For patients, hospital emergency departments (EDs) are unfamiliar, institutional contexts involving high-stakes communication in heightened emotional circumstances. This study examines laughter, as one expression of emotion, in an existing 649,631-word corpus of naturally occurring clinician-patient interactions recorded in five Australian hospitals. A mixed methods approach revealed (1) the spread, frequency and producers of laughter, and (2) the functions of laughter in unfolding interactional contexts. First, a corpus analysis showed that laughter in the ED was most frequently produced by nurses and patients, but relatively infrequently by doctors. Secondly, two case studies comprising all the interactions of two patients for the whole duration of their ED visits were analysed in detail to explore the individuals' contrasting patterns of laughter. The analysis revealed how laughter can be a cue to the affective dynamics of patient-clinician interactions about serious matters, for example, signalling difficult topics and managing anxiety in the ED context. Laughter, and any related humour, can indicate the achievement of mutuality, which is considered a cornerstone of genuine shared decision-making and patient participation in their own care. Therefore, the findings suggest that a sensitive responsiveness to patient-initiated laughter, and any associated humour, may promote patient-centred relationships in clinical interactions. Previous research has indicated that healthcare support for sexuality after spinal cord injury appears to be neglected or inadequately provided. The perspectives of people living with spinal cord injuries regarding what is lacking and what support they would like to be provided, and why, has yet to be explored. This paper uses critical theory to explore how societal constructs of sexuality and disability shape how sexuality is supported after a spinal cord injury. To do this, we conducted in-depth semi-structured interviews with 11 people with a spinal cord injury from Australia, the United Kingdom, and the United States. We drew from the works of various critical disability theorists to analyze what discourses underpinned the experiences people with spinal cord injuries had when receiving sexuality support. Analysis suggested that the following discourses were apparent: (1) broken bodies require repair, (2) sex is penetration, ejaculation then reproduction, and (3) sexual autonomy is required. Findings indicated healthcare professionals need to recognize people with spinal cord injuries as sexual beings with a diverse range of desires, lives, relations, and bodies. Provision of individualized comprehensive high-quality sexuality support is therefore warranted. Researchers have found that many post-secondary music students suffer from physical and mental health issues. However, researchers have mainly studied these problems at the individual level, with little investigation of how music students' work is shaped by the coordinating effects of policies, texts, and discourses at and beyond their local site. This paper describes a protocol for an international project that will explore the health of music students in the context of their daily work. Using an institutional ethnography approach, we will examine the social organization of music students' work at three universities: two in Canada and one in Australia. This will be the first set of studies that use institutional ethnography specifically for the purpose of understanding how the social organization of music students' work shapes their health. Data will be collected using several methods common to institutional ethnography: interviews, focus groups, observations, and collecting texts. Data analysis will begin during the data collection process and proceed in two fluid stages. The first stage will involve a detailed investigation of the pertinent work activities at each music school. The second will involve linking that work to specific social relations within and beyond the institution. After data analysis has been completed at all three sites, findings will be compared to one another to identify commonalities and differences in how students' work is organized. Findings of the entire project may inform policy-making and lead to positive change at the institutions studied, as well as others where similar social organization may occur. The novel approach described here will provide opportunities to expand current knowledge about music students' work and health beyond what has been learned through approaches that focus on students' individual behaviours and attributes. In Australia, the marriage-equality plebiscite of 2017 prompted significant debate over appropriate limits to free speech in the context of expressions of religious belief. This article reports on the results of a political discourse analysis of media coverage in prominent national newspapers from 2017 to 2021. Findings show the debate was dominated by three representations: discrimination against Christians, disagreement as rights abrogation, and the meaning of 'free speech' as unbridled and unlimited. These representations were deployed in an (ultimately unsuccessful) endeavour to override long-held views about the appropriate place of freedom of speech in Australian political culture and law, and to misrepresent the meaning and purpose of vilification laws. Objective Physical therapy is generally considered to be a white profession in Western nations. Australia's increasingly diverse population, driven largely by growth in immigration, invites Australian health systems to be responsive to factors that may impact on non-white Australian population's access to health care. Here, the authors sought to explore non-Indigenous Black people's perspectives of physical therapy in Australia. Methods Eligible participants (n  = 12) took part in semistructured, in-depth interviews and were prompted to discuss their experiences and perceptions of physical therapy. Interview data were analyzed using critical discourse analysis, underpinned by critical race theory. Results Participants were on average 29.4 years (SD = 12.9) from diverse cultural and linguistic backgrounds. Participants described mostly positive experiences with physical therapists, but they noted that it was not widely accessed by their communities, with 2 key discourses underpinning these discussions. Discourse 1, "physiotherapy is a solution for white people," established physical therapy as primarily accessed by and welcoming of white people. "Whiteness" and the perpetuation of Western norms in physical therapy resulted in little consideration of other cultural practices. Participants' discussions also pointed to the intersections of systemic racism and social inequities such as low income and language barriers, impacting Black people's engagement with physical therapy. Discourse 2, "physiotherapists are white," was mostly concerned with how the perceived "whiteness" of professionals and lack of Black physical therapists impacts cultural safety and comfort when accessing physical therapy. Conclusion Our results suggest that Westernization and whiteness persist among discourses surrounding physical therapy. Such discourses are likely to inform Black people's perceptions and experiences of physical therapy. Our analyses suggest avenues to enhance the cultural diversity of the profession and improve physical therapy accessibility for Black people in Australia by providing culturally appropriate material, including critical reflexivity, epistemic and cultural humility in the curricula, recruiting staff from diverse backgrounds, and providing outreach services to underserved populations. Purpose: Despite a significant evidence base illustrating the issue of housing insecurity and homelessness experienced by women over the age of 55 in Australia [Pawson et al., 2018; Australian Institute of Health and Welfare (AIHW), 2021b; Mission Australia 2022; Per Capita 2022], there continues to be a knowledge gap in the representation of older women in current Federal and New South Wales (NSW) State Government housing and homelessness policies and initiatives. This paper aims to identify the extent and ways in which older women were represented (or not represented) in the Federal and NSW State housing and homelessness policies in 2022. Design/methodology/approach: Of the total primary and supplementary Federal and NSW State policy and strategy documents, 16 were collected through a systematic review and then analysed using a Critical Discourse Analysis (CDA) framework and feminist lens to explore quotes, phrases, keywords and language choices which suggested prevalent themes, rhetorical devices and dominant discourses. Findings: Three significant themes were identified by the analysis (1) "relinquished responsibility", (2) "inconsistent rhetoric" and (3) "homogenising and ideological cherry picking". These themes presented the authors with three main discussion points to inform policy reform which we have addressed in relation to policy implications, evidence-based policy decision-making and impacts on older women. Objectives: Justice and equity-focused practices in health services play a critical but overlooked role in low back pain (LBP) care. Critical reflexivity – the ability to examine and challenge power relations, and broader social issues embedded in everyday life – can be a useful tool to foster practices that are more socially just. No research has yet explored this approach in back pain health services. This study sought to understand how clinicians construct LBP in relation to broader socio-cultural-political aspects of care and explore if those constructions changed when clinicians engaged with critically reflexive dialogues with researchers. Methods: Using critical discourse analysis methods, this qualitative study explored institutionalised patterns of knowledge in the construction of LBP care. We conducted 22 critically reflexive dialogues with 29 clinicians from two health services in Australia – a private physiotherapy clinic and a public multidisciplinary pain clinic. Results: Our analyses suggested that clinicians and services often constructed LBP care at an individual level. This dominant individualistic discourse constrained consideration of justice-oriented practices in the care of people with LBP. Through dialogues, discursive constructions of LBP care expanded to incorporate systems and health service workplace practices. This expansion fostered more equitable clinical and service practices – such as assisting patients to navigate health care systems, considering patients' socioeconomic circumstances when developing treatment plans, encouraging staff discussion of possible systemic changes to enhance justice, and fostering a more inclusive workplace culture. Although such expansions faced challenges, incorporating broader discourses enabled recommendations to address LBP care inequities. Conclusions: Critical reflexivity can be a tool to foster greater social justice within health services. By expanding constructions of LBP care beyond individuals, critical reflexive dialogues can foster discussion and actions towards more equitable workplace cultures, services and systems. In recent years, the concept of 'free speech' appears to have reached a position of significant prominence in public debate. In this context, we ask how the concept of free speech was used discursively by the Morrison-led government. Utilising a political discourse analytic approach, and examining texts from 2019 to 2021, we argue that, and show how, the government engaged in multiple discourses around free speech that were incompatible, inconsistent and incoherent. These inconsistencies and discontinuities were so significant as to render this a 'weaponisation' of free speech discourse. Thus, the article both reveals the complexities of the Morrison government's discursive positioning of free speech, and renders sensible the otherwise nonsensical way in which 'free speech' was used to garner support for a range of public policies. Increasingly, competing discourses shape tensions between the role of the contemporary university and the global markets in which universities must exist. This paper draws on the examination of interviews with nine education academics in Australia to illuminate the construction of ‘global’ in the production of the global graduate (GG). Discourse analysis is used to explore how, against the backdrop of COVID 19, participants construct different identities variously related to current and future orientations for the GG. This paper uses two big ‘D’ discourses – efficiency as centralised imperative and boundless productivity – to explore knowledge production and accountabilities, neoliberalism, internationalisation and the construction of marketised universities operating in global knowledge economies. We conclude, the GG is an elusive notion, which draws mobile and multiple positionings to reveal unsettled and often ambiguous constructions of ‘university’ and ‘teacher’, with related tensions for the role and identity of education academics. Negative, ableist discourse challenging the rights of people with Down syndrome to receive support services and even to be born regularly arise on social media. This disability community has retaliated against such hate speech with resistance narratives that promote the value of people with Down syndrome, with few other recourses available. This article explores online free speech versus ableism in Australia through a major case study of 60 Minutes Australia Facebook discussions. Analysis identifies types of ableism expressed, types of harm caused and dimensions of grassroots responses. Using Timothy Garton Ash's 2016 work Free Speech as a basis for contemporary analysis, we discuss: to what extent should harmful and offensive comments directed at vulnerable populations be tolerated in the name of free speech; might negative, ableist discourse contribute to any social good; and what mechanisms beyond grassroots resistance narratives might effectively respond to harmful, online ableist discourses. The article looks at whether the right to free speech is more important than the harmful effects of online hate speech towards people with Down syndrome. We use the ideas in a book on Free Speech by Timothy Garton Ash to think about whether online hate speech should be allowed or silenced. Our analysis is based on a case study of Facebook discussion threads from a 60 Minutes Australia television segment. People in the disability community speaking up to create resistance narratives is the main way that online disability hate speech is challenged. Low back pain (LBP) directives provide information about how LBP should be managed, communicated and navigated in complex health systems, making them an important form of policy. This study aimed to examine how LBP is problematised (represented) in Australian directives. We employed an analysis of discourse of LBP directives drawing on Bacchi's "What's the problem represented to be?" policy problematisation approach. Our analysis suggests that LBP is problematised as a symptom that tends to improve when individuals take responsibility for themselves, but may require care at times. The way in which LBP is represented in the directives excludes important aspects, such as the uncertainties of scientific knowledge, paradigms other than (post)positivist, multimorbidity, social and structural determinants of health. LBP directives may benefit from problematisations of LBP that consider the ongoing nature of LBP and broader contextual factors that impact on both LBP outcomes and care, beyond individual responsibility. Consideration of a wider range of paradigms and expanded evidence base may also be beneficial, as these are likely to enable individuals, clinicians and the Australian healthcare system to address LBP while dealing with its complexities, enabling real-world changes to lessen the LBP burden. Healthcare professionals who work with people who experience low back pain (LBP) may benefit from critically reflecting about discourses embedded in policy directives. Healthcare professionals may consider engaging in policy changes processes to expand the discourses on which LBP policy directives rely. Healthcare professionals' ability to enact policy recommendations may be enhanced by consideration of the fluctuating nature of LBP, uncertainties, multimorbidity and determinants of health. In the context of this special issue's inquiry into whether it is possible to decolonise Australian international relations, this article investigates the service of Indigenous people in the Australian Defence Force (ADF). The military is a crucial site to investigate the colonial state of Australian international relations not only because it is an institution that performs key international relations practices such as war and diplomacy, but also because it defines and projects the identity of the state both domestically and internationally. In the past two decades, there has been a sustained effort to include Indigenous people in the ADF. An inclusive and multicultural defence purports to represent a post‐colonial state where Indigenous and non‐Indigenous people stand next to one another for the defence of their shared country. However, in Australia, Indigenous people do not enjoy the wealth of the nation equally and remain dispossessed from their land and economically disadvantaged. Using discourse analysis of publicly available materials praising Indigenous military inclusion, this article argues that the inclusion of Indigenous people in the Australian Defence Force risks further entrenching the settler colonial project. This article shares findings from a small, largely qualitative empirical study of elite Australian boys' school alumni's perspectives on feminine gender and gender justice. The article focuses on the purported absence in these men's memories of learning about gender at school and the paradox that the research interviews are full of gender-related memories. Using the metaphor of the '(w)hole' to guide feminist poststructuralist analysis of data, the study considers how these interviews and playful, multiple forms of analysis and representation of data might provide resources for thinking differently about gender in schools and societies of the future. Gay and bisexual men (GBM) report higher rates of sexualised and injecting drug use (IDU) than heterosexual men. Injecting-related stigma is linked to negative health outcomes among people who inject drugs (PWID). This paper describes the ways in which stigmatisation manifests in the narratives of GBM who inject drugs. We conducted in-depth interviews with Australian GBM with IDU histories, exploring drug use, pleasure, risk, and relationality. Data were analysed using discourse analytical approaches. Interviewees (n = 19), aged 24–60 years, narrated their experiences of IDU practice over 2–32 years. Most (n = 18) injected methamphetamine, and used other (non-injected) drugs, in sexual contexts. From participants' narratives, we developed two themes related to stigmatisation of PWID that illustrate the limitations of conventional drug discourses to narrate GBM's experiences. The first theme captures participants' attempts to pre-empt stigmatisation, outlining the layering of stigma faced by GBM who inject drugs. Linguistically, participants transformed injecting stigma by distinguishing their personal practice from that of more discreditable drug users. Practically, they mitigated stigmatisation by keeping discrediting information from others. The second theme illustrates how by complicating the stereotypes of IDU, participants took up prominent discursive practices linking IDU with trauma and pathology. Participants exerted agency by broadening available interpretive repertoires for understanding IDU among GBM, thus creating a counter-discourse. We argue that mainstream discursive practices reverberate through gay communities, perpetuating stigmatisation of PWID and inhibiting care-seeking. More narration of unconventional experiences, beyond insular social groups and critical scholarship, is needed in public discourse to effect destigmatisation. [ABSTRACT FROM AUTHOR]

Drawing on a discourse analysis of emotions in national human research ethics guidelines from Australia, Singapore, the United Kingdom and United States, I argue that such guidelines treat emotions as superfluous, harmful, risky and threats to rational decision-making. Such uncritical appreciation of emotions sees instructions to show 'respect' position non-Western participants as 'the other', sees directives to consider 'emotional welfare' undermine the autonomy of people from ostensibly vulnerable groups, and risks undermining qualitative research's cathartic potential. These findings underpin a call to revise guidelines to position emotions as part of everyday life; and to encourage researchers to adopt embodied, caring and emotionally reflexive approaches to human research, where researchers draw on guidelines and emotions in deciding how to produce ethical research. Global awareness about an increase of chronic diseases and premature mortality due to 'unhealthy eating' and 'sedentary lifestyles' is embedded in various discourses shaped by relationships and power. In this article, I investigate the role of physical activity in the lives of middle-aged women in Australia and how their experiences with exercise influence the way they position themselves within the context of inter-discursivity regarding fitness and 'healthy ageing'. Results reveal how 'knowledge' about 'healthy lifestyles' is created and accessed, and how women make sense of the healthism discourse, the obesity crisis, and discourses around menopause and ageing. The participants for this study are nine women in their forties to sixties who volunteered to participate in semi-structured interviews after completing an online survey about physical activity that was part of a larger project. Their accounts of health and fitness, healthy eating, weight management, mental wellbeing and ageing are categorised and interpreted in a post-structuralist framework through the lens of feminist relational discourse analysis. Results show that all women are influenced by healthism discourses as well as being affected by assumptions and recommendations for ageing, menopausal women. They shape female identity by adopting, but also by resisting, discourses around their bodies and minds. [ABSTRACT FROM AUTHOR]

Background: An epidemic of type 2 diabetes in remote Aboriginal people in Central Australia, contributes to high rates of morbidity and mortality. Remote non-Aboriginal Health Care Workers (HCW) and the Aboriginal people they serve inhabit a complex cultural interface. This study aimed to recognise racial microaggressions in the everyday discourse of HCWs. It proposes a model of interculturality for remote HCWs that avoids racialisation and essentialising of Aboriginal people's identities and cultures. Methods: Semi-structured in-depth interviews were undertaken with HCWs from two Primary Health Care services in very remote Central Australia. Fourteen interviews were analysed from seven Remote Area Nurse, five Remote Medical Practitioners and two Aboriginal Health Practitioners. Discourse analysis was employed to explore racial microaggressions and power relations. NVivo software assisted in the thematic organisation of microaggressions according to a predefined taxonomy. Results: Seven microaggression themes were identified - racial categorization and sameness, assumptions about intelligence and competence, false colour blindness, criminality and dangerousness, reverse racism and hostility, treatment as second-class citizens and pathologizing culture. A model of interculturality for remote HCWs was based on concepts of the third space, deCentred hybrid identities and small culture formation on-the-go combined with a duty-conscious ethic, cultural safety and humility. Conclusions: Racial microaggressions are common in the discourse of remote HCWs. The model of interculturality proposed could improve intercultural communication and relationships between HCWs and Aboriginal people. Background: Australian data has indicated that the frequency and severity of family and domestic violence (FDV) tends to increase with remoteness. Rural communities rely on Emergency Departments (ED) within public hospitals for general health and safety needs. Public health departments within Australia are strongly influenced by Government policies which can define 'health problems' and limit institutional responses to patients presenting with FDV. The current study therefore aimed to critically examine FDV Australian Government policies to explore how policy meanings could potentially impact on ED staff and individuals within rural communities. Methods: Foucauldian Discourse Analysis and Policy Narrative Analysis were used to examine 9 policy documents which represented national, state/territory and clinical practice levels. Publication dates ranged from 2006 to 2020. Results: A total of 8 discourses were identified, with each one providing a unique construction of the target problem and determining the potential agency of health professionals and subjects of FDV. Discourses combined to produce an overall narrative within each policy document. Narrative constructions of the target problem were compared which produced three narrative themes: 1) Deficit Subject Narratives; 2) Object Oriented Narratives; and 3) Societal Narratives. Conclusion: The results reflected a transition in the meaning of FDV within Australian society and over the past decade, with policies trending away from Deficit Subject Narratives and towards Object Oriented or Societal Narratives. Institutional systems, sociohistorical context and broader societal movements may have shaped this transition by stagnating policy meanings or introducing new insights that expanded the possibilities of understanding and action. Narratives produced assumptions which significantly altered the relevance and agency of individuals and groups when applied to a rural ED setting. As FDV was moved out of the clinical space and into the public domain, the agency of health professionals was reduced, while the values and strengths of FDV subjects and rural communities were potentially recognised. Later policies provided contextual specificity and meaning fluidity that could benefit diverse groups within rural areas; however, the expectation for ED staff to learn from their communities and challenge institutionalised approaches to FDV requires careful consideration in relation to rural hospital systems and resources. The Murray–Darling Basin Plan, a major initiative to return water from irrigators to the environment, has been lauded as world-class water reform. The enabling legislation for the Basin Plan, the Water Act, gains its constitutional legitimacy from international treaties such as the Ramsar Convention on Wetlands. This Act mandated that water be returned from consumptive uses to the environment. An allocation of 2,750 GL/yr was set but has been reduced by the 'Sustainable Diversion Limit Adjustment Mechanism' (SDLAM), intended to achieve equivalent environmental benefits with less water. We present a synthesis of changes in decision contexts that have led to water reforms being 'watered down'. We analysed the policy discourse of water reform to assess whether SDLAM projects will achieve outcomes congruent with Australia's international treaty obligations. We found little or no alignment between the purpose of the SDLAM projects and the principles of the treaties and the Water Act. As water scarcity increases under climate change, attempting to conserve wetlands (including rivers) with less water while maintaining or increasing irrigation diversions is likely to prove maladaptive. A major reframing of environmental water policy and management is required to enable meaningful and effective adaptation to climate change. [ABSTRACT FROM AUTHOR]

In this article we analyze images of sportswomen from four media outlets over the course of the 2018 Gold Coast Commonwealth Games in Australia. Through visual discourse analysis we find that despite structural changes to increase gender equality at the Commonwealth Games—which for the first time ensured equal opportunities for men and women to win medals—sportswomen were still depicted in a very narrow way, and intersectional representations were mainly excluded. Though the quantity of images of women had increased, the 'quality' of these images was poor in terms of representing sportswomen in their diversity. We still have far to go if we are to embrace women in their multiplicity—and to recognize that women can be strong, capable, butch, femme, and varied in their range of expressions of gender, sexuality, race, and ethnicity. In the past four years, Australia has undergone a major change in the way it funds supports and services for people living with disability. Part of this change has, of course, occurred in the discourse of disability, and the emergence of the term "support" rather than "care" has been a feature of this discursive shift. In this paper we used aspects of Systemic Functional Linguistic (SFL) theory to analyse how the term "support" is used in a seven key policy and program documents related to disability written over a ten-year period. We aim to understand what is meant by the term "support" and how its meaning is built up, deepened and made more complex over time through its use in Australia's National Disability Insurance Scheme. The findings reveal that the meaning of support is relatively flexible, and as such develops meaning and complexity over time, reflecting a shift from a values and rights-based understanding of the concept to the operationalisation of the term in policy and practice. In Australia and globally, there is a push for accountability systems that focus on impact on student learning as an indicator of teacher quality. Therefore, how the impact is defined in Australian policy documents in teacher education must be made clear before it can be enacted in practice. This study uses Foucauldian archaeology and associated discourse analysis techniques to trace impact through three Australian policy documents. Findings reveal that the evidence base for prioritising impact in the recent lexicon of Australian initial teacher education (ITE) policy is based on specific and shifting ways of thinking around the concept, with only certain voices being heard in the debate. Purpose: The aim of the paper is to create a greater understanding of how people who are blind or vision impaired describe their use of audio-based reading technologies, with a particular focus on how they reason about whether the use of these technologies can be understood in terms of reading. Design/methodology/approach: The study is part of the emerging research area Critical Studies of Reading and draws theoretical inspiration from Document Theory, New Literacy Studies and Critical Disability Studies. The article presents a discourse analysis of how 16 university students in Australia who are blind or vision impaired and use audio-based reading technologies describe this use in semi-structured interviews. Findings: The participants relate to a division between 'real' reading and reading by listening, where the latter is constructed as an exception and is connected to the subject position of being blind or vision impaired. However, resistance is also noticeable, where reading by listening is constructed as something that is normal, and as a right. Originality/value: The article is a theoretical and empirical contribution to the ongoing discussion on the use of audio-based reading technologies. Consensus moderation, where collaboration and discussion take place to reach an agreement on mark allocation, is a frequently used approach to quality assurance in higher education. Unit coordinators play a vital role in facilitating consensus moderation, yet limited research has focused on their role in moderation practices. This study explored unit coordinators' perceptions and experiences of consensus moderation in Australian higher education through five focus groups. Using Foucauldian discourse analysis, data analysis identified three discursive constructions of consensus moderation situated in the wider discourse of the neoliberal university: a truly collaborative process, an illusion, and a process to manage markers. Unit coordinators in this study were positioned as either supportive, compliant but ineffective, or powerful. In contrast, markers were positioned as helpful and compliant although inexperienced and needing support; uncooperative, resistant, troublesome and demanding; or inexperienced and malleable. This paper has identified varied knowledge and understanding of consensus moderation processes and practice. Conditional welfare policies are frequently underpinned by pejorative representations of those they target. Vulnerable children, under physical or moral threat from their welfare-dependent parents, are a mainstay of these constructions, yet the nuances of this trope have received little focused attention. Through a discourse analysis of parliamentary debates at the introduction of compulsory income management (CIM) to Australia, this article explores the complexities of the vulnerable child trope. It shows how the figure of the child was leveraged to justify hard-line welfare reforms in Australia, and offers a deeper and more intersectional understanding of how social and economic marginalisation is reproduced through welfare discourse. Background: Communication about managing medications during transitions of care can be a challenging process for older patients since they often have complex medication regimens. Previous studies highlighted that links between communication breakdowns and medication incidents in older patients occur mainly at discharge or in the post‐discharge period. Little attention has been paid to exploring communication strategies facilitating patient‐centred medication communication at transitions of care from a discourse‐analytic perspective. Objectives: To explore, through a discursive lens, strategies that enable patient‐centred medication communication at transitions of care. Design: A focused ethnographic study was employed for this study. The study was reported according to the COREQ checklist. Methods: Interviews, observations and focus groups were analysed utilising Critical Discourse Analysis and the Medication Communication Model following thematic analysis. Data collection was undertaken in eight wards across two metropolitan hospitals in Australia. Results: Patient preferences and beliefs about medications were identified as important characteristics of patient‐centred communication. Strategies included empathetic talk prioritising patients' medication needs and preferences for medications; informative talk clarifying patients' concerns; and encouraging talk for enhancing shared decision‐making with older patients. Challenges relating to the use of these strategies included patients' hearing, speech or cognitive impairments, language barriers and absence of interpreters or family members during care transitions. Relevance to clinical practice: To enhance medication communication, nurses, doctors and pharmacists should incorporate older patients' preferences, previous experiences and beliefs, and consider the challenges faced by patients across transitions. Strategies encouraging patients' contribution to decision‐making processes are crucial to patient‐centeredness in medication communication. Nurses need to engage in informative talk more frequently when administering the medications to ensure older patients' understanding of medications prescribed or altered in hospital settings. Intersections of human and animal lives regarding human suicide are just beginning to be explored. Given that the investigation of human suicide poses major ethical concerns, seeking out publicly accessible information in the first instance is important. Public media reports of suicide are limited in Australia (as in many countries) by voluntary adherence to recommendations that suicides should only be reported if of substantial public significance. While macro data on suicide in Australia is available, the presence or role of animals in these human scenarios is not reported. However, some detailed coroners' investigations of suicide scenarios are available via state coroner websites. Publicly available reports between 2016 and 2020 from all Australian states and territories were manually searched for mention of pets/animals using a range of keywords in scenarios investigated as suicide. Fewer than 30 reports were identified. A critical discourse analysis was undertaken of the mentions of pets/animals in these reports. This included theming of core qualitative data. The overarching theme identified was the insignificance of pets within coroners' reports of suicide. Four subthemes were identified: pets as beloved; potential aggressors; incidental to reporting; and enmeshed in human domestic violence. Despite the increasing inclusion of pets within definitions of kin, nonhuman family members are virtually invisible in Australian coronial discourses investigating possible suicides. Even when pets were noted by informants as being extremely important in the life of the deceased, there was no evidence of this relationship being seen as part of the legitimate enquiry in terms of causation or future preventative strategies. While current data are limited, given emerging research on the role of pets in human suicidality it is possible that this humancentric bias may be overlooking potentially protective and preventive approaches to suicide. In this paper we consider parents as unofficial evaluators of success in primary bilingual programmes that are not subject to any standardised bilingual outcomes. Small-scale, opt-in programmes that are reliant on parental buy-in are increasing in Australia. Given the importance of parental choice on the sustainability of these programmes, we seek to understand parents' evaluation of success in their implementation. We apply Mehisto's ([2015]. End-of-day debriefs are reported to offer students opportunities to reflect and consolidate learning. However, there is little evidence about how clinical facilitators encourage student participation that leads to refection and learning, particularly in debrief sessions with linguistically diverse students. This research investigated how the pedagogic practices of clinical facilitators enabled or constrained student participation during debrief. This study used an ethnographic approach combined with linguistic analysis of audio recordings of debrief in two metropolitan hospitals in Australia. The study found that several key factors contributed to student participation during debrief. Factors included: establishing a space that offered visual and aural privacy; using strategies that encouraged student talk; and adopting roles of expert teacher, facilitator, clinical expert, and therapeutic agent. Conducting debrief in appropriate settings and adopting strategies and roles that encourage student talk can lead to opportunities for students to reflect on their day, and for facilitators to make judgements about students' knowledge. Facilitators can enable student participation by using spaces that offer physical and aural privacy for debrief, focusing on knowledge within students' scope of practice, and using communication strategies that encourage talk. Background: Communicating about medications across transitions of care is important in older patients who frequently move between health care settings. While there is increasing interest in understanding patient communication across transitions of care, little is known about older patients' involvement in formal and informal modes of communication regarding managing medications. Objective: The aim of this paper was to explore how older patients participated in managing their medications across transitions of care through formal and informal modes of communication. Methods: The study was conducted across two metropolitan hospitals: an acute hospital and a geriatric rehabilitation hospital in metropolitan Melbourne, Australia. A focused ethnographic design was used involving semi‐structured interviews (n = 50), observations (203 h) and individual interviews or focus groups (n = 25). Following thematic analysis, data were analysed using Fairclough's Critical Discourse Analysis. Results: Data analysis revealed two major discursive practices, which comprised of an interplay between formal and informal communication and environmental influences on formal and informal communication. Self‐created patient notes were used by older patients to initiate informal discussion with health professionals about medication decisions, which challenged traditional unequal power relations between health professionals and patients. Formal prompts on electronic medication administration records facilitated the continuous information discourse about patients' medications across transitions of care and encouraged health professionals to seek out older patients' preferences through informal bedside interactions. Environmental influences on communication comprised health professionals' physical movements across private and public spaces in the ward, their distance from older patients at the bedside and utilization of the computer systems during patient encounters. Conclusion: Older patients' self‐created medication notes enabled them to take on a more active role in formal and informal medication communication across transitions of care. Older patients and family members did not have continuous access to information about medication changes during their hospital stay and systems often failed to address older patients' key concerns about their medications, which hindered their active involvement in formal and informal communication. Depression has been the subject of increased awareness and concern in Australia, but there has been little research into how depression is constructed on mental health websites, which have become a major resource for mental health information among the general public. In this study, critical discursive psychology was employed to analyse the informational content of eight major Australian mental health websites concerning depression. Four interpretative repertoires were identified – a biomedical, a self-optimization, a normal-natural and a societal-structural repertoire. The biomedical and self-optimization repertoires were the most prevalent, constructing depression as an illness within an individual occurring as a result of a biological or psychological deficit. Whilst previous studies have identified the predominance of a biomedical repertoire of depression on official websites, this study highlights the growing prominence of a self-optimization repertoire alongside the biomedical. Whilst it appeared that the aim of the websites was to challenge stigma and encourage help-seeking, it is argued that this way of understanding depression may have counter-productive effects in that the problem is located within the individual rather than with society, and individuals may be positioned as responsible for managing their own mental health, under the guidance of experts. Objective: Vaccine hesitancy is a persistent barrier to vaccination uptake, and health professionals report interactions with such parents to be difficult. Using discourse analytic techniques, we examine the foundation of a therapeutic relationship: the display of empathy and attempts to build rapport, in consultations between immunisation specialists and vaccine reluctant parents.Methods: Consultations between consenting clinicians and parents in two Specialist Immunisation Clinics in Australia were recorded. Twelve conversations between the clinicians and parents were analysed using interactional sociolinguistic (IS) discourse analytic methods.Results: This paper takes a case study approach by citing two interactions that exemplify the interactional work of the consultants as they strive to engender mutual understanding and goodwill, noting examples of discursive choices that demonstrate empathy and the building of rapport.Conclusion: Awareness of discourse strategies that interweave relational and clinical goals enable a more nuanced understanding of communication skills that support a guiding partnership in vaccine related decisions with parents.Practical Implications: Through highlighting the strategic interactional work that displays empathy and builds rapport, we can inform educational approaches and build a repertoire of communication choices that strengthen the communication skills of health professionals. 'Intercultural understanding' (ICU) and its core concept 'interculturality', was introduced in the new national curriculum, implemented across Australia from 2013 (Australian Curriculum, ACARA. This paper draws on a study conducted in Victoria, Australia, which asked: how history teachers conceptualise interculturality for history teaching and learning? The study used two methods to gather data: textual analysis and focus group interviews. This paper only reports on specific findings from the focus group interviews, analysed through a methodology of crystallisation and discourse analysis, framed by four modes of historical thinking; traditional, exemplary, critical and transformative. The paper argues that interculturality is a significant challenge to history education often located in discourses constructed over time which disrupt how things have always been taught. By analysing the teachers' talk through a lens whereby the construction of language reveals the educational problematic, the research looks for a way in for interculturality into history teaching and learning. The book is organized into seven chapters and utilizes a corpus-assisted approach and discourse analysis to explore teacher identities and media representations. It focuses on media coverage of teacher-related topics in Australia from 1996 to 2020, but also provides insights into media studies worldwide. Although health services in Australia have an aim to provide inclusive care for their patients/clients, this study highlights how barriers to care can lie at the centre of patient-provider interactions. Racial microaggression is a subtle form of racism that can occur in health settings, leading to further exclusion for First Nations Australians, immigrants and refugees. This paper is guided by Derrida's approach to deconstructionism by unpacking how language is used by health professionals – as holders of organisational power – and how they construct 'truths' or discourses about clients that historically have been marginalised by health services and system. Data comprise 21 interviews with staff from two rural health services. It identified three racial microaggressions were used to justify the challenges of providing care to people from First Nations, immigrant and refugee backgrounds: (1) Participants problematised culture(s) of service users; (2) participants implied cultural superiority in their conceptualisation of 'other' cultures; and (3) participants shared stories of inactions, discomfort and relegating of responsibility. Alcohol is a modifiable risk factor for cancer. Public awareness of the link between alcohol and cancer risk is poor; thus, alcohol consumers may be unknowingly putting themselves at increased risk of cancer. One way to raise awareness of alcohol-related cancer is through placing labels warning of cancer risk on alcoholic beverage containers; however, little is known about the impact of such labels. We conducted seven focus groups, comprising participants who self-identified as low-to-moderate alcohol consumers, to gauge public attitudes towards the labels and messages relating to alcohol-related cancer risk. Transcripts of discussions were coded to identify emergent themes. Participants expressed a negative response to the alcohol warning labels, and their talk worked to challenge the legitimacy of alcohol-related cancer messages, and the entities responsible for disseminating the information. These responses functioned to counter any implied recommendation for reduction in speakers' alcohol consumption. These findings illustrate how the general population make sense of information about health risks, using this knowledge to make decisions about personal behaviour. In combination with other public health initiatives, alcohol-warning labels have the potential to increase awareness of cancer risk and help in the fight against cancer, but any messaging will need to account for probable consumer resistance. In recent years, a discourse of 'ethical artificial intelligence' has emerged and gained international traction in response to widely publicised AI failures. In Australia, the discourse around ethical AI does not accord with the reality of AI deployment in the public sector. Drawing on institutional ethnographic approaches, this paper describes the misalignments between how technology is described in government documentation, and how it is deployed in social service delivery. We argue that the propagation of ethical principles legitimates established new public management strategies, and pre-empts questions regarding the efficacy of AI development; instead positioning implementation as inevitable and, provided an ethical framework is adopted, laudable. The ethical AI discourse acknowledges, and ostensibly seeks to move past, widely reported administrative failures involving new technologies. Public health responses to the vaccine‐refusal (VR) movement are hindered by inadequate research about the movement's aims, identity and perceived value for its members. This study examined how members of the VR movement in Australia described the movement and what being part of it meant to them. Descriptions of the VR movement by 696 members from across Australia were collected between January and May in 2017 via an online survey. The data were analysed using thematic discourse analysis. Members' understandings of the movement and the beliefs underpinning these understandings were examined. Vaccine refusal was underpinned by distinct epistemic beliefs. Participants believed that mainstream vaccine promotion relies on dishonest communication of compromised research. They saw the VR movement as a science‐based movement, researching both 'mainstream' and 'hidden' knowledge, promoting scientific values and advocating for better vaccine studies. Participants believed responsible parenting requires personally researching healthcare choices. Participants constructed the movement's identity in relation to common criticisms of vaccine refusal. These were discredited and repurposed to portray the movement as being brave and righteous. Participants believed people in the movement are astute, informed, responsible and courageous. They believed many members were impacted by vaccine‐related harms, from which the movement now saves others. They saw themselves as fighting for an inconvenient truth that the mainstream ignores. Vaccine promotion needs to address the epistemic beliefs associated with vaccine refusal, yet these have been inadequately understood. Our findings contribute to understanding these beliefs. Furthermore, our findings suggest what messages targeting vaccine‐refusing people should focus on. This may include acknowledging the significant effort that vaccine‐refusing people invest in trying to protect their children, catering to vaccine‐refusing people's high engagement and desire for detailed information, and avoiding stigmatising or confrontational vaccine‐promotion strategies. There have been at least 100 governmental inquiries into teacher education in Australia since the 1970s, making thousands of recommendations, the majority of which have never been fully actioned or realised. Additionally, there is minimal evidence about the efficacy of this continuous review cycle. Here, we problematise these issues by conducting a comparative analysis of the Bassett report, Review of Teacher Education in Queensland, the first review of independent teacher education in Australia conducted in 1978, and the most recent review by the Teacher Education Ministerial Advisory Group, Action Now: Classroom Ready Teachers. Applying discourse analysis techniques associated with Foucauldian archaeology, findings reveal: a self-referential use of policy is accelerating; those involved are less likely to have knowledge of the discipline of education; similarities in the discourses despite the 40-year timeframe; and increased marginalisation of teacher educators. In the light of these findings, the authors highlight the need for teacher educators to be politically savvy about the mythologising and canonising of allowable problems in teacher education. The aim of this study is to investigate ways in which interpreting practice in health care settings can be further developed to better facilitate communication with Aboriginal and Torres Strait Islander patients. Data used include 15 hours of transcribed audio recording from semi-structured interviews with interpreters and Aboriginal Liaison Officers who discuss their perceived experiences of interpreting in medical settings. They offer insights into how language is used to resolve communication differences that may confound the interpreting process and in doing so identify roles interpreters assume and discourse patterns that emerge in interpreting practice. As evidenced through the findings, provisions need to be made for cultural differences. Interpreters report they have to "unpack" medical terminology and explain such terminology and related concepts in tangible terms to ensure patient understanding. Other strategies include talking about taboo topics using culturally appropriate terms, avoiding certain question–answer routines, and being aware of nonverbal aspects of communication. This research aimed to understand how the policy was represented as a 'problem' in food regulatory decision-making in Australia, and the implications for public health nutrition engagement with policy development processes. Bacchi's 'what's the problem represented to be?' discourse analysis method was applied to a case study of voluntary food fortification policy (VFP) developed by the then Australia and New Zealand Food Regulation Ministerial Council (ANZFRMC) between 2002 and 2012. As a consultative process is a legislated aspect of food regulatory policy development in Australia, written stakeholder submissions contributed most of the key documents ascertained as relevant to the case. Four major categories of stakeholder were identified in the data; citizen, public health, government and industry. Predictably, citizen, government and public health stakeholders primarily represented voluntary food fortification (VF) as a problem of public health, while industry stakeholders represented it as a problem of commercial benefit. This reflected expected differences regarding decision-making control and power over regulatory activity. However, at both the outset and conclusion of the policy process, the ANZFRMC represented the problem of VF as commercial benefit, suggesting that in this case, a period of 'formal' stakeholder consultation did not alter the outcome. This research indicates that in VFP, the policy debate was fought and won at the initial framing of the problem in the earliest stages of the policy process. Consequently, if public health nutritionists leave their participation in the process until formal consultation stages, the opportunity to influence policy may already be lost. The Australian government demonstrates strategic anxiety about the 'crowded and complex' geopolitics of the Pacific Islands region. This reflects its broader concerns about geostrategic competition in the 'Indo-Pacific', and its perception that Pacific states are 'small' and 'weak' and therefore vulnerable to influence from potentially hostile powers. Simultaneously, the government has vowed to 'step-up' its engagement with its 'Pacific family', emphasising that its relationships with Pacific states will be characterised by respect for, and listening to them, as equals. But while the government has articulated its intention to improve its relationships with Pacific states, puzzlingly, it adopts policies that undermine this goal. This article analyses how and why this occurs. It outlines what this analysis demonstrates about how leaders and officials perceive the Pacific, what assumptions and habits inform those beliefs, and as the 'step-up' moves from announcement to implementation, how they are translated into behaviour via government policy. It concludes by arguing that Australian leaders and officials should seek consistency in their discourse about, and policies toward, the Pacific, guided by the discourse of the 'Blue Pacific'. This article explores the attitudes and beliefs of 38 people who made claims of anti-white racism in a national survey that measured the extent and variation of racism in Australia. Quantitative analysis of survey data reveals that those who make claims of anti-white racism are nearly twice as likely as the rest of population to hold negative views about cultural diversity and immigration, to identify 'out-groups', and to self-identify as racist. They are also much less likely to recognise the existence of white privilege in Australia. Discourse analysis of these participants' open-ended survey responses reveals their attitudes stem from a belief that their white national identity, and the privilege and ownership that accompanies this, is under threat. They view this 'threat' as a form of racism they are victims of. These discourses of anti-white racism reflect key mechanisms of institutional racism, in particular white privilege. The Northern Territory (NT) government operates remote clinics which are primarily staffed by Aboriginal Health Practitioners and Remote Area Nurses (RANs). RAN practice has been described as particularly complex due to high health needs, workforce shortages and high levels of turnover in remote Aboriginal communities. While individual incentives are offered, there has been little examination of the role and why the work takes such a toll on RANs. This study aims to identify dominant discourses underpinning RAN practice and how these discourses reflect tensions and reinforce power relations that impact on the RAN role. Discourses were identified from a Foucauldian‐inspired discourse analysis of 29 interviews with RANs in six remote NT communities. Five dominant discourses were identified, namely, that permanent RANs are preferred to agency RANs, RANs portray themselves as experienced and certain, RANs use autonomous clinical judgement, Aboriginal staff are important and RAN's belief in making a difference. However, the experience of RANs suggested that there are many types of employment that learning from was also important, RANs often struggled to work with Aboriginal staff and they were unsure if they were making a difference. Furthermore, these discourses created tensions between RANs who were permanent–agency, older–younger, experienced–newer and certain–reflexive. Deconstructing these rigid discourses could allow the RAN role to be reconstructed in ways that lead to better retention, job satisfaction and health outcomes. [ABSTRACT FROM AUTHOR]

In a globalising world, people might be expected to be less nationalistic about their personal consumption decisions. Yet support for Trump-style protectionism and 'buy national' campaigns appears to be spreading. How do people talk about such purchasing decisions? Applying thematic content analysis and discourse analysis to qualitative data from 27 focus groups (n = 223) conducted across Australia, this article explores the key features of discussions about 'buying Australian', comparing migrant/ethnic minority responses with non-migrant/mainstream responses. Perhaps surprisingly, migrants adopt the taken-for-granted imperative that nationalist consumption is good for the national economy, and articulate this choice as a demonstration of loyalty and reciprocity to their adopted country. Perhaps counter to expectations, mainstream participants are far more ambivalent in the ways they express this nationalist imperative. While nationalist consumption remains the bottom line as the appropriate purchasing orientation, their position as charter group enables them to offer more critical arguments about the authenticity of the 'Australian-made' logo and the quality and price of goods. For both, however, the taken-for-granted default position is that consumer nationalism is 'the right thing to do'. Buying national is thus an instance of what Billig identified as 'banal nationalism', where the nation-state, as opposed to other collectives such as 'workers' or 'global citizens', is reproduced as the appropriate category for identification and collective responsibility. Purpose: The adolescent developmental task of establishing autonomy from parents is supported through various aspects of executive functioning, including critical thinking. Our aim was to investigate younger and older adolescent language performance in form, content, and use in response to a moral dilemma task. Method: Forty-four typically developing adolescents completed a language sampling task, responding to stories that contained a moral dilemma for one of the characters. Two age groups participated: younger adolescents (n = 24, 12;2-13;11 [years; months]) and older adolescents (n = 20, 16;1-17;11). Participants produced a monologue in response to an open-ended question prompt. Responses were transcribed and analyzed for discourse production on measures of form (verbal productivity and syntactic complexity) and content (semantic diversity and word percentages in three semantic domains: affective, social, and cognitive). Language use was evaluated using a coding system based on Bloom's revised taxonomy of thinking. Results: There were no significant group differences in performance on measures of syntactic complexity and semantic diversity. Significant differences were found in adolescents' language using Bloom's revised taxonomy. The younger adolescents demonstrated a significantly higher proportion of utterances at Level 1 (remembering and understanding) compared to older adolescents, while the older age group produced a higher proportion at Level 3 (evaluating and creating). Conclusions: The moral dilemma task was effective in demonstrating the growth of adolescent language skills in use of language for critical thinking. The results highlight the clinical utility of the moral dilemma task in engaging adolescents in discourse involving critical thinking, whereas the associated coding scheme, based on Bloom's revised taxonomy of thinking, may differentiate levels of critical thinking and provide direction for intervention. Spirituality has been a part of nursing for many centuries and represents an essential value for people, including nurses and patients. Cumulative evidence points to the positive contribution of spiritually on health and wellbeing. However, there is little clarity about what spirituality means. The literature reveals that nurses have ascribed a diversity of interpretations to spirituality. However, no studies have investigated how registered nurses construct their understanding of spirituality using a critical discourse analysis approach. Therefore, the aim of this study was to uncover how registered nurses construct their understanding of spirituality using a critical discourse analysis approach. Twenty registered nurses from a non‐denominational public hospital and a faith‐based private hospital were interviewed about their understanding of spirituality and practice of spiritual care. A critical discourse analysis approach was used in the examination of the interview texts to uncover underlying social and power features. Links were made between the linguistic features the registered nurses used in their interviews and the broader social context of the study. Three discourses emerged from the interview texts. These include constructing spirituality through personal religious beliefs discourse, holistic discourse and empathetic care discourse. Suicide is a significant issue worldwide and despite comprehensive prevention activities, suicide stigma remains. To explore this issue, we used critical discourse analysis to examine how Australian suicide prevention documents (n = 8) constructed people living with thoughts of suicide. We found that risk and biomedical discourses dominated, with people experiencing suicide ideation constructed as dangerous, different, lacking coping skills, and burdensome. We propose that future suicide prevention activities address potentially stigmatizing language, broaden support and advocacy options, and meaningfully include people with lived experience of suicidal ideation or behavior in the development of policy and interventions. This study explored how psychologists and psychiatrists working in Australian youth mental health services constructed their professional identity, and whether and how implementing Open Dialogue transformed this. Nine clinicians (psychologists, clinical psychologists and psychiatrists) were interviewed after completing Open Dialogue training. Interviews were subjected to discourse analysis. First, two general pre‐existing discursive professional identity positions were constructed: (i) psychiatrists rhetorically distancing themselves from the medical model as 'fixers' of mental illness; and (ii) psychologists and psychiatrists rhetorically embracing their personal identity. Second, participants' responses about implementing Open Dialogue revealed opportunities and discomforts, including: (i) dialogical approaches offering psychiatrists an alternative identity to 'fixers'; and (ii) dialogical approaches generating discomfort at the risk of exposing participants' own vulnerability. Participants' professional identities comprised contrasting positions.
