Abstract: International audience; Background: Speech disorders are amongst the first symptoms to appear in Parkinson's disease (PD). Objectives: We aimed to characterize PD voice signature from the prodromal stage (isolated rapid eye movement sleep behavior disorder, iRBD) to early PD using an automated acoustic analysis and compare male and female patients. We carried out supervised learning classifications to automatically detect patients using voice only. Methods: Speech samples were acquired in 256 French speakers (117 participants with early PD, 41 with iRBD, and 98 healthy controls), with a professional quality microphone, a computer microphone and their own telephone. High-level features related to prosody, phonation, speech fluency and rhythm abilities were extracted. Group analyses were performed to determine the most discriminant features, as well as the impact of sex, vocal tasks, and microphone type. These speech features were used as inputs of a support vector machine and were combined with classifiers using low-level features. Results: PD related impairments were found in prosody, pause durations and rhythmic abilities, from the prodromal stage. These alterations were more pronounced in men than in women. Early PD detection was achieved with a balanced accuracy of 89% in males and 70% in females. Participants with iRBD were detected with a balanced accuracy of 63% (reaching 70% in the subgroup with mild motor symptoms). Conclusion: This study provides new insight in the characterization of sex-dependent early PD speech impairments, and demonstrates the valuable benefit of including automated voice analysis in future diagnostic procedures of prodromal PD.

Abstract: We propose a framework for optimizing personalized treatment outcomes for patients with neurological diseases. A typical consequence of such diseases is gait disorders, partially explained by command and muscle tone problems associated with spasticity. Intramuscular injection of botulinum toxin type A is a common treatment for spasticity. According to the patient’s profile, offering the optimal treatment combined with the highest possible benefit-risk ratio is important. For the prediction of knee and ankle kinematics after botulinum toxin type A (BTX-A) treatment, we propose: (1) a regression strategy based on a multi-task architecture composed of LSTM models; (2) to introduce medical treatment data (MTD) for context modeling; and (3) a gating mechanism to model treatment interaction more efficiently. The proposed models were compared with and without metadata describing treatments and with serial models. Multi-task learning (MTL) achieved the lowest root-mean-squared error (RMSE) (5.60°) for traumatic brain injury (TBI) patients on knee trajectories and the lowest RMSE (3.77°) for cerebral palsy (CP) patients on ankle trajectories, with only a difference of 5.60° between actual and predicted. Overall, the best RMSE ranged from 5.24° to 6.24° for the MTL models. To the best of our knowledge, this is the first time that MTL has been used for post-treatment gait trajectory prediction. The MTL models outperformed the serial models, particularly when introducing treatment metadata. The gating mechanism is efficient in modeling treatment interaction and improving trajectory prediction.

Abstract: Many articles have used voice analysis to detect Parkinson's disease (PD), but few have focused on the early stages of the disease and the gender effect. In this article, we have adapted the latest speaker recognition system, called x-vectors, in order to detect an early stage of PD from voice analysis. X-vectors are embeddings extracted from a deep neural network, which provide robust speaker representations and improve speaker recognition when large amounts of training data are used. Our goal was to assess whether, in the context of early PD detection, this technique would outperform the more standard classifier MFCC-GMM (Mel-Frequency Cepstral Coefficients - Gaussian Mixture Model) and, if so, under which conditions. We recorded 221 French speakers (including recently diagnosed PD subjects and healthy controls) with a high-quality microphone and with their own telephone. Men and women were analyzed separately in order to have more precise models and to assess a possible gender effect. Several experimental and methodological aspects were tested in order to analyze their impacts on classification performance. We assessed the impact of audio segment duration, data augmentation, type of dataset used for the neural network training, kind of speech tasks, and back-end analyses. X-vectors technique provided better classification performances than MFCC-GMM for text-independent tasks, and seemed to be particularly suited for the early detection of PD in women (7 to 15% improvement). To increase the system reliability, several biometric devices are often used. Such a combined system is known as a multimodal biometric system. This paper reports a benchmarking study carried out within the framework of the BioSecure DS2 (Access Control) evaluation campaign organized by the University of Surrey, involving face, fingerprint, and iris biometrics for person authentication, targeting the application of physical access control in a medium-size establishment with some 500 persons. While multimodal biometrics is a well-investigated subject in the literature, there exists no benchmark for a fusion algorithm comparison. Working towards this goal, we designed two sets of experiments: quality-dependent and cost-sensitive evaluation. The quality-dependent evaluation aims at assessing how well fusion algorithms can perform under changing quality of raw biometric images principally due to change of devices. The cost-sensitive evaluation, on the other hand, investigates how well a fusion algorithm can perform given restricted computation and in the presence of software and hardware failures, resulting in errors such as failure-to-acquire and failure-to-match. Since multiple capturing devices are available, a fusion algorithm should be able to handle this nonideal but nevertheless realistic scenario. In both evaluations, each fusion algorithm is provided with scores from each biometric comparison subsystem as well as the quality measures of both the template and the query data. The response to the call of the evaluation campaign proved very encouraging, with the submission of 22 fusion systems. To the best of our knowledge, this campaign is the first attempt to benchmark quality-based multimodal fusion algorithms. In the presence of changing - - image quality which may be due to a change of acquisition devices and/or device capturing configurations, we observe that the top performing fusion algorithms are those that exploit automatically derived quality measurements. Our evaluation also suggests that while using all the available biometric sensors can definitely increase the fusion performance, this comes at the expense of increased cost in terms of acquisition time, computation time, the physical cost of hardware, and its maintenance cost. As demonstrated in our experiments, a promising solution which minimizes the composite cost is sequential fusion, where a fusion algorithm sequentially uses match scores until a desired confidence is reached, or until all the match scores are exhausted, before outputting the final combined score., This evaluation was made possible thanks to the EU funding from the following projects: BioSecure (www.biosecure.info) and Mobio (www.mobioproject.org). Abstract: International audience; In this paper, ultrasound measurements of 1:1 scale three-dimensional (3D) printed trabecular bone phantoms are reported. The micro-structure of a trabecular horse bone sample was obtained via synchrotron x-ray microtomography, converted to a 3D binary data set, and successfully 3D-printed at scale 1:1. Ultrasound through-transmission experiments were also performed through a highly anisotropic version of this structure, obtained by elongating the digitized structure prior to 3D printing. As in real anisotropic trabecular bone, both the fast and slow waves were observed. This illustrates the potential of stereolithography and the relevance of such bone phantoms for the study of ultrasound propagation in bone.

Abstract: International audience; This paper presents the design and performance of a single-ended transimpedance amplifier (TIA) for gigabit passive optical networks (GPON) implementing orthogonal frequency division multiplexing (OFDM) modulation format. The circuit is realized using an industrial GaAs integrated technology. Low power consumption and small chip area are the main challenges in the TIA design. On-wafer characterization in terms of S-parameters, noise figure, gain compression and intermodulation are presented and compared to simulated results showing a good agreement. The TIA is linked with a broadband PIN photodiode and introduced in a radio over fiber (RoF) system. The optical link is simulated in a microwave software and characterized in terms of error vector magnitude (EVM) by varying the radio frequency (RF) input power and the laser bias current.

Abstract: International audience; Although underexplored, the idea of using brain computer interfaces (BCI) for behavioral and cognitive rehabilitation is based on recent evidence suggesting that not only self-regulated brain signals, but also involuntary brain signals may provide useful information about the BCI user. The aim is to promote the recruitment of neural networks involved in specific brain functions and to facilitate Central Nervous System (CNS) plasticity. Critically, CNS plasticity has been observed in early-stages of dementia, thus constituting a great challenge for the development of "cognitive BCIs" focused on the rehabilitation of brain functions in neurological patients. The main goal of this project is to promote CNS plasticity, and therefore cognitive reserve, through neurofeedback training in subjects with Subjective Memory Complaints (SMC) related to attentional deficits. Abstract: International audience; The future technology migration in access networks compels the development of key innovative transmitters operating at 10 Gb/s around 1550 nm and capable of transmitting data in extended reach passive optical networks (>60 km). A laser modulated directly appears to be a low cost and a simple solution to address these needs. However, the extinction ratio and the distance of transmission are limited by frequency chirping inherent to high bit rate modulation at 1550 nm. In this letter, we demonstrate the monolithic integration of a directly modulated lasers and a ring resonator (RR) and show the possibility to engineer the RR with specific frequency modulation efficiency. The ring is used as an optical eye reshaper, which enables the increase of the extinction ratio above 9 dB. Transmissions up to 65 km with 5.8 dBm of modulated optical power coupled into the fiber are demonstrated, proving the feasibility of our transmitter for the next generation passive optical network stage 2

Abstract: International audience; Flex-grid technology is an effective mean to improve the spectral efficiency of optical communications. For a given amplifier spectral bandwidth, it gives rise to the increase of the number of optical channels as it reduces the channel spacing. Therefore, in order to reap full benefits from flex-grid saved spectrum, further amplification power is required with respect to conventional fixed grid. This is a strong limitation if the legacy amplifiers cannot meet this new requirement due to their optical power limits. In this work, we demonstrate that exploiting the link margins allows supporting this increase while maintaining in use legacy amplifiers.

Abstract: National audience; Parmi les manifestations cliniques de la maladie de Parkinson, les troubles de la voix surviennent particulièrement tôt dans la maladie. Ils prennent principalement la forme d'une insuffisance prosodique, articulatoire et phonatoire. La voix des patients devient monotone, à débit variable, éraillée ou soufflée, et les articulations deviennent imprécises. Notre objectif est de dresser un état de l'art concernant les études réalisées sur l'analyse de la voix comme outil de diagnostique précoce de la maladie de Parkinson. Nous nous intéressons aux études qui utilisent des paramètres acoustiques vocaux pour détecter la maladie de Parkinson chez des parkinsoniens récents, et chez des parkinsoniens en phase préclinique. L'ensemble des résultats porte à croire que la voix pourrait à terme être utilisée en clinique pour établir un diagnostique précoce de la maladie de Parkinson. Cependant le faible nombre d'études acoustiques au stade préclinique, et le faible nombre de patients dans chaque étude nous incitent à être plus prudents quant aux conclusions. Des études longitudinales, avec des groupes plus importants de patients, semblent nécessaires pour valider ces résultats

Abstract: International audience; We demonstrate that the birefringence of the liquid-crystal cell (LCC) can be varied by applying different frequency values of a single applied ac square voltage. For the experimental evaluation of the birefringence, associated with a certain wavelength λ, as a function of the frequency FLCC of the electrical signal applied to the LCC, we use, for the first time to our knowledge, what we call here a frequency-dependent transmission technique. It consists in measuring the transmission responses between crossed and parallel polarizers as a function of the frequency FLCC. Experimental tests were carried out using a 7 μm-thick E63 nematic LCC and a laser source emitting at λ=1.55 μm with a launch power of −3 dBm. The tuning voltage VLCC applied to the LCC is an alternative square wave electrical signal whose frequency ranges from 0.5 to 15 kHz. The peak to peak amplitude of the electrical signal is 5 V. The curve of the measured variations of the optical path difference of the LCC versus the frequency FLCC has a positive slope. Application to the tuning of the center wavelength of the transmission response of a one stage hybrid birefringent filter is shown as a proof-of-principle test

Abstract: International audience; OBJECTIVES: The relationship between anemia, renal insufficiency, and the outcomes of TAVI patients has not been thoroughly studied. We aimed to evaluate the influence of pre- and post-procedural anemia on the incidence of renal insufficiency, especially AKI, and on the outcomes of TAVI. METHODS: Data from the French national TAVI registry were collected in 3,472 patients who underwent TAVI between January 2010 and December 2012. Of these 2,137 were in the no/mild anemia group, 748 were in the moderate anemia group, and 587 were in the severe anemia group before TAVI. Furthermore, we divided the 3,472 patients into three groups according to post-procedural anemia, measured as post-procedural hemoglobin (Hb) drop: \textless2 g/dl (n=1,633, group 1), 2 to \textless4 g/dl (n=1,458, group 2), and \textgreater4 g/dl (n = 381, group 3). Procedure and outcome variables were compared. RESULTS: Increased severity of anemia before TAVI was associated with significantly different rates of 1-year mortality (15%, 19%, and 24%, P\textless0.01), with similar differences in the incidence of AKI (5%, 8%, and 10%, P\textless0.01). Increased severity of Hb drop was associated with significantly different rates of 1-year mortality (16%, 18%, and 23%, P\textless0.01), and with similar differences in the incidence of AKI (6%, 7%, and 10%, P=0.04). Both pre- and post-procedural anemia were predictors of the incidence of AKI (OR 1.82, P\textless0.01; OR 1.82, P\textless0.01, respectively) and 1-year mortality (HR 1.44, P\textless0.01; HR 1.50, P\textless0.01, respectively). CONCLUSIONS: Both pre- and post-procedural anemia were significantly associated AKI and 1-year mortality

Abstract: International audience; Flexible grid optical networks allow an efficient utilization of spectrum resources using 12.5-GHz frequency slot multiples instead of a fixed spacing, introducing however spectrum fragmentation (SF). In the literature, SF is often assumed to be a serious problem specifically in a dynamic traffic context. It is mostly related to the bandwidth blocking ratio due to the lack of relevant comparison criteria and efficient metrics. Besides, in operator core network, traffic behavior is instead incremental and it is forecasted for short periods of time in addition to some operational constraints that make of it a specific context. In this work, we present an exhaustive analysis and an accurate evaluation for SF issue in flexible optical networks. We also propose new metric for fragmentation measurements and some approaches to address such a problem.

Abstract: This paper address the development of a new technic in the sleep analysis domain. Sleep is defined as a periodic physiological state during which vigilance is suspended and reactivity to external stimulations diminished. We sleep on average between six and nine hours per night and our sleep is composed of four to six cycles of about 90-minutes each. Each of these cycles is composed of a succession of several stages of sleep, more or less deep. The analysis of sleep is usually done using a polysomnography. This examination consists of recording, among other things, electrical cerebral activity by electroencephalography (EEG), ocular movements by electrooculography (EOG) and chin muscle tone by electromyography (EMG). The recording is done mostly in a hospital, more specifically in a service for monitoring the pathologies related to sleep. The readings are then interpreted manually by an expert to generate a hypnogram, a curve showing the succession of sleep stages during the night in 30-second epochs. The proposed method is based on the follow-up of the thermal signature that makes it possible to classify the activity into three classes: "awakening", "calm sleep" and "agitated sleep". The contribution of this non-invasive method is part of the screening of sleep disorders, to be validated by a more complete analysis of the sleep. The measure provided by this new system, based on temperature monitoring (patient and ambient), aims to be integrated into the tele-medicine platform developed within the framework of the Smart-EEG project by the SYEL - SYstèmes ELectroniques team. Analysis of the data collected during the first surveys carried out with this method showed a correlation between thermal signature and activity during sleep. The advantage of this method lies in its simplicity and the possibility of carrying out measurements of activity during sleep and without direct contact with the patient at home or hospitals.

Abstract: The exponential traffic growth in optical networks has triggered the evolution from fixed-grid to flex-grid technology. This evolution allows better spectral efficiency and spectrum usage over current optical networks in order to facilitate huge dynamic traffic demands. The promise of flex-grid technology in terms of increasing the number of optical channels established over optical links, however, may not be sustainable because of the associated increase in optical amplification power. In this work, we detail a power control process that takes advantage of link optical power and channel optical signal-to-noise ratio (OSNR) margins to allow network operators to support this optical power increase while maintaining the use of legacy optical amplifiers. New generalized multiprotocol label switching (GMPLS) protocol extensions are proposed on which to integrate the optical power control process in the control plane. The performance of the process is evaluated in terms of the blocking ratio and network throughput over fixed-grid and flex-grid networks. Results show that controlling optical power benefits from the flex-grid technology in terms of spectrum and capacity gain and reduces optical connection blocking.
