Abstract Introduction The License, Master and Doctorate (LMD) reform that structured high studies in three cycles, has been instituted since the Bologna declaration in 1999. To be conformed to international standards, the LMD system has been instituted in University of Lomé in 2009 to foster pathways between medical and paramedical training. The purpose of this study was to evaluate the strengths and weaknesses of the LMD reform since its introduction in medical school of Lomé. Method It was an opinion survey conducted during four months in University of Lomé among the medical school’s teachers about strengths and weaknesses of LMD reform since its application. The strengths were defined as all facilities brought by LMD reform in organization of courses and practices, evaluations, new Information and Communication Technologies (ICTs) (internet, video projector, courses on line). The LMD weaknesses were defined as any problem that it could generate. Results Of 113 resident teachers of the medical school of Lomé, seventy-six have completed the questionnaire (67.2%). The majority of teachers (74) thought that the introduction of LMD reform will make Lomé medical school fit into international standards. The availability of the video projectors was mentioned by 90.8% of the teachers and 82.9% of them used it for teaching. Online course was not available. The main strengths of LMD were: a better evaluation system (33.3%), the organization of training in units with credit (28.6%), the usage of new ICTs (23.8%). Respondents also reported many weaknesses of LMD reform: the plethoric number of students (36.2%), the absence of an intermediate diploma and pathways between studies (29.3%). The Insufficiency of human resources and material was also mentioned. Conclusion This study highlights that LMD reform needs adaptation to local realities and improvement to ensure that students will get better training in conformity with international standards.

Background and Importance: Overdrainage is a complication of ventriculoperitoneal shunt but adjustable valves and anti-siphon devices can prevent it. These very expensive valves are most often inaccessible, so that the majority of the valves available in Togo are fixed differential pressure valves. Although overdrainage is a widely-known issue, we aimed to introduce a new risk factor and the way we manage this complication. Case Presentation: This case series study included all patients who had overdrainage or a high potential risk of overdrainage (hydranencephaly). Our technique consisted of partial ligation of the peritoneal catheter at the level of the thorax by non-resorbable wire while controlling the drainage rate at the slots. The goal was to transform this fixed differential pressure valve into a pressure-controlled and flow-regulated one. Patients were followed for 1, 3 and 6 months, postoperatively. Conclusion: Hydranencephaly predisposes patients to overdrainage. The partial ligature of the catheter is an effective technique for treating or preventing overdrainage.

L’objectif était de relever les aspects anesthésiologiques et les complications périopératoires des méningiomes intracrâniens opérés au CHU Sylvanus Olympio de Lomé (CHU SO). Il s’agissait d’une étude rétrospective sur dossiers de patients opérés de méningiomes intracrâniens durant la période de Décembre 2010 à décembre 2015 (5 ans) au CHU SO. 21 (45,6%) méningiomes recensés sur 46 tumeurs cérébrales opérées. Age moyen: 49 ± 20 ans; prédominance masculine (52,4%), ratio M/F: 1,1. Classification ASA: ASA II: 16 patients, ASA III: 4 patients, et ASA IV: 1 patient. L’anesthésie a été intraveineuse totale avec du Propofol (100%) comme hypnotique et du fentanyl (76,2%) comme morphinique le plus disponible. Complications peropératoires: saignement, pertes sanguines moyennes: 1750 ± 584 ml; hypotension avec Pression artérielle moyenne (PAM) 60 mmHg: 10 (47,6%) patients; choc hémorragique 2 (9,5%) patients, arrêt cardiocirculatoire: 01(4,7%) patient réanimé avec succès. Complications postopératoires: convulsions 5 (23,8%) cas, hyperthermie 4 (19%) cas, choc hémorragique 2(9,5%) cas, décès: 2 (9,5%) cas. La morbidité et la mortalité périopératoire de la chirurgie du méningiome intracrânien au CHU SO de Lomé reste élevée. L’amélioration du plateau technique et une consultation précoce permettrait de réduire ces complications.

Le but de notre étude était de déterminer la fréquence des fractures de membres liées à l'exercice de la fonction militaire au sein des Forces de Défense et de Sécurité en milieu africain en vue de ressortir l'impact des différentes circonstances de survenue. Nous avons entrepris une étude rétrospective descriptive allant du 1er janvier 2004 au 31 décembre 2013. Elle a concerné les agents des forces de défense et de sécurité traités pour des fractures de membres au cours de notre période d'étude. Sept cent quatre (704) cas de fractures de membres ont été dénombrées. La moyenne d'âge des patients était de 30,57 ans avec des extrêmes de 19 et 55 ans et une nette prédominance masculine (95,71%). L'Armée de Terre (51,05%) et la Gendarmerie Nationale (38,86%) étaient les plus représentées. Les hommes du rang étaient majoritaires (43,08%), suivis des sous-officiers (32,59%). La fréquence annuelle des fractures de membres en rapport avec la profession militaire était de 63 cas. Les fractures de jambe étaient les lésions les plus recensées (32,96 %). Les Formations et les stages militaires ont été les circonstances de survenue les plus rencontrées (42,60 %), suivies des accidents de la circulation (39,43%). La perte des journées de service liée à ces lésions était estimée à 14009 jours par an. Les fractures de jambes occupent le premier rang des fractures de membres en rapport avec l'exercice de la profession militaire. Les formations-stages militaires et les accidents de la voie publique en sont les deux grandes circonstances de survenue.
