Abstract

A study published in the European Journal of Heart Failure compared the effectiveness of sacubitril/valsartan, an angiotensin receptor–neprilysin inhibitor (ARNI), with angiotensin-converting enzyme inhibitors/angiotensin receptor blockers (ACEi/ARB) in patients with newly diagnosed heart failure (HF) and preserved or mildly reduced ejection fraction. The study used a large real-world dataset from the United States and found that patients initiated on sacubitril/valsartan had significantly lower rates of all-cause and cardiovascular hospitalizations compared to those initiated on ACEi/ARB. These findings support the potential benefit of early initiation of sacubitril/valsartan in a diverse and high-risk HF population. However, the study acknowledges limitations, including its retrospective nature and the possibility of unmeasured confounding.
