Detailed analysis of skeletal muscle architecture provides insights into skeletal muscle function. To date, measurements of the human subscapularis architecture have been limited to cadaveric measurements. In this study we demonstrate the feasibility of using anatomically constrained fibre tractography to reconstruct and quantify the 3D architecture of the human subscapularis muscle , and provide the first quantitative measurements of the architecture of the human subscapularis muscle in vivo. mDixon and diffusion tensor magnetic resonance images were obtained from the right shoulders of 20 healthy young adults. Anatomically constrained fibre tractography, in which fascicle reconstructions were forced to terminate on the internal aponeurosis of the subscapularis, was used to reconstruct muscle fibre architecture of the subscapularis muscles. Qualitatively, architectural reconstructions resembled the known subscapularis anatomy well, demonstrating face validity of the reconstructions. Muscle architectural parameters (means ± SDs) were: muscle volume 138 ± 42 cm3, fascicle length 63.6 ± 5.9 mm, physiological cross-sectional area (PCSA) 22 ± 6 cm2, and pennation angle 16 ± 2°. Architectural measurements of the subscapularis fell within the range reported in cadaver studies and were relatively insensitive to variations in fibre tractography parameters. The anatomically detailed whole-muscle reconstructions can be used to quantify the effects of joint surgery on muscle architecture and to advance computational models of the human shoulder. Background. Most data on the architecture of the human soleus muscle have been obtained from cadaveric dissection or two-dimensional ultrasound imaging. We present the first comprehensive, quantitative study on the three-dimensional anatomy of the human soleus muscle in vivo using diffusion tensor imaging (DTI) techniques. Methods. We report three-dimensional fascicle lengths, pennation angles, fascicle curvatures, physiological cross-sectional areas and volumes in four compartments of the soleus at ankle joint angles of 69±12° (plantarflexion, short muscle length; average ± SD across subjects) and 108 ± 7° (dorsiflexion, long muscle length) of six healthy young adults. Microdissection and three-dimensional digitisation on two cadaveric muscles corroborated the compartmentalised structure of the soleus, and confirmed the validity of DTI-based muscle fascicle reconstructions. Results. The posterior compartments of the soleus comprised 80 ± 5% of the total muscle volume (356 ± 58 cm³). At the short muscle length, the average fascicle length, pennation angle and curvature was 37 ± 8 mm, 31 ± 3° and 17 ± 4 /m, respectively. We did not find differences in fascicle lengths between compartments. However, pennation angles were on average 12° larger (p<0:01) in the posterior compartments than in the anterior compartments. For every centimetre that the muscle-tendon unit lengthened, fascicle lengths increased by 3.7 ± 0.8 mm, pennation angles decreased by -3.2 ± 0.9° and curvatures decreased by -2.7 ± 0.8 /m. Fascicles in the posterior compartments rotated almost twice as much as in the anterior compartments during passive lengthening. Discussion. The homogeneity in fascicle lengths and inhomogeneity in pennation angles of the soleus may indicate a functionally different role for the anterior and posterior compartments. The data and techniques presented here demonstrate how DTI can be used to obtain detailed, quantitative measurements of the anatomy of complex skeletal muscles in living humans. This study tested the hypothesis that the ratio of changes in muscle fascicle and tendon length that occurs with joint movement scales linearly with the ratio of the slack lengths of the muscle fascicles and tendons. We compared the contribution of muscle fascicles to passive muscle-tendon lengthening in muscles with relatively short and long fascicles. Fifteen healthy adults participated in the study. The medial gastrocnemius, tibialis anterior, and brachialis muscle-tendon units were passively lengthened by slowly rotating the ankle or elbow. Change in muscle fascicle length was measured with ultrasonography. Change in muscle-tendon length was calculated from estimated muscle moment arms. Change in tendon length was calculated by subtracting change in fascicle length from change in muscle-tendon length. The median (IQR) contribution of muscle fascicles to passive lengthening of the muscle-tendon unit, measured as the ratio of the change in fascicle length to the change in muscle-tendon unit length, was 0.39 (0.26-0.48) for the medial gastrocnemius, 0.51 (0.29-0.60) for tibialis anterior, and 0.65 (0.49-0.90) for brachialis. Brachialis muscle fascicles contributed to muscle-tendon unit lengthening significantly more than medial gastrocnemius muscle fascicles, but less than would be expected if the fascicle contribution scaled linearly with the ratio of muscle fascicle and tendon slack lengths. There are few comprehensive investigations of the changes in muscle architecture that accompany muscle contraction or change in muscle length in vivo. For this study, we measured changes in the threedimensional architecture of the human medial gastrocnemius at the whole muscle level, the fascicle level and the fiber level using anatomical MRI and diffusion tensor imaging (DTI). Data were obtained from eight subjects under relaxed conditions at three muscle lengths. At the whole muscle level, a 5.1% increase in muscle belly length resulted in a reduction in both muscle width (mean change -2.5%) and depth (-4.8%). At the fascicle level, muscle architecture measurements obtained at 3,000 locations per muscle showed that for every millimeter increase in muscle-tendon length above the slack length, average fascicle length increased by 0.46 mm, pennation angle decreased by 0.27° (0.17° in the superficial part and 0.37° in the deep part), and fascicle curvature decreased by 0.18 m-1. There was no evidence of systematic variation in architecture along the muscle's long axis at any muscle length. At the fiber level, analysis of the diffusion signal showed that passive lengthening of the muscle increased diffusion along fibers and decreased diffusion across fibers. Using these measurements across scales, we show that the complex shape changes that muscle fibers, whole muscles, and aponeuroses of the medial gastrocnemius undergo in vivo cannot be captured by simple geometrical models. Precise estimates of muscle architecture are necessary to understand and model muscle mechanics. The primary aim of this study was to estimate continuous changes in fascicle length and pennation angle in human gastrocnemius muscles during ramped plantar flexor contractions at two ankle angles. The secondary aim was to determine whether these changes differ between proximal and distal fascicles. Fifteen healthy subjects performed ramped contractions (0-25% MVC) as ultrasound images were recorded from the medial ( MG, eight sites) and lateral ( LG, six sites) gastrocnemius muscle with the ankle at 90° and 120° (larger angles correspond to shorter muscle lengths). In all subjects, fascicles progressively shortened with increasing torque. MG fascicles shortened 5.8 mm (11.1%) at 90° and 4.5 mm (12.1%) at 120°, whereas LG muscle fascicles shortened 5.1 mm (8.8%) at both ankle angles. MG pennation angle increased 1.4° at 90° and 4.9° at 120°, and LG pennation angle decreased 0.3° at 90° and increased 2.6° at 120°. Muscle architecture changes were similar in proximal and distal fascicles at both ankle angles. This is the first study to describe continuous changes in fascicle length and pennation angle in the human gastrocnemius muscle during ramped isometric contractions. Effects of Isokinetic Eccentric Training on the Human Achilles Tendon. JEPonline 2016;19(2):46-54. This study investigated the effects of isokinetic eccentric training on the cross-sectional area and thickness of the Achilles tendon. Thirty healthy adults were randomized into an Eccentric Training Group and a Control Group. Subjects in the Eccentric Training Group performed an isokinetic eccentric exercise of the gastrocnemius 3 times·wk-1 for 5 wks. Testing occurred at baseline and after training. Achilles tendon cross-sectional area and thickness were determined using ultrasound imaging. After training, there was no significant difference in the cross-sectional area or the tendon thickness between groups (mean difference in cross-sectional area 3.2 mm2, 95% CI -3.3 to 9.7; thickness -0.2 mm, 95% CI -0.5 to 0.2). Thus, the findings indicate that short-term isokinetic eccentric training in healthy young men does not result in tendon hypertrophy. [ABSTRACT FROM AUTHOR]

The length and pennation of muscle fascicles are frequently measured using ultrasonography. Conventional ultrasonography imaging methods only provide two-dimensional images of muscles, but muscles have complex three-dimensional arrangements. The most accurate measurements will be obtained when the ultrasound transducer is oriented so that endpoints of a fascicle lie on the ultrasound image plane and the image plane is oriented perpendicular to the aponeurosis, but little is known about how to find this optimal transducer orientation in the frequently-studied medial gastrocnemius muscle. In the current study, we determined the optimal transducer orientation at 9 sites in the medial gastrocnemius muscle of 8 human subjects by calculating the angle of misalignment between three-dimensional muscle fascicles, reconstructed from diffusion tensor images, and the plane of a virtual ultrasound image. The misalignment angle was calculated for a range of tilts and rotations of the ultrasound transducer relative to a reference orientation that was perpendicular to the skin and parallel to the tibia. With the transducer in the reference orientation, the misalignment was substantial (mean across sites and subjects of 6.5°, range 1.4 to 20.2°). However for all sites and subjects a near-optimal alignment (on average 2.6°, range 0.5° to 6.0°) could be achieved by maintaining 0° tilt and applying a small rotation (typically less than 10°). On the basis of these data we recommend that ultrasonographic measurements of medial gastrocnemius muscle fascicle architecture be obtained, at least for relaxed muscles under static conditions, with the transducer oriented perpendicular to the skin and nearly parallel to the tibia. In vivo measurements of muscle architecture provide insight into inter-individual differences in muscle function and could be used to personalise musculoskeletal models. When muscle architecture is measured from ultrasound images, as is frequently done, it is assumed that fascicles are oriented in the image plane and, for some measurements, that the image plane is perpendicular to the aponeurosis at the intersection of fascicle and aponeurosis. This study presents an in vivo validation of these assumptions by comparing ultrasound image plane orientation to three-dimensional reconstructions of muscle fascicles and aponeuroses obtained with diffusion tensor imaging (DTI) and high-resolution anatomical MRI scans. It was found that muscle fascicles were oriented on average at 5.5 + 4.1° to the ultrasound image plane. On average, ultrasound yielded similar measurements of fascicle lengths to DTI (difference < 3 mm), suggesting that the measurements were unbiased. The absolute difference in length between any pair of measurements made with ultrasound and DTI was substantial (10 mm or 20% of the mean), indicating that the measurements were imprecise. Pennation angles measured with ultrasound were significantly smaller than those measured with DTI (mean difference 6°). This difference was apparent only at the superficial insertion of the muscle fascicles so it was probably due to pressure on the skin applied by the ultrasound probes. It is concluded that ultrasound measurements of deep pennation angles and fascicle lengths in the medial gastrocnemius are unbiased but have a low precision and that superficial pennation angles are underestimated by approximately 10°.
