1. Human dignity from social attribute to universal potentiality : the Latin reception of Origen's thought in the 4th Century

Abstract

The dissertation assesses the meanings attributed to the term dignitas ("dignity") in the Latin translations of Origen of Alexandria made by Rufinus and Jerome, as well as in other Latin Christian texts of the 4th century which, to different degrees, show the reception of Origen's exegesis of Genesis 1:26-27. Particular focus is placed on instances where "dignity" refers to humanity as a whole, as is the case with Rufinus' translation of Origen, De Principiis 3 .6.1: this usage of dignitas appears as an exception when compared to most other occurrences of the term in Latin literature, as dignitas is normally attributed only to individuals or to specific groups of people. The aim of this study is thus to shed light on the way in which Origen' s thoughts on the relationship between God and human beings, as mediated by the Latin translators, contributed to a shift towards a new universalizing understanding of dignitas. The investigation has shown that the connection between terms signifying "dignity" and views on humankind expressed in Origen' s exegesis of Genesis 1 :26-27, although occasionally present already in Origen himself, was established in the second half of the 4th century by writers such as Jerome, Rufinus, and Hilary of Poitiers. These Christian authors are the first Latin writers to employ the term dignitas to denote the universal potential of humans as reasonable and free beings created according to the image of God. The contribution offered by the Latin reception of Origen to the history of the idea of human dignity was, based on Orig en's notion of the universal reach of God's love encompassing all his creation, to problematise the elitism and individualism associated with Classical views on dignitas and thus to connect the traditional individual and hierarchical understanding of dignitas as rank to this new universalising and egalitarian perspective.

Abstract

In his commentary on Psalm 118(LXX):73a, "Your hands made me and fashioned me" (Tract.118Ps. 10.1–8), Hilary of Poitiers provides a unique perspective on the creation of humanity and on its exceptional dignity. I will examine key aspects of Hilary's exegesis of this verse, namely, the identification of the hands of God, the interpretation of the two verbs "made" and "fashioned", and the connections with Genesis 1:26 and 2:7. I will also compare Hilary's interpretation of Ps 118:73a to the views expressed by other authors such as Theophilus, Irenaeus, Origen, Tertullian, Ambrose, and Didymus, with the purpose of contextualising Hilary's novel understanding of human dignity within the larger framework of Early Christian interpretations of Genesis. 3. ‘You Are Gods’ (Ps 81:6): Jerome and the Legacy of Origen’s Anthropology

The paper discusses Jerome’s attack against the belief that human beings share the same substance as the heavenly powers and even as the Trinity, according to the dignity (dignitas) of the soul: in polemical texts such as Ep. 124.14, Jerome attributes this belief to Origen. Jerome’s intent clearly to demarcate the difference in nature between human and divine beings is also reflected in his exegetical writings, especially when dealing with Psalm 81, where human beings are addressed as “gods.” The paper investigates Jerome’s understanding of the dignitas of humanity as it emerges from his Homily on Psalm 81: the comparison with Origen’s own passages on Psalm 81 reveals that Jerome closely follows Origen’s exegetical argument. However, through a careful definition of human dignitas Jerome intentionally distances himself from Origen when it comes to associating human beings with immortal beings, most notably Christ.

Perspectives on Origen and the History of his 'Reception The present volume contains papers on Origen and the history of his reception which were presented at a series of workshops at the Eighteenth International Conference on Patristic Studies held at Oxford in August 2019. They provide multifarious insights into various aspects of Origen’s thought and his impact on different topics of theology, exegesis and philosophy from Late Antiquity to Early Modern Times. By connecting the Alexandrian’s legacy with recent developments in Patristics and Classics, they open up new perspectives for Origen scholarship in the new millenium. Research on Origen can be connected with studies, e.g., on rhetoric and power, on individuality and diversity, on gender and equality issues, on determinism and freedom and on questions of cultural transfer and transformation. The contributions to this volume can thus be taken as starting points for future studies on Origen within the broader context of contemporary research in science and the humanities. Abstract

none 22 no Background: Few studies evaluated the clinical outcomes of Community Acquired Pneumonia (CAP), Hospital-Acquired Pneumonia (HAP) and Health Care-Associated Pneumonia (HCAP) in relation to the adherence of antibiotic treatment to the guidelines of the Infectious Diseases Society of America (IDSA) and the American Thoracic Society (ATS) in hospitalized elderly people (65 years or older). Methods: Data were obtained from REPOSI, a prospective registry held in 87 Italian internal medicine and geriatric wards. Patients with a diagnosis of pneumonia (ICD-9 480-487) or prescribed with an antibiotic for pneumonia as indication were selected. The empirical antibiotic regimen was defined to be adherent to guidelines if concordant with the treatment regimens recommended by IDSA/ATS for CAP, HAP, and HCAP. Outcomes were assessed by logistic regression models. Results: A diagnosis of pneumonia was made in 317 patients. Only 38.8% of them received an empirical antibiotic regimen that was adherent to guidelines. However, no significant association was found between adherence to guidelines and outcomes. Having HAP, older age, and higher CIRS severity index were the main factors associated with in-hospital mortality. Conclusions: The adherence to antibiotic treatment guidelines was poor, particularly for HAP and HCAP, suggesting the need for more adherence to the optimal management of antibiotics in the elderly with pneumonia. none Raffaella Rossio; Carlotta Franchi; Ilaria Ardoino; Codjo D. Abstract

The primary aim of this study was to evaluate the prevalence of opioid prescriptions in hospitalized geriatric patients. Other aims were to evaluate factors associated with opioid prescription, and whether or not there was consistency between the presence of pain and prescription. Opioid prescriptions were gathered from the REgistro POliterapie Societa` Italiana di Medicina Interna (REPOSI) data for the years 2008, 2010 and 2012. 1,380 in-patients, 65+ years old, were enrolled in the first registry run, 1,332 in the second and 1,340 in the third. The prevalence of opioid prescription was calculated at hospital admission and discharge. In the third run of the registry, the degree of pain was assessed by means of a numerical scale. The prevalence of patients prescribed with opioids at admission was 3.8 % in the first run, 3.6 % in the second and 4.1 % in the third, whereas at discharge rates were slightly higher (5.8, 5.3, and 6.6 %). The most frequently prescribed agents were mild opioids such as codeine and tramadol. The number of total prescribed drugs was positively associated with opioid prescription in the three runs; in the third, dementia and a better functional status were inversely associated with opioid prescription. Finally, as many as 58 % of patients with significant pain at discharge were prescribed no analgesic at all. The conservative attitude of Italian physicians to prescribe opioids in elderly patients changed very little between hospital admission and discharge through a period of 5 years. Reasons for such a low opioid prescription should be sought in physicians’ and patients’ concerns and prejudices.

Abstract

Background It is well known that atrial fibrillation (AF) and chronic kidney disease (CKD) are associated with a higher risk of stroke, and new evidence links AF to cognitive impairment, independently from an overt stroke (CI). Our aim was to investigate, assuming an underlying role of atrial microembolism, the impact of CI and CKD in elderly hospitalized patients with AF. Methods We retrospectively analyzed the data collected on elderly patients in 66 Italian hospitals, in the frame of the REPOSI project. We analyzed the clinical characteristics of patients with AF and different degrees of CI. Multivariate logistic analysis was used to explore the relationship between variables and mortality. Results Among the 1384 patients enrolled, 321 had AF. Patients with AF were older, had worse CI and disability and higher rates of stroke, hypertension, heart failure, and CKD, and less than 50% were on anticoagulant therapy. Among patients with AF, those with worse CI and those with lower estimated glomerular filtration rate (eGFR) had a higher mortality risk (odds ratio 1.13, p = 0.006). Higher disability levels, older age, higher systolic blood pressure, and higher eGFR were related to lower probability of oral anticoagulant prescription. Lower mortality rates were found in patients on oral anticoagulant therapy. Conclusions Elderly hospitalized patients with AF are more likely affected by CI and CKD, two conditions that expose them to a higher mortality risk. Oral anticoagulant therapy, still underused and not optimally enforced, may afford protection from thromboembolic episodes that probably concur to the high mortality.
