of ICU patients with a central venous catheter. A baseline period of 19 months was followed by an intervention period of 9 months. FINDINGS A total of 5008 patients were included. In the baseline period a mean CLABSI rate of 2.45/1000 catheter days (95% confidence interval (CI) 1.93-3.07) was observed, followed by 1.00/1000 catheter days (95% CI 0.55-1.67; P<0.001) in the intervention period. Introduction of chlorhexidine bathing was independently associated with a reduced risk of CLABSI (adjusted odds ratio 0.47, 95% CI 0.26-0.84, P=0.011). We did not observe a significant change in aetiology except for an increase of Serratia marcescens in the intervention period. CONCLUSIONS Introduction of daily chlorhexidine bathing resulted in a decline in CLABSI incidence on ICUs. Starting from a baseline CLABSI rate that can be considered standard in a high-income setting and several measures for CLABSI prevention implemented, chlorhexidine bathing proved helpful for a further reduction.

Abstract

Systems biology has experienced dramatic growth in the number, size, and complexity of computational models. To reproduce simulation results and reuse models, researchers must exchange unambiguous model descriptions. We review the latest edition of the Systems Biology Markup Language (SBML), a format designed for this purpose. A community of modelers and software authors developed SBML Level 3 over the past decade. Its modular form consists of a core suited to representing reaction-based models and packages that extend the core with features suited to other model types including constraint-based models, reaction-diffusion models, logical network models, and rule-based models. The format leverages two decades of SBML and a rich software ecosystem that transformed how systems biologists build and interact with models. More recently, the rise of multiscale models of whole cells and organs, and new data sources such as single-cell measurements and live imaging, has precipitated new ways of integrating data with models. We provide our perspectives on the challenges presented by these developments and how SBML Level 3 provides the foundation needed to support this evolution.
