Aim: Our aim was to find different radiological markers for the diagnosis of coexistence of pseudotumor cerebri and Chiari malformation Type 1. Method: Patients who were referred to the Department of Radiology with a clinical preliminary diagnosis of Chiari malformation Type 1 between 2007 and 2020 and whose diagnosis was confirmed radiologically by magnetic resonance imaging were evaluated retrospectively. 49 Chiari malformation Type 1 patients with both cervical and cerebral examinations and 49 control groups of the same age and sex without Chiari malformation Type 1 diagnosis were included in the study. Presence of pseudotumor cerebri radiologically from Cerebral MRI images of patients with Chiari malformation Type 1 diagnosis, presence of syringomyelia from cervical spinal MRI images, and distance of cerebellar tonsils and obex according to McRae line were evaluated in millimeters. Results: In cases with Chiari malformation Type 1 clinical and radiological diagnosis, patients with radiological diagnosis of pseudotumor cerebri had lower cerebellar tonsils and Obex than those who were not, and the rate of accompanying syringomyelia seemed to be higher. However, there was no statistically significant difference between the two groups. Conclusion: Coexistence of pseudotumor cerebri and Chiari malformation Type 1 was more common than expected. The treatment protocol is different in the coexistence of pseudotumor cerebri and Chiari malformation Type 1, therefore this increases the importance of diagnosing the coexistence. Further radiological imaging studies are needed to find different radiological markers in the diagnosis of pseudotumor cerebri and Chiari malformation Type 1 coexistence., Amaç: Amacımız Psödotümör serebri ve Chiari malformasyonu Tip 1 birlikteliğinin tanısı için farklı radyolojik belirteçler bulabilmektir. Yöntem: 2007-2020 tarihleri arasında radyoloji anabilim dalına Chiari malformasyonu Tip 1 klinik ön tanısıyla yönlendirilip magnetik rezonans görüntüleme ile radyolojik olarak tanısı doğrulanan hastalar geriye dönük olarak değerlendirildi. Hem servikal hem serebral tetkikleri bulunan 49 Chiari malformasyonu Tip 1 hastası ve aynı yaş ve cinsiyetten olan Chiari malformasyonu Tip 1 tanısı olmayan 49 kontrol grubu çalışmaya dahil edildi. Chiari malformasyonu Tip 1 tanılı hastaların Serebral MRG görüntülerinden radyolojik olarak Psödotümör serebri varlığı, Servikal spinal MRG görüntülerinden şiringomyeli varlığı, McRae çizgisine göre serebeller tonsillerin ve obexin mesafesi milimetre cinsinden değerlendirildi. Bulgular: Chiari malformasyonu Tip 1 klinik ve radyolojik tanılı olgularda radyolojik olarak Psödotümör serebri tanısı konulan hastaların konulmayanlara göre serebeller tonsilleri ve Obexi daha aşağı yerleşimlidir ve siringomyeli eşlik etme oranı daha fazla gözükmektedir. Ancak her iki grup arasında istatistiki olarak anlamlı bir farklılık izlenmemektedir. Sonuç: Psödotümör serebri ve Chiari malformasyonu Tip 1 birlikteliği tahmin edildiğinden daha sık görülmektedir. Psödotümör serebri ve Chiari malformasyonu Tip 1 birlikteliğinde tedavi protokülünün farklı olması bu birlikteliğin tanısını koyabilmenin önemini arttırmaktadır. Psödotümör serebri ve Chiari malformasyonu Tip 1 birlikteliğinin tanısında farklı radyolojik belirteçler bulabilmek için daha fazla sayıda radyolojik görüntüleme çalışmalarına ihtiyaç vardır.

Aim:The aim of this study was to determine the presence and ratio of septum or doublehypoglossal canals in human fetuses. Method:In the inventory of the Department of Anatomy, Faculty of Medicine, twenty-fivefetuses (14 males and 11 females) with a mean age of 23.36 ± 2.63 weeks ofgestation were included in the study. Results:Double hypoglossal canal was detected in 9 (36%) of the 25 fetuses investigated.The double hypoglossal presence was bilateral in 2 (%8) fetuses, while 7 (%28)fetuses were unilateral. The presence of double hypoglossal canal incidence was12% on the right and 16% on the left. There was norelationship between the presence of a septum and gestational age, gender orside. Conclusion: The division ofthe hypoglossal canal with a bony septum is of great importance forneurosurgeons and radiologists, especially for posterior cranial fossaoperations such as hypoglossal canal schwannomas. Additionally, the data presentedin this study may be valuable for anatomists and anthropologists inunderstanding the development of the skull base., Amaç: Buçalışmanın amacı, insan fetüslerinde septum veya çift hipoglossal kanal insidansınıtespit etmektir. Yöntem: TıpFakültesi Anatomi Anabilim Dalı envanterinde bulunan ortalama 23.36±2.63 haftalık 25 fetüs (14 erkek, 11kız) çalışmaya dahil edildi. Bulgular:Fetüslerin 9’unda (%36) septum vardı. Fetüslerin 2’sinde (%8) bilateral, 3’ünde(%12) sağ ve 4’ünde (%16) sol tarafta çift hipoglossal kanal tespit edildi. Gestasyonalyaş, cinsiyet ve taraflar ile septum varlığı arasında bir ilişki görülmedi. Sonuç: Hipoglossal kanalın bölümlenmepaterni özellikle hipoglossal kanal schwannoma’ları gibi posterior kraniyalfossa operasyonları açısından önem taşıdığından nörocerrahlar ve radyololariçin büyük önem taşımaktadır. Ayrıca çalışmamızdaki veriler, kafa tabanınıngelişiminin anlaşılması açısından anatomistler ve antropologlar için kıymetliolabilir.

Chiari sendromu, arka fossa yapılarının foramen magnumdan üst servikal spinal kanala doğru değişen derecelerde yer değiştirmesi ile birlikte olan, kraniovertebral bileşkenin ve art beynin (hindbrain) konjenital bir anomalisidir. Cerrahi tedavi uygulanan hastaların büyük çoğunluğuna posterior fossa dekompresyonu yapılır. Biz bu çalışmada, cerrahi tedavi yapılan Chiari sendromlu hastalarda cerrahinin klinik semptomlar ve bulgular, görüntüleme parametreleri üzerindeki etkisini retrospektif olarak değerlendirmeyi amaçladık.Temmuz 2008 ve Ağustos 2015 yılları arasında cerrahi tedavi uygulanmış 70 Chiari sendromu tip 1 hastasının dosya bilgileri ve radyolojik görüntülemeleri retrospektif bir şekilde incelenmiştir. Cerrahi tedavi uygulanmış 70 Chiari sendromu tip 1 hastasından 64 hastaya (%91,4); foramen magnum dekompresyonu ve C1 laminektomi yapılırken; 6 hastaya (%8,6), foramen magnum dekompresyonu, C1 laminektomi ve duraplasti yapılmıştır. Hastalar duraplasti yapılanlar ve yapılmayanlar olarak 2 gruba ayrılmıştır. Grup 1'de duraplasti yapılan 6 hasta (%8,6) ve grup 2'de duraplasti yapılmayan 64 hasta (%91,4) mevcuttu. Grup 2'deki hastaların, grup 1'deki hastalara göre iyileşme oranları daha yüksek saptanmıştır. Chiari sendromu tip 1 hastalarının operasyonunda, duraplasti olmaksızın yapılan foramen magnum dekompresyonu ve C1 laminektomi güvenli ve etkili bir cerrahi yöntemdir. Duraplasti yapılan Chiari tip 1 sendromlu hastaların operasyon sırasında kan transfüzyon oranı, postoperatif komplikasyon oranı, postoperatif yoğun bakım takip süresi oranı ve hastanede yatış süresi oranları; duraplasti yapılmayan Chiari tip 1 sendromlu hastalara göre daha yüksek saptanmıştır. The chiari syndrome is a congenital anomaly of the craniovertebral junction and hindbrain. It is characterised by the herniation of posterior fossa structures through the foramen magnum into the cervical spinal canal. Vast majority of patients undergoing surgical treatment receive posterior fossa decompression. We aimed at evaluating retrospectively effects of surgery on the clinical symptoms and signs and imaging parameters in the patients with Chiarisyndrome undergoing surgical treatment.Medical charts and radiological images of the 70 patients with Chiari syndrome type I who had surgical treatment between July 2008 and August 2015 were reviewed retrospectively. 64 patients (%91,4) who applied surgical treatment of 70 patients with Chiari syndrome type 1, underwent foramen magnum decompression and C1 laminectomy. 64 patients (%8,6) underwent foramen magnum decompression, C1 laminectomy and duraplasty. The patients are divided in two groups based on presence or absence of associated duraplasty. 6 patients (%8,6) in group 1 based on presence of duraplasty and 64 patients (%91,4) in group 2 based on absence of duraplasty was available. The recovery rates of the patients in Group 2 were significantly higher than in Group 1 patients.Foramen magnum decompression and C1 laminectomy made without duraplasty are safe and effective surgical procedure in the operation of patients with Chiari syndrome type 1. Blood transfusion rate during surgery, postoperative complication rate, hospitalization rate and duration of the postoperative intensive care unit rates of patients with Chiari syndrome type 1 based on presence of duraplasty were significantly higher than patients with Chiari syndrome type 1 based on absence of duraplasty. 63
