2. School Nurse Practice: A Descriptive Spanish Study

- Abstract
This study aimed to describe the profile and practices of school nurses working at a network of educational centers in Spain. This was a descriptive study of the documented actions of 107 school nurses between September 2018 and June 2021 in 54 educational centers (55.6% private and 44.4% subsidized). The profile of the school nurses was young (average age 33.8 [standard deviation (SD) = 7.7] years) and predominantly female (91.6%) with a diverse and multidisciplinary education, primarily at the postgraduate level (specialized mainly in emergency care, nursing/school health, and pediatrics/neonatology). They carried out 256,499 interventions. The most frequent types of incidents they treated were accidents (30.4%) and disease-related episodes (22.2%). The interventions were usually brief (average time 7.7 min) and were resolved by the school nurse (99.1%), and the main recipients were students (87.3%). The highest incidence of interventions occurs during breaks between classes. Acute interventions occupied most of the school nurses' time, leaving little opportunity for health education (0.3%). School nurses played an important role in preserving and promoting the health of school populations and cost-savings to healthcare systems with the actions that they performed. 5. Students with Disabilities at University: Benefits and Challenges from the Best Faculty Members' Experiences

- Abstract
This article presents the results of a study that gave a voice to 119 faculty members of 10 Spanish universities, who were recommended by their students with disabilities for carrying out an inclusive pedagogy. Following a qualitative methodology, two interviews were held to analyse different areas of inclusive pedagogy: beliefs, knowledge, designs and actions. The data were analysed through an inductive system of categories and codes. The results show the positive aspects and benefits highlighted by these faculty about their experiences regarding the presence of students with disabilities in their classrooms, as well as the main challenges they encounter when attempting to achieve the social and educational inclusion of these students. more...

- Abstract
Purpose: This paper aims to show how the Green Campus Program has been implemented at the Faculty of Sociology of the University of A Coruña (Spain). It describes the criteria used to create teaching sustainability actions related to community engagement to introduce education for sustainable development into college curricula. Design/methodology/approach: Drawing on a human-centered design model approach, as well as on transformative teaching theories, this study explores the criteria used to build the Free Classroom based on a participatory model. Findings: The authors argue that the success of this activity depends on how it relates to the theme-based specialization of the different academic degrees through which they are managed. Equally important is the creation of permanent spaces that enable the collaboration of other organizations, such as non-governmental associations and local public administrations. Originality/value: The findings provide valuable insights into how the social dimension of sustainability in higher education institutions can be emphasized. A model of implementation of the activities is offered under which academic, political, student and community agents are coordinated to favor the change of attitudes and behaviors to strengthen SD. more...

- Abstract
COVID-19 has overloaded national health services worldwide. Thus, early identification of patients at risk of poor outcomes is critical. Our objective was to analyse SARS-CoV-2 RNA detection in serum as a severity biomarker in COVID-19. Retrospective observational study including 193 patients admitted for COVID-19. Detection of SARS-CoV-2 RNA in serum (viremia) was performed with samples collected at 48-72 h of admission by two techniques from Roche and Thermo Fischer Scientific (TFS). Main outcome variables were mortality and need for ICU admission during hospitalization for COVID-19. Viremia was detected in 50-60% of patients depending on technique. The correlation of Ct in serum between both techniques was good (intraclass correlation coefficient: 0.612; p < 0.001). Patients with viremia were older (p = 0.006), had poorer baseline oxygenation (PaO
2 /FiO2 ; p < 0.001), more severe lymphopenia (p < 0.001) and higher LDH (p < 0.001), IL-6 (p = 0.021), C-reactive protein (CRP; p = 0.022) and procalcitonin (p = 0.002) serum levels. We defined "relevant viremia" when detection Ct was < 34 with Roche and < 31 for TFS. These thresholds had 95% sensitivity and 35% specificity. Relevant viremia predicted death during hospitalization (OR 9.2 [3.8-22.6] for Roche, OR 10.3 [3.6-29.3] for TFS; p < 0.001). Cox regression models, adjusted by age, sex and Charlson index, identified increased LDH serum levels and relevant viremia (HR = 9.87 [4.13-23.57] for TFS viremia and HR = 7.09 [3.3-14.82] for Roche viremia) as the best markers to predict mortality. Viremia assessment at admission is the most useful biomarker for predicting mortality in COVID-19 patients. 10. In the name of the people: The populist redefinition of nature conservation in post-crisis Spain.

• Debates on the political ecology of populism has not reached the conservation field. • Yet a new populist conservation model is appearing in different locations in Spain. • Populist conservation builds on neoliberal and participatory conservation. • It permits the co-optation and privatization of conservation. • It can be resisted from an environmental justice rationale. The global surge of right-wing populism and its impact on environmental policies are attracting the attention of political ecologists. However, little of this debate has reached the nature conservation literature. In this paper, I explore the emergence of populist logics in conservation as a result of the entrenchment of neoliberalism after the 2008 economic crisis. On the one hand, neoliberalism has incited the roll-back of public institutions that hitherto monopolised the management of protected areas (PAs), as well as the roll-out of market-based and network-based forms of PA governance. At the same time, it has also had a significant impact on rural communities, imposing austerity policies that have caused a great deal of social and economic precarity and vulnerability, generating feelings of abandonment, dispossession and disenfranchisement. To illustrate how these two phenomena intersect and motivate the surge of right-wing populism in conservation, this paper dissects a number of parliamentary debates on a recent policy reform that seeks to decentralise the management of PAs in Asturias, a region in the north of Spain. These debates brought together stakeholders and members of several rural groups with different political orientations and views, including public administrators and policy-makers, farmers and livestock breeders, farmers' unions, landowners, tourism business owners, hunters and academics. I analyse how right-wing populist discourses framed this policy change, scapegoated the public management of conservation for all the problems suffered in rural areas, and co-opted popular demands of rural communities, reducing them to the economic interest of private landowners. I will also describe the various attempts made to negotiate a left-wing, progressive democratic alternative that hinges on the recognition of the social complexity of rural communities and the diversity of problems that affect them. The 2008 economic crisis and the introduction of austerity policies and values after the crisis are transforming the management of natural protected areas in the global North. Communities of amenity migrants living in these areas have been impacted by such issues as budgetary cuts and state disinvestments in conservation. Important threats to their expectations of an alternative life in a natural idyll are changing the attitudes of amenity migrants towards conservation and rural regions. In this paper, I analyse the main changes in their livelihood and lifestyles in two different natural parks in Spain. Carbonyl sulfide (OCS) is used to quantify the carbon capture potential of the biosphere because of its direct correlation with CO2 uptake during photosynthesis. However, to constrain the urban biosphere signal, it is necessary to evaluate potential anthropogenic sources. We conducted two sampling campaigns in the Metropolitan Area of Barcelona (AMB), Spain, during May (full COVID lockdown) and October 2020 to measure the spatial distribution and variability of OCS in four urban land uses as follows: built, urban forest, urban park, and peri‐urban agriculture. The OCS background levels determined at Tibidabo (442 m asl) were approximately 484 ± 20 ppt and 407 ± 8 ppt for May and October 2020, respectively, and agreed with other seasonal surveys conducted in Europe during that same period. Averaged emissions were in the range of +Δ12 ± 40 ppt for the city and were +Δ9.4 ± 40.9 ppt for urban +Δ22.1 ± 48 ppt for urban green +Δ20.7 ± 42.9 ppt for agricultural and −Δ4.8 ± 19.6 ppt for forest. The urban values ranged from neutral to above background, suggesting nearby anthropogenic and marine emissions such as +Δ150 ppt in Montjuic, which is downwind of Barcelona's harbor. During the crop‐growing season in May, the agricultural areas consistently showed values below the background (up to −Δ76 ppt in Gavà, uptake) at 7:00 UTC when the land breezes were dominant, while later in the morning, when the sea breeze are developed, the plant sink is masked by the transport of marine emissions. Urban forests located north of Tibidabo showed OCS values up to −Δ70 ppt, suggesting significant uptake by urban forests. We conclude that determining the urban biosphere signal using OCS as a tracer is more complex than expected because the marine and anthropogenic emissions from the port strongly impact the spatial‐temporal distribution of OCS. In most Western countries, there is clear evidence of a reduction and even a reversal of the gender gap in education. However, there are doubts about the effect of this reversal on family life and about its impact on the distribution of gender roles within couples. Using data from the Spanish time use survey of 2009–10, this paper explores whether the improved education of women is translating to better occupations and higher incomes and whether it is related to a more egalitarian distribution of gender roles. The greater human capital that women are developing as a result of their improved education is being wasted because it is not reflected in better positions in terms of occupational class and income. The barriers of 'doing gender' also prevent a fully egalitarian division of roles, and only the most advantaged women are in couples with a relatively egalitarian division of time. Antibiotic resistance is one of the major challenges that humankind shall face in the short term. (Bacterio)phage therapy is a valuable therapeutic alternative to antibiotics and, although the concept is almost as old as the discovery of phages, its wide application was hindered in the West by the discovery and development of antibiotics in the mid-twentieth century. However, research on phage therapy is currently experiencing a renaissance due to the antimicrobial resistance problem. Some countries are already adopting new ad hoc regulations to favor the short-term implantation of phage therapy in clinical practice. In this regard, the Phage Therapy Work Group from FAGOMA (Spanish Network of Bacteriophages and Transducing Elements) recently contacted the Spanish Drugs and Medical Devices Agency (AEMPS) to promote the regulation of phage therapy in Spain. As a result, FAGOMA was asked to provide a general view on key issues regarding phage therapy legislation. Maxillary sinus membrane perforation has been reported as the most common intraoperative complication during sinus augmentation, potentially leading to postoperative infection and consequent loss of graft or even implant failure. Numerous anatomical factors have been demonstrated to affect membrane tearing. However, careful use of proper instrumentation, such as a reamer, seems to play an important role in minimizing the incidence of these complications. Hence, the aim of the present study was to (1) investigate the reliability of reamer drilling for lateral window preparation; (2) examine the incidence of membrane perforation; and (3) study the factors that might influence membrane perforation. Results from this study showed the safety and effectiveness of using a reamer to perform lateral window approach sinus augmentation. The sinus membrane perforation rate was found to be 12.5%. A slightly higher perforation rate was noted in thinner maxillary lateral walls (< 1.25 mm). Antimicrobial agents are crucial for the treatment of many bacterial diseases in pigs, however, the massive use of critically important antibiotics such as colistin, fluoroquinolones and 3rd–4th-generation cephalosporins often selects for co-resistance. Based on a comprehensive characterization of 35 colistin-resistant Escherichia coli from swine enteric colibacillosis, belonging to prevalent Spanish lineages, the aims of the present study were to investigate the characteristics of E. coli clones successfully spread in swine and to assess the correlation of the in vitro results with in silico predictions from WGS data. The resistome analysis showed six different mcr variants: mcr-1.1 ; mcr-1.10 ; mcr-4.1 ; mcr-4.2 ; mcr-4.5; and mcr-5.1. Additionally, bla
CTX–M– 14 , blaCTX–M– 32 and blaSHV– 12 genes were present in seven genomes. PlasmidFinder revealed that mcr-1.1 genes located mainly on IncHI2 and IncX4 types, and mcr-4 on ColE10-like plasmids. Twenty-eight genomes showed a gyrA S83L substitution, and 12 of those 28 harbored double-serine mutations gyrA S83L and parC S80I, correlating with in vitro quinolone-resistances. Notably, 16 of the 35 mcr- bearing genomes showed mutations in the PmrA (S39I) and PmrB (V161G) proteins. The summative presence of mechanisms, associated with high-level of resistance to quinolones/fluoroquinolones and colistin, could be conferring adaptive advantages to prevalent pig E. coli lineages, such as the ST10-A (CH11-24), as presumed for ST131. SerotypeFinder allowed the H-antigen identification of in vitro non-mobile (HNM) isolates, revealing that 15 of the 21 HNM E. coli analyzed were H39. Since the H39 is associated with the most prevalent O antigens worldwide within swine colibacillosis, such as O108 and O157, it would be probably playing a role in porcine colibacillosis to be considered as a valuable subunit antigen in the formulation of a broadly protective Enterotoxigenic E. coli (ETEC) vaccine. Our data show common features with other European countries in relation to a prevalent clonal group (CC10), serotypes (O108:H39, O138:H10, O139:H1, O141:H4), high plasmid content within the isolates and mcr location, which would support global alternatives to the use of antibiotics in pigs. Abstract: The aim of this study was to analyse health inequalities in the immigrant population in Spain in 2014, while differentiating between immigrant and native‐born men and women. We have designed a cross‐sectional study on the population aged over 15 years resident in Spain and the data were obtained from the 2014 European Health Survey in Spain (n = 22,842). Among immigrant men and women, we observed a lower risk of having a Chronic Physical Problem (CPP) or a Mental Health Problem (MHP) and a lower consumption of psychiatric drugs. We also observed a higher risk of lack of medical care in immigrant men compared to native‐born. The country of origin was not significantly related to self‐perception of health or use of Primary Care (PC) and Emergency Care services. In conclusion, we observed that now that the peak of the crisis has passed it seems that the “healthy immigrant” effect is being recovered, although the gender inequalities observed in the general population are transferred to the immigrant population. Background. Aims were to assess the efficacy of metacognitive training (MCT) in people with a recent onset of psychosis in terms of symptoms as a primary outcome and metacognitive variables as a secondary outcome. Method. A multicenter, randomized, controlled clinical trial was performed. A total of 126 patients were randomized to an MCT or a psycho-educational intervention with cognitive-behavioral elements. The sample was composed of people with a recent onset of psychosis, recruited from nine public centers in Spain. The treatment consisted of eight weekly sessions for both groups. Patients were assessed at three time-points: baseline, post-treatment, and at 6 months follow- up. The evaluator was blinded to the condition of the patient. Symptoms were assessed with the PANSS and metacognition was assessed with a battery of questionnaires of cognitive biases and social cognition. Results. Both MCT and psycho-educational groups had improved symptoms post-treatment and at follow-up, with greater improvements in the MCT group. The MCT group was superior to the psycho-educational group on the Beck Cognitive Insight Scale (BCIS) total (p = 0.026) and self-certainty (p = 0.035) and dependence self-subscale of irrational beliefs, comparing baseline and post-treatment. Moreover, comparing baseline and follow-up, the MCT group was better than the psycho-educational group in self-reflectiveness on the BCIS (p = 0.047), total BCIS (p = 0.045), and intolerance to frustration (p = 0.014). Jumping to Conclusions (JTC) improved more in the MCT group than the psycho-educational group (p = 0.021). Regarding the comparison within each group, Theory of Mind (ToM), Personalizing Bias, and other subscales of irrational beliefs improved in the MCT group but not the psycho-educational group (p < 0.001-0.032). Conclusions. MCT could be an effective psychological intervention for people with recent onset of psychosis in order to improve cognitive insight, JTC, and tolerance to frustration. The aim of this paper is to analyze the relationship between immigration and redistributive public spending by using the recent, massive arrival of immigrants in Spain. Specifically, we focus our analysis on the effect of 1998-2006 changes in local immigrant density on contemporaneous changes in municipal spending in social services. To address the potential endogenous location of immigrants, we adopt an instrumental variables approach that uses the distribution of rental housing in 1991 to predict the location of immigrant inflows. The results indicate that (per capita) social spending increased less in those municipalities that recorded the largest increases in immigrant density. In the Balearic Islands, as in other areas in southern Europe, there are a significant proportion of asymptomatic Leishmania infantum-infected blood donors. Theoretically, these donors may represent an important challenge for blood transfusion safety. However, despite an active search of multiply transfused patients, there have been, so far, no cases of transfusion-transmitted leishmaniasis (TTL) in our region. On the other hand, there is scarce evidence of the TTL in the literature. Background: Opisthorchis felineus and Metorchis bilis are trematodes that cause opisthorchiasis and metorchiasis, respectively. The freshwater snails Bithynia hispanica and B. tentaculata are the respective intermediate hosts for these parasites in the Iberian Peninsula, where both parasites are present. Methods: To study the distribution of these snail species, an exhaustive literature review and revision of museum collections was performed. Results: A total of 370 localities were compiled and mapped. B. tentaculata were found throughout the Iberian Peninsula, both in Spain and Portugal, while B. hispanica was found only in the Mediterranean coast of Spain. Conclusion: Knowing the distribution of the Bithynia species found in the Iberian Peninsula is highlighted in terms of public health, as the presence of the snail is related to the presence of the disease and can be used as a tool to face future outbreaks. Background: Metacognitive training (MCT) has demonstrated its efficacy in psychosis. However, the effect of each MCT session has not been studied. The aim of the study was to assess changes in cognitive insight after MCT: (a) between baseline, post-treatment, and follow-up; (b) after each session of the MCT controlled for intellectual quotient (IQ) and educational level. Method: A total of 65 patients with first-episode psychosis were included in the MCT group from nine centers of Spain. Patients were assessed at baseline, post-treatment, and 6 months follow-up, as well as after each session of MCT with the Beck Cognitive Insight Scale (BCIS). The BCIS contains two subscales: self-reflectiveness and self-certainty, and the Composite Index. Statistical analysis was performed using linear mixed models with repeated measures at different time points. Results: Self-certainty decreased significantly (p = 0.03) over time and the effect of IQ was negative and significant (p = 0.02). From session 4 to session 8, all sessions improved cognitive insight by significantly reducing self-certainty and the Composite Index. Conclusions: MCT intervention appears to have beneficial effects on cognitive insight by reducing self-certainty, especially after four sessions.
