The inter-rater reliability of the Quebec Task Force (QTF) classification system for Whiplash-Associated Disorders (WAD) remains unknown. Our objective was to determine the inter-rater reliability of the WAD classification between an experienced chiropractic clinician and two chiropractic residents.We conducted an inter-rater reliability study using baseline clinical data from 80 participants assessed for inclusion in a randomized clinical trial of the conservative management of WAD grades I and II. We reported reliability using Cohen's kappa (k) and 95% confidence intervals (CI).The mean duration of WAD symptoms was 7.6 days (s.d.=5.2). In our study, the interrater reliability of the WAD grade classification varied from k=0.04 (95% CI -0.04 to 0.12) to k=0.80 (95% CI 0.67 to 0.94).Inter-rater reliability of the WAD classification varied greatly across raters and may be associated with the experience of the raters and with their understanding of the criteria. Our results suggest that clinicians may benefit from training to standardize how they classify WAD. Furthermore, our results need to be tested in a different sample of patients and with a range of clinicians from different clinical disciplines.La fiabilité inter-utilisateur du système de classification des troubles associés au coup de fouet cervical (TACF) établi par le Groupe de travail du Québec (GTQ) demeure inconnue. Notre étude visait à établir la fiabilité inter-évaluateur du système de classification des troubles associés au TACF utilisé par un chiropraticien clinicien d’expérience et deux résidents en chiropratique.On a effectué notre étude à l’aide de données cliniques de départ sur 80 participants à un essai clinique, à répartition aléatoire, sur le traitement conservateur du TACF de stades I et II. On a utilisé le coefficient kappa (k) de Cohen et des intervalles de confiance (IC) à 95 % pour évaluer la fiabilité.La durée moyenne des symptômes du TACF était de 7,6 jours (écart-type :5,2). La fiabilité inter-utilisateur de la classification des TACF a varié de k = 0,04 (IC à 95 % – de 0,04 à 0,12) à k = 0,80 (IC à 95 % de 0,67 à 0,94).La fiabilité inter-utilisateur de la classification des TACF a beaucoup varié d’un évaluateur à l’autre; l’écart pourrait être lié à l’expérience de l’évaluateur et à sa compréhension des critères de classification. Selon les résultats de notre étude, les cliniciens pourraient bénéficier d’une formation servant à normaliser leur méthode de classification des TACF. Nos résultats devraient être confirmés par une autre étude utilisant un autre échantillon de patients et un éventail de cliniciens appartenant à diverses disciplines.

Background In the midst of the coronavirus pandemic, the International Chiropractors Association (ICA) posted reports claiming that chiropractic care can impact the immune system. These claims clash with recommendations from the World Health Organization and World Federation of Chiropractic. We discuss the scientific validity of the claims made in these ICA reports. Main body We reviewed the two reports posted by the ICA on their website on March 20 and March 28, 2020. We explored the method used to develop the claim that chiropractic adjustments impact the immune system and discuss the scientific merit of that claim. We provide a response to the ICA reports and explain why this claim lacks scientific credibility and is dangerous to the public. More than 150 researchers from 11 countries reviewed and endorsed our response. Conclusion In their reports, the ICA provided no valid clinical scientific evidence that chiropractic care can impact the immune system. We call on regulatory authorities and professional leaders to take robust political and regulatory action against those claiming that chiropractic adjustments have a clinical impact on the immune system.

Objectives: To: 1) develop/adapt and validate an instrument to measure patient safety attitudes and opinions of community-based spinal manipulative therapy (SMT) providers; 2) implement the instrument; and 3) compare results among healthcare professions.Methods: A review of the literature and content validation were used for the survey development. Community-based chiropractors and physiotherapists in 4 Canadian provinces were invited.Results: The Agency for Healthcare Research and Quality's (AHRQ) Medical Office Survey on Patient Safety Culture was the preferred instrument. The survey was modified and validated, measuring 14 patient safety dimensions. 276 SMT providers volunteered to respond to the survey. Generally, SMT providers had similar or better patient safety dimension scores compared to the AHRQ 2016 medical offices database.Discussion: We developed the first instrument measuring patient safety attitudes and opinions of community-based SMT providers. This instrument provides understanding of SMT providers' opinions and attitudes on patient safety and identifies potential areas for improvement.

A cross-sectional survey.The purpose of this study was to identify characteristics of Canadian doctors of chiropractic (DCs) associated with their number of workers' compensation patients.It has been previously hypothesized that DCs that treat a relatively high volume of workers' compensation cases may have different characteristics than the general chiropractic community.Secondary data analyses were performed on data collected in the 2011 survey of the Canadian Chiropractic Resources Databank (CCRD). The CCRD survey included 81 questions concerning the practice and concerns of DCs. Of the 6,533 mailed questionnaires, 2,529 (38.7%) were returned. Of these, 652 respondents did not meet our inclusion criteria, and our final study sample included 1,877 respondents. Bivariate analyses were conducted between predetermined independent variables and the annual number of workers' compensation patients. A negative binomial multivariate regression was performed to identify significant factors associated with the number of workers' compensation patients.On average, DCs received 10.3 (standard deviation (SD) = 17.6) workers' compensation cases and nearly one-third did not receive any such cases. The type of clinic (other than sole provider), practice area population (smaller than 500,000), practice province (other than Quebec), number of practice hours per week, number of treatments per week, main sector of activity (occupational/ industrial), care provided to patients (electrotherapy, soft-tissue therapy), percentage of patients with neuromusculoskeletal conditions, and percentage of patients referred by their employer or a physician were associated with a higher annual number of workers' compensation cases.Canadian DCs who reported a higher volume of workers' compensation patients had practices oriented towards the treatment of injured workers, collaborated with other health care providers, and facilitated workers' access to care.Une enquête transversale.Cette étude visait à déterminer les caractéristiques des chiropraticiens canadiens associées à leur nombre de patients traités pour accidents de travail.Selon des hypothèses émises précédemment, les chiropraticiens qui traitent un nombre relativement élevé de patients pour accidents de travail peuvent avoir des caractéristiques différentes de l’ensemble des chiropraticiens.Des analyses secondaires de données ont été effectuées sur des données recueillies dans l’enquête de 2011 de la banque de données de ressources chiropratiques canadiennes (CCRD). Cette enquête comportait 81 questions relatives à l’exercice et aux préoccupations des chiropraticiens. Sur les 6 533 questionnaires envoyés, il y a eu 2 529 (38,7 %) réponses. Parmi celles-ci, 652 ne répondaient pas à nos critères d’inclusion. Donc, l’échantillon final utilisé pour notre étude comprenait 1 877 répondants. Les facteurs suivants ont été associés à un nombre annuel plus élevé de patients traités pour accidents de travail : type de clinique (autre que fournisseur unique), population de la ville de la clinique (inférieure à 500 000), province de la clinique (autre que le Québec), nombre d’heures de cabinet par semaine, nombre de traitements par semaine, principal secteur d’activité (professionnelle / industrielle), soins prodigués aux patients (électrothérapie, thérapie des tissus mous), pourcentage de patients atteints de maladies neuromusculo-squelettiques, et pourcentage de patients référés par leur employeur ou un médecin.Les chiropraticiens canadiens qui ont déclaré un nombre plus élevé de patients traités pour accidents de travail avaient des pratiques axées sur le traitement des travailleurs blessés, ont collaboré avec d’autres fournisseurs de soins de santé, et ont facilité l’accès aux soins des travailleurs.
