IgM-enriched immunoglobulins (e-IgM) may be useful in patients with severe acute bacterial infections. The evidence for the administration of e-IgM is not extensive and a definitive consensus has never been reached on its best use in patients with acute infections as well as in critically ill patients. However, the official indication in several countries, including Italy, is quite wide and mainly refers to supportive treatment of patients with acute severe bacterial infections. A multidisciplinary meeting of Italian Experts in Infectious Diseases, Anesthesia and Critical Care, Pneumology, Microbiology and Oncohaematology aimed to produce a statement on the best practical methodological score that could improve the use of e-IgM in patients with different infections, variable severity of disease and etiology. The Expert Panel reviewed the literature and the available guidelines, discussed the experience and eventually proposed to adapt the PIRO score to the practical methodological needs of a simple tool that could guide the administration of e-IgM.

The most recent conceptualizations of empathy recognize affective empathy as distinct from cognitive empathy. Consequently, instruments that assess these 2 types of empathy have been developed. Among them, the Questionnaire of Cognitive and Affective Empathy (QCAE) is a particularly promising, relatively new, self-report measure consisting of 31 items. To examine the cross-cultural adaptability of the QCAE, we investigated the psychometric properties of an Italian version in 2 samples and with 2 different formats of administration. Study 1 (n = 407) used archival data collected via paper and pencil; Study 2 (n = 285) used newly collected data, obtained with an online format. In these studies, in addition to the QCAE, 6 other instruments measuring empathy-related constructs (i.e., interpersonal competence, well-being, personality traits, emotion regulation, alexithymia, and emotion recognition) were administered, too. Data analysis focused on factor structure, internal consistency, and convergent validity. The findings of both studies provide support for the cross-cultural applicability of the QCAE, and reveal interesting associations between empathy and the other constructs under examination.

The conservation of historic libraries can be referred towards both the ancient book collections and the buildings themselves. Heritage collections made of paper are threaten by climate-induced deterioration risks such as cellulose hydrolysis. Several studies have investigated the microclimate inside historic libraries but comparisons are difficult due to the lack of long-term microclimate observations and uniformity in the use of standards and risk assessment methods. For the first time, the long-term microclimate observations collected in three historic libraries in Italy were comparatively studied to outline differences and similarities of their microclimates in terms of paper preservation. A multidisciplinary approach was applied to assess the building performance (a) and the deterioration risks for the collections (b). As for a), a common feature of the libraries was the high thermal inertia and low indoor-outdoor air exchanges. As for b), the Time Weighted Expected Lifetime (TWEL) was defined to account for an average chemical risk on a seasonal and yearly basis. TWEL allowed to highlight the impact of the most adverse conditions on the overall chemical risk for acidic paper preservation (e.g., temperatures above 20 °C reached naturally in summer/artificially in winter). It resulted that the measured microclimate conditions in the libraries would lead to the loss of their acidic collections in less than 300 years. Demographic plots were finally used to inform about the risk resulting from the synergy between handling and microclimate as well as to explore the effectiveness of possible preservation measures such as the deacidification of 10% of the collections. Cardiac rehabilitation (CR) is the subspecialty of clinical cardiology dedicated to the treatment of cardiac patients, early and in the long term after an acute event. The aim of CR is to improve both quality of life and prognosis through prognostic stratification, clinical stabilization and optimization of therapy (pharmacological and non), management of comorbidities, treatment of disability, as well as through the provision and reinforcement of secondary prevention interventions and maintenaince of adherence to treatment. The mission of CR has changed over time. Once centered on the acute phase, aimed primarily at short-term survival, the healthcare of cardiac patients now increasingly involves the chronic phase where the challenge is to guarantee continuity and quality of care in the medium and long-term. The aim of the present position paper is to provide the state-of-the-art of CR in Italy, discussing its trengths and weaknesses as well as future perspectives.

Purpose: To develop, on behalf of Associazione Italiana di Oncologia Medica and Società Italiana di Malattie Infettive e Tropicali, evidence-based and practical recommendations for the management of cancer patients who are Hepatitis C virus (HCV)-positive and are undergoing antitumor treatment., Methods: Recommendations were generated by panel of experts selected by the boards of the Societies Associazione Italiana di Oncologia Medica and Società Italiana di Malattie Infettive e Tropicali (4 oncologists and 6 infectious disease and hepatology specialist). The level of evidence and grade or recommendation was assessed according to the Grading of Recommendations Assessment, Development and Evaluation for practice guidelines [5]: A (high), B (moderate), and C (low), together with 2 recommendation levels: 1 (strong), and 2 (weak). Experts provided additional information, which helped greatly in clarifying some issues in the absence of clear-cut information from the literature. The final draft was then submitted to the evaluation of experts and the text modified according to their suggestion and comments., Results: HCV screening rates are low in patients with malignancies. The risk of reactivation or exacerbation of hepatitis C is higher in patients receiving immunosuppressive agents. It may be difficult to discriminate naturally occurring cancer-related complications from true reactivation or exacerbation of hepatitis C and hepatotoxicity due to cancer treatment. No conclusive data are available concerning the appropriate monitoring of liver function and when an antiviral regimen should be proposed., Conclusions: Patients at risk of any flare of HCV-related liver disease during active therapy for cancer should be managed with a multidisciplinary approach where all relevant diagnostic techniques and therapeutic resources are available. This paper describes the digital acquisition and restoration of a Super 8 documentary on the origins of citizens' awareness on paper recycling in the city of Brescia, Lombardy, northern Italy. This film is currently stored in Fondazione ASM archives, and its acquisition is part of a larger project of preservation of industrial memories in Italy. Here we describe the tools used for the acquisition and the post-processing with a Spatial Colour Algorithm (SCA) for unsupervised colour and contrast enhancement. This method provides a semi-automatic image restoration, whose quality has been tested through different evaluations. In this paper, some advantages of the use of SCAs in the restoration pipeline are presented, together with a preliminary discussion on the problem of assessing frame restoration results. To this aim, we present objective vs. subjective evaluation, and some issues associated with this complex task. In order to make our cities more sustainable, the paper describes a research aimed at defining a new urban tool in the context of Agenda 2030. It is the 'Sustainability Charter', a tool that already exists at the international level, but here is analyses at the local level as a result of decision-making process which refers to a set of interactions between academia (the university), industry and local community. One of the priority of the Sustainability Charter is to analyse all the sustainable services that an administration has to implement in order to help citizens to have virtuous lifestyles. The authors developed a methodological approach to better define, implement and asses these sustainable actions related to different urban topics (mobility, waste m paper proposes to adopt different indicators to measure the sustainability of each action using a specific chart defined as sustainability map. The method proposed by the authors ai ms to be easily adaptable and scalable to different local administrations. To give a concrete example of the results that can be obtained from its application, the paper proposes the experience of the Sustainability Charter of Sestri Levante, a medium-sized municipality in Italy in the Ligurian Riviera. Although direct oral anticoagulants (DOAC) do not require dose-adjustment on the basis of laboratory test results, the measurement of their anticoagulant effect is useful in special situations. This position paper issued by the Italian Scientific Societies that are mainly involved in the management of patients on DOAC is aimed at providing guidance to care-givers on which tests should be used and the situations in which testing is useful. The guidance is based on the data from the literature so far available and/or on consensus among experts.

Transcatheter aortic valve implantation (TAVI) has revolutionized the management of patients with symptomatic severe aortic stenosis and has become the standard of care for inoperable patients and the preferred therapy for those at increased surgical risk with peculiar clinical and anatomic features. Technology advances, growing experience and accumulating data prompted the update of the 2011 Italian Society of Interventional Cardiology (SICI-GISE) position paper on institutional and operator requirements to perform TAVI. The main objective of this document is to provide a guidance to assess the potential of institutions and operators to initiate and maintain an efficient TAVI program.

The introduction of proton pump inhibitors (PPIs) into clinical practice about thirty years ago has greatly improved our therapeutic approach to acid-related diseases for their well-recognized efficacy and safety. Despite the well-defined indications, however, the use of PPIs continues to grow every year in both western and eastern countries and this phenomenon poses serious queries that include the onset of potential adverse effects and the increase in health care costs. The major reason explaining this worrying market expansion is the inappropriate use of PPIs. The Mediterranean Diet Foundation, in collaboration with the International Menarini Foundation, organized the "International Conference on Mediterranean Diet and Health: A Lifelong Approach." The Conference was held in Ostuni (Puglia, Italy) from March 30 to April 1, 2017. The event received the endorsement of the American Federation for Aging Research, the Research Consortium "Luigi Amaducci," the European Nutrition for Health Alliance, the European Union Geriatric Medicine Society, the Clinical Section of the International Association of Gerontology and Geriatrics-European Region, the National Research Council Research Project on Aging, the Italian Society of Gerontology and Geriatrics, and the Italian Society of Clinical Nutrition and Metabolism. During the conference, results were presented from major studies on dietary interventions aiming to assess the efficacy of the Mediterranean diet in the prevention of chronic diseases and the potential underlying mechanisms. Background: Interest in vegetarian diets is growing in Italy and elsewhere, as government agencies and health/nutrition organizations are emphasizing that regular consumption of plant foods may provide health benefits and help prevent certain diseases., Methods and Results: We conducted a Pubmed search, up to September, 2015, for studies on key nutrients (proteins, vitamin B12, iron, zinc, calcium, vitamin D, and n-3 fatty acids) in vegetarian diets. From 295 eligible publications the following emerged: Vegetarians should be encouraged to supplement their diets with a reliable source of vitamin B12 (vitamin-fortified foods or supplements). Since the plant protein digestibility is lower than that of animal proteins it may be appropriate for vegetarians to consume more proteins than recommended for the general population. Vegetarians should also be encouraged to habitually consume good sources of calcium, iron and zinc - particularly vegetables that are low in oxalate and phytate (e.g. Brassicaceae), nuts and seeds, and calcium-rich mineral water. Calcium, iron, and zinc bioavailability can be improved by soaking, germination, and sour-dough leavening that lower the phytate content of pulses and cereals. Vegetarians can ensure good n-3 fatty acid status by habitually consuming good sources of a-linolenic acid (walnuts, flaxseeds, chia seeds, and their oils) and limiting linoleic acid intake (corn and sunflower oils)., Conclusions: Well-planned vegetarian diets that include a wide variety of plant foods, and a reliable source of vitamin B12, provide adequate nutrient intake. Government agencies and health/nutrition organizations should provide more educational resources to help Italians consume nutritionally adequate vegetarian diets., (Copyright © 2017. The aim of the study was to perform dimensional, morpho-structural, mechanical and chemical characterization of four materials based on paper currently in use in Romania (RO), Italy (IT), Spain (SP) and Slovenia (SLO) designed for the temporarily wrapping of meat. SEM-EDXS, FTIR and DSC showed that RO is laminated on one side with polyethylene, SP is waxed, IT is modified with a multilayered polyethylene film and SLO is coated with hydrophobized starch. RO, IT and SLO comply with the standards of determination of density, roughness, breaking length and bending resistance. The highest content of metals found in the SP explains the highest electrical conductivity and the poor quality of the recycled material used for manufacturing. The highest values of total dissolved solids (TDS) and chemical oxygen demand (COD) resulted for SP sample expressed its water solubility and availability of migration into foodstuff. RO, IT and SLO are suitable for food packaging. In contrast, SP do not meet the requirements for safety and mechanical resistance. The publication of updated works on the distribution, breeding and conservation status of Italian birds has stimulated an analysis of the factors that have so far guided the research. This was done through a bibliometric analysis of one of the largest scientific databases on the web. Two publication metrics were used, the total number of papers and the h-index. They express the quantity and the quality of research efforts through their impact on the scientific community. 791 articles concerning the 270 species reported in the Italian Atlas of Breeding Birds were selected and analysed by univariate statistics and negative binomial GLMs. Eight multilevel factors (origin of species, breeding phenology, main occupied habitat, population trends, degree of threat, national interest relative to population management, functional grouping and geographic range size) were used as potential predictors of species publication metrics. These 791 papers attracted 20,982 citations and had an overall h-index of 48. The publication years ranged from 1975 to 2023 with a significant increase in slope through time. The Barn Swallow leads the top ten of both publication metrics followed by the Lesser Kestrel and the Golden Eagle in the case of number of papers, while the Red-backed Shrike, and again the Lesser Kestrel follow the Barn Swallow in the first places of the h-index top ten. Main habitat, functional grouping and geographic range size are modelled as significant factors predicting a change in publication metrics, instead, the other five factors do not predict a significant change in both response variables. The lack of focus on research on species in numerical decline, threatened, or of national interest for population management reveals a main gap in Italian ornithological research. Another one is the skewed distribution of studies, with a not negligible 17% of breeding species that have never been the subject of a paper. These weaknesses are likely due to the low presence of ornithologists in local/national environmental and wildlife management bodies and to the uneven distribution of research groups among the Italian regions. Increasing the number of professional ornithologists and including them in local authorities and regional administrations is the best strategy to grow the levels of research and protection of Italian birds. Purpose: This study aims, first of all, to analyze the extent to which Italian higher education institutions declare their mission statements in their official documents; then, to examine their content; and finally, to investigate whether mission statements include considerations on sustainability dimensions. Design/methodology/approach: After a theoretical background on the new strategic approach adopted by higher education institutions and the related disclosure of their mission statements, the study deepens their sustainable perspective presenting the concepts of social engagement and knowledge transfer from a literary point of view. Then, a documentary analysis on the content of Italian universities' official mission declarations will be conducted, to understand the role they play in society, by individuating their actual interest in sustainable dimensions. Findings: Findings suggest that 36% of the 98 Italian universities state their engagement in societal issues, but only 3 of them mention sustainability targets in their mission declarations. Research limitations/implications: The study is limited, exclusively based on what higher education institutions share and, in terms of future perspective, the aim can be to investigate further official documents, especially for what concerns sustainability reports. Practical implications: Practical implications regard the denounce of weak attention paid by Italian universities to sustainability, with the aim to foster them to increase connections with the external world, aware of the centrality of their role within society. Originality/value: In a context that is not yet completely investigated, the originality of the paper regards the intent to study the sustainable propensity that characterizes the Italian higher education system.

The study presents a global research scenario in the domain of quantum sensing in quantitative and qualitative terms. The study is based on an analysis of 588 global publications in the field, sourced from the Scopus database for the period 1991-2020. The study identified key countries, organizations and authors, network collaborative linkages at national, institutional, and author level. In addition, it identified broad subject areas intersecting quantum sensing research, key journals for research communications, and broad characteristics of highly-cited papers. The study finds that the USA and Germany lead the world ranking in quantum sensing research with a combined share of 50% to the global output. The USA, Germany, and Italy are the home countries to 13 of the top 15 most productive organizations, and also the home countries to 14 of the top 15 most productive authors in the subject. The top journals publishing most research publications are Physical Review A, Physical Review Letters and New Journal of Physics. Printed books would be about to die in front of the advantages offered by the digital world. But for 750 million illiterates over 15 worldwide, for two-thirds women (UNESCO, 2017), literature does not go through digital or printing on paper. There is also a disturbing degree of returning or functional illiteracy, significantly affecting a part of European population. Starting from an idea of literature that is above all the art of the word, the article analyzes the (in)utility of digital media compared to the emotional impact and prestige possessed by printed books; the relationship between e-books and printed books in the self-publishing sector; the importance of the human voice as an ancient and very modern means of telling stories and conveying emotions. Recent statistical data (e.g. Pepe Research, 2020) show the success of audiobooks and podcasts in Covid times, in Italy and in the "Westernworld", and for some types of disabilities. The article underlines the need to recompose the competition between digital and printed books into a fruitful collaboration. Finally, it is highlighted that the voice is, still today and everywhere - from the hi-tech world to African storytellers -an extraordinary tool to produce and promote knowledge about literature. Telemedicine is the provision of health care services, through the use of information and communication technology, in situations where the health care professional and the patient, or 2 health care professionals, are not in the same location. It involves the secure transmission of medical data and information, through text, sound, images, or other forms needed for the prevention, diagnosis, treatment, and follow-up of a patient. First data on implementation of telemedicine for the diagnosis and treatment of acute myocardial infarction date from more than 10 years ago. Telemedicine has a potential broad application to the cardiovascular disease continuum and in many branches of cardiology, at least including heart failure, ischemic heart disease and arrhythmias. Telemedicine might have an important role as part of a strategy for the delivery of effective health care for patients with cardiovascular disease. With the ageing of the population in the Western world, an increasing proportion of patients seen in cardiology practice is represented by the elderly. Although approximately one third of patients admitted with acute coronary syndrome (ACS) are >75 years old and the mortality rate in this age group is doubled compared with younger patients, this population is underrepresented in randomized controlled trials and, consequently, clinical guidelines do not always provide clear indications for the management of elderly patients. Therefore, there is an unmet need for clinical guidance regarding this rapidly growing subset of ACS patients, also considering that decisions about optimal antithrombotic treatment strategies in the elderly are often challenging, mostly due to age-related organ dysfunction, the frequency of comorbidities and concomitant medications and an increased risk of both ischemic and bleeding events. A panel of Italian cardiology experts assembled under the auspices of the Italian Society of Interventional Cardiology (SICI-GISE) for comprehensive discussion and consensus development, with the aim to provide practical recommendations, for both clinical and interventional cardiologists, regarding optimal management of antithrombotic therapy in patients with ACS aged ≥75 years. In this position paper, various clinical scenarios in patients with ST-elevation and non-ST-elevation myocardial infarction or unstable angina are presented and discussed, including special subsets (e.g., patients aged ≥85 years, patients with chronic renal disease or previous cerebrovascular events, patients requiring triple therapy or long-term antithrombotic therapy), with the panel's recommendations being provided for each scenario.

Many unsolved practical issues, from technical and scientific to ethical, legal and economic topics, are slowing down the translation of Personalized Medicine principles into medical practice. The Italian Society of Personalized Medicine exposes here its point of view, based on the real-world practice of precision medicine carried-out in Italian healthcare structures.

Introduction: Magnesium exerts analgaesic effects in several animal pain models, as well as in patients affected by acute postoperative pain and neuropathic chronic pain. There is no evidence that magnesium can modulate pain in patients with peripheral arterial occlusive disease (PAOD). We describe the protocol of a single-centre randomised double-blind clinical trial aimed at assessing the efficacy of oral magnesium supplementation in controlling severe pain in patients with advanced PAOD., Methods and Analysis: Adult patients affected by PAOD at stages III and IV of Lèriche-Fontaine classification, who are opioid-naïve, and who have been admitted to our Acute Pain Service for intractable pain, will be eligible. Patients will be randomised to the control group, treated with standard therapy (oxycodone and pregabalin) plus placebo for 2 weeks, or to the experimental group (standard therapy plus magnesium oxide). Patients will be evaluated on days 0, 2, 4, 6, 8, 12 and 14; the following information will being collected: daily oxycodone dose; average and maximum pain (Numerical Rating Scale); pain relief (Pain Relief Scale); characteristics of the pain (Neuropathic Pain Scale); impact of pain on the patient's daily activities (Brief Pain Inventory). The primary outcome will be oxycodone dosage needed to achieve satisfactory analgaesia on day 14. Secondary outcomes will be pain relief on day 2, time needed to achieve satisfactory analgaesia and time needed to achieve a pain reduction of 50%. Available data suggest a steep increase in stable coronary artery disease with age. Its prevalence reaches a peak of almost 12-14% in men aged 65-84 years with an annual mortality ranging from 1.2% to 2.4%. The diagnosis of stable angina is primarily based on history and therefore relies on clinical judgment. In addition, its diagnosis can be extremely challenging because of the frequent transition from unstable to stable angina. Current European guidelines on the management of stable coronary artery disease give increased importance to the pre-test probability, which strongly affects the diagnostic algorithms. Imaging techniques play a greater role in the diagnosis of stable angina than in the past. Conversely, despite recent advances in technology and in the physiological assessment of coronary stenosis, an ever decreasing relevance is conferred to coronary angiography. Another difficult and controversial issue relates to the prognostic benefit of myocardial revascularization. The aim of this position paper is to review the most relevant clinical aspects of the European guidelines on the management of stable coronary artery disease.

In the last few years, the activity of cath labs has undergone some notable changes, at present largely focusing on diagnosis and invasive therapy of a broad spectrum of cardiovascular diseases. Technological and pharmacological advances have allowed for procedures to be performed in patients who are increasingly complex, and cath labs have become the preferred venue for endovascular treatment of coronary artery disease, in particular acute coronary syndrome, as well as the treatment of structural heart disease and peripheral vascular disease. This position paper is an update of the 1996 and 2008 versions, given the present level of experience and the situation in Italy. It aims to provide the quality standards required to maintain adequate conditions of know-how and safety, as well as the structural and organizational requirements that are fundamental to obtain the best possible use of human and technological resources. Position papers should be a stimulus and guide for operators in the field as well as for those who govern health policies. This should allow for an improved and more rational allocation of cath labs in Italy, based on the real need for procedures and an optimal distribution and organization of the cardiovascular emergency networks while respecting the minimum standards of care.

A series of measurements of particulate samples collected on paper filters in Palermo (Italy) during 1995–2005 has been performed to detect 210 Pb air activity concentration and, along with previous similar data on 7 Be, to evaluate their activity ratio. Average values for the daily activity concentrations of 7 Be and 210 Pb are found to be 4.7 and 0.6 mBq m − 3 , respectively, 9.8 being the average activity ratio. Data confirm a seasonal behavior for both radionuclides. A correlation between 7 Be and 210 Pb air activity concentrations seems to exist, despite their different origins. The time behaviors of both 7 Be and 210 Pb air concentrations and 7 Be/ 210 Pb activity ratio have been compared with some meteorological parameter time series. The analysis of the data shows a weak correlation with rainfall, while no correlation can be established with other parameters. Monthly and weekly average values have been reconstructed taking into account missing data, in order to carry out homogeneous comparisons with available data from other geographical areas. Data series correlations and the frequency analysis show the importance of atmospheric transport processes and the influence of meteorological and climatic parameters (in particular rainfall) in determining the concentrations in air of both radionuclides. This paper discusses craft and design practices through their impact on the environment. We consider how to act concerning the consequences of the craft and design industry. Also, we reflect on the agency of our field of practice in changing how we perceive the environment. We present three case studies of the European glass industry sites in Sweden, Italy and Finland, where we study contamination of the soil with participatory, speculative and craft methods. Through these cases, we reflect on our role in soil communities and ask how we may act in them with responsibility, hope and care. We conclude by proposing to act locally, to share our practices and make them visible, expanding our situated, personal skills and knowledge towards the political. [ABSTRACT FROM AUTHOR]

Worldwide, tropospheric ozone (O3) is a potential threat to wood production, but our understanding of O3 economic impacts on forests is still limited. To overcome this issue, we developed an approach for integrating O3 risk modelling and economic estimates, by using the Italian forests as a case study. Results suggested a significant impact of O3 expressed in terms of stomatal flux with an hourly threshold of uptake (Y = 1 nmol O3 m−2 leaf area s−1 to represent the detoxification capacity of trees), i.e. POD1. In 2005, the annual POD1 averaged over Italy was 20.4 mmol m−2 and the consequent potential damage ranged from 790.90 M€ to 2.85 B€ of capital value (i.e. 255–869 € ha−1, on average) depending on the interest rate. The annual damage ranged from 31.6 to 57.1 M€ (i.e. 10–17 € ha−1 per year, on average). There was also a 1.1% reduction in the profitable forest areas, i.e. with a positive Forest Expectation Value (FEV), with significant declines of the annual national wood production of firewood (− 7.5%), timber pole (− 7.4%), roundwood (− 5.0%) and paper mill (− 4.8%). Results were significantly different in the different Italian regions. We recommend our combined approach for further studies under different economic and phytoclimatic conditions. Brentuximab vedotin (BV) is approved for the treatment of patients with relapsed or refractory CD30-positive Hodgkin lymphoma, and relapsed or refractory systemic anaplastic large-cell lymphoma. Several uncertainties remain regarding the optimal use of the drug in its approved indications as well as outside them. This article reports recommendations on the use of BV issued during a consensus project, sponsored by the Italian Society of Hematology (SIE) and its affiliate societies, Società Italiana di Ematologia Sperimentale (SIES) and Gruppo Italiano Trapianto di Midollo Osseo (GITMO). Scientific evidence on BV was evaluated by a panel of experts, and consensus was developed by group discussion for key questions selected according to the clinical relevance. The following key issues were addressed: testing CD30 positivity to assess eligibility to BV; assessing practice indications of BV in Hodgkin lymphoma and systemic anaplastic large-cell lymphoma; providing pretreatment evaluation of patients candidates to BV; monitoring the response to BV; managing patients treated with BV; and assessing the role of BV in other CD30-positive lymphomas., (Copyright © 2015 Elsevier Inc. Ischemic heart disease remains the leading cause of death worldwide. Ischemic pre-, post-, and remote conditionings trigger endogenous cardioprotection that renders the heart resistant to ischemic-reperfusion injury (IRI). Mimicking endogenous cardioprotection by modulating genes involved in cardioprotective signal transduction provides an opportunity to reproduce endogenous cardioprotection with better possibilities of translation into the clinical setting. Genes and signaling pathways by which conditioning maneuvers exert their effects on the heart are partially understood. This is due to the targeted approach that allowed identifying one or a few genes associated with IRI and cardioprotection. Genes critical for signaling pathways in cardioprotection include protectomiRs (e.g., microRNA 125b*), ZAC1 transcription factor, pro-inflammatory genes such as cycloxygenase (COX)-2 and inducible nitric oxide synthase (iNOS), antioxidant enzymes such as hemoxygenase (HO)-1, extracellular and manganese superoxidase dismutases (ec-SOD and Mg-SOD), heat shock proteins (HSPs), growth factors such as insulin like growth factor (IGF)-1 and hepatocyte growth factor (HGF), antiapoptotic proteins such as Bcl-2 and Bcl-xL, pro-apoptotic proteins such as FasL, Bcl-2, Bax, caspase-3 and p53, and proangiogenic genes such as TGFbeta, sphingosine kinase 1 (SPK1), and PI3K-Akt. By identifying the gene expression profiles of IRI and ischemic conditioning, one may reveal potential gene targets responsible for cardioprotection. In this manuscript, we review the current state of the art of gene therapy in cardioprotection and propose that gene expression analysis facilitates the identification of individual genes associated with cardioprotection. Introduction: Treatment of pediatric malignancies is becoming progressively more complex, implying the adoption of multimodal therapies. A reliable, long-lasting venous access represents one of the critical requirements for the success of those treatments. Recent technical innovations-such as minimally invasive procedures for placement, new devices and novel materials-have rapidly spread for clinical use in adult patients, but are still not consistently used in the pediatric population., Methods: The Supportive Therapy Working Group of Italian Association of Hematology and Oncology (AIEOP) reviewed medical literature focusing on new aspects of central venous access devices (VADs) in pediatric patients affected by oncohematological diseases., Results: Appropriate recommendations for clinical use in these patients have been discussed and formulated., Conclusions: The importance of the correct choice, management and use of VADs in pediatric oncohematological patients is a necessary prerequisite for an adequate standard of care, also considering the increased chances of cure and the longer life expectancy of those patients with modern therapies.

Functional assessment of coronary lesions has become an integral part of routine practice in most cath labs. Such evaluation is performed using a pressure wire that allows measurement of fractional flow reserve (FFR). The latter has received a class I indication with level of evidence A according to the most recent European guidelines on myocardial revascularization for the assessment of angiographically moderate coronary lesions. The present document has the following objectives: 1) to summarize the theoretical basis of FFR; 2) to provide a guideline for vasodilator therapy; 3) to summarize scientific evidence supporting FFR; 4) to provide a model of health economy evaluation focusing on resource sparing associated with the use of FFR.

The recent availability of biosimilars as a result of the expiry of the patents of first-generation biotechnological drugs may theoretically reduce the direct costs of such treatments, making their use accessible to a larger number of patients. However, the currently available clinical data refer to a relatively small number of patients, and do not provide sufficient information concerning long-term efficacy and safety or the frequency of rare adverse events. Given the importance of the introduction of biosimilar drugs and the limitations of our current knowledge of their efficacy and safety profiles, we believe it is mandatory to draw up a position paper for Italian Rheumatologists. Moreover, in order to guarantee their safety, it is mandatory to indicate behavioural rules for the involved specialists and competent authorities, and perform ad hoc clinical trials and appropriate drug surveillance.

Purpose: To develop recommendations for the management of acute hepatitis B by the Italian Society for the Study of Infectious and Tropical Diseases., Methods: Development of the recommendations divided into three levels of evidence according to the GRADE system: A (high), B (medium) and C (low experts opinion), together with three recommendation levels: 1 (strong), 2 (medium), 3 (weak)., Results: The treatment with antivirals is in selected cases the mainstay of management of severe acute hepatitis, and should be started as a matter of urgency in order to prevent death., Conclusions: These recommendations are meant to provide the rationale and practical indications for the management of acute hepatitis B (AHB).

The efficacy data obtained with boceprevir and telaprevir for persons with hepatitis C virus (HCV) genotype 1 infection raise the question of whether HCV protease inhibitors should be used in human immunodeficiency virus (HIV)/HCV co-infected persons. The Italian Association for the Study of Infectious and Tropical Diseases has made these recommendations to provide the rationale and practical indications for the use of triple anti-HCV therapy in persons living with HIV (PLWHIV). A Writing Committee of experts indicated by the President of the Association and a Consulting Committee con- tributed to the document. The final draft was submitted to the evaluation of external experts and the text modified according to their suggestions and comments. Treatment of HCV co-infection should be considered for all HCV RNA positive PLWHIV. Response-guided therapy with pegylated interferon and ribavirin is the standard treatment of PLWHIV with infection by HCV genotype 2, 3, 4, 5 and 6. Boceprevir and telaprevir should be used to treat HCV genotype 1 infection in HIV/HCV co-infected patients for 48 weeks on an individual basis, with close monitoring of their efficacy and tolerability with concur- rent antiretroviral therapy, taking into account potential drug-drug interactions. The decision to treat a patient or to wait for better treatment options, or to discontinue treatment should be made on an individual basis taking into account pre-treatment variables and the on-treatment HCV RNA kinetics.

Traumatic wounds are one of the most common problems leading people to the Emergency Department (ED), accounting for approximately 5,4 % of all the visits, and up to 24 % of all the medical lawsuits. In order to provide a standardized method for wound management in the ED, we have organized a workshop, involving several Italian and European experts. Later, all the discussed statements have been submitted for external validation to a multidisciplinary expert team, based on the so called Delphi method. Eight main statements have been established, each of them comprising different issues, covering the fields of wound classification, infectious risk stratification, tetanus and rabies prophylaxis, wound cleansing, pain management, and suture. Background: The aim of this research was to study the epidemiology, microbiology, prophylaxis, and antibiotic therapy of surgical site infections (SSIs), especially those caused by methicillin-resistant Staphylococcus aureus (MRSA), and identify the risk factors for these infections. In Italy SSIs occur in about 5 % of all surgical procedures. They are predominantly caused by staphylococci, and 30 % of them are diagnosed after discharge. In every surgical specialty there are specific procedures more associated with SSIs. Methods: The authors conducted a systematic review of the literature on SSIs, especially MRSA infections, and used the Delphi method to identify risk factors for these resistant infections. Results: Risk factors associated with MRSA SSIs identified by the Delphi method were: patients from long-term care facilities, recent hospitalization (within the preceding 30 days), Charlson score > 5 points, chronic obstructive pulmonary disease and thoracic surgery, antibiotic therapy with beta-lactams (especially cephalosporins and carbapenem) and/or quinolones in the preceding 30 days, age 75 years or older, current duration of hospitalization >16 days, and surgery with prothesis implantation. Protective factors were adequate antibiotic prophylaxis, laparoscopic surgery and the presence of an active, in-hospital surveillance program for the control of infections. MRSA therapy, especially with agents that enable the patient's rapid discharge from hospital is described. Conclusion: The prevention, identification and treatment of SSIs, especially those caused by MRSA, should be implemented in surgical units in order to improve clinical and economic outcomes. Background. Gender gap is a neologism that identifies the disparity between social and professional conditions experienced by females compared to males. The disparity increases as one ascends the academic hierarchy. In recent years, the debate has expanded, and more options have been planned for the elimination of the current gender gap. Methods. This research was conducted by examining the landscape of the gender gap, particularly in the academic forensic medicine field. Our analysis involved reviewing papers published between 2006 and 2024, identified through electronic database searches (PubMed). The search terms used were: "gender gap" AND "academic" AND "medicine" AND "leadership." In total, we analyzed 85 papers. Additionally, we examined data from forensic medicine residency programs. Conclusions. The representation of women in medicine is well-known. Despite the increasing number of women in leadership positions in medicine, they still lag significantly behind men. These data highlight a situation that could be seen as grounds for an accusation of "academic abuse". In the Italian forensic residency programs, less than 20% are led by women, and among these, not all hold the rank of full professor. Although a certain rebalancing is already underway, the gap is still significant. There are already regulations obliging local authorities to promote gender equality in councils, companies, and institutions under their jurisdiction. It would be desirable to consider minimum quotas for female participation in university competitions. Purpose: This study deals with the main issues concerning the interplay between homeownership and labour market outcomes, namely (1) the relation between homeownership and labour market outcomes, at both the individual level and the aggregate level, and (2) the relation between homeownership and human capital. Design/methodology/approach: This paper is both theoretical and empirical. A search and matching model of the labour market is developed to explain the strong relation between mortgage markets and wages. A regional panel analysis in Italy is used to verify the interplay between homeownership and wages. Findings: Homeownership is not, by itself, a condition for receiving higher wages, but rather higher wages increase the probability to become a homeowner, since they positively affect the probability of acquiring a mortgage from the bank. Eventually, wages cause homeownership, but the reverse may not be true. Research limitations/implications: The paper focuses on the labour market, while the housing market model is restricted to the mortgage market. Practical implications: The positive effect of homeownership on wages is hard to theoretically formalise and is not empirically proven. Before investigating a (potential) bidirectional relationship between homeownership and labour market outcomes, therefore, the related literature should assume a new theoretical link between homeowners and wages. Social implications: The result that "homeownership is not, by itself, a condition for receiving higher wages" has positive implications for human and social development. If homeownership could lead to better labour market outcomes, indeed, socio-economic inequalities would increase in the society, because homeownership would be the starting point of a "lucky" circle that increases the well-being of people who are already wealthy. Originality/value: First, this study clearly explains why the microeconomic result that homeowners are more likely to be employed than tenants is consistent – at the aggregate level – with a negative relation between homeownership and better labour market outcomes. Second, the related literature has largely ignored the social implications of the topic. A potential bidirectional relation between homeownership and (better) labour market outcomes, indeed, could imply an increase in the well-being of people who are already wealthy. This article contextualises the Higher Education in Italy from the outset by describing deeply rooted but accepted practices, such as the long-term dominance of academic groups in decision making--baroni and their "court"--and also the widespread custom of "Raccomandazioni" as the means to lead mechanisms of recruitment among "peers", being these academics or professional staff. However, assuming that forms of professionalism exist in this sector, our aim is to look for evidence of this professionalism; this will be done by looking at the most recent reform and at how this has been implemented within one of the professional communities. Later on, after taking into account the case-study of this group of professionals within the wider community, we explain why this can be regarded as a good practice of professionalism in HE management in the sector: in this case, modernisation and changes, backed by collective attitudes aiming at valorising domains of expertise, have been implemented across parts of the community and have also affected the whole institution. Overall, these collective understandings have facilitated the process of modernisation and its assimilation, since they have arisen from professionalism opportunely contextualised. Findings show how forms of professionalism are more likely to be found in small communities; and also how these, acting as communities of practice, can valorise one's knowledge and be driving forces at the time of any restructuring. Whilst, implications from this study aim to back further research into higher education management in Italy as well as into professional staff.
