Abstract

The objective of this study was to identify comorbidity between PTSD and MDD in people who are victims of forced displacement and associated factors. A research with a quantitative approach, a nonexperimental cross-sectional design and a descriptive-analytical scope was carried out. 120 people, of both genders, from 18 to 57 years old, selected through a non-probabilistic convenience sampling, were evaluated. A structured clinical interview for Axis I diagnoses (SCID-I) was used based on the DSM IV criteria for PTSD and MDD, after the application of a questionnaire of sociodemographic data and clinical history with which the factors of risks and associated symptoms were registered. The prevalence for PTSD was 48%, 32% for MDD, and 21% for comorbidity, associated with suicidal ideation and attempt and rumination symptoms. The results were discussed in light of other prevalence studies and the importance of associated risk factors in understanding the comorbidity between PTSD and MDD., O objetivo deste estudo foi identificar a comorbidade entre TEPT e TDM em pessoas vítimas de deslocamento forçado e fatores associados. Realizou-se uma pesquisa com abordagem quantitativa, delineamento transversal não experimental e escopo analítico-descritivo. Foram avaliadas 120 pessoas, de ambos os sexos, com idade entre 18 e 57 anos, selecionadas por meio de amostra não probabilística de conveniência. A entrevista clínica estruturada para diagnósticos do Eixo I (SCID-I) foi utilizadacom base nos critérios do DSM IV para TEPT e TDM, após a aplicação de um questionário de dados sociodemográficos e história clínica com os fatores de riscos e sintomas associados. A prevalência de TEPT foi de 48%, 32% para TDM e 21% para comorbidade, associada a ideação suicida e sintomas de tentativa e ruminação. Os resultados foram discutidos à luz de outros estudos de prevalência e da importância dos fatores de risco associados no entendimento da comorbidade entre TEPT e TDM., El objetivo del presente estudio fue identificar comorbilidad entre TEPT y TDM en personas víctimas del desplazamiento forzado y factores asociados. Se realizó una investigación de enfoque cuantitativo, diseño no experimental de corte transversal y alcance descriptivo-analítico. Fueron evaluadas 120 personas, de ambos géneros, entre 18 a 57 años, seleccionadas a través de un muestreo no probabilístico por conveniencia. Se utilizó una entrevista clínica estructurada para diagnósticos del Eje I (SCID-I) basada en los criterios del DSM IV para el TEPT y TDM, posterior a la aplicación de un cuestionario de datos sociodemográficos y antecedentes clínicos con el que se registraron los factores de riesgos y síntomas asociados. La prevalencia para TEPT fue del 48%, para TDM del 32% y 21% para comorbilidad, asociada con ideación e intento suicida y síntoma de rumiación. Los resultados fueron discutidos a la luz de otros estudios de prevalencia y la importancia de los factores de riesgo asociados en la comprensión de la comorbilidad entre TEPT y TDM.

7. MitraClip para el reparo de insuficiencia mitral severa

Abstract

Resumen Objetivo: Reportar el primer caso de implante de MitraClip realizado en la costa atlántica. Material y métodos: Paciente femenina de 69 años de edad, con diagnóstico de: cardiopatía dilatada de 10 años de evolución con aumento progresivo de deterioro funcional. Ha presentado edema pulmonar recidivante. Resultados: Se realiza procedimiento de implante del dispositivo. Mediante cateterismo transeptal, se procede a introducir el MitraClip con maniobras del sistema de posicionamiento pasando la válvula mitral guiado por el ETE 3 D y se realiza la captura de ambos bordes de la válvula en el sitio de mayor regurgitación A2/P2 sujetando los bordes por él. Se observa persistencia de la insuficiencia moderada por lo que se coloca un segundo MitraClip en forma satisfactoria. Se realizan las medicines Doppler observando una insuficiencia residual mínima. Se corrobora por la angiografía y el ETE su adecuada implantación. Se considera un éxito pudiendo realizar el implante del dispositivo en forma rápida, adecuada y sin complicaciones. Conclusiones: Se reporta el primer caso de implante de MitraClip para el manejo de la IM severa, en la ciudad de Barranquilla, siendo la segunda ciudad de Colombia, después de Cali, donde se implanta este tipo de dispositivos para el reparo de la insuficiencia mitral severa. Abstract Objective: To report the first case if MitraClip implantation performed in the Atlantic coast. Material and methods: 69 year-old female patient diagnosed with dilated cardiomyopathy with a 10-year course suffering from gradual functional deterioration. She developed relapsing pulmonary edema. Results: Device implantation is performed. By means of a transseptal catheterization MitraClip is inserted with positioning system operations below the mitral valve guided by 3 D TEE and captures both vale leaflets on the place where most A2/P2 regurgitation is located, thus holding the leaflets. Mild insufficiency is still observed, so a second MitraClip is implanted succesfully. Doppler measurements reveal minimal residual insufficiency. Angiography and TEE confirm correct implantation. Procedure is considered a success, being able to conduct device implantation in a fast, proper and uncomplicated way. Conclusions: The first case of MitraClip implant for management of severe MI in Barranquilla is presented, this being the second Colombian city after Cali where this type of device is implanted for the repair of severe mitral insufficiency

Abstract

Aims The objective of the Aliskiren Trial on Acute Heart Failure Outcomes (ASTRONAUT) was to determine whether aliskiren, a direct renin inhibitor, would improve post-discharge outcomes in patients with hospitalization for heart failure (HHF) with reduced ejection fraction. Pre-specified subgroup analyses suggested potential heterogeneity in post-discharge outcomes with aliskiren in patients with and without baseline diabetes mellitus (DM). Methods and results ASTRONAUT included 953 patients without DM (aliskiren 489; placebo 464) and 662 patients with DM (aliskiren 319; placebo 343) (as reported by study investigators). Study endpoints included the first occurrence of cardiovascular death or HHF within 6 and 12 months, all-cause death within 6 and 12 months, and change from baseline in N-terminal pro-B-type natriuretic peptide (NT-proBNP) at 1, 6, and 12 months. Data regarding risk of hyperkalaemia, renal impairment, and hypotension, and changes in additional serum biomarkers were collected. The effect of aliskiren on cardiovascular death or HHF within 6 months (primary endpoint) did not significantly differ by baseline DM status (P = 0.08 for interaction), but reached statistical significance at 12 months (non-DM: HR: 0.80, 95% CI: 0.64-0.99; DM: HR: 1.16, 95% CI: 0.91-1.47; P = 0.03 for interaction). Risk of 12-month all-cause death with aliskiren significantly differed by the presence of baseline DM (non-DM: HR: 0.69, 95% CI: 0.50-0.94; DM: HR: 1.64, 95% CI: 1.15-2.33; P < 0.01 for interaction). Among non-diabetics, aliskiren significantly reduced NT-proBNP through 6 months and plasma troponin I and aldosterone through 12 months, as compared to placebo. Among diabetic patients, aliskiren reduced plasma troponin I and aldosterone relative to placebo through 1 month only. Abstract

The neurobiology of depression involves structural changes in the nervous system associated with cognitive processes such as memory and attention. This study sought to evaluate the long-term effect of depressive symptomatology (DS) using a task of visual working memory with or without attentional interference in college students. A 2 × 2 factorial design was used wherein the independent variables were the clinical condition (with and without DS) and the Memonum version (with and without interference of color). The Memonum consists of software to evaluate working memory through retention of direct digits, in two versions: a black-white (without interference) and another with a color attentional distractor (with interference). Depressive symptomatology was evaluated by applying the CES-D to 76 undergraduates. Two week after the diagnosis of DS, the test version was randomly applied (with or without attentional interference). The DS did not alter long-term performance of the tasks involved in visual working memory with or without attentional interference. However, it was found that the use of strategies for mnemonic task performance depended on the clinical condition, being the strategy of grouping the least used by the participants with DS.
