The purpose of this research was to identify the clinical and diagnostic characteristics of Somalian patients suffering from swollen lymph nodes.Methods: Under the purview of this study, the diagnoses, patient clinical courses, and treatment outcomes were ascertained retrospectively by analyzing the biochemical, microbiological, radiological, and pathological data of the cases in all age groups who underwent therapy for lymphadenitis at Mogadishu Somali Turkey Recep Tayyip Erdoğan Training and Research Hospital between January 2016 and September 2023.Results: During the study period, 317 patients were followed up, with 53.3% of the patients as female, 46.7% as male, 28.1% as pediatric, 63.1% as adult, and 8.8% were elderly patients. With 58.7% of diagnoses, tuberculous lymphadenitis was the most common, pursued by malignant causes (21.8%). The most commonly affected lymph node was the cervical region, the symptom detected was lymph node swelling, the radiological method used was ultrasonography, and the biopsy method was fine needle aspiration biopsy. It was determined that benign conditions, including tuberculosis were more common in pediatric group and younger adults (p< 0.001), and weight loss was a significant in terms of malignancy and tuberculosis (p< 0.001). White blood cell value (p< 0.001), erythrocyte sedimentation rate (p< 0.001), and C-reactive protein (p:0.001) revealed differences across the diagnostic groups.Conclusion: Almost two-thirds of patients were diagnosed with tuberculosis lymphadenitis. Our finding revealed tuberculosis as the commonest cause of lymphadenopathy followed by malignant causes. Therefore, before screening for malignant causes, tuberculosis should be the first diagnosis considered in Somalia, particularly in a patient presenting with weight loss and swelling of the lymph nodes.Keywords: immunization, lymphadenopathy, Somalia, sub-Saharan Africa, tuberculosis

Therefore, it is high mortality rates compared to other departments of the hospital. Estimating the prognostic risk of hospitalized patients may be useful in mortality for patients. İn this study, we evaluated the level of Systemic Immune Inflammation Index (SII) and Systemic Inflammation Response Index (SIRI) and its association with mortality in inpatients.Patients and methods: This study was performed in 2218 patients who were hospitalized between January 1st–December 31th of 2019. Patients were followed up for three years about primary endpoint as all-cause (except for unnatural deaths) mortality. Participants were divided into 4 equal groups according to their increasing levels of SII and SIRI. (Quartile 1– 4) Age, gender, diabetes mellitus, hypertension, coronary artery disease, chronic kidney disease, malignancies (solid), white blood cell, neutrophil, lymphocyte, monocytes, hemoglobin, hematocrit, platelet, CRP, albumin, Systemic Inflammation Response Index (Quartile 1– 4), Systemic Immune Inflammation Index (Quartile 1– 4) were compared between survival and non-survival groups.Results: There were 1153 female and 1065 male participants enrolled. Compared with surviving patients, patients who died were older and had a higher prevalence of diabetes mellitus, hypertension, malignancy, chronic kidney disease and coronary artery disease (p < 0.001). There was a lower proportion of female patients among the patients who died. Compared to the survivor group, group who died exhibited a significant increase in CRP level, neutrophil, white blood cell and monocyte counts, but had a lower lymphocyte count, albumin level and hemoglobin count (P < 0.001). Results of Cox regression analysis showed that age, chronic kidney disease, malignancy, SIRI quartile 3, 4 and SII quartile 3, 4 pointed out a close relationship with mortality risk. The global data movement over Internet has an estimated energy footprint of 100 terawatt hours per year, costing the world economy billions of dollars. The networking infrastructure together with source and destination nodes involved in the data transfer contribute to overall energy consumption. Although considerable amount of research has rendered power management techniques for the networking infrastructure, there has not been much prior work focusing on energy-aware data transfer solutions for minimizing the power consumed at the end-systems. In this paper, we introduce a novel application-layer solution based on historical analysis and real-time tuning called GreenDataFlow, which aims to achieve high data transfer throughput while keeping the energy consumption at the minimal levels. GreenDataFlow supports service level agreements (SLAs) which give the service providers and the consumers the ability to fine tune their goals and priorities in this optimization process. Our experimental results show that GreenDataFlow outperforms the closest competing state-of-the art solution in this area 50% for energy saving and 2.5x for the achieved end-to-end performance.

8. Pelvic inlet closure with bladder peritoneal flap reduces postoperative ileus after abdominoperineal resection.

Background: Abdominoperineal resection (APR) is the standard treatment for locally advanced distal rectal cancer (LADRC) following neoadjuvant treatment when sphincter-preserving procedures are inapplicable. However, complications such as perineal site infection, perineal hernia and postoperative ileus remain ongoing challenges. We aimed to compare the rate of postoperative ileus due to pelvic incarceration in APR patients with/without pelvic inlet closure. Methods: LADRC patients, who underwent APR with/without pelvic inlet closure between January 2016 and September 2022 at a comprehensive cancer center were analyzed retrospectively. After laparoscopic APR, the pelvic inlet was closed with a bladder peritoneal flap. A U-shaped peritoneal flap, up to approximately 15 × 20 cm in size, was peeled off from the dome of the bladder with its base at the posterosuperior part of the bladder. Results: Of the 80 patients included in the analysis, 27 (34%) underwent laparoscopic APR with pelvic inlet closure and 53 (66%) had no pelvic inlet closure. There was no significant difference in age, mean body mass index and American Society of Anesthesiologists score among both groups. Patients with a pelvic inlet closure had a significantly lower postoperative ileus rate because of incarceration [0 vs. 17%, n = 9/53; (P = 0.03)] and a shorter hospital stay [6.74 ± 4.21 vs. 9.00 ± 5.99 days; P = 0.03]. The rate of perineal surgical site infection (SSI) [5/27 (18.5%) vs. 14/53 (26.4%); P = 0.61) was lower in the laparoscopic APR with pelvic inlet closure group, but this was not significant. Conclusion: Pelvic inlet closure using a bladder peritoneal flap following APR is a safe and feasible procedure associated with reduced postoperative ileus due to pelvic incarceration rates, emergent surgical interventions due to pelvic incarceration and shorter hospital stay. In this study, various concentrations of Dried Apricot Pulp (DAP) were added to butter at levels of 0, 15, 20, and 25%, and its effects on antioxidant capacity, oxidative stability, color, and sensory attributes were assessed during 45 days of storage. Butter samples were labeled as A0 (Control), A15, A20, and A25 corresponding to the respective levels of DAP incorporation. The addition of DAP resulted in significant reductions in pH, while titratable acidity (expressed as lactic acid %) and free fatty acids (FFA) content (mg KOH g-1 butter) increased in a dose-dependent manner (P< 0.05). Fortification with DAP delayed peroxide formation in the butter samples. Additionally, DAP supplementation significantly (P< 0.05) increased Total Phenolic Content (TPC), improved DPPH (2,2-Diphenyl-1-Picrylhydrazyl) scavenging activity, and increased Total Antioxidant Capacity (TAC) during storage. Panelists assigned higher sensory scores to the DAPenhanced butters. These fortified butters exhibited lower fat content (ranging from 65.25 to 65.50%) compared to the control butter (73.75%). Notably, the total sugar content of samples A15, A20, and A25 was 3.69±0.06, 4.70±0.06, and 5.64±0.08, respectively. Overall, this study demonstrates the potential of all DAP ratios, particularly 25%, as rich sources of antioxidants. However, further formulation adjustments and comprehensive analyses are warranted for industrial and marketing applications. [ABSTRACT FROM AUTHOR]

- Abstract
Background: The optimal treatment of anastomotic leak after rectal cancer resection is unclear. This worldwide cohort study aimed to provide an overview of four treatment strategies applied. Methods: Patients from 216 centres and 45 countries with anastomotic leak after rectal cancer resection between 2014 and 2018 were included. Treatment was categorized as salvage surgery, faecal diversion with passive or active (vacuum) drainage, and no primary/secondary faecal diversion. The primary outcome was 1-year stoma-free survival. In addition, passive and active drainage were compared using propensity score matching (2: 1). Results: Of 2470 evaluable patients, 388 (16.0 per cent) underwent salvage surgery, 1524 (62.0 per cent) passive drainage, 278 (11.0 per cent) active drainage, and 280 (11.0 per cent) had no faecal diversion. One-year stoma-free survival rates were 13.7, 48.3, 48.2, and 65.4 per cent respectively. Propensity score matching resulted in 556 patients with passive and 278 with active drainage. There was no statistically significant difference between these groups in 1-year stoma-free survival (OR 0.95, 95 per cent c.i. 0.66 to 1.33), with a risk difference of -1.1 (95 per cent c.i. -9.0 to 7.0) per cent. After active drainage, more patients required secondary salvage surgery (OR 2.32, 1.49 to 3.59), prolonged hospital admission (an additional 6 (95 per cent c.i. 2 to 10) days), and ICU admission (OR 1.41, 1.02 to 1.94). Mean duration of leak healing did not differ significantly (an additional 12 (-28 to 52) days). Conclusion: Primary salvage surgery or omission of faecal diversion likely correspond to the most severe and least severe leaks respectively. In patients with diverted leaks, stoma-free survival did not differ statistically between passive and active drainage, although the increased risk of secondary salvage surgery and ICU admission suggests residual confounding.

It was reported that increased production of the main proinflammatory cytokines, such as interleukin-1, interleukin-6, and tumor necrosis factor-alpha, and that of acute phase reactants may play a role in the etiopathogenesis of depression. Stress and depression were reported to increase leukocyte and neutrophil counts and to decrease lymphocyte count. Biological determinants affecting the diagnosis, therapy, and prognosis of depression are quite limited. Therefore, new etiological models are needed to explain the pathophysiology of depression. In recent years, neutrophil–lymphocyte ratio (NLR) was determined to be a good indicator of inflammatory status. There is no study in the literature investigating NLR in MDD. This study aims to examine the role of inflammation in the etiology of depression based on the NLR in MDD patients who are undergoing no pharmacological therapy. A total of 41 patients diagnosed with MDD, who received no antidepressant therapy within the past 1 month, were included in the study, which took place between January and March 2015. The control group consisted of 47 healthy subjects with no psychiatric disorders. A sociodemographic information form and a Beck Depression Scale were administered, and the blood was taken for biochemical analysis. A 52-year-old male patient was admitted to our eye clinic with the complaint of sudden visual loss and a visual acuity of 20/50 in the right eye and 20/800 in the left eye. Fundus examination revealed common flame-shaped hemorrhages, venous dilatation and tortuosity, Roth spots, serous macular detachment, and yellow macular deposits in both eyes. Evaluation with fundus fluorescein angiography, fundus autofluorescence, and spectral-domain optical coherence tomography resulted in suspicion of hyperviscosity retinopathy and referral to the hematology clinic. After hematology consultation confirmed a diagnosis of multiple myeloma, chemotherapy and plasmapheresis were initiated. Four months after presentation, best-corrected visual acuity was 20/20 in both eyes and improvement in hyperviscosity retinopathy, serous macular detachment, and yellow macular deposits was observed. İlgi çekici rekreasyon alanlarından birisi de dağlık alanlardır. Bu alanların rekreasyonel amaçlı kullanımıyla, özellikle zirve tırmanışı, yürüyüş yolları ve kamp alanları gibi yoğun ziyaretçi faaliyetlerinin olduğu lokasyonlarda çevresel ve sosyal etkiler meydana gelebilir. Bu çalışma, Ağrı Dağı Milli Parkı’ında kullanıcılar tarafından algılanan sosyal ve çevresel etkileri ortaya koymayı amaçlamaktadır. Veriler, alanda yapılan ziyaretçi anketlerinden elde edilmiştir (n = 181). Algılanan kalabalık (sosyal etki), çevresel etkiler ve kullanıcı memnuniyeti göstergeler olarak seçilmiş ve analiz edilmiştir. Çalışmanın bulguları, ziyaretçilerin büyük bir kısmının çevresel etkileri rapor ettiğini göstermektedir; çöp varlığı, toprak ve bitki örtüsü hasarı, yürüyüş yollarının mevcut koşulları büyük bir sorun olarak görülmüş ve ziyaretçilerin büyük bir çoğunluğu ziyaretleri sırasında alanda biraz kalabalık hissettiklerini belirtmiştir. Memnuniyetin, ziyaretçilerin algıladığı kalabalık ile pozitif yönde ilişkili olduğu görülmüştür. Rekreasyon faaliyetleri sonucu oluşan çevresel ve sosyal etkilerin azaltılması ve kontrol edilebilmesi, ziyaretçi yönetimi ve çevresel eğitim programlarıyla mümkün kılınabilir ve bu doğrultuda alanla ilgili çözüm önerileri geliştirilmiştir., One of the attractive outdoor recreation settings is mountain environments. However, recreational use of these areas may degrade their natural environment, particularly in areas of concentrated visitor activities like the summits, trails, and campsites. This study aims to explore perceived recreational social and environmental impacts on Mount Ararat. Data were obtained from on-site surveys of visitors (n = 181). Perceived crowding, perceived environmental impacts, and satisfaction was chosen as indicators to analyze. The study's findings showed that most visitors reported environmental impacts; litter, soil and vegetation damage, trail conditions as a big problem in the area, and most visitors reported they felt slightly crowded during their visit. Satisfaction was positively related to visitors' perceived crowding. Reducing and controlling the environmental and social impacts resulting from recreational activities can be made possible by visitor management and environmental education programs, and solutions have been developed in this direction.

- Abstract
People across the world and throughout history have gone to great lengths to enhance their physical appearance. Evolutionary psychologists and ethologists have largely attempted to explain this phenomenon via mating preferences and strategies. Here, we test one of the most popular evolutionary hypotheses for beauty-enhancing behaviors, drawn from mating market and parasite stress perspectives, in a large cross-cultural sample. We also test hypotheses drawn from other influential and non-mutually exclusive theoretical frameworks, from biosocial role theory to a cultural media perspective. Survey data from 93,158 human participants across 93 countries provide evidence that behaviors such as applying makeup or using other cosmetics, hair grooming, clothing style, caring for body hygiene, and exercising or following a specific diet for the specific purpose of improving ones physical attractiveness, are universal. Indeed, 99% of participants reported spending >10 min a day performing beauty-enhancing behaviors. The results largely support evolutionary hypotheses: more time was spent enhancing beauty by women (almost 4 h a day, on average) than by men (3.6 h a day), by the youngest participants (and contrary to predictions, also the oldest), by those with a relatively more severe history of infectious diseases, and by participants currently dating compared to those in established relationships. The strongest predictor of attractiveness-enhancing behaviors was social media usage. Other predictors, in order of effect size, included adhering to traditional gender roles, residing in countries with less gender equality, considering oneself as highly attractive or, conversely, highly unattractive, TV watching time, higher socioeconomic status, right-wing political beliefs, a lower level of education, and personal individualistic attitudes. 44. Perceived benefits of retrofitted residential secondary glazing: an exploratory Australian study

PurposeRetrofitting single glazing with double glazing saves energy and enhances indoor comfort, however replacement can be expensive. Secondary glazing is an easier-to-install, cost-effective retrofit alternative. There is a gap in the literature about post-retrofit occupant satisfaction with secondary glazed windows in Australia.Design/methodology/approachThis research addresses this gap, with an online survey completed by 56 respondents in New South Wales and the Australian Capital Territory who retrofitted secondary glazing. This research seeks a deeper understanding of occupier views. Issues of validity and internal and external reliability were addressed in the research.FindingsStrong satisfaction and significant improvement in indoor comfort was found. Over 80% of respondents felt their home had better thermal and noise insulation. Nearly 79% found a positive impact on their property value and 77% would retrofit secondary glazing if they moved. Occupants considered secondary glazing a suitable technical, economic and environmental alternative to double glazing in retrofits.Research limitations/implicationsThere is a chance of sampling bias; only satisfied respondents answer the survey. Survey data was collected from occupants located in a small geographic area and who used the same manufacturer. Different climatic conditions, varying specifications and quality of installed systems could influence perceptions.Practical implicationsMany reported health and wellbeing improvements, higher comfort, higher property value and willingness to invest in residential energy efficiency measures. These findings need to be more widely known in the Australian market.Originality/valueFew studies focus on retrofitting secondary glazing to Australian residential buildings. This study highlights the social, economic and environmental benefits perceived by occupants post-retrofit.

National parks provide settings for outdoor recreational activities. Mount Ararat is one of the well-known mountainous national parks for summit climbs and functions as a habitat variety of plants and animals in Europe. Besides its essential recreational and ecological services, the mountain has a cultural and spiritual heritage. With the growth in recreational use, decisionmakers confronted with this increasing complexity and uncertainty about keeping the park maintained sustainably. This study examines the park's recreational potential by analysing its landscape value, climatic value, accessibility, recreative convenience, and negative factor indicators to help decisionmakers make long-term plans. According to the analysis results, Mount Ararat was found as a high levelled outdoor recreation setting with a rate of 63% as predicted. Recommendations and implications were discussed. This study can give managers useful information to plan and design recreational settings that support sustainability in the national park.

The conflict between the use of the world's resources and development goals has created significant pressures on the sustainable development of human settlements. This situation has increased the duties and responsibilities of spatial planning and design disciplines about creating sustainable living spaces.As a spatial planning and design discipline, the landscape architecture profession works on the creative transformation of the natural and built environment. In addition, the profession should also provide spatial solutions suitable for science, policies, institutional organizations, resources owned by the community, the way these resources are evaluated and their changes over time. The training of landscape architects who will present these solutions should be integrated into the conditions created by the complex problems facing the world. The process of policy-building in landscape architecture education has come into the agenda due to the need to determine the minimum qualifications in landscape architecture education in line with both national and international standards as well as to the need to create a national policy for increasingly changing/transforming student profile and institutional approaches.The present Landscape Architecture Education Policy, prepared by the Council of Heads of Landscape Architecture Schools of Turkey, is a roadmap for updating itself within the framework of changing national and international dynamics, and for building the future of both education and the profession. In the study; the need for a landscape architecture education policy, the process of emergence of the text and the explanation of this process, the purpose and scope of the policy, its foundations, its vision, its equivalents in higher education, the basic criteria for opening a new program and the accreditation are discussed.The policy text, which aims at the definition of guiding principles with critical mind and stakeholder involvement and the provision of visibility and recognition of strategies and guiding principles and, which will be updated at appropriate intervals, is intended to contribute to the current and future situation of landscape architecture education., Dünyanın sahip olduğu kaynakların kullanımı ile kalkınma hedefleri ve arasındaki çelişkiler, insan yerleşimlerinin sürdürülebilir gelişimi üzerinde önemli baskılar yaratmıştır. Bu durum, mekan planlama ve tasarım disiplinlerinin sürdürülebilir yaşam alanlarının oluşturulması konusundaki görev ve sorumluluklarını arttırmıştır. Bir mekan planlama ve tasarım disiplini olarak peyzaj mimarlığı mesleği, doğal ve yapılı çevrenin yaratıcı biçimde dönüşümü üzerine çalışmalar yürütmektedir. Background: Alterations in ceruloplasmin are currently assumed as one of the mechanisms underlying the development of a number of neurodegenerative disorders. Several studies indicate that elevated serum ceruloplasmin levels may play a role in schizophrenia by exacerbating or perpetuating dopaminergic dysregulation. No study investigating the relationship between ceruloplasmin and obsessive-compulsive disorder (OCD) has been published to date. Nowadays OCD is increasingly speculated to be a different disorder than other anxiety disorders, and rather is considered to be more similar to psychotic disorders. The objective of this study to explore whether there is an association of ceruloplasmin with OCD as in schizophrenia. Method: 26 pure OCD and 9 co-morbid OCD patients from Gaziantep University Sahinbey Research Hospital, Psychiatry Clinics, diagnosed according to the DSM IV and 40 healthy controls were included in the study. Blood samples were collected; ceruloplasmin levels were measured. Results: The mean ceruloplasmin level in pure OCD patients, co-morbid OCD patients, and control group persons were 544.46 +/- 26.53, 424.43 +/- 31.50 and 222.35 +/- 8.88 U/L respectively. Results of all 3 groups differ significantly. Positive predictive value of ceruloplasmin for that cut-off point is 31/31 (100%) and negative predictive value is 40/44 (91%) in our group. Conclusion: Although the nature of relationship is not clear there was an association between ceruloplasmin levels and OCD in our study.
