Objective: Cases of Cytomegalovirus (CMV) infection are encountered in the early stages of life in developing countries. The aim of this study is to specify the seroprevalence of CMV-IgM and IgG in pediatric patients and to indicate its relationship with certain hematological, serological, and biochemical parameters.Material and Methods: Serological test results of CMV-IgM and CMV-IgG in children aged 0-14 with CMV as a causative agent were analyzed retrospectively in the blood samples sent to the Microbiology Laboratory of Dursun Odabas Medical Center of Van Yuzuncu Yil University between 2013 and 2015. The relationship with hematological and biochemical parameters was investigated in the cases with positive CMV-IgM results.Results: CMV-IgM and IgG tests were studied in 1.385 children. It was determined that 58% of these children were boys and 42% were girls. A total of 112 (8.2%) of the 1.363 patients who were tested for CMV-IgM were found to be positive. Objective: Headache is a common complaint in childhood. Imaging methods are frequently used in evaluation. This study aimed to determine the frequency of findings detected by imaging methods in children presenting withheadache and to discuss the importance of Magnetic Resonance Imaging (MRI) in diagnosis by emphasizing the possible necessity of SWI (Susceptibility Weighted Imaging) and DWI (Difusion Weighted Imaging) sequences in headache.Material and Methods: 119 patients (51 M/68 F; mean 11.63±3.69 years) who presented with headache and underwent MRI were included in the study. The age of onset, duration, type, frequency of attacks, physical and neurological examination findings of the patient were recorded. Pathologies observed in cranial MRI were evaluated.Results: Headache was primary in 89 (74.8%) patients, secondary in 19 (16%), and unspecified headache in 11 (9.2%) patients. Although 63.9% of patients who underwent MRI had positive imaging findings, very few of them were intracranial pathology explaining the headache symptom (5/76; 6.5%). The most common positive finding was sinusitis (46/76; 61%). Developmental venous anomaly was detected in 2 patients and cavernoma was detected in 1 patient on SWI images. No pathological finding was detected in any of the patients on DWI images. It was observed that the type of headache, duration of attack, gender and age did not significantly change the rate of pathology detected in MRI. On the other hand, the duration of the attack and the presence of additional pathology were found to be significant. The rate of of pathology in MRI was found to be significantly higher in patients who were examined due to clinical findings or due to examination findings, compared to those who were examined due to family anxiety.Conclusion: In the presence of headache in childhood, MRI should be recommended to patients with abnormal neurological examination findings, and in other patients, a detailed clinical evaluation before imaging should be performed together with consultations including otorhinolaryngology. There is potential benefit in adding SWI and DWI sequences to MRI. Finally, parents concern should not be an indication for an MRI request., Amaç: Baş ağrısı çocukluk çağında sık karşılaşılan bir yakınmadır. Değerlendirmede görüntüleme yöntemlerine sıklıkla başvurulmaktadır. Bu çalışmada, baş ağrısı ile başvuran çocuklarda görüntüleme yöntemleri ile saptanan bulguların sıklığını saptamak ve baş ağrısında SWI (Susceptibility Weighted Imaging) ve DWI (Difusion Weighted Imaging) sekansların olası gerekliliğine vurgu yaparak Manyetik Rezonans Görüntüleme (MRG)’nin tanıdaki yerininin tartışılması amaçlandı.Gereç ve Yöntemler: Çalışmaya baş ağrısı yakınması ile başvuran ve MRG yapılan 119 hasta (51 E / 68 K; ortalama 11.63 ± 3.69 yıl) dahil edildi. Baş ağrısının başlangıç yaşı, süresi, tipi, atak sıklığı ile hastanın fizik ve nörolojik muayene bulguları kaydedildi. Kranial MRG’de izlenen patolojiler değerlendirildi. Bulgular: Hastaların 89’unda (%74.8) primer, 19’unda (%16) sekonder, 11’inde ise (%9.2) spesifiye edilemeyen baş ağrısı mevcuttu. MRG yapılan hastaların, %63.9’unda pozitif görüntüleme bulguları olmakla birlikte bunların çok azı başağrısı semptomunu açıklayıcı intrakraniyel bir patolojiydi (5/76; %6.5). En sık rastlanan pozitif bulgu sinüzitti (46/76; %61). SWI görüntülerde 2 hastada gelişimsel venöz anomali, 1 hastada kavernom tespit edildi. DWI görüntülerde ise hiçbir hastada patolojik bulgu saptanmadı. Baş ağrısının tipinin, atak süresinin, cinsiyetin, yaşın MRG’de patoloji tespit edilme oranını anlamlı şekilde değiştirmediği görüldü. Buna karşın atak süresi, ek patoloji varlığı anlamlı bulundu. Klinisyenin gerekli görmesi sonucu veya muayene bulgusu olduğu için tetkik edilenlerde MRG’de patoloji tespit edilme oranı, aile anksiyetesi sebebiyle tetkik edilenlerden anlamlı şekilde yüksek bulundu.Sonuç: Çocukluk çağında başağrısı varlığında MRG anormal nörolojik muayene bulguları olan hastalara önerilmeli diğer hastalarda görüntüleme öncesi ayrıntılı klinik değerlendirme gerekirse Kulak Burun Boğaz’ı de içeren konsültasyonlarla birlikte yapılmalıdır. MRG’ye SWI ve DWI sekansların eklenmesinde potansiyel yarar vardır. Son olarak ailelerin endişesi MRG istemi yapmak için bir endikasyon olmamalıdır.

Guillain Barre Syndrome (GBS) is an acute inflammatory polyneuropathy, often characterized by symmetrical weakness and areflexia. It is considered to be an autoimmune disease triggered by previous bacterial or viral infections. Miller Fisher’s syndrome is one of the most common variants of GBS, and it is characterized by trauma with areflexia, ataxia, and ophthalmoparesis.Mortality is 1-2% in childhood GBS and generally develops due to respiratory failure. Here, a case with Guillain-Barre Syndrome (GBS) developed after parvovirus B19 infection was reviewed because the symptom of the application was syncope, which is thought to be developed due to autonomic involvement., Guillain Barre Sendromu (GBS), sıklıkla simetrik güçsüzlük ve arefleksi ile karakterize akut inflamatuar polinöropatidir. Geçirilmiş bakteriyel veya viral enfeksiyonların tetiklediği otoimmün bir hastalık olduğu kabul edilmektedir. Miller Fisher sendromu, GBS'nin en sık görülen varyantlarından biri olup, arefleksi, ataksi ve oftalmoparezi ile seyretmesi özelliğidir.Çocukluk çağı GBS'de mortalite %1-2 oranındadır ve genellikle solunum yetersizliğine bağlı olarak gelişir. Burada parvovirus B19 enfeksiyonu sonrası gelişen Guillain-Barre Sendromlu (GBS) bir olgu başvuru semptomunun otonom tutuluma bağlı gelişmiş olabileceği düşünülen senkop olması nedeniyle gözden geçirildi.

Amaç: Tüberoskleroz kompleksi (TSK) vücutta birçok organın tutulumu ile karakterize, otozomal dominant kalıtım gösteren genetik bir rahatsızlıktır. Bu çalışmada TSK tanı kriterlerinin ve tanı kriterleri dışındaki bulguların sıklığını belirlemek amaçlanmıştır.Hastalar ve Yöntemler: TSK tanılı 35 hastanın verileri geriye dönu?k olarak incelendi. Hastaların tanısı, 2012 yılında güncellenen TSK'nin tanı kriterlerine göre değerlendirildi. Tanısal olmayan kriterler olarak; epilepsi, ilaca dirençli epilepsi, elektroensefalografi (EEG) tiplerini (fokal, diffüz-multifokal ve hiperaritmi) ve TAND'ı (TSC ile ilişkili nöropsikiyatrik bozukluklar) (zihinsel yetersizlik ve / veya otizm ve öğrenme yetersizliği) inceledik.Bulgular: Yirmi bir hasta (%60) nöbet geçirme, 9 hasta (%26) hipopigmente lekelenmeler ve 5 hasta (%14) kardiyak rabdomiyomlar nedeniyle başvurmuştu. Beyin magnetik rezonans göru?ntu?leme (MRG) ile en sık saptanan bulgu kortikal tüberlerdi (%85). Nöbet geçiren hastaların EEG incelemelerinde 5’inde (%24) yaygın ve multifokal epileptik bozukluk, 8’inde (%38) fokal epileptik bozukluk ve 8’inde (%38) hipsaritmi paterni saptandı. Epilepsi tanısı ile izlenen olguların %38’i dirençli epilepsi tanısına sahipti. Ağır derecede entelektüel yetersizlik ve/veya otizm 11 (%32) hastada saptandı. Dirençli epilepsi grubunda böbrek anjiomiyolipomaları olan hasta sayısı anlamlı olarak fazlaydı (p:0.001) ve aynı zamanda tüberoskleroz ile ilişkili nöropsikiyatrik bozuklukların oranı anlamlı olarak epilepsi grubunda yüksekti (p:0.001).Sonuç: Hastalığın multidisipliner bir yaklaşım ile takip edilmesi gerekmektedir. Tanı kriterlerinde yer almasa da epilepsi, entelektüel yetersizlik ve nöropsikiyatrik bozuklukların sık eşlik ettiği akılda tutulmalıdır. Aim: Tuberous sclerosis complex (TSC) is a multisystem genetic, autosomal-dominant disorder predisposing to multiple organ manifestations. The aim of this study is to determine TSC the frequency of findings including diagnostic and non-diagnostic criteria.Patients and Metod: Thirty-five patients diagnosed with tuberous sclerosis complex were examined retrospectively. The diagnosis of the patients were evaluated according to the diagnostic criteria of TSC that were updated in 2012. As non-diagnostic criteria, we reviewed epilepsy, drug-resistant epilepsy, electroencephalography (EEG) types (focal, diffuse-multifocal and hypsarrhythmia) and TAND (TSC-associated neuropsychiatric disorders) (intellectual disability and/or autism and learning disability).Results: Twenty-one cases (60%) presented with seizures, 9 cases (26%) with hypopigmented patches and 5 cases (14%) with cardiac rhabdomyomas. The most common finding with brain magnetic resonance imaging (MRI) was cortical tubers (85%). EEG examinations revealed diffuse and multifocal epileptic disorder in 5 (24%), focal epileptic disorder in 8 (38%), and hypsarrhythmia in 8 (38%) patients. 38% of the patients with epilepsy were diagnosed with refractory epilepsy. Severe intellectual disability and / or autism were detected in 11 (32%) patients. The number of patients with renal angiomyolipoma (p:0.001) were significantly higher in drug resistant epilepsy patients and also TSC-associated neuropsychiatric disorders (TAND) (p:0.001) rate was significantly higher in epilepsy patients.Conclusion: The disease should be followed with a multidisciplinary approach. Although not included in the diagnostic criteria, it should be kept in mind that epilepsy, intellectual disability and neuropsychiatric disorders frequently accompany.

In present case report, four-months-old boy who referred to our pediatric endocrinology outpatient clinic from department of ophthalmology due to evaluation of endocrine and metabolic disorders for cataract was discussed. The characterized features of patient were hypotrichosis, microphthalmia, nystagmus, strabismus, congenital cataract, beaked nose, micrognathia,scaphocephaly, frontal and parietal bossing. The case has typical dysmorphic physical examination findings that appropriate diagnostic features to rare Hallermann-Streiff syndrome.

Ecthyma gangrenosum is a cutaneous lesion often associated with pseudomonas aeruginosa bacteremia, even though it may develop without bacteremia and may originate from other bacterial and fungal organisms. Pseudomonas aeruginosa bacteremia or sepsis, which mainly affects immunocompromised patients, frequently occurs in hospitals. This lesion typically occurs on the extremities and gluteal and perineal regions. In this report we present a case of ecthyma gangrenosum in a premature newborn occurring secondary to pseudomonas sepsis causing sucking dysfunction due to tissue loss in the lip, soft palate, and tongue.

Background: Hepatitis A is a common infectious disease during childhood worldwide. Recently, great deal of changes in the epidemiology has been reported. The seroepidemiologic studies of this infection are not sufficient in Eastern region of Turkey. Objective: To investigate the seroprevalence and association with socio-demographic variables of hepatitis A in 1-15 year old children in Van. Patients and Methods: This study was performed on 510 one to fifteen year old children from outpatient pediatric clinics in Yuzuncu Yil University, Faculty of Medicine during last three months of 2009. Anti-HAV IgG was measured in sera by enzyme-linked immunosorbent assay. The information about subjects was recorded on standardized forms and a chart review survey was performed. Results: The overall ratio for seropositivity was 54.9%. Statistical significance was found between hepatitis A seroprevalence and age, collective use of domestic items, fresh water resources, localization and type of toilet and the number of households. Conclusion: This study provided the most recent data of seropositivity and revealed the preliminary indication of epidemiological shift in seroprevalence of Hepatitis A virus in a region with high endemicity.

BackgroundBrucellosis is the most common zoonotic disease worldwide and remains an important human disease especially in developing countries. The aim of the present study was to evaluate clinical manifestations and laboratory findings of childhood brucellosis in Van province of Eastern Turkey. To our knowledge, this is the largest series of childhood brucellosis reported in the literature.

Objective: We aimed to compare maternal and perinatal findings of mild and severe preeclampsia, also to define considerable predictive factors for early diagnosis. Material and Methods: Study included total of 124 patients that consist of 28 mild and 96 severe preeclampsia patients. 150 singleton pregnants, as a similar demographic features with preeclampsia group were added to study as control group. Age, birth weight, laboratory values and perinatal outcomes were compared between three groups. Results: Hemoglobin, maternal (free triiyodothyronin) fT3, (free thyroxine) fT4 and birth weight values were determined significantly lower in severe preeclampsia group. Blood urea nitrogen (BUN), creatinine, aspartate transaminase (AST), alanine transaminase (ALT) and maternal thyroid stimulan hormone (TSH) values were determined significantly higher in severe preeclampsia compared with other two groups. Cesarean ratio were determined higher in entire preeclampsia patients and newborns of preeclampsia patients required more overhaul in newborn intensive care unit. There were no significant differences determined between three groups in white blood cell and platelet count. Conclusion: Preeclampsia is one of the most important pregnancy complication must be early diagnosed and interfered. Early diagnosis and consistent approach will produce better results for perinatal maternal morbidity and mortality, Amaç: Hafif ve şiddetli preeklampsi olgularında maternal ve perinatal bulguların karşılaştırılması, erken tanı için dikkate alınabilecek prediktif faktörlerin tanımlanmasını amaçladık. Gereç ve Yöntem: Çalışmaya 28 hafif ve 96 ağır toplam 124 preeklampsi olgusu dahil edildi. Benzer demografik özelliklere sahip herhangi bir gebelik komplikasyonu olmayan 150 tekil gebeler kontrol grubu olarak çalışmaya alındı. Hastaların yaş, gebelik haftası, doğum ağırlığı, laboratuvar değerleri ile perinatal sonuçlar gruplar arası karşılaştırıldı. Bulgular: Hemoglobin, maternal (serbest triiyodotironin) sT3, (serbest tiroksin) sT4 ve doğum ağırlıkları ağır preeklampsi grubunda belirgin olarak düşük izlenirken; kan üre azotu (BUN), kreatinin, alanin aminotransferaz (ALT), aspartat aminotransferaz (AST) ve maternal tiroid stimulan hormon (TSH) değerleri ise ağır preeklampsi olgularında anlamlı şekilde yüksek izlendi. Preeklampsi olgularında sezaryen oranı yüksek, ağır preeklampsi olgularının bebeklerinde yeni doğan yoğun bakım ihtiyacının daha fazla olduğu saptandı. Beyaz küre, trombosit değerleri açısından gruplar arasında farklılık saptanmadı. Sonuç: Preeklampsi; erken tanı konulması ve erken müdahele gerektiren en önemli gebelik problemlerinden birisidir. Erken tanı ve uygun tedavi ile perinatal maternal morbidite ve mortalitede açısından daha iyi sonuçlar elde etmemizi sağlayacaktır
