32. Abstract 13147: Association of Electroanatomic Mapping Derived Left Atrial Bipolar Voltage With Clinical Characteristics and Recurrence of Arrhythmia in Patients Undergoing Ablation for Atrial Fibrillation

Abstract

Introduction:Low voltage measured by electroanatomic mapping (EAM) in the left atrium (LA) has been correlated with myocardial fibrosis. Low voltage thresholds of 0.1-0.5mV and 0.2-0.45 have been proposed for the LA. Increased atrial fibrosis is associated with increased AF recurrence post ablation. However the extent and appropriate thresholds of low LA voltage to utilize remain unclear.Objective:To determine association of LA bipolar voltage thresholds and extent of LA low voltage areas with atrial arrhythmia (AA) recurrence and risk factors for recurrence.Methods:Patients undergoing AF ablation were followed for 12 months. Characteristics including age, comorbidities, and AF type were recorded. EAM (Carto: Biosense Webser, CA) was used to measure LA shells and bipolar voltage (mV) at each point. Bipolar voltages thresholds <0.1, <0.2 <0.5 mV were used to identify LA low voltage regions. These areas were measured and percent area of LA (%LA) was calculated. Recurrence of AA was defined as any AF, atrial tachycardia, or atrial flutter > 30 seconds after a 3-month blanking period. Analyses were performed using Fisher?s exact test for dichotomous and 2-tailed t-tests for continuous variables.Results:Among 30 patients, 21 and 9 subjects underwent first and repeat ablation, respectively. A mean of 1843 ? 206 points per patient were collected. Low voltage areas were 5.3 ? 3.3 % (<0.1mV), 11.1 ? 4.3 %(<0.2mV), and 20.4 ? 5.3 % (< 0.5mV) of LA area. Presence of diabetes mellitus, hypertension, obesity and dyslipidemia was not correlated with %LA. Mean low voltage %LA was lower in patients with AA recurrence only at 0.1mV: (0.1mV: 5.1 ? 1.4 vs 5.4 ? 3.6; p = 0.002; 0.2mV: 23 ? 6.4 vs. 8.1 ? 3.6; p = 0.4; 0.5 mV: 38.8 ? 8.6 vs. 15.8 ? 4; p = 0.79). Using low voltage areas of >10%, 15% and 20% at <0.1, 0.2 or 0.5mV, larger low voltage areas at lower thresholds were associated with AA recurrence (0.1mV: 10% p=0.001, 15% p=0.018, 20% p=NS; 0.2mV: 10% p=0.1, 15% p=0.04, 20% p=0.001; 0.5mV: 10% p=0.72, 15% p=0.6, 20% p=0.4).Conclusions:Increased AA recurrence is associated with larger areas of low LA voltage at <0.1 and <0.2 but not <0.5mV. Low LA voltage was not associated with clinical risk factors for AA recurrence. Lower thresholds to define low LA voltage were associated with AA recurrence.

33. Successful permanent pacemaker explantation after diagnosis and treatment of Lyme carditis complete heart block: a case report.

Abstract

Background: Lyme carditis (LC) complete heart block (CHB) is typically treated with i.v. antibiotics without requiring permanent pacing. In patients with high degree atrioventricular (AV) block, suspicious index in Lyme carditis (SILC) scoring is highly sensitive for diagnosing LC., Case Summary: We present a case of CHB where a permanent pacemaker (PPM) was implanted prior to LC diagnosis. Suspicious index in Lyme carditis score was 2 at the time of exam, indicating a low risk for LC. However, per further discussion at follow-up, his score was retroactively increased to an intermediate risk of 4 and Lyme titres returned positive. An outpatient oral antibiotic regimen was given, and 2 months later, the patient had <0.1% V-pacing on interrogation with a subsequent unremarkable event monitor. The pacemaker was removed after considerations ensuring full conduction recovery. The patient is doing well at follow-up > 1 year., Discussion: Lyme carditis spontaneous resolution of CHB is common. Once safe extraction parameters have been established, it is appropriate to engage patients without ongoing pacer requirements about explantation of their PPM. For CHB patients without clear aetiology, SILC scoring may be a predictive measure to help prevent unnecessary PPM placement in the future., Competing Interests: Conflict of interest: None declared., (© The Author(s) 2023. 35. Cardioembolic stroke in a young male with cor triatriatum sinister: a case report.

Abstract

Background: Cor triatriatum sinister (CTS) is a rare congenital cardiac anomaly defined by a fibromuscular membrane which bisects the left atrium. Cor triatriatum sinister has been associated with cardioembolic stroke through mechanisms including stagnation of blood flow within the left atrium, an association with atrial fibrillation (AF), and/or an accompanying atrial septal defect (ASD) or patent foramen ovale. We describe a case highlighting the role that CTS may play in cardioembolic stroke, provide high-quality computed tomography angiography and two- and three-dimensional echocardiography of the CTS membrane, and outline management strategies for this uncommon clinical scenario., Case Summary: A 35-year-old man with no prior medical history presented with acute onset weakness and aphasia. He was found to have an embolic stroke with left M1 and A1 occlusions and received tissue plasminogen activator followed by mechanical thrombectomy with successful recanalization. A thorough stroke workup revealed CTS with an associated ASD as well as potential protein C deficiency. He was managed with indefinite anticoagulation with apixaban., Discussion: This is the 13th reported case of CTS associated with stroke. In most previous cases evidence of blood stasis or frank thrombus was associated with the CTS membrane, and/or existing AF was noted. In this case, none of these were identified, particularly highlighting the surreptitious risk of CTS. In addition, the presence of potential protein C deficiency in this case compounded the risk for thromboembolism and factored into multidisciplinary management decisions., (© The Author(s) 2020.
