Background: Branch pulmonary artery (PA) stenosis is one of the most common indications for percutaneous interventions in patients with transposition of the great arteries (TGA), tetralogy of Fallot (ToF), and truncus arteriosus (TA). However, the effects of percutaneous branch PA interventions on exercise capacity remains largely unknown. In addition, there is no consensus about the optimal timing of the intervention for asymptomatic patients according to international guidelines. This trial aims to identify the effects of percutaneous interventions for branch PA stenosis on exercise capacity in patients with TGA, ToF, and TA. In addition, it aims to assess the effects on RV function and to define early markers for RV adaptation and RV dysfunction to improve timing of these interventions. Methods: This is a randomized multicenter interventional trial. TGA, ToF, and TA patients ≥ 8 years with a class IIa indication for percutaneous branch PA intervention according to international guidelines are eligible to participate. Patients will be randomized into the intervention group or the control group (conservative management for 6 months). All patients will undergo transthoracic echocardiography, cardiac magnetic resonance (CMR) imaging, and cardiopulmonary exercise testing at baseline, 6 months, and 2–4 years follow-up. Quality of life (QoL) questionnaires will be obtained at baseline, 2 weeks post intervention or a similar range for the control group, and 6 months follow-up. The primary outcome is exercise capacity expressed as maximum oxygen uptake (peak VO2 as percentage of predicted). A total of 56 patients (intervention group n = 28, control group n = 28) is required to demonstrate a 14% increase in maximum oxygen uptake (peak VO2 as percentage of predicted) in the interventional group compared to the control group (power 80%, overall type 1 error controlled at 5%). Fontan patients undergo multiple cardiothoracic surgeries in childhood. Following these procedures, ventricular function is temporarily decreased, and recovers over months. This is presumably related to cardiopulmonary bypass, but this is incompletely understood. Throughout the Fontan palliation, cardiac function is also affected by volume unloading. We aimed to gain insight into the biological processes related to impaired ventricular function and recovery following Fontan palliations using a panel of biomarkers. Furthermore, we described changes in ventricular function across the Fontan palliation due to volume unloading. We performed a prospective multicenter observational study in patients undergoing partial (PCPC) or total cavo-pulmonary connection (TCPC). Patients underwent assessment—including echocardiography and blood sampling—before surgery (T1), at first follow-up (T2), and 1 year after their procedures (T3). Blood samples were analyzed using a biomarker panel (OLINK CVD-III). Ninety-two biomarkers were expressed as principal components (PC) to limit multiple statistical testing. We included 32 PCPC patients aged 7.2 [5.3–10.3] months, and 28 TCPC patients aged 2.7 [2.2–3.8] years. The single ventricular longitudinal strain (SV GLS) temporarily decreased for PCPC patients at T2 (−15.1 ± 5.6 (T1) to −13.5 ± 5.2 (T2) to −17.3 ± 4.5 (T3), p < 0.047 for all differences), but not following TCPC. The serum biomarkers were expressed as 4 PCs. PC1, including biomarkers of cell–cell adhesion, was not related to any patient characteristic. PC2, including biomarkers of superoxide anion regulation, increased at T2. PC3, including biomarkers of cardiovascular development, related to the stage of Fontan palliation. Ralstonia solanacearum (fylotype II), de veroorzaker van bruinrot in aardappel (Solanum tuberosum), is een quarantainebacterie die sinds de jaren ’90 voorkomt in het Nederlandse oppervlaktewater. Bitterzoet (Solanum dulcamara), een inheemse wilde plant die langs waterwegen groeit, is ook een waardplant van de ziekteverwekker en speelt een belangrijke rol in de vermeerdering van de bacterie en de overleving tijdens de winterperiode. In het voorjaar komt de bacterie vanuit de besmette bitterzoet planten in het water terecht. De bacterie kan ca. 5 weken buiten de plant in het oppervlaktewater overleven (Janse 1996). Er wordt jaarlijks een grote survey uitgevoerd om besmetting van de Nederlandse watergangen in kaart te brengen (Janse et al. 2009). Vondsten kunnen leiden tot een veregeningsverbodsgebied; het oppervlaktewater in deze gebieden mag niet gebruikt worden voor de teelt van aardappelen en tomaten (zie www.nvwa.nl/onderwerpen/plantenziekten-en-plagen/bruinrot/verbodsgebieden-gebruik-oppervlaktewater). In 2020 werd tijdens deze survey in twee afzonderlijke gebieden (omgeving Midden-Regge en omgeving Breukelen) niet R. solanacearum (fylotype II), maar een andere Ralstonia soort aangetoond, namelijk Ralstonia pseudosolanacearum.

Brugada syndrome (BrS) is a rare inherited arrhythmia syndrome. Affected children may experience life-threatening symptoms, mainly during fever. The percentage of SCN5A variant carriers in children is higher than in adults. Current diagnostic and follow-up policies for children with (a family history of) BrS vary between centres. Here, we present a consensus statement based on the current literature and expert opinions to standardise the approach for all children with BrS and those from BrS families in the Netherlands. In summary, BrS is diagnosed in patients with a spontaneous type 1 electrocardiogram (ECG) pattern or with a Shanghai score ≥ 3.5 including ≥ 1 ECG finding. A sodium channel-blocking drug challenge test should only be performed after puberty with a few exceptions. A fever ECG is indicated in children with suspected BrS, in children with a first-degree family member with definite or possible BrS according to the Shanghai criteria with a SCN5A variant and in paediatric SCN5A variant carriers. In-hospital rhythm monitoring during fever is indicated in patients with an existing type 1 ECG pattern and in those who develop such a pattern. Genetic testing should be restricted to SCN5A. Children with BrS and children who carry an SCN5A variant should avoid medication listed at www.brugadadrugs.org and fever should be suppressed. Ventricular arrhythmias or electrical storms should be treated with isoproterenol infusion.

METHODS AND RESULTS: Of 137 children we included in a prospective cohort, 36 (26%) reached the study endpoint (SE: all-cause death or HTx), 15 (11%) died at a median of 0.09 years [inter-quartile range (IQR) 0.03-0.7] after diagnosis, and 21 (15%) underwent HTx at a median of 2.9 years [IQR 0.8-6.1] after diagnosis. Median follow-up was 2.1 years [IQR 0.8-4.3]. Twenty-three children recovered at a median of 0.6 years [IQR 0.5-1.4] after diagnosis, and 78 children had ongoing disease at the end of the study. Children who reached the SE could be distinguished from those who did not, based on the temporal evolution of four risk factors: stunting of length growth (-0.42 vs. -0.02 length Z-score per year, P < 0.001), less decrease in N-terminal pro-B-type natriuretic peptide (NT-proBNP) (-0.26 vs. -1.06 2log pg/mL/year, P < 0.01), no decrease in left ventricular internal diastolic dimension (LVIDd; 0.24 vs. -0.60 Boston Z-score per year, P < 0.01), and increase in New York University Pediatric Heart Failure Index (NYU PHFI; 0.49 vs. -1.16 per year, P < 0.001). When we compared children who reached the SE with those with ongoing disease (leaving out the children who recovered), we found similar results, although the effects were smaller. In univariate analysis, NT-proBNP, length Z-score, LVIDd Z-score, global longitudinal strain (%), NYU PHFI, and age >6 years at presentation (all P < 0.001) were predictive of adverse outcome. In multivariate analysis, NT-proBNP appeared the only independent predictor for adverse outcome, a two-fold higher NT-proBNP was associated with a 2.8 times higher risk of the SE (hazard ratio 2.78, 95% confidence interval 1.81-3.94, P < 0.001). Second, we test whether the effects of individual job insecurity on individual relationship satisfaction depends on partner job insecurity. Background: Research has established that job insecurity is negatively associated with relationship quality at the individual level, but little is known about how partner's job insecurity shapes individual perceptions of relationship quality. Although one's partner's job insecurity is expected to be negatively associated with relationship quality on average, it may be protective in cases in which individual job insecurity is also high. We expand spillover–crossover and family stress models to develop and test hypotheses about the independent and interactive effects of individual and partner job insecurity on relationship quality and further consider potential gender and educational differences in the independent effects of individual and partner job insecurity on individual perceptions of relationship quality. Method: Dutch dyadic longitudinal panel data was used (Dutch Longitudinal Internet Studies for the Social Sciences panels 2008–2015) composed of 11,536 observations of 3,668 individuals. Random and fixed effects models were estimated to study differences between individuals (random effects) and changes over time within individuals (fixed effects). Results: Men and women were less satisfied with their relationship when they felt more job insecurity, and women were less satisfied when their male partner experienced job insecurity. Men's relationship satisfaction was less strongly related to job insecurity when his partner experienced job insecurity as well. Tiivistelmä Oulussa järjestettiin 13.6.2018 lääketieteen ja hammaslääketieteen tohtorintutkinnon konsensuskokous, jossa käytiin läpi vallitsevia käytäntöjä ja muutostarpeita. Ohjeellinen tutkinnon laajuus on neljä vuotta kokopäiväistä työtä, ja muodollisesti tutkinnon myöntää yhtä yliopistoa lukuun ottamatta aina vastaava tiedekunta. Ohjaajia on tyypillisesti kahdesta kolmeen, ja yksi on pääohjaaja. Seurantaryhmä on käytössä tai ollaan ottamassa käyttöön kaikissa yliopistoissa. Aktiivinen seurantaryhmä tukee merkittävästi ohjausprosessia. Väitöskirjojen asiantuntijoina toimivat esitarkastajat ja vastaväittäjät ovat yleensä muualta kuin suorituspaikan yliopistosta. Osittain tästä syystä suuria eroja eri yliopistojen tohtorintutkinnon vaatimusten välillä ei todettu. Haasteena väitöskirjatyössä on kliinisen työn paine ja nuorten kollegoiden ruuhkavuosien kuormittuminen, mitkä heikentävät mahdollisuuksia tutkimustyöhön. Kaikki osallistuneet kannattivat käytäntöjen vakiinnuttamista ja yhtenäistämistä säännöllisellä konsensuskokoustyöskentelyllä., On June 13, 2018 a consensus meeting of the PhD degree in medicine and dentistry was held in Oulu to review the prevailing practices and assess the needs for change. The PhD degree should be completed in four years of full-time work and, except for one university, is formally awarded by the respective faculty. There are typically two or three supervisors, one acting as the principal supervisor. The follow-up group is or is being implemented in all universities and provides support to the process. To ensure objectivity, pre-examiners and opponents are not usually affiliated to the university granting the degree. This in part explains the relatively similar requirements. One of the challenges in PhD studies is how to combine time-consuming clinical work and requirements of the daily family life with research work. Recent positive changes include the systematization of doctoral studies and improvement in the level of guidance and support from both supervisors and follow-up committees. All participants of the consensus meeting supported the goal to consolidate and harmonize the doctoral training practices in Finland.

Expectations were formulated that people who experienced or expected economic hardship would be less satisfied with their family life. Additionally, it was hypothesized that current and expected economic hardship would amplify each other's consequences on satisfaction, and that current and expected economic hardship was more harmful for people with children and when the rise of unemployment in a country was larger. Multilevel analyses were conducted using a sample from the European Quality of Life Survey 2012 (N = 13,013 in 30 countries). Results indeed indicated that people who experienced or expected economic hardship were less satisfied with their family life. Expecting a financial decline was (slightly) more harmful for people in larger families. Generally, current and expected economic problems were not more harmful for parents or when a country's rise of unemployment was larger.

Aims: To clarify the clinical characteristics and outcomes of children with SCN5A-mediated disease and to improve theirrisk stratification. Methods and results: A multicentre, international, retrospective cohort study was conducted in 25 tertiary hospitals in 13 countries between1990 and 2015. All patients <_16 years of age diagnosed with a genetically confirmed SCN5A mutation wereincluded in the analysis. There was no restriction made based on their clinical diagnosis. A total of 442 children{55.7% boys, 40.3% probands, median age: 8.0 [interquartile range (IQR) 9.5] years} from 350 families wereincluded; 67.9% were asymptomatic at diagnosis. Four main phenotypes were identified: isolated progressive cardiacconduction disorders (25.6%), overlap phenotype (15.6%), isolated long QT syndrome type 3 (10.6%), and isolatedBrugada syndrome type 1 (1.8%); 44.3% had a negative electrocardiogram phenotype. During a medianfollow-up of 5.9 (IQR 5.9) years, 272 cardiac events (CEs) occurred in 139 (31.5%) patients. Patients whose mutationlocalized in the C-terminus had a lower risk. Compound genotype, both gain- and loss-of-function SCN5A mutation,age <_1 year at diagnosis in probands and age <_1 year at diagnosis in non-probands were independent predictorsof CE.Conclusion In this large paediatric cohort of SCN5A mutation-positive subjects, cardiac conduction disorders were the mostprevalent phenotype; CEs occurred in about one-third of genotype-positive children, and several independent riskfactors were identified, including age <_1 year at diagnosis, compound mutation, and mutation with both gain- andloss-of-function.
