- Abstract
This study (FIS-PI18/00912) was supported by the Instituto de Salud Carlos III (Plan Estatal de I+D+i 2013–2016) and cofinanced by the European Development Regional Fund ‘‘A way to achieve Europe’’ (ERDF) and B2017/BMD-3804 MITIC-CM., Objective: To compare pelvic floor disorders between vaginal delivery (VD) and cesarean delivery (CD). Methods: For this study, a PUBMED database search was used, utilizing a combination of relevant medical subjects’ headings (MeSH) terms, with the following keywords: “Pelvic floor disorders” or “Pelvic floor morbidity” and “Delivery”. Search limits were articles in English or Spanish, about women, published from December 2009 to December 2019. The STATA 16 package was used for meta-analysis and data heterogeneity assessment. Results: Thirteen studies meeting eligibility criteria were identified comprising 1,597,303 participants. Abstract: Pelvic floor morbidity prevalence was Urinary Incontinence (UI) 27.9% (5411 patients in 7 studies with reported cases), Pelvic Organ Prolapse (POP) 14.2% (6019 patients in 8 studies with reported cases), and Anal Incontinence (AI) 0.4% (1,589,740 patients in 5 studies with reported cases). Our meta-analyses revealed significantly higher rates of all three morbidities and overall morbidity in the VD versus CD group: UI OR = 2.17, 95% CI 1.64–2.87, p for heterogeneity ≤ 0.0001, I2 = 84%; POP OR = 3.28, 95% CI 1.91–5.63, p for heterogenicity ≤ 0.043, I2 = 63%; AI OR = 1.53, 95% CI 1.32–1.77; p for heterogeneity ≤ 0.291, I2 = 20%; and overall morbidity (OR = 2.17, 95% CI 1.64–2.87; p for heterogeneity ≤ 0.0001, I2 = 84%). Conclusion: Vaginal delivery is directly related to the appearance of pelvic floor disorders, mainly UI, POP, and AI. El riesgo de Pérdida del Bienestar Materno Perinatal (RPBMP) supone la aparición de cualquier entidad de muerte o morbilidad en el binomio materno-fetal, que se relacionan entre sí. En esta Memoria, para la definición y el análisis de la muerte y morbilidad materna se han seguido los criterios planteados por la OMS, el Near Miss y el Euro- Peristat. Mediante un análisis retrospectivo de los partos atendidos en la Maternidad del Hospital General Universitario Gregorio Marañón durante 2011-2015, de las variables clínicas maternas pregestacionales, gestacionales, durante el parto y el postparto así como las neonatales, se establecen la correlación de dichas variables con el riesgo de padecer cada uno de los eventos mórbidos maternos descritos con anterioridad. Dicho análisis se realiza mediante un regresión logística univariada y multivariada, esta última para ajustar la magnitud de efecto de cada variable y la de averiguar un modelo óptimo que minimice el número de variables alcanzando el mayor índice de predicción. Entre los resultados encontrados en un total de 24.059 madres y 24.874 recién nacidos queremos destacar una tasa de muerte fetal de 3-4 por mil partos y dos muertes maternas durante el periodo (8,08 por cien mil recién nacidos vivos). En cuanto al perfil de pacientes relacionados con eventos mórbidos concretamente los trastornos hemorrágicos fueron más frecuentes en multíparas, con patología gestacional que han requerido ingreso, antes de la semana 37 de embarazo, cuyo parto fue asistido durante el turno no programado (guardia) que ha requerido anestesia general, con alteraciones durante el alumbramiento y en recién nacidos con anomalías del crecimiento intrauterino..., The risk of loss of maternal perinatal well-being (RPBMP) implies the appearance of any entity of death or morbidity in the maternal-fetal binomial, which are related to each other. In this Report, the criteria set out by WHO, Near Miss and Euro-Peristat, have been followed for the definition and analysis of death and morbidity. A retrospective analysis of the deliveries attended at the Maternity Hospital of the General University Hospital during 2011-2015, of the clinical variables (maternal pregestational, gestational, during delivery and postpartum as well as neonatal), and the correlation of variables is established. Risk of suffering each of the material morbid events previously. This analysis is performed using a univariate and multivariate logistic regression, the latter to adjust the magnitude of effect of all variables and that of a model that minimizes the number of variables that reached the highest prediction index. Among the results found in a total of 24,059 mothers and 24,874 newborns we want to highlight a fetal death rate of 3-4 per thousand births and two maternal deaths during the period (8.08 per one hundred thousand live births). The profile of patients related to morbid events, specifically the hemorrhagic disorders were more frequent multiparous, with gestational pathology that have required admission, before the 37th week of pregnancy whose delivery was attended during the unscheduled (guard) shift that required general anesthesia, with alterations during childbirth and newborns with intrauterine growth abnormalities...

WOS:000613461600006 PubMed ID: 32926494 Objectives To evaluate the maternal and perinatal outcomes of pregnancies affected by SARS-CoV-2 infection. Methods This was a multinational retrospective cohort study including women with a singleton pregnancy and laboratory-confirmed SARS-CoV-2 infection, conducted in 72 centers in 22 different countries in Europe, the USA, South America, Asia and Australia, between 1 February 2020 and 30 April 2020. Confirmed SARS-CoV-2 infection was defined as a positive result on real-time reverse-transcription polymerase chain reaction (RT-PCR) assay of nasopharyngeal swab specimens. The primary outcome was a composite measure of maternal mortality and morbidity, including admission to the intensive care unit (ICU), use of mechanical ventilation and death. Results In total, 388 women with a singleton pregnancy tested positive for SARS-CoV-2 on RT-PCR of a nasopharyngeal swab and were included in the study. Composite adverse maternal outcome was observed in 47/388 (12.1%) women; 43 (11.1%) women were admitted to the ICU, 36 (9.3%) required mechanical ventilation and three (0.8%) died. Of the 388 women included in the study, 122 (31.4%) were still pregnant at the time of data analysis. Among the other 266 women, six (19.4% of the 31 women with first-trimester infection) had miscarriage, three (1.1%) had termination of pregnancy, six (2.3%) had stillbirth and 251 (94.4%) delivered a liveborn infant. The rate of preterm birth before 37 weeks' gestation was 26.3% (70/266). Of the 251 liveborn infants, 69/251(27.5%) were admitted to the neonatal ICU, and there were five (2.0%) neonatal deaths. The overall rate of perinatal death was 4.1% (11/266). Only one (1/251, 0.4%) infant, born to a mother who tested positive during the third trimester, was found to be positive for SARS-CoV-2 on RT-PCR. Conclusions SARS-CoV-2 infection in pregnant women is associated with a 0.8% rate of maternal mortality, but an 11.1% rate of admission to the ICU.
